Valet Parking Service Now Permanent Floyd Memorial established temporary, free valet parking in February of 2015 for our Wound, Pain and Spine Center patients because of the parking changes related to the construction of the new CVS on the corner of State Street and Green Valley Road. The service was so well received, Floyd Memorial has decided to continue offering free valet parking on a permanent basis. The valet parking service is located at both the Hospital’s main entrance and the Wound Center entrance. Valet service is available Monday through Friday from 8 am to 5 pm at the main entrance, and from 8 am to 4 pm at the Wound Center entrance.

Board certified family medicine physician and obstetrician Mark Adams, Sr., MD, graduated from Southern Illinois University School of Medicine in 2010, and then went on to complete his family medicine residency at Spartanburg Regional Hospital in Spartanburg, SC. Dr. Adams is experienced in all aspects of women’s health. Floyd Memorial would like to welcome podiatric medicine physician Joseph Spires, DPM, to Floyd Memorial Medical Group – Orthopedics. Dr. Spires completed medical school, magna cum laude, at Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science in North Chicago. He then completed his podiatric surgical residency at Norton Healthcare in Louisville with an emphasis on foot/ ankle trauma and reconstruction. During his residency, he participated in a variety of volunteering opportunities, including aiding the Louisville Ironman and Kentucky Derby medical tents. Dr. Spires is ACLS certified and is an active member of the American College of Foot and Ankle Surgeons and American Podiatric Medical Association. Each year, the Floyd Memorial Foundation, the Floyd Memorial Medical Staff and the Floyd Memorial Auxiliary come together to award $45,000 in educational scholarships for current employees to further their medical and allied health education. The scholarship committee interviewed 17 applicants this year and nine were chosen to receive a $5,000 scholarship for the 2015/2016 school year. At minimum, employees must have been employed for 12 months; accepted into an accredited nursing or allied health program; maintain a minimum 2.7 GPA and have no work disciplinary actions within the past 12 months. As an additional scholarship opportunity this year, the committee chose 17 dependent recipients to receive $500 scholarships for the 2015/2016 school year. Monies for the dependent scholarships were from previous years’ remaining scholarship funds.

WHAS Crusade for Children Provides Generous Grant The WHAS Crusade for Children recently granted $35,000 to the Floyd Memorial Foundation in order for Floyd Memorial to purchase essential pediatric medical supplies. The grant funding will allow the Hospital to purchase an Avalon cordless maternal and fetal heart monitor, a Philips HeartStart MRx defibrillator/monitor and a Natus NatalCare incubator for newborn thermal regulation. The generous support of the WHAS Crusade for Children allows the Floyd Memorial Birthing Center to remain on the cutting edge of infant healthcare, and continue to provide a vital service to the children of our community. The Floyd Memorial Foundation awarded the Floyd Memorial Women’s Imaging Center a grant for $101,000 to purchase the Affirm Breast Biopsy System® to compliment the Hologic® Tomosynthesis 3D mammography unit that was purchased in June 2015. This is the first 3D mammography unit with the Affirm Biopsy System in metro Louisville with this capability. The advantages of having both components available for our patients and the medical oncology community include:

Floyd Memorial Cancer Center of Indiana Hosting Bone Marrow Donor Registry Drive

Be the match. Save a life. Thousands of patients with blood cancers, such as leukemia or lymphoma, depend on the National Bone Marrow Donor Registry to find a match to save their lives. The Floyd Memorial Cancer Center of Indiana will host a bone marrow registry drive to sign up community members interested in joining the registry on Thursday, October 22 from 3 to 7 pm. If you are between 18 and 44 years of age, meet health guidelines and are willing to donate bone marrow if a genetic match is found, you are encouraged to participate. Joining the registry is quick and painless - it only takes a simple cheek swab to potentially save a life. No appointment is necessary. Georgetown resident, Dianne Stewart, has a no nonsense approach to life. Originally from New York, Dianne and her family were living in Mississippi when Hurricane Katrina hit. The disaster sparked their decision to move to Southern Indiana. This was just the first life-changing chapter for Dianne.

BREAST CANCER Just Another Chapter for Dianne Stewart About four years ago, Dianne was performing her daily routine of showering and shaving under her arms. She dropped the razor and felt a sharp pain in her armpit. Dianne thought she had cut herself, but instead, noticed a very small lump. Although she had yearly mammograms, Dianne knew she needed to figure out why the lump had developed. Her OB/GYN suggested she have a breast ultrasound. That’s when she found out she had breast cancer. Dianne explained, “I was really surprised to find out I had breast cancer. Having a small chest, I didn’t think I could even get breast cancer, or if I did, I would see it.” She went to see Floyd Memorial Cancer Center of Indiana’s medical director and board certified medical oncologist/ hematologist, Naveed Chowhan, MD. Dianne said, “Dr. Chowhan gave me my options. He took his time and explained everything.” Dr. Chowhan elaborated, “Each patient who is diagnosed with breast cancer is different, and their treatment plan is unique to them. By targeting specific proteins on an individual’s cancer cells, treatments are designed that are less toxic to the patient and more toxic to the cancer cells by overcoming the resistance that cancer cells often develop. Huge progress has been made in the fight against cancer over the last few years using these very specialized techniques. Dianne’s breast cancer was in the early stage, but the mass was big enough that surgery alone would not take care of it. She underwent chemotherapy, and then I placed her on hormone therapy. We also discussed a mastectomy.” Dianne decided to have a mastectomy on her left breast. Board certified plastic surgeon Michelle Palazzo, MD, did a

full left breast reconstruction and an augmentation on the right breast. During the procedure, nearby lymph nodes were removed and came back with negative margins – it did not spread. Dianne explained, “Dr. Palazzo asked me what size I wanted to be; I told her, surprise me (but not too big)! She gave me a perfect size for my body frame and shape, and now I wear tight shirts and show the world. I’m not embarrassed or ashamed of what I went through.” Dianne is a true survivor, and in a no nonsense style, added, “We search the web when we’re diagnosed with something like this, and all you see are negative stories, and it really raises your expectations of how bad the treatments are going to be. Sure, I got a little sick with chemo and lost my hair, but you do what you have to and move on. I’m not one to whine and worry. It’s just another chapter in my life. God helps those who help themselves.” “Dr. Chowhan is amazing, and his nurses and staff at the Cancer Center of Indiana are angels sent from above. They were all wonderful to me and the best I could ever hope for,” said Dianne. Dianne will continue to see Dr. Chowhan every six months until she reaches the five year mark. After that, she will have yearly follow up visits. Dr. Chowhan noted, “Floyd Memorial now has the latest 3D mammography technology. Breast cancers like Dianne’s may now be found even earlier. 3D Mammography Now Available at Floyd Memorial Women should have yearly screening mammograms after age 40. In order to help detect cancer early, the Floyd Memorial Women’s Imaging Center now offers 3D mammography at a total cost of $60. It is fully covered by Medicare. Golf Game Back in His Future Thanks to Spine Surgery

He was a pitcher for Indiana State University and went on to become the football coach and athletic director at New Albany High School, along with teaching biology and driver’s education. After retiring in 1998, he was able to pursue his love of golf. Living right off a golf course gave him easy access to the game. Michael J. Developing Problems In late 2014, over the course of about five months, Gary had subtly developed weakness in his arms and legs, numbness in both arms and hands and balance problems that became increasingly worse. He tried epidural shots and other conservative treatments that were not working. “I had to stop playing golf, and living right off a golf course, it isn’t easy to sit and watch,” said Gary. After asking his primary care doctor and many friends and medical professionals for a surgeon they recommended, the name he kept hearing was board certified neurosurgeon and spine surgeon at the Floyd Memorial Spine Center, Michael J. Doyle, MD. Dr. Doyle ordered an MRI of Gary’s cervical spine and immediately discovered the problem. He explained, “As soon as I saw his MRI, I was concerned, especially knowing Gary had balance issues. The MRI showed a large disc herniation. Gary had previous back surgeries, so his situation was fairly complex. He had a big fragment of disc that had worked its way into his spinal 6

canal, causing cervical myelopathy, or pressure on the spinal cord. He was at risk for spinal cord damage, even paralysis, if we didn’t get this taken care of. It can cause all kinds of symptoms from the neck down.” He added, “Cervical myelopathy is sometimes underdiagnosed because as people age, their balance is off, their gait is uneven, they stumble and they attribute it to age, when it could possibly be a medical issue that can be fixed. Some patients don’t even have neck pain or arm symptoms. It’s something we try to keep an eye out for.” Fixing the Problems “During his anterior cervical discectomy surgery, we were able to enter through the front of his neck, and fortunately, tease the fragment out of his spinal canal without having to remove too much bone,” said Dr. Doyle. “We were able to take some of the bone spurs from his spine to use as bone graft for a spinal fusion, then applied a thin metal plate to stabilize the vertebrae.” “Gary had made pretty good progress since the surgery, with much of the arm

weakness and pain alleviated after surgery. I felt physical therapy could help him regain strength and mobility, as well as help his balance issues,” he said. Physical Therapy After surgery, Floyd Memorial Physical Therapy - Highlander Point’s, Tyler Helm, PT, DPT, worked on increasing Gary’s strength, stability and endurance. He explained, “I focused mainly on the lower extremities because they take longer to heal. Gary was great to work with because he is very motivated and picked up on therapy so quickly.” After six months of therapy, Gary is able to start putting and chipping golf balls. Dr. Doyle expects a near full recovery within one year. “Dr. Doyle and his team at Floyd Memorial were so great to me. I can’t thank them enough for helping me get back into my life and onto the course,” said Gary. Diabetes is a disease of metabolism, which is the process by which our bodies use digested food for energy. The hormone, insulin, allows the body to absorb glucose, a form of sugar, and use it for energy. People with diabetes have high blood glucose (blood sugar) levels due to inadequate insulin production, unresponsiveness of the body’s cells to insulin or both. There are three main types of diabetes: type 1, type 2 and gestational diabetes. With type 1 diabetes, your body does not make insulin. This type of diabetes is typically diagnosed in children and young adults, but can happen at any age, and was previously known as juvenile diabetes. With type 2 diabetes, the more common type, your body does not make or use insulin well. Type 2 diabetes is associated with excess weight, physical inactivity, family history of diabetes, previous history of gestational diabetes (in women who are pregnant) and certain ethnicities. According to Vasdev Lohano, MD, medical director at the Joslin Diabetes Center Affiliate at Floyd Memorial, there are several risk factors to consider when screening for diabetes, including age, family history, high blood pressure, high triglycerides, low HDL, obesity, sedentary

lifestyle and African American and Hispanic ethnicities. Dr. Lohano also labels the following as signs and symptoms of diabetes: fatigue, increased thirst, increased need to urinate, weight loss and recurrent infection, like yeast infections. “If someone is experiencing these symptoms to an abnormal degree or is experiencing them in pairs, I suggest talking to a doctor and checking the fasting blood sugar or the hemoglobin A1c level,” said Dr. Lohano. “It is important to be aware of your body and act quickly when you notice something has changed. Studies have shown that if you have pre-diabetes, you can work on changing your lifestyle, and if you lose seven percent of your body weight, you reduce your risk of developing type 2 diabetes by about 60 percent.” The Joslin Diabetes Center Affiliate at Floyd Memorial offers a comprehensive management program for all types of diabetes. Along with board certified endocrinologists, there are also certified diabetes educators and registered dietitians on staff to help you manage your diabetes with a number of classes and support groups. “Unique to the Center, we can also take care of other metabolic and endocrine disorders, specifically the full spectrum of thyroid

Once a patient decides to seek help for diabetes, the Center offers a comprehensive course on diabetes, along with an initial assessment, which is a one-on-one appointment tailored to determine the patient’s education needs. Then the patient chooses to attend one of three class formats that are available:

The course covers the proper way to check blood sugar, dosage of insulin and ensuring the patient is up-to-date with the standards of care published by the American Diabetes Association. Other topics include high vs. low blood sugar monitoring, meal planning, stress management, healthy coping skills, “sick day” rules, acute complications and prevention and chronic complications and prevention, such as eye or kidney disease and nerve damage. Beth Ackerman, RD, CDE, program coordinator for the Joslin Diabetes Center Affiliate at Floyd Memorial said, “We try to emphasize the importance of staying plugged in with whatever avenue the patients choose to take. We focus on providing our patients with a support plan that includes appropriate encouragement and resources.” There is also a refresher class, which is a two-part series for people who have been to a comprehensive course before, but it has been several years. The first, two-hour class focuses on reviewing medication and monitoring signs and symptoms of acute

Some of it is confidence, knowing what path to take and how to do things correctly. The other part is the relationships people build, and the feeling that they aren’t alone,” said Beth. In addition, the Center offers a monthly Balancing Life with Diabetes Support Group, a Pre-Diabetes class and a Gestational Diabetes class.

FLOYD MEMORIAL’S IMPACT ON OUR COMMUNITY Each year, Floyd Memorial Hospital and Health Services invests millions of dollars in the health and wellness of our community. Healthier Community Initiative (HCI) The Healthier Community Initiative and all its activities are wholly funded and made possible by the generous support of the Floyd Memorial Foundation.

1. 112 people participated in local skin cancer screenings provided by area dermatologists. 2. 54 people participated in the annual Peripheral Arterial Disease screening. 3. 478 people participated in the monthly Heart & Stroke Screenings. 4. Organized blood drives with over 71 units of blood donated. 5. Provided occult blood screening kits to the community. 6. Flu vaccines provided for at-risk population. 7. Held monthly screenings at the Floyd County YMCA. 8. The Floyd Memorial Foundation funded three community health coalitions: Physical Activity, Nutrition and Tobacco Prevention & Cessation. 9. The Physical Activity Coalition sponsored the 2nd annual Family Fitness Day at the waterfront, provided monthly Fitness Fun Facts, developed a Wound Center research study and produced 10-minute physical activity videos that are located on Wellnesscsi.com/give-me-10. 10. The Nutrition Coalition gave away 1,000 tomato plants through eight different organizations, developed healthy cooking videos that are located on Wellnesscsi.com/5-a-day and provided monthly fun facts and healthy food samples in Floyd Memorial’s cafeteria. 11. The Tobacco Coalition worked with Floyd Memorial to refer hospital inpatients to the Quit Line, if requested. We are the first hospital in Indiana to coordinate a bi-directional communication system between a hospital Electronic Medical Record and the state Quit Line. 12. The Tobacco Coalition continued to sponsor Student Health 101, an online health and wellness magazine for all IUS students, hosted Red Ribbon Week activities and provided Cooper-Clayton smoking cessation programs to the community. 13. 209 lung cancer screenings were performed. 14. Sponsored seven mini-grants ($1,000 each) to local organizations for projects related to physical activity, nutrition and tobacco prevention and cessation. 15. Reached 433 people through community seminars. 16. Hosted 16 community support groups. 17. Provided free screenings to over 3,000 people. 18. Collaboration of enrollment events for the Marketplace Exchange.

The Floyd Memorial Foundation Community Giving

The Floyd Memorial Foundation is a tax-exempt organization established solely to support the mission and needs of Floyd Memorial Hospital and Health Services and its affiliate organizations. The Foundation acts as a vital link between the hospital’s vision of excellence and its financial capacity to provide the best for the community it serves. Financial contributions from individuals, families, organizations and corporations help support advanced and effective resources in order to promote quality health and healing through sophisticated technology, community health screenings, medical and nursing scholarships, as well as support for the Floyd County Family Health Center.

Grant Giving Floyd Memorial Cancer Center of Indiana – $2,000 Funds to help support the Cancer Survivors Reunion on June 6, 2014. Approximately 200-250 cancer survivors attended with family members.

Floyd Memorial Named a Winner of the Healthy Communities Achievement Award On March 6, 2014, Floyd Memorial Hospital and Health Services was one of two winning healthcare facilities from across the nation recognized by the Healthy Communities Institute (HCI), a leading provider of technologies and services for community health analysis, population risk profiling and evidence-based health improvement programs, at their annual Healthy Communities Achievement Awards. Floyd Memorial was chosen for demonstrating how data can be effective in driving strategic vision, identifying at-risk populations, prioritizing local health needs, building effective partnerships and strategically targeting resources where they can have greatest impact within the community. Floyd Memorial’s primary community benefit focus is in Floyd County, a community of over 75,000. Over the past two years, Floyd Memorial has dramatically shifted their approach to community health improvement. In the past, programs focused mainly on screenings and informational booths at local health fairs. Now, fueling the hospital’s strategic direction, they have completed a community health needs assessment, available at FloydMemorial.com, identified priority areas,

launched targeted action plans, engaged community coalitions and funded a grant-giving program aligned with focus areas. For example, Floyd County ranks 83 out of 92 counties in Indiana for tobacco use, and by diving into the data, they identified related at-risk populations for intensifying their health improvement efforts. Floyd Memorial also saw, in many cases, that resources were being applied to populations with relatively good health indicators. With their new approach and a mini-grant funding program, Floyd Memorial was able to launch three community-based coalitions specifically chartered to make a measurable impact: Physical Activity, Improving Nutrition and Tobacco Prevention & Cessation. Floyd Memorial is currently working on their 2016-2019 community health needs assessment.

Marketplace Exchange and HIP 2.0 Help Available In the fall of 2014, Floyd Memorial Marketplace and HIP 2.0 navigators began assisting the community with their enrollment needs of these two insurance programs. Enrollment sessions are held every other month, by appointment, and are free to the public. Home Healthcare – $4,528.08 Pill organizers to distribute to home health patients to help reduce medication errors and prevent readmission to the hospital. Family Health Center – $25,000 Support operating expenses and staff so FHC can continue to serve uninsured, low income residents of Floyd County. Women’s Services – $8,304 Purchased 18 over-bed tables and 18 night stands for patient rooms. Food & Nutrition – $9,010 Purchased grab-n-go vending machine with healthy food options provided by the cafeteria (sandwiches, salads, etc.). Respiratory – $8,716.25 Purchased two Vapotherm Hi-Flow Oxygenation systems. Vapotherm provides high flows to relieve the work of breathing, and control accurate FiO2 to maintain oxygenation on patients that present to the hospital in respiratory distress. Medical Care Management – $1,054.80 Purchased 10 new Evenflo Embrace car seats. Families at high risk for social and economic adversity who cannot afford a car seat will be provided a safe and reliable car seat for their newborn. Joslin Diabetes Center – $1,000 Purchased approximately 300 diabetes cookbooks in order for the Joslin Diabetes Center Affiliate at Floyd Memorial to provide diabetes friendly recipes, developed by Joslin educators, to patients finishing Diabetes Self-Management Education so they may prepare diabetes appropriate foods. Physical Medicine - $2,500 Funds continued to support the Pink Ribbon Program for Breast Cancer Survivors. Specifically, funds support a certified Pilates instructor who offers free Pilates programs twice a week for breast cancer survivors to increase and strengthen self-esteem and quality of life. Nurse’s Day – $5,379.78 This celebration is an opportunity to honor nurses who provide care to patients in our community. Healthier Community Initiative - $85,425 Scholarships - $30,000

Cardiac CT Calcium Scoring Screenings

By appointment Floyd Memorial Hospital A CT scan of the heart to detect calcium deposits found in plaque buildup in the coronary arteries. The level of calcium can signal the presence of arthrosclerosis, a disease of the vessel wall, also called coronary artery disease (CAD). Quinoa with Ginger Carrots – Serves 8 1 tablespoon extra-virgin olive oil 1/2 medium onion, finely diced 4 cloves of garlic, minced 2 large carrots, cubed 1 tablespoon minced fresh ginger root 2 cups quinoa 4 cups water Sea salt (optional)

Using a 3-quart saucepan over medium-high heat, sauté onion, garlic, carrots and ginger root in olive oil until the onions begin to brown; about three minutes. Stir in quinoa and water; bring mixture to a boil. Cover, reduce heat and simmer on low for 15 to 20 minutes. Remove from heat. Allow to stand 10 minutes. Fluff with a fork.
