Therapy for Parkinson’s changes West Knoxville man’s life Just a few years ago, Joseph Lessard couldn’t walk. A Parkinson’s disease diagnosis two decades earlier had restricted him to a wheelchair. His New England accent had become a whisper. Lessard had pursued physical therapy to manage his Parkinson’s, but no significant improvements were made. He was taking medications and going to the doctor regularly when he heard about the BIG and LOUD program at the Parkwest Therapy Center. BIG and LOUD are offshoots of the Lee Silverman Voice Training (LSVT) technique that was established in 1987. The four-week programs are meant to combat the limitations Parkinson’s causes in terms of movement and speech. Four times a week, patients spend an hour exaggerating common motions to increase mobility and using speech software to complete vocal exercises. “By the third week, I was up and walking. It was a huge improvement,” Lessard said. “I started with standing up without falling, 4 2015 WEST KNOX MEDICAL FOCUS

then moved to walking and running.” “It changed his whole outlook when he saw that it was working,” Michele Waak, Lessard’s daughter, said. “He had something to look forward to and made him excited to go to his appointments.” Much of the success that can come from the BIG and LOUD program stems from a commitment to doing your homework. “You can’t miss any practices at home,” Lessard said. “You have to do it every day or it won’t work.” Beyond his mobility, Lessard cites a renewed sense of balance as the real treasure of the BIG and LOUD program. “It made me more confident and able to move more steadily, but I also developed a lifestyle that allowed me to enjoy things again,” he said. Lessard no longer comes to the Therapy Center for appointments, but diligently does his exercises each day. He only uses a wheelchair at night. “You can’t be wishy washy,” he said. “You have to dedicate your life to it.”

Dr. Perkins indicated that over the last decade the use of intraocular injections has become a major advance in the treatment of retinal disease and is a highly successful, relatively painless in-office procedure. Patients typically have a series of monthly treatments. However, the exact number of treatments depends on the particular disease and the individual response to treatment. Since the early 2000s, the use of intraocular injections has accelerated, initially fueled by clinical trials in which SERA participated. Today, intraocular injections have become the first-line of therapy for treating and stabilizing most cases of neovascular age-related macular degeneration as well as many cases of diabetic retinopathy and retinal vascular occlusion. Symptoms that may indicate a retinal problem exists include floaters, blurred vision, waviness, distortion or dark gray spots in the central vision. Patients are usually initially evaluated by their primary eye-care provider and, if appropriate, referred to SERA. SERA physicians do not replace, but instead, work alongside the 6 2015 WEST KNOX MEDICAL FOCUS

patient’s primary eye care provider to deliver specialty care. If you notice any of these potentially concerning symptoms, see your primary eye care provider to help determine if you might have a retinal problem. Did you know? Eye examinations are recommended to identify potential vision problems in their infancy stages. Many eye experts advise that the average person should have an eye exam every one to three years, depending on his or her age. The American Optometric Association says children should have their first eye exam at six months of age. Subsequent exams can take place at age 3, and then when the child is beginning school. If an eye doctor determines a person has a particular condition that requires monitoring, more frequent eye exams will be advised. Children require less frequent eye exams, but as a person gets older, he or she should schedule eye exams every year. Consult with an eye doctor if you have questions on when to schedule an eye examination or if you are experiencing vision problems.

Computer Vision Syndrome People who sit in front of a computer for long periods of time often encounter a variety of uncomfortable symptoms. Headaches, neck strain, backaches and wrist pain are common, but sadly, the most prevalent symptoms of prolonged computer use – eye strain, blurred vision and dry eyes - are often overlooked. Computer Vision Syndrome, also referred to as Digital Eye Strain, describes a group of eye and vision-related problems that result from extended computer, tablet, e-reader and cell phone use. At greatest risk for developing CVS or Digital Eye Strain are those persons who spend two or more continuous hours at a computer or using a digital screen device every day. Accordingly, the level of discomfort appears to increase with the amount of digital screen use. Many of the visual symptoms experienced by users are only temporary and will decline after stopping computer work or use of the digital device. However, some individuals may experience continued reduced visual abilities, such as blurred distance vision, even after stopping work at a computer. If nothing is done

to address the cause of the problem, the symptoms will continue to recur and perhaps worsen with future digital screen use. In most cases, symptoms of CVS or Digital Eye Strain occur because the visual demands of the task exceed the visual abilities of the individual to comfortably perform them. Even if you don't need glasses for driving, reading or other things you do, you still may have a minor vision problem that can be aggravated by computer use. You may need a mild prescription to wear only for use of electronic devices to reduce vision stress. A thorough eye exam every year for computer users is a good idea. If you wear glasses for distance vision, reading, or both, they may not give you the most efficient vision for viewing your computer screen, which is usually about 20 to 30 inches from your eyes. Tell your doctor at The Eye Group about your job tasks and measure your on-the-job seeing distances. Oral health impacts overall health A healthy mouth is good for more than just a pretty smile. Oral health can affect the entire body, making dental care more than just a cosmetic concern. Many people know that poor oral hygiene can lead to gum disease, tooth decay and even lost teeth. But are you aware that failing to brush or visit the dentist regularly also can lead to more serious health issues? According to Colgate, recent research suggests that there may be an association between oral infections, particularly gum disease, and cardiovascular disease and preterm birth. Gum disease also may make diabetes more difficult to control, since infections may cause insulin resistance and disrupt blood sugar. Your mouth also can serve as an infection source elsewhere in the body. Bacteria from your mouth can enter the bloodstream through infection sites in the gums. If your immune system is healthy, there should not be any adverse effects. However, if your immune system is compromised, these bacteria can flow to other

areas of the body where they can cause infection. Other links have been found between oral health and overall health. In 2010, researchers from New York University who reviewed 20 years of data on the association concluded that there is a link between gum inflammation and Alzheimer’s disease. Researchers in the UK also found a correlation. Analysis showed that a bacterium called “Porphyromonas gingivalis” was present in brains of those with Alzheimer’s disease but not in the samples from the brains of people who did not have Alzheimer’s. The P. gingivalis bacterium is usually associated with chronic gum disease and not dementia.
