STEPPING THROUGH THE DOOR Defining the word ageless is not as simple as it sounds. Some definitions are influenced by Hollywood’s world of flawless glamor shots. Photoshop hides the reality of time and truth. Age-defying has become the “it” term that instantly places rose-colored glasses on viewers and paints an unrealistic picture of what really is.

Why are we so afraid of aging? Can society not come to terms with this simple fact of life—that aging is inevitable? Some of us have trouble dealing with the thought of our past lives catching up with us. With the growing population of baby boomers and their parents, can we do no better than to wage war on the ever-present facial lines and wrinkles? I had a milestone birthday this year—I turned 60. And with that came a moment of truth and assessment of where I am with my life, my health and what is remaining of my future. I’d be lying if I stated I was initially at peace with the number 60. The truth is, this milestone moment challenged me on several levels—to look at myself with eyes wide open, step through the door of love and self-acceptance and understand what is really important and precious to me. This milestone provided a lot of wonderful answers. In listening to my heart, I feel that what I have learned over the years from personal triumphs and challenges has provided the foundation for how I grow older. When I look in the mirror I see my past, my lessons, a peaceful resolve in aging with grace and dignity, and being my personal best. Love for my family and friends and providing time for self-care are the most important and precious things to me. In this issue we introduce Doctors Saj Vivraj and Mamaly Reshad of Anacapa Dental; an update on stem cell therapy and its benefits, and looking your personal best with a beautiful smile. Bone Marrow Stem Cell Therapy Several different types of stem cell therapies have routinely been limited to blood-forming cells derived from bone marrow. Bone marrow transplantation is the most common form of stem cell therapy and is used to treat leukemia and other disorders occurring in the bone marrow and blood. During bone marrow transplantation, bone marrow and white blood cells are destroyed using radiation and chemotherapy. Afterward, a sample of bone marrow from a healthy donor is injected into the patient. The transplanted bone marrow and stem cells continue to populate and these, in turn, produce healthy new blood cells.

With this therapy, peripheral blood stem cells and umbilical cord blood stem cells can be used in place of bone marrow samples.

Embryonic Stem Cell Therapy One of the most useful forms of stem cell therapy is embryonic. It has been proposed for tissue replacement and for regenerative medicine. Many diseases can potentially be treated with embryonic stem cells including juvenile diabetes, blindness, spinal cord injuries, immune system related diseases and cancers. However, the ethical concerns of embryonic stem cell therapy have restricted it from being legal and accessible.

Stem Cell Therapy from Fat Stem cells found in adipose, or fat, are available in high numbers and are relatively easy to harvest. They are similar to bone marrow stem cells. With this type of stem cell therapy, a donor’s fat is harvested. The stem cells are then separated from the fat cells and activated. Afterward, the stem cells are returned to the body through an IV or injected into a joint.

HOW IS PEP STEM CELL THERAPY CONDUCTED? PEP stem cell therapy is one of the least invasive types of therapy. Because there are stem cells in the circulatory system, a patient simply has their blood drawn. The blood is then processed to activate the stem cells and the separated plasma fraction is reintroduced into the same patient. Patients undergoing therapy have noticed improvement of symptoms within days, and the benefits accumulate over the next weeks and months. If the condition is more severe, additional treatments may be required. Patients do not experience down time or prolonged, painful healing. The most important safety element is the fact that the cells belong to the 16

Too much bone loss may require bone replacement, a process in which a donor’s bone is grafted onto the patient. If you lose a tooth for any reason, Greenman recommends getting a temporary replacement within four to six weeks to prevent the bone from weakening while you decide on the next step.

W H AT A R E Y O U R OPTIONS IF YOU ARE LOSING YOUR TEETH? In the past, the only options for tooth replacement were removable dentures or bridges. But dentures move around, are uncomfortable and those who wear them have 20 percent less chewing force. If you can’t chew, you’re not getting a healthy diet.

“I’ll tell a patient with dentures to bite down on my fingers and I can barely feel it; they can’t hurt me,” said Greenman. The newest and best solution to tooth loss is dental implants. Rather than just filling the space where a gap was before, implants are a long-term solution that protect important jaw and teeth function. An important benefit to implants is the prevention of bone loss and deterioration. Extensive photography, molds and scans are performed prior to the surgery so the implants fit perfectly like your own teeth. The golden years mean different things to different people, but rarely do those visions include the nagging physical ailments or distressing cognitive impairment that leave you and your loved ones upset, confused or even hopeless.

A geriatric care manager (GCM) is a non-medical specialist who advocates for older adults and the families who are providing for their care. The GCM is educated and experienced in any of several fields related to care management, including but not limited to gerontology, social work or psychology, with a specialized focus on issues related to aging and elder care.

The good news is that healthy aging is attainable even in the face of a multitude of challenges. As the U.S. population ages—nearly one in four adults will be older than 65 by the year 2020—geriatric care managers can provide expertise, advocacy and referrals to resources that help older adults continue to live vibrantly and safely for many golden years.

As we age, it’s helpful to define the term healthy, not necessarily as vigorous exercise or having the same physical shape we had half a lifetime ago, but rather how each individual, older adult can create an environment that embraces his interests while managing the physical and cognitive challenges that can potentially come with aging.

A GCM can assist in creating an appropriate care plan for older adults facing the aforementioned challenges, among others. Through a detailed clinical assessment and monitoring, a GCM sees the client as a whole. The assessment tools provide the GCM with insight into the client’s current and past medical history, indicators of any social and economic red flags, as well as family dynamics. This comprehensive overview enables the care manager to help the client and family plan, problem-solve, educate and advocate for the client’s overall health and well-being.

PROVIDING EXPERTISE AND MANAGING RESOURCES Adult children taking care of aging parents or loved ones are facing a new societal challenge. With Americans living longer than ever before, we have to provide care for our elders while often still juggling a job and our own children. Caregiver stress can decrease a family caregiver’s life by as much as 10 years. With a GCM in place, much of this stress can be alleviated. For example, by attending doctor appointments and communicating among doctor, client, and family (if appropriate), a GCM stays abreast of the older adult’s medical issues and makes the necessary suggestions on how to foster healthy aging. Since a GCM is a non-medical professional, these suggestions to promote optimum health are based solely on doctor/patient feedback.

M E N TA L S T I M U L AT I O N A N D S O C I A L I Z AT I O N Examples of possible changes to promote brain health are memory games or puzzles. Engaging in such activities with a friend or caregiver may slow memory loss. Researchers continue to study dementia and cognitive impairment, but there appears to be a consistent link between brain training and a decreased risk of mental decline. Providing opportunities for clients to engage in social, recreational or cultural activities are time-tested and proven to enrich quality of life and stimulate healthy aging. Too often families mistakenly conclude that their aging family member is “too physically compromised,” “too cognitively impaired” or “anti-social” to reap the benefits of socialization. Scholarly articles clearly state that mental stimulation and socialization are tools to promote healthy aging. The objective of such fine-tuning is better quality of life.

A G O L D E N L I G H T AT T H E END OF THE TUNNEL Older adults frequently feel that a medical or cognitive diagnosis is the start of a journey with no light at the end of the tunnel. A GCM is knowledgeable on a wide variety of communitybased programs and services that encourage seniors to maintain their safety and, in turn, their dignity. Knowing an aging parent is safe and well cared for is a common concern for adult children. A GCM monitors the client’s 22

adherence to medical orders and instructions while keeping family members (if authorized) and professionals informed of the well-being and changing needs of the client. There are many reasons an adult child may be unable to provide around-the-clock care-giving. In many situations, the adult child or caregiver has never dealt with an aging loved one and has no idea where to turn. To say aging is overwhelming is an understatement. When a condition is diagnosed or an elderly spouse passes away, lives feel as if they’ve been turned upside down. Too often families are faced with all of the business at hand and aren’t afforded the opportunity to take it all in or mourn the loss at hand—the loss of independence, the loss of routine, the loss of a loved one. Days on end can be spent researching solutions. A GCM conducts a comprehensive assessment and helps the family plan for the current and future needs of their aging loved one, taking potential guesswork off the family’s plate and allowing that family member’s time and attention to be focused on the family. A GCM can’t take away the fear that often accompanies aging, but she can hold your hand and travel the road beside you. GCMs can help you find glimpses of light at the end of the tunnel and in the process, help you and your aging loved one redefine healthy aging.

This makes it important to know the true cause of your headaches and not just what you can do to hide its symptoms. The best step to take is to understand the most common types of headaches.

HEADACHE C AT E G O R I E S The most common headache type is the tension headache. This is the general headache most people refer to when their head hurts. For the

majority, these headaches are episodic, which means they occur less than 15 days per month. They also tend to affect the whole head and not just one side of the head because nonlocalized muscle contractions are the cause of this type of headache. Someone experiencing a tension headache is most likely to feel mild to moderate pain that can last for a greater part of the day. It’s rare for episodes of tension headaches to cause severe pain or to last for more than a day or two. If this type of headache occurs for more than 15 days per month, then it is considered a chronic headache. The next most common type of headache is the migraine. Migraines plague millions of Americans every day. These headaches are identified as the number one reason Americans take time off of work. Migraine headaches are extremely debilitating as they cause many unwanted symptoms. Depending on the episode, a victim of migraines also may feel nausea, a loss of appetite, and sensitivity to light, loud noise and strong odors. Above all, these symptoms are complemented by a severe head throbbing that is quite painful. Worst of all, these episodes can last for many hours or even days.

Regardless, many believe there are certain triggers for their migraines, prompting them to eliminate many foods and/or activities from their lives. Taking such a measure does indeed work for some.

A large portion of the population has migraines triggered by tempromandibular joint (TMJ) issues. Unknown to many, misalignment of the jaw is a very common cause for migraine headaches. It is common for these migraines to be accompanied by other symptoms not normally experienced with regular migraines, such as ringing in the ear or pain near the jaw and temples. TMJ is often overlooked as a cause of headaches, but conservative estimates show it is a major cause. Up to 25 to 30 percent of the population suffers from TMJ. When the tempromandibular joint is not aligned properly, it puts tension on the nerves in the head. A cascade effect puts pressure on these nerves and can trigger headaches and migraines. The good news is there are ways to ease a TMJ headache. One can stretch and relax their facial muscles, try soft food or partake in yoga. Sleeping with a very proper form can also help. These measures may help ease tension in the joint, but they still do not reach the root of the problem. The best method to prevent such headaches is to cure one’s TMJ problems by finding the optimal bite position and then crafting a customized oral appliance (known as a neuromuscular orthosis) to gradually reposition the bite. This type of treatment requires specific computer scans, nerve stimulation therapy,

bite registration and most importantly, a neuromuscular-trained TMJ specialist. One small step toward the right direction can mean a lifetime of relief.

OTHER HEADACHES Other types of headaches include mixed headache syndrome, sinus headaches and cluster headaches. Mixed headache syndrome is a combination of a migraine and tension headache, while a sinus headache is due to sinus congestion. Cluster headaches are the least common type of headache, yet are known to be the most severe. A cluster headache is characterized by its unique pain located behind the eyes, but its cause is mostly unknown. While it is believed to be the result of an abnormality in the hypothalamus, this is yet to be fully determined. Considering the many types of headaches, it is important to take into consideration many factors when diagnosing and to seek the help of a specialist. Be aware of duration, location, severity, time of day, changes in vision, frequency and more.

But you may be surprised and happy to know these symptoms are actually related to inflammation and lifestyle. They are related to chronic illness, not aging. The U.S. has the highest rate of chronic illnesses in the world.

Recent studies and surveys show the brains of those well over 60 years old want and enjoy sex

Myths of Aging 1. Lifestyle changes late in life have no effect. Some people think if you don’t take care of yourself when you’re young, there’s nothing you can do to fend off health problems later in life. But that’s not true. While taking care of your body with exercise and a good diet from a young age does have lasting benefits, getting a late start on a healthy lifestyle can still do the body a world of good—no matter how late that start may be. Exercise, even during advanced years, has many benefits, including increased strength and improved mobility. It can also help reduce joint pain from arthritis. Even if you’ve never exercised a day in your life, starting now can still improve your health. 2. Your brain power declines. The truth is most older people don’t experience a significant decline in their mental abilities. “Verbal and math abilities and spatial and abstract reasoning all improve in middle age,” according to Melissa Lee Phillips in the American Psychological Association’s Monitor on Psychology.

A degenerative disease such as Alzheimer’s may cause a serious impaired memory or declining cognitive ability. But not all people get Alzheimer’s. According to the Alzheimer’s Association, about one in eight people over the age of 65 has the disease. The brain is similar to your muscles—use it or lose it. We maintain our cars with gas and changing the oil. We must maintain or bodies and our brain with healthy nutrition, exercise, laughter and socializing! 3. You won’t be able to take care of yourself. Only about 5 percent of elderly people live in full-time care facilities. Most elderly people live on their own or with minimal in-home care. 4. You’ll stop having sex. According to the Mayo Clinic, “most aging men and women produce enough testosterone to maintain their interest in sex.” If a man or woman has low testosterone, testosterone therapies can increase libido and erectile function. A survey by the National Council on Aging found almost half the people over the age of 60 were having regular sex. Thirty nine percent wished they were having more sex and 43 percent said that sex is just as good, if not better, than it was when they were younger. “Recent studies and surveys show the brains of those well over 60 years old want and enjoy sex,” observed Judith Horstman in her book, “The Scientific American Healthy Aging Brain: The Neuroscience of Making the Most of Your Mature Mind.” 5. You’ll feel old. According to the 2009 Pew Research Survey, “the older people get the younger they feel, relatively speaking.” Individuals with healthy aging are active, vibrant, social, engaging and many are starting new hobbies and careers.

6. Your happiness will decline. Surveys have found that while happiness dips in the 40s, people start to feel more content with life after the age of 50. A study recently published in the Journal of Economic Behavior and Organization found that people are “at their happiest at retirement age.” 7. You’ll stop learning and growing. The adult brain is no longer assumed to be an organ that becomes stagnant with age. Instead, the idea of neuroplasticity, or “the [adult brain’s] ability to change its structure and function in response to experience,” has become widely accepted in the world of science and in mainstream knowledge. The idea of self-directed neuroplasticity—the power of thought to retrain the brain is indeed exciting news. 8. You’ll become less social and more isolated. Social isolation at any age is a serious issue especially among the housebound elderly; however the elderly tend to have stronger social skills than expected. “As we get older, our social intelligence keeps expanding,” according to Dr. Margaret Gatz, professor of psychology, gerontology and preventive medicine at the University of Southern California. 9. Your destiny is out of your control. “One may have greater personal control over one’s bio psychosocial health after retirement than previously recognized,” concluded a study conducted over a 60-year period, published in the American Journal of Psychiatry. A recent study of the lives of 500 people concluded seven factors have a major impact on how we age: marital status, alcohol and tobacco use, body mass index, physical and social activities, education and how we cope with everyday stresses. According to the American Society of Plastic Surgeons (ASPS) 15.1 million cosmetic procedures were performed in the U.S. in 2013. About 1 percent of our population opts for cosmetic surgery (currently more than 3 million people). A WebMD interview with Sarah Thorne, co-leader of the ASPS study in 2005, revealed that people want “to improve imperfections so they can feel better about themselves.” Thorne explained, “[People] felt they would be happier, that others would respond to them better, that they would have improved social lives. Men in particular thought they would have improved career opportunities.” The American Academy of Facial Plastic and Reconstructive Surgery’s annual survey reported that one in three facial plastic surgeons have more business (and more patients under 30) because patients are more self-aware of their appearance on social media. Upon seeing their selfie photographs, people want to look better and younger. It’s a competition. In the U.S., we have an aversion to aging and our imperfect appearance. We are suffering from vanity and insecurity.

A Longer-term Solution There are better, more permanent ways than surgery to achieve a happier life filled with meaningful relationships. We can come to terms with the fact that we’re aging, and enhance our self-worth in many ways.

The alternatives are better for us psychologically. We’ll feel more confident and whole—connected to ourselves and to others. Fortunately many of us are on this path and more are jumping on board.

Contrasting Approaches to Aging We generally face life situations in one of two ways: either with confidence and courage or with fear (we may switch modes depending on the situation). Fear-based living is treacherous and ineffective. It tears us down and pits people against each other rather than fostering cooperation and support. Fear holds us back and makes life miserable for ourselves and others. A fear-based approach to aging includes frowning at yourself in the mirror; constant fretting; feeling lessthan; making endless attempts at covering lines, sags, bags and wrinkles with cosmetics, surgery, clothing, and hair styles and colors; rigorous dieting; and tortuous exercise rather than fun, h ealthy exercise. There are worse approaches—excess alcohol, cutting oneself off from society, and major moodiness and depression. In contrast, a confident, conscious approach to aging includes keeping your mind active with a variety of interests; seeking like-minded, positive friends; being appreciative of life’s gifts and making a point of helping others.

good news is, we have a choice. We can choose how we think— whether we approach situations from a place of inner strength or we let someone’s superficial judgment from the outside knock us off balance.

From Fear to Fortitude Our self-esteem and self-worth come from the inside, not outside. In fact, making a major, yet simple shift to emphasizing our inner being can go a long way toward helping us adjust our thoughts. The shift helps us make choices that boost how we value ourselves, how we see ourselves relative to others, and how we conduct our everyday lives. There is a huge world outside of ourselves. We get caught up in it because we are so attached to it. The people and situations in our lives affect us in whatever way we let them—and it’s often a habit we can change. Concerning our appearance—if we have a low self-worth and notice the skin on our face is sagging as we age, we might compare ourselves to more youthful-looking people and feel self-conscious and inadequate. If instead we focus our mind on the value of our years—that we are now wiser and more accomplished—we will recognize age as a great advantage. We know everyone must face aging and we will decide to approach it with appreciation and an inner feeling of love and self-worth. Next time a situation that brings you down, check to see if you are using a fear-based approach from within. Also assess whether you’re letting something or someone from the outside take away your inner power. If so, switch your own thinking to positive or get some counseling to help change your approach.

How to Live More Consciously Conscious living enhances our whole self—our mind, body and spirit. Mind, body and spirit are not separate areas. They are inextricably intertwined. What affects one affects the others. Living consciously involves developing a stronger relationship with your inner thoughts and your inner self. It’s a matter of self-assessment, improving your approach, finding your balance and living from that point of strength. These are concepts of spiritual yoga. Our minds are a lot more powerful than we give them credit. We have the power to decide to think for ourselves in positive rather than negative, fear-based ways. We can even learn to view life’s difficulties as opportunities. A big problem is often what it takes to make us seek better outcomes. When the pain of staying the same is worse than the pain of changing, it can be a pivotal point to improved living. We can seek and find better ways of thinking and acting to produce better results.


Take some time now to assess how you feel about getting older. Create a table on a piece of paper and write the advantages and disadvantages sideby-side. Pat yourself on the back for the advantages. Then assess each disadvantage carefully to develop a positive way of thinking about it. If it’s something you can change, such as your health, write down the steps you will take to improve it. This will motivate you to make changes. Feeling better after making the changes will motivate you further. If a disadvantage involves your attitude, you can change that, too—with self-help or counseling. Living consciously involves seeking real answers to life’s major questions. If your spiritual or religious path isn’t giving you the answers you need to feel happy, peaceful, balanced and connected, it might be time to seek further. The question is, do you know why you’re in your relationship? If you don’t, then the pain can be severe and pointless. But if you know why, the pain may be a signal for the two of you to examine how you can improve your participation so you can actually have the relationship you’d like. The pain in the butt I’m referring to is the work required to cohabitate for the long haul. If you’re just looking for fun, then have your short-term recreational relationships and get out at the first hint of relationship issues. But if you want a long-term partner, if you want to nurture an intimate and loving relationship where you both listen to each other and support each other to grow, to heal, and to find joy, that’s gonna take work.

Old Issues Surfacing It’s work because as the honeymoon fades and the full-dimensional truth of who you both are becomes more and more apparent, you’re each confronted with childhood issues that are begging to be examined and healed. And until they are, they’ll continue to wreak havoc in your relationship. If one of you is afraid of conflict and the other addicted to chaos, the chasing and the running will lead to endless arguing and possibly more violent forms of confrontation. If one of you is a bully and the other never had a voice as a child, the frustration levels could rise to unbearable levels. While these old issues are played out in your relationship, your kids are witness to the blow-by-blow of their parent’s contentious marriage. As they’re watching, storing each and every memory, they’re developing their definitions of what a marriage is, what a man and woman are, and what they’ll likely create for themselves as they enter the dating and marriage world.

Work to Be Done That’s why it’s so important for you to know who you are, what you want and why you’re in your relationship. Without that clear information you’ll struggle to make sense of it all while your partner’s lack of clear information muddies the already muddied waters.

But there is an upside. When you do this important work, you have the potential to share a spiritual journey with someone you adore, are sexually mad about, trust and respect. That is worth all the work. With a strong foundation, the two of you can build a family and support one another through the challenging stages of your kids’ growth and your own maturation as parents, empty nesters and then grandparents. So, read, go to counseling individually and as a couple, listen to podcasts, eat well, exercise, be spontaneous, listen to each other, go out of your way to love each other, and never forget that becoming complacent is the first step toward losing what could be the most wonderful part of your life.
