WRITING GUIDANCE FOR FIELD ASSESSMENT REPORT Sustainable Plantation Forest Management Certification

1. Introduction Field assessment is an important part of the Sustainable Plantation Forest Management (SPFM) certification process. Field assessment reports containing clear, complete and systematic field information will help a great deal in the decision making process by Expert Panel II.

2. Scope This guide provides reference to Field Assessors in report writing. Field assessment reports needs to be clear and orderly to assist Expert Panel II in conducting performance evaluation and SPFM certification decision making.

4. Information Presented in the Report 4.1 Document Identity 4.1.1

Document Identification

Documents presenting field assessment results is labeled FIELD ASSESSMENT REPORT. The report consist of two unseparable parts: Book I, Field Assessment Report of a Management Unit; and Book II, Supplement of Field Assessment Report of a Management Unit. Book I contains detailed method and procedures to conduct field assessment, field assessment result on each indicator, and relevant additional information. Book II contains supplements of documents related to field assessment result. 4.1.2

Page Numbering

Every page is numbered which shows parts of the whole document. Every page must be numbered and printed at the lower center of the page, including the page for chapter title, by printing the page number and of the number of pages in the book.

4.2 Identity of a Certification Institution and Team of Field Assessors This part contains descriptions of the Certification Institution, address, head of the institute, and Field Assessment Team organization that states how many personnel involved and responsible to the assessment. The document is signed by the head team of Field Assessor or one who has the authorization to control every factor affecting field assessment results. 4.3 Identity of the Certification Applicant This part contains a brief statement which includes the name of the management unit, address of head and branch offices, location to be assessed, names and jobtitles of persons with authorization in controling the management unit at the head level or at the assessment site, and other general information concerning the unit.

4.4 Field Assessment Method Field assessment process refers to LEI Guide 99-31 regarding SPFM Certification Field Assessment Guide. 4.5 Field Assessment Result The result contains detailed field data analyses presented properly and clearly to demonstrate systematic decision making flow of every indicator. The field assessment results consist of: 4.5.1

Management Unit Typology Principle

Identification of factors that drives the formation of a management unit typology based on every principle (production function sustainability, ecology/environment, and social, economy and culture), and the assessed principles of typology of the management unit. The typology drivers on each criteria and its rationale should be detailed clearly. 4.5.2

Field Assessment Results of Each Indicator

A precise and clear description of data analyses of each indicator. Field data, maps, written documents used in the analyses or explication of the assessment result of each indicator must be stated in the supplement. Supplement numbers must be printed on each field assessment result. 4.5.3

Decision Making of Each Indicato

A detailed information on the value or scale of intensity of each indicator that states every good, sufficient or bad condition of the indicator and its reasons must be clearly presented. 4.6 Additional Information This part means that a relevant additional information within the context of SPFM certification found in the field that needs to be reported to Expert Panel II to assist in the decision making must be presented.

4.7 Supplement The supplement contains field data, maps, and other written documents used in the analysis or explains field assessment results for each indicator. The list of supplements showing the location of supplement of an indicator in the supplement document must be presented at the beginning of the document. 5. Correction Corrections on the field assessement report document can be done at the clarification process between Expert Panel II and Field Assessors, and the assessed management unit. The result must be added as supplement at the final report documented by the Certification Institution.
