Request for admission to Waipuna HospiceInpatient unit: (current Waipuna patient or new referral)
• IPU admission requests and handovers are required to be Doctor to Doctor. Contact the Specialist Medical Officer (SMO) at Waipuna Hospice on 07 552 4380
• If a new patient to Waipuna Hospice, please complete an electronic referral form found on ‘One Place’ and forwardany supporting documentation e.g. Transfer of Care.
Electronic Referral to Waipuna Hospice for new patient:
• The electronic referral form is found on TPH ‘One Place’ (refer Appendix 1)
• Important to provide as muchinformation as possible (Waipuna Hospice does not have access to records at TPH)
• Referralprocess will start as soon as possible and will be triaged against Midlands Specialist Palliative Care criteria (refer Appendix 2)
• All referred patients accepted to service are contacted within 72 hours (if not urgent)
• If the referralis URGENT (Patientcarerequire within 48 hours): In Working hours (8.00am to 4.00pm):
Please follow up the electronic referral with a phone call to Waipuna Hospice 07 552 4380 to discuss the referral and patient needs. Out of hours:
If the referralis out of hours, please phone Waipuna Hospice 07 552 4380 select 1 and talk to a Registered Nurse.
o Nurse may request that youemail referral through to a different email address.
• Notification of referral status:
o The referrer may be contactedto gather further information or to support with planning
o The referrer will be advised if the referralis accepted or declined
Medications on Discharge:
Waipuna Hospice currently use Medi-mapfor all their syringe driver and PRN subcutaneous prescribing. This is then prepared by a pharmacy contractedby Waipuna Hospice that is credentialed as an aseptic dispensing service. Oneof our community nurses will then deliver to the patient along with appropriate education
On discharge from TPH:
• Syringe driver and / or Parenteral PRN medications:
o Prescribe required medications on an ‘Authority for Administration of Medication by a Waipuna Hospice Palliative Care Nurse’ form (refer Appendix 3)
o Email to Waipuna Hospice: referrals@waipunahospice.org.nz (Email box is monitored Monday to Friday 8.00am to 4.00pm)
NOTE: NoPrescriptionis requiredfor these medications for the patient
• On receivingthis form Waipuna Hospice will have one of the MedicalOfficers/Nurse Practitioners prescribe the medication on Medi-mapfor our contractedpharmacy to dispense.
• All other medications:
o Please prescribe all other medications as per the normal process for you at TPH.
