Like clockwork, Matters of the heart emerge during this time each year. The universal symbol of love is front and center on Valentine’s Day, when many of us express feelings of devotion and romance toward our significant others. But most important, February is a time for the entire community (not just those in the healthcare industry) to share news and strategies to promote cardiovascular health. At Saint Barnabas Medical Center, we are dedicated to fulfilling the mission of Heart Health month all year long. Beginning on page 28 is a comprehensive list of the cardiac services available at Saint Barnabas Medical Center, including emergency care, diagnostics, clinical trials and surgery. As technology advances, so do we. Our cardiac surgeons use cuttingedge and state-of-the-art equipment and treatments, such as the transcatheter aortic valve replacement for heart valve repair and the Watchman to help those with atrial fibrillation. Read more about the Watchman on page 30. New technology has also led to improved treatment of aortic aneurysms, a condition that claims the lives of about 13,000 Americans each year. Vascular surgeons can detect and repair these aneurysms earlier than ever thanks to advanced imaging and the new hybrid operating room at Saint Barnabas Medical Center. If you’re a snow lover, you don’t care what Punxsutawney Phil predicts—you want more snow! That’s especially true for sleigh riders and snow tubers, who during this time of year can’t get enough of the fluffy white stuff. After the next snowfall, be sure to check out some of the following neighborhood sledding hot spots and fall in love with the winter. Lewis Morris Park—This county park in Morristown is a hot spot following every major snowfall. “There’s nothing more invigorating than sledding down the Doe Meadow hill,” says Reynold Fauci, director of recreation for the Morris County Park Commission. “You have to be looking for excitement to attempt it from the top!”

Tourne County Park—The Boonton section of the park is a long-time destination for sledders in Morris County. On snowy days, a portion of the park road is closed off to vehicles and designated for sleigh riding only. Flood’s Hill—Next to the Baird Community Center in South Orange, this locale is one of the most popular sleigh riding stops in North Jersey. It’s suitable for families but the hill can be intimidating so keep an eye on the young ones.

Color Glen Ridge Once solely found in the domain of youngsters, coloring books, colored pencils and crayons have been elevated from child’s play to grown-up fun. The burgeoning adult coloring craze has hit towns in Morris and Essex counties—clubs and groups meet regularly at libraries and centers. The Friends of the Glen Ridge Library has jumped on the trend, creating Color Glen Ridge, an adult

coloring book featuring residents’ photos-turneddrawings. The pages contain dozens of images from the community, including homes, parks, schools, churches and other familiar locations. Newark hosted Abraham Lincoln twice—first in 1861 when he, as President-elect, stopped there on his way to Washington for his inauguration. In 1865, thousands were in tears when Lincoln’s remains were brought to the city days after the president was assassinated. His body was en route to Illinois for burial. George H. W. Bush made several stops in Newark, including a 1992 appearance at Lincoln Technical Institute where he met and spoke with students.

Montclair has been a destination for several presidents-turnedauthors and those on the campaign trail. Bill Clinton stopped by in 2007 to promote his book and support his wife during her first run for POTUS.

Rockaway Township received a $77,000 grant from the state Department of Environmental Protection to continue its recycling efforts, which have been cited as some of the most successful in New Jersey. While the state average recycling rate sits around 41 percent, Rockaway’s hit 59.5 percent in 2014. Percentages are calculated by comparing total recycling tonnage with the weight of solid waste. “The more we work toward this goal, the better off we will be in the future both ecologically and economically,” Mayor Michael Dachisen said.

The county seat of Morris County, this burg we take for granted isn’t just a great place to live near. It’s a great place to spend time in too—for shoppers, diners, museum-goers and nightlife seekers. Morristown’s roughly 18,500 in-town residents have direct access to Hoboken and New York Penn Station on New Jersey Transit from the Morris Avenue train station, and proximity to routes 287, 10 and 24 makes traveling by car a cinch. The Green is where George Washington made the first of his two Morristown encampments during the Revolutionary War. From January to May 1777, he maintained his headquarters at Arnold’s Tavern. The general’s second encampment, in the winter of 1779–1780, was in Jockey Hollow, where he stayed at the Ford Mansion. The mansion, Washington’s Headquarters Museum, the Jockey Hollow Visitor Center, the Jockey Hollow Encampment Area and Fort Nonsense are all part of the Morristown National Historical Park. History buffs can also take in many other town attractions, including the Morris County Historical Society’s Acorn Hall, Historic Speedwell Village, Fosterfields Living Historical Farm, and MacCulloch Hall Historical Museum. The imposing Greco-Roman Morris County Courthouse, built in 1827, was actually the third courthouse building to serve the county.

Getting to...

The heart of the matter

When it comes to cardiovascular care, we offer everthing— from new medicine to new treatments—a patient needs. Heart disease is still the number one killer in America—that hasn’t changed. But just about everything else involving the heart and cardiac care has in the past decade. “Ten years is ancient history in cardiac care,” says Gary Rogal, M.D., chief of Cardiology at RWJBarnabas Health and at Saint Barnabas Medical Center. “We are seeing a big shift in how we deal with cardiac disease. There is a lot of movement to outpatient care, new medicines and treatments are being discovered that really work, and the bottom line is dramatic: mortality from heart disease is dropping significantly.” Barnabas Health Heart Centers provide every option for cardiac patients of all ages, he says. “Even if it’s not in the four walls of the medical center, we integrate with our sister institutions, like Newark Beth Israel Hospital, to offer everything from basic cardiology to full heart transplants.” Indeed, as a tertiary care center, Saint Barnabas receives patients from other area hospitals who need specialized diagnostic or therapeutic procedures that those hospitals don’t offer. The comprehensive list of services includes: Emergency care. When the medical center receives patients showing symptoms of heart attack, specialists on call rush to diagnose and open blocked

arteries within an hour (and no later than 90 minutes) to restore blood flow as quickly as possible. “The team effort among EMS responders, the ER staff and the cardiac catheterization lab to make that all happen is very successful,” says Sabino Torre, M.D., an interventional cardiologist. “We have all the latest devices, including stents and tools to open blocked arteries, all available in the lab.” Cardiac imaging and diagnostic testing. The cardiac catheterization lab uses the the most advanced noninvasive cardiac and coronary computed tomographic (CT) angiography with a CT scanner, comprehensive cardiac ultrasound echocardiography and the most advanced MRI scanner to provide sophisticated images of the heart. “Soon we’ll have a 256-slice CT scanner to image coronary arteries and valves even better,” Dr. Rogal says. These imaging systems help Barnabas Health Heart Centers perform more diagnostic imaging and treatment, such as angioplasty procedures, than any other program in New Jersey. Heart failure treatment and heart transplantation. RWJBarnabas Health’s heart transplant program is the third largest in the nation with survival rates that consistently meet or exceed national

benchmarks. “The program performs about 300 cases of open-heart surgery a year,” notes Frederic Sardari, M.D., vice chair of the Cardiothoracic Surgery Department. “But in the past five years or so, more procedures are performed minimally invasively.” Surgeons now work on a still-beating heart in many cases, avoiding the highrisk heart-lung machine. The most common is coronary artery bypass graft surgery. “Beating heart surgery is more complicated, but it works very well,” Dr. Sardari says. Mechanical ventricular assist devices (VAD). For patients waiting on the transplant list, surgeons now offer implantable VADs as a bridge to transplant. These devices give patients a better quality of life until transplantation becomes available. Structural surgery. Advances in heart valve repair and replacement have been dramatic in recent years. Transcatheter aortic valve replacement (TAVR) can be done without open heart surgery, a blessing for the older patients who suffer aortic valve disease most often, says Dr. Torre. A similar procedure for mitral valve repair, called the MitraClip, will be offered beginning this year, he says. To treat atrial fibrillation, a new device called the Watchman helps prevent blood

clots from leaving the heart and potentially causing a stroke. (See page 30.) Clinical trials. When implanted in the heart, a tiny device—the Watchman—can treat atrial fibrillation, reducing the risk of stroke.

Atrial fibrillation, or AFib, is a cardiovascular disease that causes abnormal heart rhythm. AFib is caused by a combination of factors, but principally its development is attributed to overactive electrical signals from the top chambers of the heart. These signals are disorganized, causing the atria to “fibrillate” or “flutter.” A small percentage of AFib is caused by heart valve problems. The most dangerous consequence of atrial fibrillation is stroke—the average person with AFib is about five times more likely to suffer a stroke than someone with a regular heartbeat with a risk ranging from 2 to 20 percent a year. Most people with AFib are treated with a blood thinning medication such as warfarin (brand name Coumadin) to reduce the risk of forming blood clots in the heart that cause a stroke. For some, though, these medications cannot be taken long-term. Until recently, there was no FDA-approved alternative to blood thinners. Now there is: a tiny device implanted into the heart itself to block off the area where most of these clots form. It’s called the Watchman. The team of David Dobesh, M.D., cardiac electrophysiologist; Constantinos Costeas, M.D., director of Cardiac Electrophysiology at Saint Barnabas Medical Center; and Susan Simandl, M.D., director of the Saint Barnabas Medical Center Echocardiography Laboratory, implanted the first four of them at the medical center in January. “There is long-term data showing that the Watchman is equivalent to or superior to using Coumadin,” Dr. Dobesh says. “It reduces the risk of stroke without the increasing risk of internal bleeding from blood thinners. It’s a game changer in how we treat this disease.” The reason AFib can lead to stroke is because it decreases the heart’s pumping capacity in the top chambers, the atria. When blood isn’t pumped out efficiently, blood cells can pool in a part of the heart’s top left chamber called the left atrial appendage (LAA), where it forms clots that can travel to the rest of the body, including the brain. More than 90 percent of the clots that cause a stroke in patients with this type of AFib come from the LAA. “The main preventive therapy is blood thinners, but for many patients these are not suitable,” Dr. Dobesh says. They can cause bleeding in the gastrointestinal or urinary tracts, or lead to excessive surface bleeding and bruising, especially in the elderly, who make up a substantial portion of those with AFib. Those with other health conditions that

raise the risk of bleeding, such as kidney or liver disease, or blood disorders, are also poor candidates for blood thinners. “Millions of people in the United States have AFib, so a large number of people are at risk,” he says. For them, the Watchman may be the answer. The device is less than the size of a quarter. It is composed of a self-expanding, nickel-titanium frame, with a polyester fabric cover. To implant it, the surgeon makes a small cut in the upper leg and inserts a catheter, similar in many ways to procedures to implant a cardiac stent. In this case, the doctor guides the Watchman into the LAA. It expands to block off the LAA, and the heart grows its own new tissue over the device over the next 45 days to create a permanent seal, preventing blood from pooling and forming clots there. The LAA has no known role in normal heart function; it is a vestigial appendage, much like the appendix. “In fact in the original Grey’s Anatomy text it is called the left atrial appendix,” Dr. Dobesh says. “It plays a role in fetal heart development, but you don’t need it after birth.” The procedure is done under general anesthesia and takes about an hour. Patients commonly stay in the hospital overnight and leave the next day. “The recovery is very quick, and most patients will be back to normal that week,” Dr. Dobesh says. Patients are required to remain on blood thinners for 45 days, until the Watchman has permanently sealed the LAA. An echocardiogram will be performed to make sure the device is working properly. The device is permanent, and never needs to be replaced. Patients will need to take a different medicine called clopidogrel (known by the brand name Plavix) and aspirin for six months. After six months they are encouraged to continue aspirin on an ongoing basis. A very small number of patients may be encouraged to keep taking blood thinners long term, Dr. Dobesh says, but more than 90 percent of patients can stop taking the drug. What’s more, those patients will no longer need regular blood tests or to restrict their eating and drinking habits, as is required when on some blood thinners. More than 20,000 Watchman procedures have been performed worldwide since 2005, with more than 10 years of clinical trials and registry data proving its safety and efficacy. It’s the only device of its kind approved by the FDA for reducing stroke risk in people with AFib not caused by heart valve problems. —d.l.

Physicians’ gift propels project

the state-of-the-art Cooperman Family Pavilion will house a new NICU, nursing units and other outpatient testing areas. saint Barnabas medical center has embarked on the most ambitious construction project in two decades. The 270,000-square-foot, five-story Cooperman Family Pavilion, set to open in fall 2017, is the cornerstone of the campus transformation. A group of physicians who will be working in the space are playing a big role in funding the project. American Anesthesiology of New Jersey (AANJ), an affiliate of MEDNAX, has pledged a total of $2 million (over five years) to help with the construction of the space and the improvement of electronic health records technology. The physicians of AANJ are personally donating half the sum, and AANJ will provide the other half. “We are grateful to have the physicians of AANJ, along with all the anesthesiologists at the medical center, behind this both personally and professionally,” says Mehrdad Rafizadeh, M.D., MEDNAX corporate medical director, chairman of Anesthesiology at Saint Barnabas Medical Center and president of the Medical Staff. “This is our future, and it will impact us for the next 20 to 25 years.” The result will be a state-of-the-art facility. The Cooperman Family Pavilion will be home to a world-class Neonatal Intensive

Care Unit (NICU), new nursing units and a host of state-of-the-art, expansive outpatient testing areas. There is also the light-filled, two-story Eric F. and Lore Ross Lobby and a new parking garage, which has already been completed. Through collaboration with Saint Barnabas caregivers, this project is drawing on the latest ideas in evidence-based hospital design. The pavilion will be created with natural materials, soothing colors and abundant natural light, and it will provide the most advanced medical technology as well as spaces to promote privacy and healing. Such evidence-based design reflects new thinking about how to create hospital environments that improve care, including from curb-free showers to prevent falls, acoustical materials to lessen noise and hybrid operating suites that let surgeons switch easily from minimally invasive procedures to so-called open procedures. Dr. Rafizadeh hopes that other health care providers follow AANJ’s lead. “We want this project to be an example to other physicians on our medical staff that they can do the same thing,” he says. “This gift is one way to continue our commitment to our hospital partner and demonstrate that we want to be here for a long time.” —d.l.

Travel with care Saint Barnabas Medical Center reaches out to the community both near and far.

Giving back to the community locally, nationally and internationally has been a focus of Saint Barnabas Medical Center physicians, nurses and staff since the inception of the hospital back in 1865. In 2015 alone, Saint Barnabas Medical Center provided in excess of $1 million of community benefit reaching over 32,000 community members. “Whether it was providing free melanoma or lung cancer screenings, offering health lectures throughout the region or participating in mission work, we are passionate about keeping our communities healthy while providing exceptional care to our patients and their families,” says Stephen P. Zieniewicz, FACHE, President and CEO, Saint Barnabas Medical Center. This past year, a team of Saint Barnabas Medical Center physicians and staff volunteered their time to provide specialized services to a medically underserved area in the mountains of Appalachia. In the town of Wise, Va., many individuals fall into the cracks of health insurance—too poor to afford their own coverage but too well off to qualify for subsidies under the Affordable Care Act. Margie Heller, Administrative Director of Community Outreach, learned about this underserved group after watching a 60 Minutes program about Health Wagon, an organization that provides free medical care to the medically underserved in Appalachia. Heller wanted to help and reached out to Ernani Sadural, M.D., obstetrician/gynecologist and Director of Global Health for RWJBarnabas

Health. Together, they partnered with Health Wagon who has been providing care to the community for decades. “We asked them to identify five needs not being met for their patients,” Heller says. As a result, two missions were planned to address the community needs. The first three-day trip included John Russo, M.D., an internal medicine specialist along with Dr. Sadural to treat patients with diabetes as well as women’s health issues. A second three-day trip in October sent gastrointestinal and anesthesiology specialists, who saw 42 patients and performed 67 procedures. “Robert Schuman, M.D., treated several

patients with precancerous lesions,” Dr. Sadural says. The procedures removed those lesions and likely saved them from developing cancer. Along with providing care, the team is learning more about the community’s health needs to form a lasting partnership, Dr. Sadural says. “Our physicians were so happy to be there, to help on their time, and to see how much they got out of it was really special,” Heller says. Dr. Sadural adds, “We came in to help, like a neighbor would. We want to make this a reflection of who we are at Saint Barnabas Medical Center, to always remember to help for no other reason than that.” —d.l.

in battle against aneurysms Technology updates improve detection and treatment of the condition in the aorta.

About 13,000 Americans die each year from a condition they usually don’t know they have—an aortic aneurysm. An aneurysm is a bulge or “balloon” in the wall of an artery, the blood vessels that carry oxygen-rich blood from the heart to other parts of the body. Arterial walls can withstand normal blood pressure, but certain medical problems, genetic conditions and trauma—especially from smoking—can damage or injure artery walls. The force of blood pushing against the weakened or injured walls can cause an aneurysm. If an aneurysm grows large, it can rupture or dissect. A ruptured aorta, the main artery that runs from the heart through the chest and abdomen, causes massive bleeding inside the body, which is why it is usually fatal. A dissection is a split in one or more layers of the artery wall, which causes bleeding into and along the layers of the artery wall. It too can be fatal. “Einstein had a ruptured aneurysm,” says Steven Hertz, M.D., vascular surgeon and chief of the section of vascular surgery at Saint Barnabas Medical Center. “Lucille

Ball had one. George C. Scott died from one.” An aneurysm that occurs in the chest portion of the aorta is called a thoracic aortic aneurysm. More commonly, an aneurysm that forms in the abdominal portion is called an abdominal aortic aneurysm. Both can develop and become large before causing any symptoms. Often aneurysms are found by chance during tests done for other reasons. The good news is that doctors can stop aneurysms from bursting if they find and treat them early. Better news is that vascular surgeons now can offer treatment to a larger group of patients, says Dr. Hertz. “The two things have allowed that,” he says, “are improved imaging and the new hybrid operating room at Saint Barnabas Medical Center.” When he started in the field, back in 1993, the only option for aortic aneurysm repair was open surgery. “It was a very big operation,” he says. Today, the trend is to repair the damaged artery from the inside of the vessel itself. That’s a minimally invasive procedure, and often works well. But on occasion, the

Abdominal Aortic Aneurysm: Screening Recommendations The National Institutes of Health report that early diagnosis and treatment can help prevent rupture and dissection of aortic aneurysms. However, aneurysms can develop and grow large before causing any symptoms. Thus, people who are at high risk for aneurysms can benefit from early, routine screening. n Men ages 65 to 75 years who have ever smoked: The United States Preventive Services Task Force (USPSTF) recommends one-time screening for abdominal aortic aneurysm (AAA) with ultrasonography in men ages 65 to 75 years. n Men ages 65 to 75 years who have never smoked: The USPSTF recommends that clinicians selectively offer screening for AAA in men ages 65 to 75 years who have never smoked rather than routinely screening all men in this group. n Women ages 65 to 75 years who have ever smoked. The USPSTF concludes that the current evidence is insufficient to assess the balance of benefits and harms of screening for AAA in women ages 65 to 75 years who have ever smoked. n Women who have never smoked: The USPSTF recommends against routine screening for AAA in women who have never smoked. surgeon needs to switch to an open operation, and usually very quickly. That’s where the hybrid operating room come in. This special room has the surgical equipment, operating tables and other technology to allow surgeons to perform both types of surgery, without having to move the patient when time is of the essence. Saint Barnabas’ hybrid OR opened in April 2016, and Dr. Hertz was the first to use it to repair aortic aneurysms. To repair an aortic aneurysm, the surgeon places a graft inside the artery with a catheter to support the damaged arterial walls. Doing this in open surgery would require time in the intensive care unit and six to eight weeks of recovery. Minimally invasively: “One night in the hospital, and you leave with two Band-Aids in the groin,” he says. This allows older and sicker patients to partake of the surgery.

“The goal is to have none become emergencies,” he says. “Our focus is to identify aneurysms when they are small and fix them electively. The mortality during elective surgery is less than half of one percent.” Because of these changes, aortic aneurysm is “on people’s radar now more than it used to be,” he says. And screening, thanks to better imaging, is now covered by Medicare and many other insurance plans. “Physicians want to offer the best technology, and that takes the commitment of the community and the hospital to find the space and funding for a program like this,” Dr. Hertz says. “This is forward looking. It allows us to do things we couldn’t do before, which translates to the ability to help patients more, which is really what we are here for.” —d.l.

Attend a religious service Frequent church or synagogue attendees have lower blood pressure than others, says a study published in the International Journal of Psychiatry in Medicine. Why? It could be that “people who attend services are exposed to sermons that provide guidance for healthy decision-making and coping with stress,” says Harold G. Koenig, M.D., of Duke University, one of the study authors. Group singing may also help cut stress, he says.

It was her family’s many visits to the beautiful Whiteface Lodge in Lake Placid, N.Y., that served as inspiration for the décor of the great room in the Mendham Township home of Kara Bossert. “My daughter did figure skating and competed up there, so we went every January for six or seven years,” Bossert recalls. “We always thought the antler chandeliers were amazing, so we figured, ‘Why not? We have tons of deer around here; it’s not so far-fetched that we would have an antler chandelier.’ So we had one made for the room.” The great room is the main living area in the house, and Bossert wanted a natural, earthy look for the space. “I have animal skins, fur pillows, horn bowls and things like that out on the tables,” says Bossert, owner of the design firm Design in Time. “With the stone fireplace, it all lends itself to that feel.” The large scale of the room led Bossert to use long gray velvet couches from Restoration Hardware and not one but two black coffee tables from Safavieh. And the Lexington armoire in the corner was chosen for its size but turned

out to be a very handy piece. “Initially, it was just to fill that corner and give some height to the room,” Bossert explains. “But it stows the kids’ board games and puzzles that we want to keep handy.” Bossert used Benjamin Moore’s Bridgewater Tan on the walls because she loves the way that color looks against the abundant white woodwork in the house, which the family built about three years ago. Shades of black and gold can be found throughout the room, including in the Kravet fabric used for long panel drapes and roman shades. “I love black,” says Bossert. “I have a lot of black in my home. It’s very dramatic and makes an impactful statement.” The black embossed leather lamp by Ralph Lauren was a lucky Home Goods find. Overall, Bossert loves the way it all came together. “It’s a really comfortable room,” she says. “It gets a lot of natural light, and it’s a nice room to spend time in.” But her favorite thing will always be the chandelier: “I’m just in awe of it,” she says, “especially when you’re sitting on the couch and looking up at it. The way it’s constructed, it’s amazing.”

Easy pickin’ Produce pete loves his weekly TV segment, but it’s his sweetheart that’s always at the top of this north jersey favorite’s list. By Darius Amos Think you know the correct way to peel a banana? “Produce Pete” Napolitano will likely tell you to make like a chimpanzee and try a little monkey business. “Your first inclination is to open it from the top, which results in those annoying loose strings,” says Napolitano, who’s considered the New York metro area’s authority on fruits and vegetables. Our distant jungle cousins, he notes, pinch and peel from the bottom for cleaner, easier access to the fruit. “If you’re not doing that, try it.” Napolitano, 72, has been offering tidbits like these from his childhood days as a peddler for the family business to his successful run as a mom-and-pop farm stand owner to his 30-year career as a local television personality—with the past 25 years spent on WNBC’s Weekend Today in New York. And while he loves produce, he wouldn’t be where he is today without his wife, Bette. It was her foresight that brought the Produce Pete persona to life. Napolitano’s history on television traces back to 1986, when, not satisfied with work, he was on the verge of closing his family’s North Jersey-based Napolitano’s Produce Store. One day that year, a customer overheard Napolitano joyfully speaking with other shoppers. That customer was Rosemary Henri, a local TV producer who “found me very interesting and thought I would be good for television” he recalls. “I told her I was too busy,” Napolitano says. “She continued to call, but I turned her down each time.” But things changed the day Bette answered the phone. When Napolitano came home from work that afternoon, she told him to get dressed—the network was sending a car to bring him to its New York studio. “Bette convinced me that going on TV might be good for business,” he says. “She’s the one who saw something good.” He’s been talking produce on the air ever since. Fruits and veggies are Napolitano’s passion, and it’s seen every week during his regular TV appearances, when he converses about any and every piece of produce that’s in season. His long stint on Weekend Today is a testament to that passion, earning the “Produce Pete” and “Pete Your Produce Pal” nicknames along the way. “Most of the stuff I talk about is common sense, but they’re tips that people want,” he admits. “Why is a fruit that’s heavy in the hand better than others? Simple: It has more juice. If it’s light like a feather, then there’s usually nothing inside.” The segments got more and more popular over the years “because people love to eat, and they love to eat good food,” he says. “And I think it’s because of my

honesty. When I’m in front of the camera, my first line is usually thought out, but everything else, when I talk about a product or produce, that’s just me running my mouth. It also helps to throw in some humor and stories about my family, because viewers can relate even more.” Family, he says, turned him into the produce guru. He learned about fruit as a child, helping his parents’ produce stand by selling tomatoes door-to-door in Englewood and Tenafly. In 1969, he took the reins of the family business. “Bette helped me in the store for 20 years on the business end and was also there to raise the family and take our two kids to soccer and football,” he says. “She’s been a trouper, and I give credit to her for all the success I’ve had.” The couple have been side-by-side for most of their lives: The two were high school sweethearts—he was 15 and she was 14 when they fell in love. They’ve lived in Bergenfield and Oakland in Bergen County, and now reside in nearby North Haledon in Passaic County. So did it hurt when Bette decided to leave the business to pursue an administrative career with large companies such as Konica? Not at all. “She’s such a hard-working woman and so smart when it comes to business. I didn’t want her to regret not making a change, and I think she made a great decision,” he says. “But everyone at Napolitano’s missed her a lot.” Napolitano’s finally closed in 2006 after a 52-year run, but Pete and Bette are still partners in produce and in life. They will celebrate their 50th anniversary this summer. “Bette has been my rock and my foundation all these years,” he says. “None of this, the store or Produce Pete, would’ve happened without her.”

Freekeh On

Move over, quinoa—freekeh is quietly staging a takeover.

Tired of quinoa and want to shake up your meal plan? Then give freekeh a go. This ancient whole grain (pronounced free-kah) has long been a staple of Middle Eastern and Northern African diets but only recently began gaining popularity in the United States. Freekeh comes from durum wheat that’s harvested when it’s young and green and then fire-roasted. After the charred outer shell is rubbed off, the grain in the middle, which retains a subtle smoky, nutty flavor, can be boiled and used in dishes in place of rice, farro, barley, millet and other grains.

POWER UP Compared with superfood power-player quinoa, freekeh comes out on top with less fat (1 gram in ½ cup cooked compared to 2 grams in the same amount of quinoa), and more protein (6 grams vs. 4 grams) and fiber (4 grams vs. 3 grams). At only 130 calories per serving, it’s a better choice than rice or pasta for those hoping to lose weight. It’s also rich in magnesium, which helps with energy levels and digestion; iron, which oxygenates red blood cells; lutein, which promotes eye health; calcium, which benefits the bones; and vitamin B6, which boosts the immune system. Plus, freekeh ranks low on

the glycemic index, making it a smart choice for those who have diabetes or are at risk of the disease.

Buy/Store/Serve The grain is sold already removed from its shell and looks similar to barley or wheatberry. To prepare it, simply cook it as you would any other grain. Bring 2½ cups of water per 1 cup of dried freekeh to a boil. Lower the heat and allow it to simmer until the water is absorbed, and then fluff and allow to cool. You can also soak freekeh in water overnight and heat it up the next day. It’s available both cracked and whole—the cracked variety cooks in about half the time as the whole, similar to rolled oats vs. steel-cut oats. Once cooked, use the grain in salads, in soups, as a pilaf, or on its own as a side dish, served with meat or fish.

DID YOU KNOW? Freekeh is also called farik, which comes from the Arabic word for “to rub,” referring to how it is prepared. According to some sources, the grain was discovered two millennia ago after a Middle Eastern wheat field was set on fire during an ambush. When villagers recovered the young grains from the field, they discovered the center had not been destroyed and was still edible. —Liz Donovan

watercress with balsamic beet-roast chickpeas and parsley crab Yield: 4 servings

Calories: 238 per serving

Ingredients FOR THE CHICKPEAS n 1 cup beet juice n 1 Tbs. balsamic vinegar n 1 14-oz. can of chickpeas,

drained, rinsed and dried n Salt and pepper

FOR THE SOUP ½ small leek, finely chopped 1 garlic clove, chopped 1 celery rib, coarsely chopped 1 large zucchini (approximately 9 oz.), coarsely chopped n ½ Tbs. olive oil n 2¼ cups vegetable or chicken stock n 3 sun-dried tomatoes n 3½ oz. fresh white crabmeat n Few sprigs of parsley or tarragon, finely chopped n Squeeze of lemon juice n 12 oz. watercress n 1½ cups unsweetened soy milk

Directions To make the chickpeas, place the beet juice and balsamic vinegar in a small saucepan and bring to a boil. Simmer for 20 to 25 minutes, until the liquid has reduced to about ¼ cup and has a thick syrupy consistency. Preheat the oven to 400°F. Place the chickpeas in a small baking pan and pour over half of the syrup, mixing well. Season and roast for 10 minutes. Remove from the oven, pour over the remaining syrup and roast for another 10 to 15 minutes, keeping an eye to make sure the syrup does not burn. When ready, set aside. Sauté the leek, garlic, celery and zucchini in the olive oil and 1 tablespoon of water for about 5 to 8 minutes. Add a splash more water if necessary. Pour in the stock, then the sun-dried tomatoes. Bring to a boil and simmer for 15 minutes. Mix the crabmeat with the chopped parsley or tarragon, and then season with salt, pepper and a squeeze of lemon juice. Refrigerate until needed. Removed the sun-dried tomatoes then add the watercress and soy milk. Simmer for a couple of minutes to soften. Blend until smooth and season to taste. Serve, topped with a spoonful of crabmeat and a scattering of chickpeas.

carrot, coconut and ginger with cilantro sambal

Yield: 4 servings Calories: 227 per serving

Ingredients n 1¼ cups fresh coconut, grated n ½ tsp. cumin seeds n ½ tsp. coriander seeds n 2 shallots, diced n 1 garlic clove, coarsely chopped n 5 Tbs. ginger, peeled and grated n ½ tsp. chile flakes n ½ Tbs. coconut oil n 1½ lbs. carrots n 3 cups vegetable or chicken stock

Directions If using a whole coconut, prepare it by piercing the three eyes with a skewer, and drain out the liquid. Then, wrap it in a towel, place on a hard surface, such as the floor (or take it outside), and give it a firm blow with a hammer to crack it open. Use a spoon to gently lift the flesh away from the harder exterior and remove the brown skin using a potato peeler if desired.

CILANTRO SAMBAL

Add the carrots, fresh coconut and stock. Bring to a boil and simmer for 30 to 35 minutes until the carrots are soft.

Toast the cumin and coriander seeds in a small frying pan for about 2 minutes, then grind in a mortar and pestle. Sauté the shallots, garlic, ginger, chile flakes and ground spices in the coconut oil and 1 tablespoon of water for about 5 minutes, stirring occasionally. Add a splash of water if the mixture sticks to the pan.

To make the sambal, place the cilantro, grated coconut, ginger and chile into a mini food processor and pulse to a coarse, dry paste. Stir in the lime juice and season to taste. When the soup is ready, blend until smooth, season and serve topped with the sambal.

superfood chowder

Yield: 4 servings

Ingredients n 1 ½/3 cup black quinoa n Pinch of salt n 1½ cups unsweetened soy milk n 9 oz. smoked haddock n 1 bay leaf n 1 onion, peeled and halved n 6 peppercorns n 1 medium leek, finely sliced n 1 garlic clove, chopped n ½ Tbs. olive oil n 1 fresh corn on the cob n 3 cups vegetable stock n 5½ oz. spinach, finely shredded n 3 Tbs. chives, finely chopped

Calories: 241 per serving

Directions Place the quinoa in a strainer and rinse under cold water. Transfer to a small saucepan, cover with 2½3 cup water, add a pinch of salt, bring to a boil and cook with the lid on for 10 minutes. Remove from the heat and leave undisturbed for 5 minutes so that the grains absorb any remaining water. Return to the strainer and run under cold water again to cool completely. Set aside until needed. Place the soy milk in a medium saucepan with the smoked haddock, bay leaf, onion halves and peppercorns. Slowly bring to a boil and simmer for 3 minutes. Remove from the heat and let stand, covered, for 5 minutes. Strain and set the cooking broth aside. Clean the saucepan and return to the heat. Sauté the leek and garlic in the oil and 1 tablespoon of water over low heat for about 5 minutes, until soft and translucent. Stand the corn upright and slide the blade of a sharp knife along the length of the cob, removing the kernels as you do so. Add these to the saucepan, along with the reserved cooking broth and the vegetable stock. Add the cob for added flavor. Bring to a boil, immediately reduce to a simmer, and cook for 7 to 10 minutes. When ready, remove the cob and used a slotted spoon to skim any foam that has formed on the surface. Add the cooked quinoa, along with the spinach and fish. Bring to just below boiling point, season to taste, garnish with some chopped chives and serve immediately.

Beating Flay was no fluke No wonder this Verona restaurant’s proprietor bested a famous chef on TV. Her dinners are winners. These days, portion size is often inversely proportional to a meal’s quality. But that couldn’t be further from the truth at Ariane Kitchen & Bar in Verona. With seasonal menus of comfort food and palate-pleasing flavor combinations, this restaurant serves generously—and you’re grateful for every bite. Chef and proprietor Ariane Duarte, who was in the kitchen cooking for us on the night we visited, was a competitor on Top Chef in 2009 and went on to out-cook famed chef Bobby Flay on a special Food Network show some years later. Duarte previously owned CulinAriane, a BYO in Montclair. She opened Ariane in 2014 with partner John MacEvoy, proprietor of the popular Verona Inn just two blocks down Bloomfield Avenue. The plain exterior belies a spacious interior fronted by a full bar and a few rows of raised pub tables that seat about eight. At the rear of the room is a dining area punctuated with a row of modern-looking, richly colored upholstered wingback chairs. We were seated on this side of the space at a wooden two-seater topped with woven placemats. Our charming server, Courtney, swiftly retrieved our two requested glasses of pinot grigio and relayed the night’s specials. Shortly thereafter, a basket of three small croissants with a pat of butter arrived at the table. The croissants were incredible—a flaky, firm shell and soft, buttery goodness inside. When the trio disappeared, Courtney asked if we wanted more. We said no, but our faces must’ve been saying the opposite: Another basket magically appeared. My companion immediately expressed her pleasure with the menu offerings, particularly the starters, and chose for herself the pretzel salad. After much internal debate, I went with the oysters. Neither of us was disappointed when the appetizer course arrived. True to its name, the salad was topped with pretzel chunks that were perfectly crunchy on the outside and soft on the inside, if a bit oversalted. Served on a bed of kale with Gruyere cheese and apples,

the salad was lightly dressed with a grain mustard vinaigrette. My plate held six cornmeal-crusted oysters, served on the half shell on a pillow of airy horseradish cream, a lovely flavor combination. For my main course, I selected the AKB burger—a perfectly medium-rare patty of American Kobe beef and a healthy slab of fried green tomato topped with cheddar, pickled shallots and a harissa aioli on a sesame bun. The flavors blended wonderfully on the palate, and the lightly seasoned, thick-cut fries and homemade ketchup were the perfect accompaniment. The portion was so large, I just had to take half home. My dining partner couldn’t wait to try the juniper-cured porterhouse pork chop, which our server called the best thing on the menu. A beast of a chop if I ever saw one, it was served to order medium-well and the meat was juicy despite the size of the cut. The Brussels sprouts came a bit scorched on the outside but with just the right amount of crunch, and the cheddar grits were creamy. The apple butter bacon onion jam (say that three times fast!) perfectly finished the plate. For a side, we split the Gruyere mac and cheese, which Courtney proclaimed to be the best she’s ever had. Smooth and incredibly cheesy, it had a pleasantly smoky taste. We stopped halfway through generously portioned entrées in anticipation of sampling the dessert menu. We each quickly found a favorite. I had the deliciously creamy dark chocolate panna cotta topped with malted whipped cream and slightly bitter cocoa bits; my companion couldn’t resist the Meyer lemon curd, served atop crushed vanilla wafer and topped with a meringue she likened to “eating a lollipop with Fluff.” The menu pricing is reasonable for food of this caliber and plate size. Ariane’s top-notch cuisine, décor and servers perfectly harmonize for a pleasant dining experience.
