Patient and Family Advisory Committee

Over the past decade, the Waterloo Wellington Local Health Integration Network (LHIN) has worked to significantly improve the quality and availability of local health care. Now, we’re focused on making it easier. We want to make it easier for you to be healthy, easier for you to get the care and support you need, and easier for you and your family to live the healthiest lives possible. To make meaningful system change and improve the patient experience, we are putting the voice of patients and families at the centre of everything we do as a system and at every stage of a patient’s health care journey. The Waterloo Wellington LHIN is committed to partnering and listening to patients and families to ensure that changes in the health care system reflect the needs and opinions of those it serves.

What is a Patient and Family Advisor? A Patient and Family Advisor is someone who: •

wants to improve patient care and the experience for patients and families in our community

shares advice based on personal experience as a patient or caregiver

partners with staff to help improve the patient and caregiver experience

volunteers his/her time (approximately 2-3 hours per month)

What Patient and Family Advisors Do? Over the next year members of the Patient and Family Advisory Committee (PFAC) will help design a Patient Experience Program – a program that puts the voice of patients at the centre of health care planning, ensures greater health equity by elevating the voice of patients, specifically francophone and indigenous residents and influences models of care to create better health outcomes and improve the patient experience.
