When is the best time to intervene to ensure a healthy pregnancy?

Interventions to improve a pregnancy outcome are most effective when implemented prior to pregnancy. Maternal health and wellness allows for healthy conception. In the weeks prior to a missed menstrual cycle, major organ systems are formed in the fetus. Maternal circulation and hormones support the newly conceived fetus and implantation of placenta. Exposure to toxins and infectious agents will also be exposed to the fetus.

What is the best way for a woman to prepare herself for pregnancy?

We make the following recommendations in accordance with our governing professional bodies (American College of Osteopathic Obstetricians and Gynecologists and the American College of Nurse-Midwives). We assure adequate folic acid intake through a diet rich in folate and multivitamins containing folic acid 400 micrograms. We stress avoiding alcohol and illicit substances before trying to achieve pregnancy as both increase the risk for serious birth defects and developmental disabilities. A woman must quit smoking and avoid secondhand smoke as this too can decrease fertility. Smoking during pregnancy limits a baby’s growth and increases risk for premature birth and placental abnormalities. Be sure and wash all fruits and vegetables as pesticides are used on many farms. We also suggest using natural cleaning agents whenever possible.

Summertime is finally here!

It’s Summertime! What a wonderful thing to say after what felt like a long six months. Each year, I look forward to the carefree season when the sun is brighter and the days are longer. Whether it is picking apart crabs with a few friends or relaxing down the Shore with my family, Summertime is the perfect setting for everything fun. The 3 C’s: Covid, Custody & Co-Parenting By Marla Marinucci, Esq.

No doubt these are unprecedented times. Between the Covid-19 pandemic, the upcoming election, recent crazy weather events (a tornado touched down in Ocean City a few days ago), and a slew of other strange events, it has been difficult to find a silver lining in anything recently. I have always tried to find at least one positive aspect in everything, regardless of how negative a situation is. For example, as family law attorneys, something we have been wanting for years has been the ability to file documents electronically. Other courts and divisions have had electronic filing for many years, but not in the family part. Thanks to the pandemic, we now have electronic filing, and it is awesome. It has made life so much easier, and for the most part has had the effect of expediting things. Additionally, while I was initially not a big fan of having court appearances done via Zoom or other digital platform, I cannot imagine ever going back to having all in-person court hearings and am enjoying not paying $10 to park every time I have court in Atlantic City. I also cannot remember the last time I wore a suit, which has been another huge plus. Granted, in no way do I believe that this pandemic has been a good thing. The point I am trying to make is that out of bad, can come good. Let’s talk about custody and the effects Covid-19 is having on coparenting. Take, for example, the Governor’s order to close all schools starting in mid-March. I was so thrilled to learn of many situations of parents (divorced or just not together anymore) who successfully, and without court intervention, co-parented with each other to modify parenting time in light of the new normal: remote learning. In nearly every case of this kind that I heard about where the parents worked things out themselves, the noncustodial parent’s time increased substantially. No doubt there were plenty of cases that had the opposite result whereby one party used the pandemic as a shield in an attempt to prevent the other party from having any parenting time at all. However, I found that such a tactic was immediately shut down by the Court absent certain unique circumstances that warranted a suspension of parenting time to one party.

There are many cases that, due to distance (i.e., the parents reside in different states far apart), one parent is not able to enjoy much parenting time during the school year because it’s either too expensive (requires plane tickets) or there are not enough school breaks to warrant parenting time beyond Christmas break, spring break, and the summer. Even if an agreement states that one party is entitled to all three-day weekends during the school year, that is hardly enough time for anyone, nor is it fair to the child, because if it involves flying, there will be substantial time spent in the airport on two of the three days, and it’s just plain exhausting. With Covid-19 and the advent of remote learning, now is the time for the non-custodial parent, who rarely gets to see their child/children during the school year due to geography, to enjoy more parenting time. This applies to parents who live near each other as well. From what I have gleaned online, most school districts nationwide are offering a few different options for the 2020-2021 school year, including a hybrid schedule that involves a combination of in-person learning and remote learning, as well as an all-remote-learning option. As of the time of this writing, it appears possible that the Governor in New Jersey could once again order that schools in New Jersey close again out of safety precautions for the staff and the students. Assuming there are no parental fitness issues involved, and the pre-pandemic parenting schedule was determined based on each party’s work schedules at the time, and of course the child actually attending school, if one or both parents are now working from home, AND your child’s school is either one that involves two or three days of remote learning and two or three days of in-person learning, or all-remote learning, now is the time to flex those atrophied co-parenting muscles and take advantage of these unprecedented times. It is the child’s right to spend as much time as possible with both parents. More often than not, one parent spends far too much time and energy limiting the other parent’s time with the child for reasons that have nothing to do with the safety of the child, and for the most part are founded in spitefulness. Remote learning can be done anywhere, near and far. A child who lives with Mom and attends school in New Jersey and whose school has implemented an all-remote learning plan can follow that same plan with Dad, who lives in California, and with whom the child may not spend a lot of time due to distance and school. Temporarily modifying a parenting plan does not require a court order provided both parties agree. This is what co-parenting is all about.

Health & Wellness Yogic Freedom Heidi Farber What does freedom mean to you? Does it conjure words like autonomy, independence, liberty, and selfdetermination? Choosing the way we live our lives is how we expect to find happiness. From a yogic perspective, true freedom is to decide that you do not want to suffer anymore and there is no reason for stress, inner pain or fear. We all experience insecurity, selfconsciousness and anxiety. When Buddha said that all of life is suffering this is what he was referring to. All that you perceive and experience is in the mind. If you can look into the contents of the mind and turn your attention inward, you can begin to free yourself from unwholesome thoughts and experience yogic freedom. All you have to stop expecting the mind to fix what’s wrong inside of you. That is the core, the root of it all. When your mind starts telling you what you have to do to make everything inside okay, don’t buy into what it is telling you. The truth is that everything will be okay as soon as you are okay with everything. That is the only time that everything will be okay. Stop trying to protect yourself from the natural unfolding of life. By watching the mind, you will notice that it is engaged in the process of trying to make everything okay. Remember that this is not what you want to do. Disengage and stop fighting your mind because you will never win. The key is becoming quieter. As you watch your thoughts pass and observe the thinking mind, you will begin to relax and recognize that you are not those thoughts. Instead, you are aware of the thinking mind. You are just there, aware of what is there. The mind only runs as you are giving it the power of your attention. Withdraw your attention, and the thinking mind falls away. Begin your journey to freedom by regularly reminding yourself that you will not get

involved in your own melodrama. Untangle yourself from the endless thoughts that consume your life. You don’t have to do anything. Just be there, noticing that you notice. It is your birthright to have opportunities to be happy, and if you choose, to have opportunities to seek freedom. As in any good yoga practice, we liberate ourselves not from a selfish point of view, but from a selfless one. Creating a life that transcends ego and benefits others is a life of happiness. Loving kindness and compassion become your watchwords on the path to freedom. With a heartfelt intentions, we make a wish for the all beings everywhere, seen and unseen. Begin with a Metta Prayer. May all beings be free of suffering and the causes of suffering. May all being have happiness and the causes of happiness. The more we release unwholesome thoughts and cultivate the wholesome, we give rise to the awakened mind, Boddhicitta. This ultimate state of being is uncontrived. This natural state of the mind is empty and beyond all conceptual views. It is complete liberation. Practice for others and the world becomes an enlightened place to be.”

Recipe Summer Quiche~ Compliments of Chef Steph

Serves 4-6

Prepared pastry for 1 pie crust 1 medium onion, finely chopped 1 medium zucchini, thinly sliced 1/2 red bell pepper, finely chopped 3 roma tomatoes, sliced 2 T basil, frech, chopped 1 cup heavy whipping cream 4 egg yolks 1 dash of cayenne 8oz Colby-Jack cheese, shredded

Preheat oven to 375°. If not using a pie shell, lightly grease a 8 inch square foil pan and set aside. Saute all the vegetables, except for the tomato. Stir in the basil and put the mixure into the crust. Top with tomatoes and cheese. In a small bowl, whisk together the egg yolks and heavy whipping cream, adding the dash of cayenne after they have been thoroughly mixed. Pour this mixture over the top of the pie shell contents and bake for 30-35 minutes or until a knife inserted in the center comes out clean.

Feel free to sub out any of the vegetables or add a protein based on what you like. Even the seasonings or cheese can be changed- the possibilities are endless!

High Blood Pressure – Facts You Should Know High blood pressure is a common condition in which the long-term force of the blood against your artery walls is high enough that it may eventually cause health problems, such as heart disease. Blood pressure is determined both by the amount of blood your heart pumps and the amount of resistance to blood flow in your arteries. The more blood your heart pumps and the narrower your arteries, the higher your blood pressure. You can have high blood pressure (hypertension) for years without any symptoms. Even without symptoms, damage to blood vessels and your heart continues and can be detected. Uncontrolled high blood pressure increases your risk of serious health problems, including heart attack and stroke. High blood pressure generally develops over many years, and it affects nearly everyone eventually. Fortunately, high blood pressure can be easily detected. And once you know you have high blood pressure, you can work with your doctor to control it. Symptoms Most people with high blood pressure have no signs or symptoms, even if blood pressure readings reach dangerously high levels. A few people with high blood pressure may have headaches, shortness of breath or nosebleeds, but these signs and symptoms aren’t specific and usually don’t occur until high blood pressure has reached a severe or life-threatening stage. Causes There are two types of high blood pressure. Primary (essential) hypertension For most adults, there’s no identifiable cause of high blood pressure. This type of high blood pressure, called primary (essential) hypertension, tends to develop gradually over many years. Secondary hypertension Some people have high blood pressure caused by an underlying condition. This type of high blood pressure, called secondary hypertension, tends to appear suddenly and cause higher blood pressure than does primary hypertension. Summer On The Jersey Shore By Lisa Ledden After a year of cabin fever, people are flocking to the Jersey Shore for some summer fun. Beachgoers, bikers, golfers, and most all of us are in dire need of some summer weather and sun. Remember sun protection is so important to an enjoyable and safe summer!

To prevent sunburn use 30+ SPF ( 2 tablespoons or 30 ml is recommended for the average sized adult body), reapply every 90 minutes, wear brimmed hats, use umbrellas and try to get out before or after the hours of 10 and 2 pm. Many of my patients feel like I am a broken record when it comes to these recommendations. Just last week, I had a patient in clinic with one of the worst sunburns I have seen in a few years. It brought to mind that I should advise readers how to manage sunburn, should (Oh No!!!) this happen to you or one that you love. Sunburn is the skins reaction to excessive UV exposure. The skin will turn red, may blister and scale, and will become painful. Typically this occurs within 8 hours of exposure. Extreme reactions may include chills, fatigue and dehydration. Some people will require hospitalization. Of course, there is a long-term risk of skin cancer associated with sunburn and sun exposure, so please don’t let this happen to you.

What should I do if I need to treat sunburn? At home therapy: 1. Apply cool wet compresses for symptom relief. 2. NSAIDS - such as naproxen, ibuprofen, aspirin- (if tolerated and not contraindicated) may help with redness and pain. 3. Drink plenty of fluids with electrolyte replacement to maintain adequate hydration. 4. Apply liberal amounts of Aquaphor or plain unscented Vaseline or emollients to affected area. 5. Take cool showers and use gentle cleansers. 6. See your healthcare provider as needed.

Recipe Caponata~Compliments of Chef Steph 3/4 cup olive oil 1 large eggplant (about 2 lbs) cut into bite size pieces 6 medium zucchini (about 2 lbs) cut into bit size pieces 1/2lb mushrooms, thinly sliced 1 1/2 cups onion, chopped 1 cup celery, sliced

In a large saucepan over high heat, cook eggplant, zucchini, mushrooms, onions, celery and garlic in the oil for 10 minutes, stirring occasionally. Stir in the vinegar, capers, sugar, salt and pepper to taste. Reduce heat to low, cover and simmer for 10 minutes or until vegetables are fork tender. Willkie According to Pew Research, there was a big increase in baby boomer retirements in 2020 - 1.2 million over the historical average to be precise! Although there are no reasons cited for the increase, it’s likely that many of those who had been holding off decided to go ahead with retirement when the work dynamics they enjoyed and were staying on for suddenly shifted as a result of the pandemic. Moving from dreaming about to considering retirement is a big step and it’s not just about being financially ready either. Retirees have a lot more free time on their hands; accordingly, even though retirements were increasing last year, the traditional activity of retirement traveling had been restricted so filling up that time became even harder for new retirees. Given that it was such a big year for retirement, one might expect to see a large decline in the historical average. Those that took early retirement can’t take it again, after all. However, a recent Bloomberg article —Affluent Americans Rush Into Retirement in New Life is Short Mindset—suggests otherwise. New work dynamics may have driven some out of the market last year, but for some that stayed, the new work dynamics were preferred. Reopening and shifting back to the previous work patterns may end up driving still more toward retirement. Today’s housing market presents another conundrum for those on the cusp of retirement. Many people will choose to sell a principal residence at retirement, taking the tax-free capital gain and downsizing to more appropriate quarters. The good news is that housing prices have been booming around the country, so many will have even more equity than expected. The bad news is that prices for retirement living arrangements are going higher as well.

If you’re one of the millions facing the decision of whether or not to retire this year, you need to understand whether you’re financially secure enough to support the retirement you are aiming for. These five steps may seem overly basic, but they create a baseline and framework that helps one switch from an accumulation mindset into a distribution mindset: Identify all the predictable income streams that you expect to have: Social Security, traditional pension, lifetime annuities, rent from real estate, and portfolio income. Compile an inventory of all financial and real assets such as: stocks, bonds, mutual funds, certificates of deposit, IRAs, 401(k) accounts and real property. Estimate your monthly and annual expenses in retirement: Divide the expenses into the essentials—food, clothing, housing, transportation, insurance and taxes—and the discretionary—travel, entertainment, gifts and so on. Compare income to essential expenses: This will reveal whether there will be an income gap that will have to be filled by touching principal. If you need to take out principal, it may affect the income stream that comes along with it, so caution is needed. Monitor the plan annually: Each year you should review your plan with your financial professional, making adjustments as needed, as your retirement circumstances change. Once you know if you’re ready to retire, then it will be time to evaluate further retirement investment decisions making sure you are keeping up with inflation to retain your purchasing power, but staying within your risk profile by making sure that your asset allocation will be aligned with your time horizon, your income needs and your liquidity needs. Decisions about required minimum distribution of retirement accounts and rolling over one account into another will become relevant. Even if you find out you’re financially secure enough to retire, that doesn’t mean it’s the right choice. It will mean that you can rest easy about doing so if it turns out you need to. The Hansen Foundation’s Enlightened Farm

Transforming Humanity, One Person at a Time, One Day at a Time Lisa Whitley, PMP – Executive Director If you asked Briana Bee at 15 years old what she thought her professional passion would be, she would have said field journalism. Little did she know that a series of impactful life experiences would eventually lead her to her role as Director of The Hansen Foundation’s Enlightened Farm ... and to a profession that is changing the lives of those battling substance use disorder (SUD). Briana’s career journey began at The Catholic University of American in Washington, DC where she studied Political Science and Public Health. “While in school I interned for the government and volunteered with my University at various nonprofits around the city” says Bee. Briana fell in love with direct service and after graduation moved to Washington state and joined a social justice network. During her first job at a community health non-profit, she “witnessed the disparity between health and the lack of access to nutritional food. She saw a huge gap in the understanding of how healthy food connected to overall physical, mental and emotional health.” At her next position with L’Arche Farm and Gardens, a non-profit organization for adults with disabilities, Briana learned the healing qualities of farming and horticulture. “Sharing a positive and growth-focused space with people disempowered by society, showed me what opportunities for human connection could look like.” It was during this time that Briana tragically lost her brother Matthew to substance use disorder. “The loss of my brother was unimaginable for my family” explained Briana. In Matthew’s memory, the Bee family established the Matthew Bee Memorial Fund (MJBMF), in partnership with The Hansen Foundation, to help individuals and families in our community battling the disease of addiction and mental health issues by providing scholarships to addiction treatment, recovery support services and resources including sober living. “It was at the fund’s second annual ‘Mocktails and Cocktails’ fundraiser in 2018 that foundation founder Jennifer Hansen spoke about the Hansen Foundation and Enlightened Farm. I knew at that moment I wanted to move back home to New Jersey and bring horticulture therapy to people healing from addiction.” As director of the 400-acre organic Enlightened Farm, Briana established a variety of skill-based and therapeutic substance use disorder and recovery programs. “We focus on cultivating a sustainable, healing environment

through Horticulture Therapy, Regenerative Agriculture, Nutrition Education, Employment Programs, Workshops and Volunteer programs.” The Enlightened Farm provides local and organic produce to all of our partners at Enlightened Solutions Treatment Center, Enlightened Solutions Detox, Serenity Recovery Houses, Enlightened Café and farmer’s markets throughout the community. “We understand that the regenerative process of nature mirrors the regenerative process of individuals healing from addiction.  My team and I work to nourish people through a variety of directives and programs, bringing those in recovery, those impacted by substance use disorder, and those who are committed to holistic wellness together,” Bee explains, “We teach people how to create a healthy life Briana Bee environment and a healthy relationship with food.” Briana is known for her charismatic personality. Many describe her as having the ability to shine “a bright light of healing power” on those who work with her. “She is always 100% present and committed to everything she does,” says alumni and colleague Shirley. “I am truly passionate about what I do,” says Bee. “I am humbled that my personal experiences are able to help people overcome challenges and grow in new ways. I am grateful to have found my purpose.”

Medical Professionals Anal Abscess And Fistula Nina Paonessa, DO, FACOS

What is an anal abscess? An anal abscess is an infected cavity filled with pus found near the anus or rectum. What is an anal fistula? An anal fistula is frequently the result of a previous or current anal abscess, occurring in up to 50% of patients with abscesses. Inside the anus are small glands. Sometimes, these glands become clogged and become infected. This leads to an anal abscess. The anal fistula is the tract or tunnel that forms between the clogged gland inside the anal canal and the infected skin/abscess outside of the anus. A fistula can present with or without an abscess. Other causes of anal fistula are: Crohn’s disease, radiation, trauma and malignancy/ cancer. How does a person develop an anal abscess or fistula? Most anal abscesses are the result of an acute infection in the glands inside of the anus. Occasionally, bacteria, fecal material or foreign matter can clog the anal gland and create a condition for an abscess to form. Once an abscess is drained, in approximately 50% of cases, a tunnel (fistula) will persist under the skin connecting the infected gland to the outside skin where the abscess was located. When this occurs, patients typically experience some type of persistent drainage from the site of where the abscess had been in the perianal skin. What are the signs or symptoms of an abscess or fistula? A patient with an abscess may have pain, redness or swelling in the area around the anus. Other common symptoms are fevers, chills, fatigue and generally not feeling well. Those patients that develop an anal fistula frequently complain of perianal irritation, moisture in the perianal area and/or persistent, episodic drainage in the perianal area. A common complaint of patients with anal fistula is the sensation that they cannot keep the perianal area clean or that they feel “wet” in the perianal area. Are there any tests performed to diagnose an abscess or fistula? A majority of anal abscesses anal fistula are diagnosed by history and physical examination alone. Occasionally, additional studies such as CT scan or MRI can assist the surgeon with the diagnosis of deeper abscesses or to help identify a deeper or complex fistula tract/tunnel. How is an anal abscess treated? The treatment of anal abscess is surgical drainage. Once the overlying and surrounding skin has been numbed with local anesthetic agents, an incision is made in the skin over the abscess to drain the pus. This can usually be performed in the surgeon’s office with local anesthetic and may sometimes require deeper anesthesia in the hospital setting. For patients, such as diabetics or patients with suppressed immune systems, hospitalization may be required for more significant infections. Are antibiotics necessary to treat anal abscess or anal fistula? Antibiotics, alone, are NOT a wise treatment for anal abscess or fistula. Studies have shown that for uncomplicated abscesses, the addition of antibiotics to surgical drainage does not improve healing time or reduce the potential for recurrences. There are, however, certain circumstances in which antibiotics may be added to surgical drainage. These would include those patients with extensive cellulitis, patients with cardiac valvular disease, and those patients with altered or suppressed immune systems. This determination is made by your surgeon after taking a careful, detailed medical history and performing a physical examination. What is the treatment for anal fistula? Surgery is almost always necessary to treat an anal fistula. This may require a single stage or multi staged procedure. Colorectal surgeons are specialists

who are trained to treat anal fistula and are well versed in the different types of surgeries to treat anal fistula. Surgery for anal fistula involves connecting the external opening (site of drainage on the skin) to the internal opening (the clogged gland in the anus). There are several different types of surgical procedures to accomplish this and a colorectal surgeon would determine which procedure is best suited to your condition. The different types of surgery for anal fistula include anal fistulotomy which connects the internal opening (clogged gland) with the external opening (site of drainage on the skin). This creates a groove that will heal from the inside out. Other procedures to treat anal fistula include filling the fistula tract with a material to occlude the fistula or disrupting the tissue around the fistula in order to close the fistula. If performed correctly, surgery for fistula is unlikely to affect bowel control. What to expect when recovering from fistula surgery? Patients will be provided pain medication to assist with the discomfort associated with fistula surgery. Taking stool softeners, laxatives and fiber supplement will help to avoid constipation in the postoperative period. Patients should plan for some time at home to recover. Discussing with your colorectal surgeon the anticipated time away from work is very important. Sitz baths also play a key role in the postoperative care. Dr. Paonessa is a fellowship trained colorectal surgeon who is skilled at treating patients with anal abscess and anal fistulas. She has a 24 hour policy in her office so that if you are in pain from an anal abscess, you can be guaranteed to be seen within 24 hours. Most anal abscesses are drained right in the office under local anesthesia. Dr. Paonessa is also well trained in a variety of techniques to treat simple and complex anal fistulas with excellent outcomes. Anyone Ready To Laugh Out Loud Yet? By Charles E. Meusburger, MD, FASAM

Perhaps an incongruent inquiry at the present time, but it is certainly what “the Doctor ordered” right now. Humor is the number 1 human defense mechanism that we possess. There are ten such mechanisms and they all exist for similar reasons—to unconsciously reduce anxiety arising from unacceptable or potentially harmful stimuli, especially if done in repetitive patterns. Or, as Dr. Sigmund Freud explains psychoanalytically, “defense mechanisms are used to manipulate, deny or distort reality in order to reduce anxiety and control impulses or to maintain dignity. “ In short, all defense mechanisms are unconscious responses to manage the stresses of many different human social interactions. This includes humor that is by far the healthiest and the most productive one of the menu list. Humor, as a mechanism to create something positive and light in the face of something potentially ‘dark,’ divisive and perhaps, frightening can assist in several ways. First, it is a distraction, even a bit of a decoy to overt unwanted or uncomfortable attention away from the discomforts of that moment. Second, it provides levity as a ‘gift’ from the provider to his/her audience that must be funny in order to succeed. This requires the gift of the comedic one to impart a sense of relativity on the subject in jest to

the audience to engage them to where you prefer them to be. Third, it can ‘buy you time’ in order to seek momentary asylum away from the reality at hand until you can regroup and possibly conjure up alternative responses so as not to give in to negative reactions. There is almost a cognitive process at play in order ‘to play’ even though humor is overtly an emotional state contributing often to a sense of ‘Happy.’ Happiness is like a fingerprint in that it’s different for every individual as far as it’s definition and as the timeless wisdom of Jefferson so aptly penned---“the pursuit of happiness” was the finale of a very powerfully influential sentence that inspired a neonate nation to take hold and grow. He clearly realized all too well that one of his three famous “inalienable rights” could not be the Happiness itself, as this is idiosyncratic to each of us, but rather that the pursuit thereof would not be interfered with by the state so long as you did it by the rules of the land. Humor is a stepping-stone and omnipresent contribution to a sense of wellbeing and provides a healthy

contribution to one’s happiness quotient. Conversely, self-deprecating and anti-self humor can lead to unconscious aggression or hostility against the self or self-perceived short fall(s) that needs to be brought forward by the individual using humorous jabs at oneself before everyone else discovers these perceived inadequacies and uses them against the individual in a manner that he/she feels would be even worse. This use of ‘dark humor’ as a defense is still understandable, but cumulatively, harmful. The best psychological and alternative antidepressants and anti-anxiety treatments are: physical exercise, stimulation of one’s mind with what it knows little to nothing about a topic, creativity in any form and yes, humor—the healthy variety. So, find an old VHS recording of your favorite film or YOU-TUBE some performance of your favorite stand-up comedians and laugh out loud—share it with others and feel the environment, internal and external, sustain a palpable improvement. So, from Buster Keaton to Chris Rock, allow their gifts to stimulate your own and find fun and relief-release. Remember how long the oppressions of COVID-19 have been lingering and attempt to use these tools for twice that duration and they will become your “new automatics” to reach for instead of other more common, but less helpful defenses going forward. Enjoy and live your life— don’t endure it. No one is born to forfeit his or her lives; that’s learned. Resist, regroup, have resilience and rejoice---you can!

Both conditions are correctable problems in almost all patients, according to Dr. Banks. When the gum ridge where teeth are missing is too thin to place a dental implant, bone grafting can be performed to correct the problem. What that means is that a bone replacement product, or even the patient’s own bone, can be placed into the deficient area, stabilized, and allowed to heal. Following healing, dental implants can be placed into that area and then restored with porcelain teeth. This procedure is called “ridge augmentation” and can be accomplished in both the top and bottom jaws. When teeth are missing, the sinus that lies above the top jaw will expand downward into the ridge and cause bone loss in the gum ridge. This is a correctable problem as well, according to Dr. Banks. “A Sub-Antral Sinus Augmentation – commonly called a ‘Sinus Lift Procedure’ – can be performed, sometimes even at the time of implant placement. This procedure increases the amount of bone in the area where we would like to place a dental implant. Again, a bone replacement product or even the patient’s own bone can be used.” A common misconception is that the bone is placed into the sinus. In reality, the sinus membrane is actually lifted up (hence to name “sinus lift”) back to where it should be, and the bone graft material is placed under the sinus membrane, restoring the gum ridge and the sinus contour together in one procedure. Tracey Lynn Triniwell is the administrative director of Women’s and Children’s Service Line at AtlantiCare Regional Medical Center. An Atlantic City native, Tracey is an experienced accounting professional that holds both a Bachelors of Business Administration, from Howard University and Masters of Accounting, from Rutgers, The State University of New Jersey. At AtlantiCare, Tracey chairs the African American Employee Resource Group, LEGACY (Leaders Empowering AtlantiCare Community and You) and was awarded AtlantiCare’s 2014 Diversity award & 2019 Diversity Champion leader award for her contributions and dedication to promoting diversity and inclusion within the community and AtlantiCare. The group engages in activities to support an inclusive workplace for our staff and also to create community partnerships in support of our vision of building healthy communities together. Tracey is also a member of AtlantiCare’s Young Professionals Steering Committee; a group that provides a fun and meaningful way to get involved and make a difference in our local community through social, educational, volunteer, and giving opportunities. Robotic Assisted Total Knee Replacement Surgery

Robotic technology has paved the way for quicker production and efficiency in our society across all industries from cars to food, to shopping and of course healthcare. Incorporating robotics leads to reproducible results, accuracy, and enhances outcomes. Healthcare incorporates robotics in pharmaceutical production, medical equipment manufacturing, and during surgical procedures. Since the 1980s, surgical procedures have been utilizing robotics to decrease trauma to surrounding tissues, precision cuts, and smaller less invasive incisions across all specialties. Joint replacements have seen a 500% increase in the use of robotic assisted surgery from 2009 to 2013 with that number continuing to rise. Almost every joint replacement manufacturer has developed a robotic system to be used with their implants, however robotic assisted surgery still only makes up less than 25% of all joint replacement surgeries. First, let us discuss what robotic surgery is NOT. The robot does not perform the surgery. My kids would love this so I could be home more, but the truth is, the robot is a robotic arm that the surgeon guides to create the cuts in the bone for a proper fit and alignment. Joint replacement surgery is still performed by the surgeon using his experience and knowledge. The advantage of robotic surgery is it brings the best technology available today to be more precise, more accurate and more predictable. Robotics enables the surgeon to cut away only the bone needed to fit the prothesis like a glove, sparing healthy tissue. This exactness creates the least amount of trauma and decreases complications. Robotics allows surgeons to place the prosthetic components in the best possible position using exact measurements determined by a pre-operative CT Scan. This leads to the ability for many joint replacements to be fixated without the need of cement which can breakdown over time. Cementless joint replacements last much longer, potentially many decades or even one’s life expectancy. Traditionally, total knee replacement surgery planning starts with an x-ray. A template is created using these 2-D images and then the surgeon uses his skill

and experience to determine the prothesis, sizing and placement. Most of these decisions are made during surgery, with template sizes that can be trialed before the actual prothesis is placed. With robotics, much of the surgical planning is done prior to the patient even stepping foot in the operating room. A special 3-D CT scan is performed a least two weeks before surgery which is then used to determine component sizing, balancing the ligaments and realignment of the leg. During surgery, the surgeon as the opportunity to make millimeter changes during the procedure to perfectly balance the knee. Current research is consistently showing superior results with the use of robotic surgery compared to traditional instrumentation. In one such study by Bone and Joint Research in 2019, the authors found that the use of robotic assisted surgery decreased hospital stays by more than a full day. Robotic total knee replacement patients had less postoperative pain, required less pain medication, had increased flexion and quad strength sooner than traditional total knee replacement. These patients also required less physical therapy and had a quicker recovery. Robotic assisted surgery carries significant benefits for the patient and the surgeon. Robotic surgery makes the surgeon better, creates reproducible results, enhances the visual surgical field, and provides a great deal of detail prior to surgery to finalize the surgical plan. The patients benefit from robotics by having less pain, quicker recoveries, and less complications. Robotic surgery will continue to grow as more and more surgeons transition to robotic assisted surgery to enhance the patient experience.

The Kitchen Queens Of Cape May

As we embark on the summer season at the Jersey shore vacationers are flooding the shore towns for a week, or longer, to embrace a well needed vacation. When it came time to start planning family vacations of my own, my husband and I landed in Cape May with our then, four year old twins. We rented on Kearney Street in a beautiful historical house for a number of years. Our family walked from Kearney into town every night to walk the mall, get ice cream or just to soak in a summer night on the town. Our route from Kearney Street to the Washington commons included walking past the Chalfonte Hotel. It is a big beautiful historical structure that is nestled in a residential area a few blocks from the beach. The Chalfonte has an incredible history not only about it’s exterior but its interior that has made it into its success since the early 1900s. Helen Dickerson was the start of what would be five generations of the “Dickersons” to dedicate a life to working at the Chalfonte. Helen was 8 years old when she first arrived at the Chalfonte with her mother, Clementine Young and the then owners. The original owners would travel from Virginia to Cape May every year to open the hotel and they would bring their staff along with them. Helen started off picking flowers with the owner’s wife for table centerpieces. She also became the owner’s baby sitter making a mere $3.00 per week. Ms. Helen worked her way up through the ranks as a waitress. Then, one fine day Helen was asked to fill the shoes of interim chef for the restaurant. This was no easy task, but Ms. Helen was up to the challenge and began preparing two seatings per day for over 180 guests per sitting. Helen not only held on to that role for over 77 years she brought her daughters, Dot and Lucille to work at the hotel. At the young ages of 7 and 9. Dot and Lucille started their employment at the hotel rinsing out the sand of the guests bathing suits and hanging them on door knobs so they would be ready for the next day at the beach. To add a bit of history to the time era, the Dickerson women were well respected as “staff” of the hotel, however they endured prejudice as women of color along the way. It has been reported that “blacks were not permitted to stay as guests” in the hotel but could be employed. It was not until the ownership changed hands to two incredible women in the 1980s that permitted all guests regardless of race. Lucille and Dot have reported in interviews that in the 1950s the beaches were also segregated. The beach on Grant street was the only beach designated as the “colored” beach which is where they were permitted to swim in the ocean. Ironically, Grant street has become the beach I frequent and my family favorite. Dot and Lucille worked their way from the front of the house to the back of the house and right into the kitchen eventually learning how to prepare the famous recipes for breakfast and dinner. Helen Dickerson was renown for her “southern specialties” and was known to have described her style as “soul food with its Sunday clothes on.” Those specialties are still known to be a part of the menu at the Chalfonte and include mouth watering Southern style fried chicken and crab croquettes. The traditional menu provided a week of daily specials that ended the week with seafood options. The recipes received so much publicity that Ms. Helen was invited to a 1984 appearance on the Phil Donahue show. In case you are a younger reader, to be a guest on the Phil Donahue show was equivalent to being interviewed by Oprah Winfrey. Ms. Helen was also were featured on the Food Network “Tyler’s Ultimate.” If that is not enough there is a cookbook entitled, “I Just Quit Stirring When the Tasting is Good” with all of the family’s generations of secrets printed on page after page. Helen worked in the hotel into her 80s and along the way she taught her daughters Dot and Lucille how to prepare each meal from scratch, meaning all fresh and prepared daily and everything from the vegetable to the meats to the mayonnaise! Keep in mind there were no microwaves, no food processors, no electric can openers. Through the years Dot and Lucille added their own recipes to enhance the menu. However, these titles did not come with ease. The Ladies would arrive by 6am and not leave until after 6pm every day during the open season of the hotel which was from Memorial Day to Columbus day. They had one pleasure which was to watch the soap opera, General Hospital on a television placed in the kitchen by Ms. Helen, which was only one of the two televisions that were in the hotel. They raised their families in the kitchen of this grand hotel and Dot’s daughter Tina Burton has also become a regular serving up the culinary traditions. Other family members raised at the Chalfonte have continued on in the food industry as well. It has been reported that Dot passed away in February Dot Burton pictured above of 2015 at the age of 87.
