What is Person-Centered Planning (PCP)?

In life there is no such thing as “one size fits all” when it comes to treatment. This means that every person over the age of 18 receiving services has the right to have a plan that reflects their opinions, preferences, values, and to control the decisions that impact their lives. Your plan is based on your strengths, includes your cultural/ linguistic/ spiritual preferences, and focuses on you as a whole person. You may choose to include other support systems or people and create connections to community activities and resources. Meetings that are used to create your person-centered plan will be facilitated in a respectful and professional manner and the process will be transparent and accessible to you and other special people in your life (if you choose) to be a part of the process. Your person-centered plan will become a ‘living document’ meaning that you can always change the plan or set new goals. It will include your ongoing feedback and participation to make sure it is followed and successful and modified as needed.

What is Family-Centered Planning (FCP)?

At Easterseals MORC we recognize that parents/guardians are the experts on their own child and family. This means that your child with the support of parents/guardians receiving services has the right to have a plan that reflects their opinions, preferences, values, and to control the decisions that impact their lives. You will work together with your clinician to create a treatment plan that is based on your child’s and family’s strengths, preferences, desires, and cultural/ linguistic/ spiritual preferences. You, your child, and family, may choose to include other family members, friends, neighbors, school personnel, religious supports, and other professionals, as your own support team to help build and carry out your plan and create connections to community activities and resources. Your planning meeting will be scheduled at a time and location that is convenient for you and include those you invite, will be facilitated in a respectful and professional manner and the process will be transparent and accessible to you and others you choose to be a part of your planning meeting. Your family-centered plan will become a ‘living document’ meaning that you/your family can always change the plan or set new goals. It will include your ongoing feedback and participation to make sure it is followed and successful and modified as needed.
