Respiratory Therapists Successfully Navigate a Reduction in COPD Readmissions

Floyd Memorial has implemented an online check-in tool for use with its Urgent Care centers and Emergency department through a platform called ER Express. This new system allows patients to reserve a spot in line using the online tool, and instead of awaiting treatment in a crowded waiting room, patients can now wait in the comfort of their own homes. When logging onto FloydMemorial.com/online-check-in, patients will see next available time options and can choose their preferred time slot by completing the reservation process. Once the reservation is made, the patient will receive text and email confirmations, and Floyd Memorial staff will be alerted. Although the reservation is made, this does not guarantee the patient an immediate time slot. However, the facility will do its best to see the patient upon arrival. This is due to the fact that patients are still seen in order of emergence and severity. One can adjust an appointment time if necessary by simply making a new online reservation; the system automatically cancels the old one. If a patient arrives later than the appointment time chosen, he or she will need to sign in again at the triage window. Should patients begin to feel worse, or think they may be experiencing a life-threatening emergency after making a reservation, they should call 911 or go to the emergency room.

Floyd Memorial has received the American Heart Association/American Stroke Association’s Get With The Guidelines®-Stroke Silver Plus Quality Achievement Award with Target: Stroke Honor Roll. The award recognizes the Hospital’s commitment and success ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines, based on the latest scientific evidence. Floyd Memorial has met quality measures developed to reduce time between the patient’s arrival at the hospital and treatment with the clot-buster, tPA, the only drug approved by the U.S. Food and Drug Administration to treat ischemic stroke. If given intravenously in the first three hours after the start of stroke symptoms, tPA has been shown to significantly reduce the effects of stroke and lessen the chance of permanent disability.

What started as a pilot program is now a successfully integrated plan put into action by the Floyd Memorial Hospital Respiratory Care Department. They initiated the RT Navigator Program with the goal of reducing readmissions of patients diagnosed with COPD (chronic obstructive pulmonary disease), a condition making it extremely hard to breathe due to a disruption in airflow in and out of the lungs. And unfortunately, lung function usually worsens for the patient over time. With 11 RT navigators now in place, they review and recommend procedures for all patients who are admitted to the Hospital and have the diagnosis or a history of COPD. Further, the navigators initiate a review of a patient’s history and medication in an attempt to assist with home therapy compliance. The team also specifically performs bedside pulmonary rehab to patients in-house. Such therapy services include purse lipped breathing, diaphragmatic breathing, band exercises and harmonica therapy. They also provide education to patients to help them understand their disease state, its process and how or when to use their respiratory medications. According to certified respiratory therapist, Julie Shultz, CRT, “COPD patients are given the tools they need to help themselves succeed. When you witness patients who realize they can now manage their symptoms and experience an improved quality of life all because we invested the extra time working alongside them – that is priceless.” As a result of this program’s implementation, RT navigators are seeing over 200 new patients a month, and since the program’s inception, COPD readmissions have been reduced by more than 43 percent.

Floyd Memorial is pleased to offer 3D mammography for breast cancer screening at its Diagnostic Imaging Center in Corydon located at 313 Federal Drive NW, Suite 020. 3D mammography produces a three-dimensional view of the breast tissue that helps the radiologist “look through” the layers of tissue to identify breast cancer at earlier stages when it is most treatable. Floyd Memorial Website Gets a New Look As most consumers are searching the web from mobile devices, FloydMemorial.com has been redesigned and is now fully mobile-responsive. Other services implemented in the new Floyd Memorial website include online check-in for urgent care centers and the Hospital’s emergency department, online bill pay, health and wellness content, pre-registration and workflow automation.

New Providers Donna Lorenzo, MD, is a fellowship trained neurologist who joins the Floyd Memorial Medical Group – Neurology practice after working as a staff neurologist for both Owensboro Health System in Kentucky and Good Samaritan Hospital in Vincennes, Indiana. Dr. Lorenzo has also worked in both solo and group private practices in the Evansville area. In 1983, she obtained a Bachelor of Science degree in biology from Creighton University. Dr. Lorenzo went on to earn her medical degree there in 1989. Dr. Lorenzo continued her education at the University of Nebraska Medical Center, completing her residency in neurology in 1994 and a clinical neurophysiology fellowship there in 1996. Dr. Ehsan graduated from Tehran University School of Medicine in 1969, where he also completed a pediatrics residency. Dr. Ehsan completed an additional residency in general medicine at the Lutheran Medical Center in Cleveland, as well as rheumatology and clinical immunology/ connective tissue disease fellowships at Alton Ochsner Medical - New Orleans and the University of Louisville respectively. He later became a clinical instructor at the renowned University of Louisville Arthritis Clinic. Most notably, Dr. Ehsan worked as a research physician for the World Health Organization from 1971-1972 and has more than 33 years in private practice. He is a current member of the American College of Rheumatology, ISMA, GLMS, and previously served for 20 years both as a board member for the Kentucky Chapter of the Arthritis Foundation as well as medical director of Southern Indiana A.C.T. arthritis support group. New High Risk Colon Cancer Clinic The Floyd Memorial Cancer Center of Indiana has implemented a High Risk Colon Cancer Clinic designed to screen patients with hereditary red flags of inherited gastrointestinal cancers and Lynch Syndrome. It identifies individuals who carry the mutation that places them at a higher risk of developing a hereditary cancer. Floyd Memorial has partnered with Gastroenterology of Southern Indiana and many other healthcare organizations in the 80 by 2018 campaign to attempt to screen at least 80 percent of the population for colon cancer by 2018. The High Risk Colon Cancer Clinic gives the population that could have genetically indicated gastrointestinal cancers an additional tool for colon cancer screenings and medical management. Genetic testing, along with recommended colonoscopies, helps the patient and family members know how aggressive they need to be with their medical management. Although no cancer

is predictable or preventable, early detection increases the rates of survival. The overall lifetime risk of developing colorectal cancer is about one in 21 (4.7 percent) for men and one in 23 (4.4 percent) for women, and three of every 100 colon cancers are caused by Lynch Syndrome, which in both cases can be genetically inheritable. If a person identifies with any known mutation in their family or family history, recommendations on their medical management are of great importance to intensely monitor any new developments. Spearheading the High Risk Colon Cancer Clinic is board certified medical oncologist and hematology specialist, Yasoda Dev, MD. She explained, “Genetic testing isn’t a futuristic idea, it is happening here and now. Floyd Memorial Enters Into a Purchase Agreement with Baptist Health

After the announcement of a potential acquisition of Floyd Memorial by Baptist Health four months ago, on June 28, 2016, Floyd Memorial, Baptist Health and Floyd County have approved an asset purchase agreement. The asset purchase agreement anticipates an Oct. 1, 2016 transition date at which time Floyd Memorial Hospital and Health Services will officially become Baptist Health Floyd. The sale price includes capital investments by Baptist Health in Baptist Health Floyd of $140 million over the next seven years to expand services, invest in improved technologies and transition to Epic, one of the highest-rated electronic health record systems in the nation. Because the Hospital was countyowned, Floyd County will benefit by receiving about $75 million at closing (after paying transaction costs). In addition, another $61 million will be paid over the next 10 years, some to be designated for ongoing healthcare-related needs and activities. The acquisition will provide greater economies of scale from a business

perspective, plus patients will have easier access to the extensive healthcare services Baptist Health offers. Southern Indiana residents can expect to receive the same excellent care and service they have come to rely on, with a staff they know and trust, now with the added benefit of being under the Baptist Health umbrella. For patients accessing services at Baptist Health Floyd on Oct. 1, 2016, the transition should be seamless. The Hospital intends to continue participation in the same insurance plans. For care received prior to Oct. 1, 2016, billing will come from Floyd Memorial or Floyd Memorial Medical Group. For any care received Oct. 1 or after, billing will come from Baptist Health or Baptist Health Medical Group.

Cardiac resynchronization therapy (CRT), also known as the use of a biventricular pacemaker implantation, is intended for heart failure patients. The special pacemaker simultaneously controls the heart rate of the left and right ventricles, or lower chambers, of the heart. It improves the heart’s efficiency and increases blood flow. Candidates for this type of pacemaker have the following: • Severe or moderately severe heart failure symptoms • Delayed electrical activation of the heart • Risk or history of cardiac arrest

• Taking long-term medications for heart failure Recently, the Food and Drug Administration gave approval for the first magnetic resonance imaging (MRI) compatible cardiac resynchronization therapy defibrillators for the treatment of heart failure. The implants are now approved for use in MRI scanners, which are used to diagnose conditions such as stroke, cancer, Alzheimer’s disease, and muscle, bone and joint pain, on any part of the body without positioning restrictions. Research has shown as many

as 40 percent of these patients will need an MRI within four years after receiving a device. Now, patients in the U.S. who receive these implants have access to MRI scans if and when they need them. The Centers for Medicare and Medicaid Services (CMS) does not cover MRI scans conducted on patients with other non-MRI approved implanted cardiac devices. This new technology will save patients thousands of dollars. Dr. Garimella implants hundreds of devices such as pacemakers, defibrillators and biventricular devices, and performs electrophysiology studies and treatments in Floyd Memorial’s specially equipped catheterization lab. “It wasn’t fun being a grandma and not being able to get down on the floor and play with the grandkids or go to their ballgames because the pain in my groin was excruciating. Getting in and out of a car, putting on socks, climbing stairs, sleeping and sitting are things you take for granted when you don’t have that kind of pain,” said Donita Eurton, an energetic and active woman. Having gone through a hip replacement two and a half years earlier, she knew she had a choice to not live in pain. Floyd Memorial Joint Replacement Center board certified and fellowship trained orthopedic surgeon Aniefiok Uyoe, MD, had performed her first anterior hip replacement in August of 2013, so when Donita started experiencing pain again, she wanted him to take care of her. “Dr. Uyoe was so sweet, understanding and very comforting. I knew he could help me again,” she said. Dr. Uyoe evaluated Donita and ordered an X-ray which showed severe arthritis with bone-on-bone in her hip joint. “It was time for her to have the second hip replaced,” he noted. Dr. Uyoe utilizes the Hana® table for anterior approach hip replacement surgeries. He explained, “This table allows for hyperextension and external rotation of the hip and leg during the hip replacement surgery, which results in smaller incisions, less muscle damage, more accurate placement of the components and quicker recovery and return to regular activities.”

One example I like to give is when we reach a wooded area, we cut down the trees to block the track with a certain type of pain medication, and we continue to block the pain pathway in other ways before it reaches the brain.” Donita delighted in saying, “I was amazed at the difference in recovery from my first hip replacement to the second. The pain wasn’t as bad, and I was in and out of the hospital with just an overnight stay.” Donita received physical therapy both the day of and the morning after surgery, then three weeks of physical therapy at home. “I was back to work in three weeks. Dr. Uyoe is awesome,” she said. “Now I’m back to boating, gardening and dancing to 70s rock and roll music.

Healthier Community Initiative

The Healthier Community Initiative (HCI) and all its activities are wholly funded and made possible by the generous support of the Floyd Memorial Foundation Healthier Community Initiative

1. 64 people participated in local skin cancer screenings, provided by area dermatologists. 2. 60 people participated in the annual Peripheral Arterial Disease screening. 3. 425 people participated in the free monthly Heart & Stroke Screening with one-on-one counseling with a registered nurse. All positive results called and discussed with individual patients, and all results sent to patients and physicians. 4. Organized blood drives where over 50 units of blood were donated. 5. Provided over 750 occult blood screening kits to the community. All positive results called and discussed with individual patients, all results sent to patients and physicians. 6. Over 100 flu vaccines were given for at risk population. 7. Provided monthly screenings for approximately 116 people at the Floyd County YMCA. 8. The Floyd Memorial Foundation funded three community health coalitions: Physical Activity, Nutrition and Tobacco Prevention and Cessation. 9. The Physical Activity Coalition sponsored the 3rd annual Family Fitness Day at the Silver Street Park, provided monthly Fitness Fun Facts,

developed a Wound Center research study and produced 10-minute physical activity videos that are located on Wellnesscsi.com/give-me-10. 10. The Nutrition Coalition gave away 1,000 tomato plants through eight different organizations, developed healthy cooking videos that are located on Wellnesscsi.com/5-a-day, provided monthly fun facts, healthy food samples in Floyd Memorial’s cafeteria and provided free healthy cooking classes for over 150 people in the community. 11. The Tobacco Coalition worked with Floyd Memorial to refer hospital inpatients and observation patients to the Indiana State Quit Line, if requested. Over 1,400 people were referred. Floyd Memorial is the first hospital in the state of Indiana to coordinate a bi-directional communication system between a hospital Electronic Medical Record and the State Quit Line. The Tobacco Coalition continued to sponsor Student Health 101, an online health and wellness magazine for all IUS students, hosted Red Ribbon Week activities and provided CooperClayton smoking cessation programs to the community.

Throughout 2015, Floyd Memorial Marketplace and HIP 2.0 navigators began assisting the community with their enrollment needs of these two insurance programs. Enrollment sessions are held every other month by appointment and are free to the public.

Uncompensated and charity care is care which Floyd Memorial provides without reimbursement to those unable to pay for the cost of their services.

Stroke & Aneurysm Vascular Screenings

By appointment at Floyd Memorial Hospital in New Albany, Floyd Memorial Cardiovascular Center – Jeffersonville and Floyd Memorial Diagnostic Imaging – Corydon Non-invasive screening for Abdominal Aortic Aneurysm, Peripheral Artery Disease and Carotid Artery Disease. Cardiac CT Calcium Scoring Screenings

By appointment at Floyd Memorial Hospital CT scan of the heart to detect calcium deposits found in plaque buildup in the coronary arteries. The level of calcium can signal the presence of atherosclerosis, a disease of the vessel wall, also called coronary artery disease (CAD). This test is one of the most sensitive approaches to detecting the disease before symptoms occur. The cost is $90. Call (812) 949-5570 to schedule your screening.

If you are a current or former smoker (quit in the last 15 years) age 55 to 74, have a 30-pack-year smoking history, a first degree family history of lung cancer and are age 50 or older, or have been exposed to asbestos and been a smoker, this screening could save your life.
