SENIOR OUTREACH
Senior Outreach offers support for the well-being, independence, and dignity of older adults through community education, mental health services, and connection to community resources. The program identifies older adults who may need emotional or physical support and/or connection to community services. Examples of support may include age-related, transitional changes such as: retirement, grief and loss, or medical conditions. It is a straightforward way to help older adults get assistance before a crisis develops.
WHAT ARE SOME BENEFITS OF THE SENIOR OUTREACH PROGRAM?
Encouraging seniors to thrive in their community by gaining support, education, tools, and resources are some benefits of Senior Outreach. Data has indicated older adults tend to isolate more and subsequently their health and self-care are impacted. Senior Outreach targets these needs to provide the help and strategies to reduce risk.
HOW DOES THE SENIOR OUTREACH PROGRAM WORK?
People in the community identify older adults who may need help and contact our call center to discuss their concerns and make a referral to Senior Outreach. Our Senior Outreach staff will then telephone the older adult to offer them services.
WHAT SERVICES DOES THE SENIOR OUTREACH PROGRAM PROVIDE?
• Counseling (in-home, office, or telehealth)
• Care Management referrals
• Psychiatry/Prescriber
• Seniors Supporting Seniors Group
• Senior Outreach Workgroup
• Connection to community resources
• Depression screening and treatment
• Identification and early intervention of prescription drug misuse or abuse
WHO IS ELIGIBLE FOR THE SENIOR OUTREACH PROGRAM?
Anyone who is 60 years of age or older and lives in Oakland County is eligible for Senior Outreach home visits. However, individuals in other counties are also eligible to participate in the program through in-office or telehealth visits, if available.
WHAT IS THE COST OF THE SENIOR OUTREACH PROGRAM?
Easterseals MORC accepts most private insurances for Senior Outreach, including Medicaid and Medicare.
HOW LONG DOES THE SENIOR OUTREACH PROGRAM RUN?
Senior Outreach is a voluntary program and services typically depend on the needs of the individual.
