La Leche League GB: offering breastfeeding information and support to all In previous years we have all heard the mantra “breast is best” however this is now being replaced with the phrase “breast is normal”. Breastmilk contains all the nutrients a baby needs and its unique structures cannot be replicated in formula. Breastmilk adapts to suit our own baby and if a baby is born prematurely it will adjust so that it is ideal for the continued growth that baby needs. Breastfed babies are less likely to suffer from a range of conditions such as gastroenteritis, chest infections, eczema, asthma, celiac disease, crohn’s disease, heart disease, multiple sclerosis, meningitis, ear infections, diabetes in childhood, sudden infant death syndrome, childhood obesity, urinary tract infections, childhood leukaemia, and other cancers.

What are the benefits for Mum?

Which is the best feeding position for baby?

Breastfeeding also reduces a mother’s risk of developing breast, ovarian, cervical and uterine cancer. It protects a mother against developing osteoporosis also reducing the risk of postnatal depression, helps the woman’s uterus to return to normal size after giving birth, and aids weight loss. Women who nurse their babies have a lower risk of developing diabetes.

Getting a baby attached well at the breast is the key to comfortable breastfeeding. Find a comfortable position where you feel wellsupported sitting, reclining, or lying down Use pillows for your own comfort to support your head, shoulders, body, arms and legs, but not under the baby.

When should baby feed? Offer the breast as soon after birth as possible. Breastfeeding is normal and natural, but it’s an art, that may take both mother and baby time and practice to learn, especially if a baby is sleepy or perhaps has a headache from a forceps delivery. It’s distressing when occasionally a baby refuses to breastfeed. However, many things can help with this, such as skin-to-skin contact and lying back with the baby on the mother’s body so that s/he uses innate instincts help find the breast. It’s normal for a baby to bob at the breast and lick the nipple when preparing to latch on. Crying is one of the last cues that a baby gives to tells us that he needs to feed and watching for earlier cues such as stirring in sleep, rooting, and fist sucking can help to get the baby latched on before he is upset. It is quite normal for a baby to want to nurse frequently and to expect a newborn to nurse at least 8 to 12 times in 24 hours.

Baby should be positioned “nose to nipple” so ensure he can tilt his head back and extend his neck. He can then take a big enough mouthful of breast (not just the nipple) and swallow easily. His body should not be twisted; his ear, shoulder and hip should be in alignment. If you need to support your breast keep your fingers well away from the areola and keep the breast at its natural level. Bring baby to the breast and lightly stroke his mouth with your breast until he opens wide like a yawn, then roll the underside of your breast into his mouth and onto his tongue. This pushes the lower jaw open wider. Your nipple is the last part of the breast to enter his mouth and when it reaches the back of the mouth it triggers active sucking. Keep his body tucked in close. Once he is attached you can make small adjustments to how he is positioned to maximize your own comfort and gently tuck his body in close if necessary.

Where can mum get help and support with breastfeeding? Breastfeeding is not always easy at first and that is why nine out of ten women who wanted to breastfeed stop in the first six weeks. Getting the right information and support in the early days can make a lot of difference and going along to breastfeeding support meetings, such as those held by La Leche League, before the baby is born helps prepare for challenges which may arise. Understanding Baby byReflux Baby Eureka Up to 65% of all babies suffer from reflux. But what is reflux? Reflux is the term used to describe stomach contents and acid being regurgitated by a baby. For most babies reflux does not upset them or causes them distress, but it may distress their Mums with the mountains of washing a reflux baby produces. However for 1 in 300 babies reflux is a serious condition that causes a lot of heartache to babies and parents with intervention required.

What causes reflux in babies? It is unclear what causes infant reflux. Some of the triggers which may contribute to infant reflux are: I I I I I

immature digestive system family history food sensitivities Gastreosophagel Disease GERD/GORD Stress e.g. vaccinations/hot weather/teething/travel...

Many babies get reflux because the muscular valve at the end of the food pipe, which acts to keep food in the tummy hasn’t fully developed yet. Therefore, when baby’s tummy is full, the stomach contents and acid can travel up the gullet - causing baby to bring up small amounts of milk or even vomit.

What are the symptoms? Hiccups and bringing up a small amount of milk after feeds is quite normal unless your baby seems otherwise unwell or distressed. Both formula-fed and breastfed babies may be affected by reflux. It is important to note that while “spitting up” is the classic symptom of reflux, it can be absent and a

baby can suffer from silent reflux. It is therefore advantageous to know all the possible symptoms of reflux. Common reflux symptoms a baby may experience: I frequent spitting up (if a baby is otherwise happy and thriving the reflux is not a concern) I crying/fussy during and after a feed I under feeding I comfort feeding I arching their back and neck I excessive bouts of hiccups I hoarse/sore throat I gagging/choking I difficulty sleeping I coughing I drooling I failure to thrive/grow I nasal congestion I recurrent ear and sinus infections I breathing problems such as bronchitis, wheezing, pneumonia, asthma I foul breath Infants with reflux may experience any number of these symptoms. If your baby is exhibiting any or a combination of these symptoms, seek advice from your GP or Health Visitor.

Orla’s daughter was diagnosed with silent reflux at four weeks. ‘’She cried all the time, tossed and turned in her sleep, had frequent bouts of hiccups, and couldn’t drink a bottle as her throat has become so sore and she simply wasn’t thriving. How to Help a Choking Infant Choking is one of the things that parents worry about most . . . and rightly so. It is amongst the top causes of accidental death of UK children. In this feature Impact First Aid Training (as per Resuscitation Council guidelines) will give a stepby-step guide of how to recognise and treat a choking child and baby.

What is choking? Choking is when there is an obstruction which is preventing air (or enough air) from passing down the windpipe (trachea) into the lungs. A blockage can be a partial blockage or a full blockage.

Recognition A baby or child who is choking may start to cough, or try to cough but not be able to. A choking baby may be crying and then go silent due to a blockage. A choking child may grasp their throat and physically show signs of struggling to breathe. Someone who is struggling to breathe may appear red in the face.

What to do

What to do

– for a baby (0 - 12 months)

G First of all, look in the mouth for visible signs of any obstruction. Only if an obvious object is seen then a single finger sweep may be performed to try and remove. Summon help if available to call an ambulance.

G Position the baby face downwards across your arm, supporting the head along the chin bone, with the head lower than the chest.

G Ensure the child is bent over so head is lower than chest. Support the child by placing your hand across the front of the chest.

G Using the free hand, give up to five backslaps with the heel of the hand between the baby's shoulder blades. (see 1 figure 1) Checking the mouth after each slap. Remember these back slaps must be hard enough to dislodge the blockage.

G Give up to 5 back blows with the heel of the hand in between the shoulder blades. Check the mouth after each back blow (see figure 3).

G If after 5 back blows there is no success in removing the blockage, lay the baby down on a hard surface and place two fingers on the lower half of the sternum (just below the nipple line on the breast-bone - see figure 2) and perform up to five ‘chest thrusts’. These must be short and sharp (one third of the chest depth), checking the mouth after each thrust.

3 G If unsuccessful after 5 back blows, stand/kneel behind the child and prepare to give abdominal thrusts (Heimlich manoeuvre). G Make a fist with one hand and place the top of the fist in between the navel (belly button) and sternum (breast-bone) the other hand may wrap round if necessary, depending on the size of the child (see figure 4). 4 G Give up to five abdominal thrusts, inwards and upwards below the ribs. G If unsuccessful, repeat up to 5 back slaps again and cycle. If the child becomes unconscious begin CPR.

2 G If this is unsuccessful, repeat the 5 back slaps and cycle between the two. G If the baby becomes unconscious start cardio-pulmonary resuscitation (CPR) – 5 breaths, 30 compressions then 2 breaths repeated. CPR protocol will be covered in more detail in a future article.

Three Steps to becoming

A Greener Parent There is no denying that having a baby is hard on the environment. New babies have many needs, food, clothes, nappies, nursery products, to name but a few. Furthermore, as any new parent knows they outgrow many of these things in a very short time. Even with the demands that a new baby makes on our planet being green doesn’t have to be overwhelming. Here are a few things that you can try, even if you only do one of them it is a step in the right direction.

Host a Green Baby Shower Throwing a baby shower is one of the nicest ways to celebrate the imminent arrival of a new baby and for a mum-to-be to share the joy of becoming a mother. So why not ditch the pink or blue theme and go green instead – send e-invites instead of paper invitations,

Breastfeeding Some Mums want to breastfeed because they’re convinced – rightly – that it’s the way to give their baby the healthiest start in life. Another way to look at it is that it’s the most natural and straightforward way to look after your child in the first few months. Whenever your little one starts to show signs of hunger, you simply let them feed. 100 percent safe and clean – and you’ll be happy your baby is getting 100 percent of the nutrition needed and NOTHING else!

Top tips for choosing and using Real Nappies 1 Get independent advice from experienced washable nappy users. Someone who has used several different nappies, not just one brand, will be able to give you more idea of the pros and cons of the different types. 2 Try a few out if you can before buying a whole set, so you can be sure you like them. 3 Don’t buy a ‘Birth-to-potty kit’ that includes bigger sizes, because you might change your mind about the nappies you prefer when your baby is older. 4 The quickest-drying nappies are made from synthetic fibres (usually a polyester microfibre). If you prefer natural fibres, one that opens out as much as possible will dry quicker (eg a terry square). 5 For heavy wetters, bamboo and hemp are very absorbent fabrics without being too bulky (they do take longer to dry though!) 6 You will usually need to use a different nappy at night time, or increase the absorbency by adding bamboo or hemp boosters. You will probably need a larger nappy and wrap, or change to the next largest setting if you use onesize nappies. 7 Use a mesh bag to line your bucket, then you don’t have to re-handle dirty nappies when you transfer them to the washing machine. 8 Do a cold rinse cycle to get the worst off the nappies before you add washing powder. At this point you could also add other items to the wash to make up a full load. What’s all the BUZZ about at Wistow Maze?

Leicestershire’s famous Wistow Maze is now open, offering an enjoyable day out for the whole family. The number of bumble bees has declined by around 70% since the 1970s, and honey bee numbers have fallen by 15% over the past two years according to official UK Government figures. Tips for Travelling with Children All families are different, but when it comes down to it there are some basic requirements for a good family trip. Most family breaks run smoothly if well planned with all members of the family taken into account. Ultimately this will mean that you can spend more time doing the things you love and less time discussing where and what you will be doing.

The pleasure is in the detail If this is your first trip with your children, plan for a slower pace than you might usually attempt. Be realistic about what you can cover with little ones in tow – the less you feel you have to pack in, the more enjoyable and stressfree the holiday.

If you are travelling with a small child a pram or buggy can be useful even if your child is walking. It will serve as a place for them to rest when covering a lot of ground or a makeshift bed when they are worn out. An all-terrain version is worth the investment if you are going to be walking a lot or on unpaved paths. If you are travelling with another family, or adults, discuss what each person wants to do before you go and how much time you would like to spend together. Come to an agreement about the way you'll split the bills and arrange how chores will be shared. Discuss taking turns childminding and dedicate childcare responsibilities during travel.

Keeping little ones safe It doesn't matter how good a parent you are, it can happen the best of us! You look away for a just a second and in an instant your child has vanished. Your heart races and your stomach sinks. It's an awful feeling. Right? Always designate a meeting point in each new place, in case anyone is separated from the group. If it's likely you'll be in dense crowds, promise a group reward if everyone stays together.

A wireless child locator system is an ideal way to keep track of your child in crowded places such as shopping centres, airports, theme parks or sporting arenas. The parent carries a tracking device – while the child wears a separate part of the device. Should the distance between the child and the tracker exceed the user-defined range, or if the bracelet is removed, an alarm sounds and a bleeper will help track down your little one. In case of emergencies, an identity bracelet detailing your child's medical condition, treatment and their doctor's name is useful if your child has a disability, hidden medical condition or serious allergy. Always use a car seat when travelling by car regardless of the regulations in your destination. A universal model is a good idea if you are using a variety of vehicles or hiring a car. Childcare Look into supervised childcare, such as kids' clubs before you go. Ask what activities the children will be involved in, how many children are cared for and whether groups are split according to age. Be prepared to check it out yourself when you arrive and make sure that all the necessary paperwork including CRB checks are in place. If anything seems amiss, be prepared to cancel your plans and start looking for alternatives. Having your own personal Nanny can open new doors: cottage/villa holidays can become realistic possibilities, instead of ‘dreams’ for the future, when the children have grown up. If you holiday with other families, you could consider sharing a Nanny to cut costs. In response to demand, Play England has launched Save Children’s Play – a resource that communities can use to establish their own local campaigns for children’s play and be part of a national movement at the same time. In a climate of massive public spending cuts, the play sector faces its most difficult challenge since the 1980’s. Local authorities are facing tough decisions on spending, and children’s play is being hit hard. Sadly, once we start seeing the impact of cuts – play sites falling into disrepair, play schemes closing and play services making staff

redundant, it will be too late. The millions of children who rely on public play facilities will lose out for a generation. But it is not just the fundamental enjoyment of childhood that is being damaged by these cuts. Evidence shows that outdoor play is essential to children’s health, well-being and future life chances. Obesity, rickets and attention deficit disorder are just some of the growing problems in children that health experts have linked to a lack of good play space, and research

also shows a clear link between play and learning. These consequences are not being fully considered as play is targeted for easy savings. Crafty Changes at Burghley Living Heritage announce a major change to their Burghley Craft, Sculpture and Art Fair on Saturday 15th & Sunday 16th October.

As always there will be our Pavilions, where over 90 of the Country’s most talented Craftsmen will be exhibiting and selling their Crafts and Skills. New for this year is the addition of individual marquees of Ceramics, Wood, Glass, Paintings and other mediums which will be the star attraction. Approximately 6 or 8 Craftsmen will be on hand in each specialist marquee demonstrating continually throughout the show.

Take a piece of twisted rayon cord and make a loop at one end. Tie a knot to secure the loop. Thread on large beads in a pattern. I chose to use wooden beads and fabric covered beads.

Once the string of beads reaches your required length, make a loop at the other side, tie a knot and cut off any excess thread. Hook one loop on to a hook, wrap the beaded tie back around the curtain and hook the other loop to hold the curtain.

Programme child safety into your ‘autopilot’ It is a sad fact that every year around one million children, the majority under the age of five, are taken to hospital after an accident at home. During Child Safety week (20 to 26 June) the Child Accident Prevention Trust launched a piece of research that showed that we all carry out everyday tasks on ‘autopilot’ to get through the day because there is simply too much to remember. Think about it, how many times have you gone back to check you have locked the front door and discovered you had? Or sat down and drunk a cup of tea but not really remembering making it? But in stark contrast the research showed that for many parents child safety was not automatic, with less than half stating they did not automatically put hot drinks out of reach of small children and only 21 per cent of parents said they strapped their child into their highchair every time.

First Day Fears Your child's first day at a new school, nursery or playgroup is a big step. However, there are plenty of practical things that you can do to prepare them. It is important not to dismiss your child's fears – things that seem obvious or silly to an adult can seem daunting to a five year old. Talking to your child

G Games, role-playing and reading at home can help your child get into the right frame of mind and boost their confidence. Samantha Cameron Opens New Specialist School for Children With Autism On June 9th 2011, the Prime Minister’s wife Samantha Cameron officially opened Baston House School in Bromley, Kent. My Family were privileged to be among the 100 guests who attended, including Patron, Esther Rantzen and the Deputy Mayor of Bromley, Cllr Reg Adams and Directors of Hillcrest Care. Baston House School is an impressive new day facility providing autism specific education for children aged from 5-16. A centre of excellence in education and development, the school which offers specialist teaching and support for children and young adults on the autistic spectrum, is one of a number of provisions that have been pioneered by autism campaigner Anna Kennedy.

After her tour of the school, Samantha Cameron and other guests enjoyed the official opening celebrations with pupils displaying their talents on the school stage. To everyone’s surprise Samantha Cameron joined them briefly on the stage alongside Anna Kennedy and Esther Rantzen as they danced to Beauty School Drop Out. All three were then proudly presented with gifts from children at the school.

On visiting the groundbreaking facilities, Samantha Cameron said:

I am delighted to open Baston House School. I have great admiration for Anna Kennedy's determination to help people affected by autism and I'm pleased to celebrate this landmark opening with her. “She was quite rightly named 'Most Inspirational Woman of the Year' by the Daily Mail in 2010 and I'm so pleased she has achieved her ambition of opening this school which stands as testament to her dedication to this important cause.

During her visit Samantha Cameron toured the school and met some of the children who will be benefiting from the impressive new facility. The children were happy to meet their VIP guest and eager to show her their classrooms,

which have been specially designed to create a therapeutic environment where pupils learn to work independently without distraction. Samantha Cameron also met some of the teachers who are supporting the children in their learning. The main focus at Baston House School is to tackle the barriers to learning that pupils on the Autism Spectrum encounter; namely communication and interaction difficulties also problems with flexibility of thought and action. Working alongside the teaching staff, specialist teams ensure that students are appropriately supported through every day and every learning experience.

Baston House was beautifully decorated in blue and yellow for the occasion with fresh flowers, balloons, and the pupils work. Guests enjoyed a tasty buffet lunch and there was a beautifully decorated cake made by friend of the school Maureen Jones. With smiling faces throughout the celebration, it was clear to see that everyone had enjoyed the event!

A Step in the Right Direction Autism campaigner Anna Kennedy is due to launch a unique dance and exercise DVD for children and young people with autism. The founder of Hillingdon Manor School produced ‘Step in the Right Direction’ in association with Pineapple Performing Arts Schools. Anna, formerly a dance teacher wants to encourage youngsters on the autistic spectrum to take a ‘Step in the Right Direction’ and find out how much fun expressing themselves through music and dance can be. Most of the filming took place at Pineapple Dance Studios and features children from Anna’s school, alongside a host of famous dance teachers including Pineapple Dance Studios founder Debbie Moore, Maggie Paterson, Jade Flannagan, Andrew Stone and 12year-old, Britain’s Got Talent sensation James Hobley. The Old Vicarage Supported Care Building Independent Lives The Old Vicarage is an eight bedroom residential home, located in a leafy road, ten minutes walk from Uxbridge town centre. Offering assisted living, specifically for adults with an autistic spectrum condition or Asperger Syndrome.

A well-equipped modern kitchen encourages as much independence as is possible. Residents are encouraged to use the space to prepare their own food and labelled cupboards give everyone their own space for shopping. Leading off the kitchen is a spacious dining room which is currently used as a breakfast area.

We met manager Michele Jackson who took time out of her busy schedule to guide us around. Michele explained that The Old Vicarage’s objectives are to provide needsled care packages for residents, whilst offering a consistent, quality staff team, who recognise an individual’s needs and understand their diagnosis. The home access and liaise with relevant healthcare professionals and constantly review the service to offer the best quality of care available.

Comfortable and Homely Communal Areas The Old Vicarage is in a beautiful setting and there is a real feeling of comfort with many of the rooms retaining their original features. Four communal rooms are located on the ground floor with modern leather couches and the latest high-tech gadgets. Michele and her team appreciate that the facility is most importantly the residents’ home and respect the privacy, choices and dignity of those residing there. Personal photos can be found throughout, along with a gallery of images including fancy dress parties and barbecues that the residents have attended.

The Garden

A bright and airy conservatory looks out onto, and provides access to, a large enclosed garden where residents can take a private moment or make the most of the outdoor space. A social hub, the large conservatory is currently the dining area in the summer months where residents can eat together or alone if they prefer. Regular house meetings, attended by residents and staff for open discussions about the home are also held in the conservatory.

Neighbours, staff, and residents regularly attend open days and barbecues in the garden, these events offer the chance to meet each other and embrace The Old Vicarage as part of the community. There is always a good turn out with lots of fun, good food and sometimes even fancy dress. Within the garden, there is a large ‘wellbeing room’ set within a Swiss chalet style building a quiet space, it offers an alternative place to relax without distraction.

Care and Support The Old Vicarage provides a level of care that respects the privacy of its residents whilst recognising and enhancing selfesteem, confidence and personal identity. Service users are supported to access and investigate new ideas, activities, experiences whilst as independently as possible, are encouraged to increase their skill base in all areas of their life. If a resident chooses to move on and live independently, they are helped every step of the way by West London Specialist Support, an outreach service for supported living. INDEPENDENT LIVING West London Specialist Support Services is a supported living and outreach provision for individuals affected by Autistic Spectrum Condition, Learning Difficulties, and Acquired Brain Injuries. Providing needs-led packages of support, the aim of the service is to maximise an individual’s potential and encourage independence skills whilst building on their self-esteem.

Independence for Karl “I moved to Hounslow in February 2009 after living away for a few years. I didn’t like where I was living before and I was excited about moving in to my new house but also a little nervous. My support workers helped me settle into my house and make it feel like home, they showed me how to set up direct debits to make sure my bills are paid.

Two clients Angelo and Karl tell us how West London Specialist Support Services have helped them to gain confidence and independence.

Angelo enjoys his time with Gloria Hello, my name is Angelo and I am 18 years old. I live with my parents and attend Hillingdon Manor School. I enjoy watching DVD’s and using the computer. I have autism which means that I find communicating, understanding others and how to express my own emotions difficult. I sometimes find it hard to imagine situations outside of my routine but love trying new experiences.

My support worker is called Gloria; I meet with her once a week except in the school holidays when we meet more often. She drives and has her own silver car, and I get to sit in the front. I have a support plan that is ‘all about me’ and helps Gloria to understand why I need support and how I would like to be supported. I like to go for walks in the countryside where I can have lots of space and fresh air. It’s a good job Gloria likes keeping fit and enjoys the countryside too! We have been on lots of outings together and my confidence is growing. I like eating out in restaurants and cafes, Gloria knows all the good places to get my favourite food.

“I like to do things for myself but sometimes I wasn’t sure how, I hadn’t done things like hoovering and washing my clothes for a long time. The support workers helped me learn how to do these things again, along with other things such as which bus to catch when I can go shopping or to the pub. I’m now learning how to cook so I can eat my dinner when I want. Where I lived before there wasn’t a choice, I had to have my dinner at the same time every day, but now I can have it whenever I want!’’

Getting your prices right If you’re a mumpreneur, putting a price on your product or service can be tougher than it looks. It may seem like a simple numbers game: just check out what everyone else is charging for that kind of product then charge roughly the same. But the way you feel about yourself can have a huge impact on what you charge if you’re a one-person business. If you are just starting out, you may lack confidence and set your prices too low. And if you have a quiet period in your business, you may start to lose faith in your product or service and drop your prices. Unless you have confidence in yourself, it’s hard to charge a fair price for what you do. But if you don’t value your product or service, nobody else will! So where do you get that confidence? Start with the facts: ask your customers what they like about your product or service. If you don’t have customers yet, ask friends and family if they know of anyone who could give you some feedback. If you sell (say) shoes, ask them how much they spend on shoes each month to get a feel for what your target group are likely to pay for your product. If you know you are offering something of real value, that you know is in

demand and that people are willing to pay for, you’ll have the confidence to set a fair price. Pay attention to your inner voice. Is there a part of you that’s saying you don’t deserve to charge that much? That you’re being greedy? Or that you can’t both earn a decent income and give your children the attention they need? Now is the time to stop listening to that voice! Then work out all your business expenses so that you know how much money you need to make each month to break even. Keep that ‘break-even number’ in your mind whenever you’re tempted to drop your prices because if you go below it, you won’t get paid! Only then should you check out your competitors. Use their prices as a guide but be careful not to compete on price. As a small business the chances are you can’t compete with huge companies because they buy in bulk and use workers overseas. And even if you can, it won’t be long before someone else beats you.
