Chairman’s and President’s Message Annual Report 2010/2011

Philanthropy is transforming care in every corner of the University of Alberta Hospital, the Mazankowski Alberta Heart Institute and the future Edmonton Clinic. As generous donors, you are directly improving the lives of patients every day. Net Revenue for 2010/2011 (Prior to Distributions - $12,295,031) $5,018,459

$1,384,783 Annual Fundraising is net of fundraising costs ($1,192,853) and includes: Annual Giving Campaign, Donor Designated, Unrestricted, and Established Fund gifts and Lottery revenue. Investment Income is net of Administrative and Amortization costs which total $1,206,824 (including investment fees)

In the fiscal year ending March 31, 2011, the Foundation’s net revenue was $12.3 million. The Foundation distributed over $3.1 million and is holding $8.3 million in cash and term deposits, which will be distributed in the near future to satisfy the Foundation’s current funding commitments to Alberta Health Services. We are pleased to provide a full accounting of the Foundation’s financial position in the Management Discussion on page 22 and in the detailed financial reports that follow. The University Hospital Foundation’s dedication to responsible stewardship was recognized by the Association of Healthcare Philanthropy, who identified the foundation as one of 16 “high performers”. This distinction was based on the AHP’s 2010 benchmarking database, which includes 54 hospital foundations in the United States and Canada. This achievement, and the tremendous impact it has on the lives of patients, would not be possible without the dedicated support of our donors and volunteer Board of Trustees. These individuals give generously of their time, financial resources and wisdom, becoming valued members of our Foundation family. 2

We extend our most sincere appreciation to Dr. J.F. (Jake) Burlet who leaves the Board after nine years as a Trustee, Board Chairman and Past Chairman. During his term as chairman, Jake provided tremendous leadership in guiding the Foundation through the development of our three-year strategic planning process to map an ambitious and successful future. John Moquin also leaves the Board this year, following six years of service. John’s commitment to the Board and his additional service on the Festival of Trees sponsorship committee greatly contributed to the success of our valued holiday friend raiser and fundraiser. Finally, we bid a fond farewell to Deb Gordon, who joined the Foundation in 2005 as the Vice President of the University of Alberta Hospital and the Mazankowski Alberta Heart Institute. Deb’s enthusiasm and passion will be missed. We wish her well in her new position as Senior Vice President and Chief Nursing and Health Professions Officer with Alberta Health Services. Accountability to you, our valued supporter, is important to us. Generous gifts from our donors have supported two recent advances in diagnostic imaging that hold extraordinary promise in transforming cardiac and neurosurgical care.

Elko Cardiac MRI

The Mazankowski Alberta Heart Institute is now home to the dedicated Elko Cardiac MRI suite. Funds raised through the Capital Campaign for the Mazankowski Alberta Heart Institute, including a leadership gift from Cathie and Ernie Elko, funded the full cost of $2.65 million for this specialized imaging technology. Cardiac Magnetic Resonance Imaging (MRI) captures detailed three-dimensional images of the beating heart, providing vital information on the structure, valves and blood flow to and from the heart. This safe, non-invasive technology is highly effective in helping clinicians to determine the amount of heart muscle damaged after a heart attack, to evaluate weakened heart muscles and to diagnose, monitor and treat irregular heart rhythms and conditions affecting blood vessels. Cardiac MRI is making great strides in assessing a patient’s condition and determining the best course of treatment as quickly as possible, often eliminating

the need for additional tests. This leadingedge technology will perform up to 1,500 cardiac scans each year, and significantly reduce patient wait times for this valuable diagnostic procedure. Cardiac MRI also holds great promise as a research tool, providing detailed imaging sequences and measurements of cardiac structures. Information obtained through these new image sequences will enhance diagnostic capabilities and patient care.


Widney Intra-operative MRI A first for the University of Alberta Hospital, the 3 Tesla Intra-operative MRI suite is a quantum-leap forward in neurosurgical imaging, which allows surgeons to view the brain with microscopic detail during surgery. This $3.45 million, specialized surgical unit was funded through a leadership gift

from Dan and Bunny Widney and a variety of fundraising initiatives including: the Festival of Trees, the Brain Trust Campaign, the Full House Lottery and the many generous donors who contributed to our 2009 Annual Campaign. The 3 Tesla Intra-operative MRI is a powerful new diagnostic tool that will provide neurosurgeons and neuroradiologists with exceptionally clear images of structures deep in the brain. This will help the neurosurgical team perform complex brain surgery with microscopic accuracy, which will dramatically improve results when treating brain tumours, movement disorders and epilepsy. Having MRI technology in the operating room will reduce the need for repeat surgery and help to ensure that patients with lifethreatening and life-impacting conditions benefit from the most precise surgery and best recovery possible.

Care Transformation Initiative

The Edmonton Clinic

Care Transformation began at the University of Alberta Hospital as a bold initiative implemented by Alberta Health Services. It is grounded in the fundamental belief that through a coordinated, cooperative effort, our staff could offer the best patient experience possible by integrating patient care and by putting the right person in the right place at the right time. Through the dedicated efforts of clinicians, front-line staff, senior management and administration, the Care Transformation Team has created an innovative model of care that aims to improve every aspect of the patientexperience from admission, through to treatment, rehabilitation and release. Patients at the University Hospital are now experiencing fewer complications and shorter hospital stays. The success of this initiative holds great promise for the transformation of care in other Alberta Health Services sites across the province.

Opening in late 2012, the Edmonton Clinic will support the emphasis on putting patients at the centre of care at the University Hospital site by consolidating specialized, outpatient clinical care and streamlining access to numerous services and specialists. Unique to Alberta, this nine-storey, 670,000 square foot facility will bring multi-disciplinary teams of specialists together under one roof including surgical, medical, family and seniors’ clinics, as well as orthopaedic and neurosciences clinics. This integration will provide seamless ambulatory care for up to one million patient visits per year. In addition this tremendous new outpatient facility will enhance the partnership between Alberta Health Services and the University of Alberta by strategicaly aligning clinical care, medical education and research.


Northern Alberta Renal Program

F

or over 40 years the Northern Alberta Renal Program (NARP) at the University of Alberta Hospital has been providing a full spectrum of care for patients with renal (kidney) disease – from monitoring and

treatment, to dialysis and transplantation. This program currently serves more than 3,600 patients from northern Alberta including over 1,040 patients who require regular dialysis treatments.

Let’s Go Home Campaign

Patients with end-stage renal disease are unable to clear toxins from their blood. Without a kidney transplant or regular dialysis treatments, these patients will die. Life-sustaining dialysis requires that patients undergo treatments a minimum of three times every week, with each treatment taking at least four hours. Intensive home hemodialysis offers patients the chance to dialyze at home, often while they sleep. Longer and more frequent dialysis has shown to have dramatic benefits including improved health, more energy, fewer restrictions on diet and fluid intake, and more time for family and loved ones. The University Hospital Foundation’s “Let’s Go Home Campaign” was created in close collaboration with doctors, nurses, program staff and patients from NARP’s Home Hemodialysis Program. To date,

donors to the campaign helped fund the purchase of 15 additional home hemodialysis machines. The unique, web-based component of the Home Hemodialysis Campaign encourages donors to give online, which enables us to reach beyond the hospital and into communities and homes, and to raise funds for additional home hemodialysis machines to “send even more patients home.” For more information on this lifesaving campaign please visit:

For the past 26 years, the Festival of Trees has added “Sparkle” to the festive season as part of Edmonton’s holiday landscape. Volunteer commitment and generous community sponsors have ensured that Canada’s longest running Festival of Trees has also grown to become the flagship fundraising event for the University Hospital Foundation. Proceeds from the 2010 Festival of Trees supported the purchase of a new neurosurgical microscope, a 3 Tesla Intra-operative MRI and specialized equipment for the Multiple Sclerosis Clinic. This year we welcomed our first-ever Festival of Trees overall presenting sponsor, Landmark Group, and celebrated a “landmark achievement” – raising over $1.3 million to advance the Brain Sciences program at the University of Alberta Hospital. We sincerely thank Reza Nasseri and the Landmark Group, as well as our Gala sponsors, Paddy Webb and Family, and our Luncheon and Fashion Show sponsor, Capital Power Corporation. 2010 Audrey Greenough Award Edmonton philanthropist C.J. Woods, FCA, was presented with the Audrey Greenough Award of Merit at the University Hospital Foundation’s 2010 Festival of Trees Gala. Each year the Foundation presents this award to a member of our community in recognition of outstanding service and charitable contribution to the University of Alberta Hospital and the Mazankowski Alberta Heart Institute. For over 15 years C.J. has generously supported the University Hospital Foundation. Since 1997 he has been a valued sponsor of the Festival of Trees. He was appointed to the Foundation Board of Trustees in 2006 and shares his time, expertise and knowledge to advance the mission of the Foundation. C.J. is currently serving in a leadership role as Co-chair of the Campaign for Prostate Health, giving generously and working tirelessly to support several successful special events that have raised significant funds for the campaign.

The Brain Trust Initiative

T

he Brain Trust was introduced in 2010 through a University Hospital Foundation Board initiative in partnership with the Festival of Trees to raise an additional $5 million over

Edmonton Heartbeat Run September 26, 2010

More than 400 participants ran or walked through Edmonton’s scenic river valley for the second annual “last great run of the season” to raise awareness and funds for the prevention of heart disease. The University Hospital Foundation thanks the organizing committee: Co-chairs Dr. Robert Welsh and Dr. Blair O-Neill, Suzanne Welsh and Trevor Soll for their hard work and commitment to advancing heart health through this challenging and fun event.

A Night at the Maz Lecture Series presented by Syncrude Winter 2010/2011

This series of free, interactive lectures was held at the Mazankowski Alberta Heart Institute in the Shaw 3i Auditorium. Guests heard from leading cardiac experts on a wide range of topics including: Novel Therapies for Heart Failure, Transforming Heart Attack Care, and Stress Management. Lectures were followed by an opportunity to share experiences and ask questions.

Heart Pledge Day – 630 CHED Radiothon and Shaw TV Telethon February 10, 2011

Patients shared stories of challenges and triumphs, and experts in cardiac care discussed advances in treatment during a 16-hour radio/ telethon, broadcast live from the Mazankowski Alberta Heart Institute. Local celebrities, businesses and numerous volunteers manned the phones, receiving over $100,000 in donations and thanking generous callers for supporting the Heart Institute.

he Campaign for Prostate Health has an ambitious goal: to change the odds for men in Alberta facing prostate disease and cancer. The University Hospital Foundation, the Royal Alexandra Hospital Foundation and the Alberta Cancer Foundation joined forces to raise $30 million to provide improved and timely access to specialized services, to support the latest surgical treatment options and to advance prostate care and research.

The Campaign for Prostate Health, now in its final phase, has raised over $25 million. Innovations in Care

In the first phase of the Campaign for Prostate Health, $4 million was directed to purchase two state-of-the art robotic surgery systems and related technology now fully operational at the University of Alberta Hospital and Royal Alexandra Hospital. The Campaign for Prostrate Health is directing $10 million to develop a rapidaccess Prostate Health Clinic within a consolidated urology centre in the new Edmonton Clinic. The new Prostate Health Clinic will support a multi-disciplinary

team of experts dedicated to transforming the delivery of patient care by providing early diagnosis and rapid access to treatment and by enhancing education and support for men with prostate cancer and disease in Alberta. Additional funds will be used to create an Innovation and Research Fund to support promising advancements in prostate research, education and patient care, and to support the province-wide Alberta Prostate Cancer Research Initiative.

The Experience – Wine, Spirits and Beer Charity Auction November 4, 2010 The Experience is presented by the Liquor Depot as part of the Rocky Mountain Wine & Food Festival. This important project is made possible through the dedication of Liquor Depot CEO and Foundation Trustee Rick Crook. In 2010, the fourth annual exclusive evening of wine, spirits and food raised over $475,000. In total, The Experience has raised over $1.7 million for the Campaign for Prostate Health. Planning a Gift for the Future

I

n October 2010, Doug Morrice had openheart surgery at the Mazankowski Alberta Heart Institute to correct a blocked artery. During the procedure, surgeons discovered that his diaphragm was malformed and faced challanges when trying to reposition his left lung. He survived the very difficult surgery and was placed in the cardiac intensive care unit where he was closely monitored for two weeks. His family maintained a round-theclock bedside vigil, never giving up hope that Doug would be well again.

Honouring a Legacy of Giving For many years Elizabeth Palate supported advancements at the University of Alberta Hospital through generous gifts to the University Hospital Foundation. She took great pride in her recognition on the Foundation donor wall. Later in life Elizabeth experienced health issues that brought her to the University of Alberta Hospital for treatment. Elizabeth Palate was a passionate and dedicated teacher. It was a passion she passed on to her niece Geri Thompson who, with the help of her aunt, also pursued a teaching career. When Elizabeth passed away in November 2000, she left a bequest to her much loved niece, and Geri used this bequest to make a gift to the University Hospital Foundation, creating an established fund in honour of her aunt. The Elizabeth Palate Fund for Nursing Education recognizes the exceptional care Elizabeth received at the University Hospital and acknowledges the value she held so dear: higher education. The fund supports those with a nursing diploma who want to earn a Bachelor of Science in Nursing (BScN), providing front-line nursing staff with opportunities for continued professional development and higher education.

edical research opens the doors to discoveries that advance all areas of patient care. The University Hospital Foundation has a long history of supporting innovative research and promising new research at the University of Alberta Hospital and the Mazankowski Alberta Heart Institute through Team Grants and the Medical Research Competition.

PROACT Research (Providing Rapid Out-of-hospital Cardiovascular Treatment) When a person has a heart attack, every second is precious. Delays in treatment can result in additional damage to the heart muscle, severely impacting quality of life or even resulting in death. A city-wide, multi-disciplinary team of experts led by Dr. Paul Armstrong is looking at new ways to treat patients with heart attacks and acute heart failure before they arrive at the hospital. Research previously conducted by some of these investigators led to the development of the innovative, early heart-attack treatment Vital Heart Protocol. This program uses ECG information transmitted from the ambulance to help determine whether or not to administer clot-busting drugs en route to the hospital to help restore blood flow and oxygen to the heart muscle.

The PROACT research team is now taking this knowledge to the next level, looking at additional methods of diagnosing and treating a broader range of heart attacks and other acute heart problems sooner, through information obtained by paramedics in a single drop of blood. A new triage meter in city ambulances will bring a laboratory-like capability that measures whether or not heart damage or heart failure has occurred, giving experts in the emergency room and receiving hospital vital information, thereby fast-tracking the patient to the most rapid and appropriate care. When “time is muscle and muscle is life,” the seconds saved could mean the difference between life and death.

Medical Research Competition

E

ach year the University Hospital Foundation Medical Research Competition distributes funding for promising medical research at the University of Alberta Hospital and the Mazankowski Alberta Heart Institute. Successful projects are selected through a process of peer review and must provide a clear link to a clinical problem with an emphasis on bringing results from the bench to the bedside.

During 2010, the Faculty of Medicine and Dentistry received 44 applications for research funding. Of these, 16 projects were selected to receive a total of $500,000 of Foundation funding. Many of these projects are led by new investigators and hold great promise in shaping the future of health care. Every day donor support for medical research is funding advancements that will help save lives and reduce the impact of illness and disease.

Using high resolution MRI and intracranial EEG images, Dr. Jeffrey Jirsch is able to pinpoint the the precise areas of the brain affected during an patient’s epileptic seizures. Pinpointing Epileptic “Hotspots”

Epilepsy is a neurological disorder in which a person experiences seizures caused by abnormal electrical activity in the brain. Two-thirds of patients with epilepsy are treated successfully with medication and are able to live less restrictive lives. For those who do not respond to medication, surgery is often the only way to reduce or eliminate seizures. In a new study funded by the University Hospital Foundation Medical Research Competition, Dr. Jeffrey Jirsch and Dr. Donald Gross are researching the use of combined technologies to pinpoint the precise area of the brain that is affected 20

during seizures. This research is examining high resolution MRI images from the highfield (4.7 Tesla) MRI in the Peter S. Allen MR Research Centre. Researchers will also be assessing intra-cranial EEG recordings to measure the electrical activity of the epileptic brain. The combined modalities will help to accurately locate epileptic “hotspots” in the brain. Information obtained using these advanced technologies will help increase the likelihood that surgery to reduce or eliminate seizures will be successful, giving patients with epilepsy the freedom to participate in activities previously limited by their condition.


Better Care for Renal Disease

In a new research study funded by the University Hospital Foundation Medical Research Competition, Dr. Robert Pauly is examining the long-term effects and advantages of nocturnal (during sleep) home hemodialysis on patients with end-stage renal disease (ESRD). End-stage renal disease is a chronic, life-threatening condition that dramatically impacts the quality of life for patients and their families. When a patient is diagnosed with kidney disease, treatment options are limited. They must either have regular dialysis treatments to remove excess fluid, waste and toxins from the body, or they must receive a donor organ and undergo a kidney transplant. To date, the best possible treatment option for patients with ESRD is to have a successful living-donor kidney transplant. Transplants using a kidney from a deceased donor offer the next most significant benefit in terms of improvement to health and quality of life. However, in many cases patients must continue to receive dialysis for a significant time until a donor organ

becomes available. Also, for some patients, hemodialysis is the preferred option. In-clinic dialysis requires that patients travel, often long distances, to a clinic to receive the four-hour treatment a minimum of three times a week. In between treatments, fluid and toxins build up in the blood, causing increased blood pressure, bloating, extreme fatigue and muscle weakness. Home hemodialysis machines now allow patients to dialyze from the comfort of their own home, often while they sleep. This means that patients have fewer dietary restrictions, fewer time constraints and feel better overall, providing them with more freedom and a higher quality of life. This study hopes to demonstrate that the positive benefits of nocturnal home hemodialysis may actually be similar to the benefits experienced by patients who recieve deceased-donor transplants. Information gained through this research will also help experts understand which patients with ESRD are most likely to benefit from home hemodialysis and which strategies are most effective in keeping patients in their homes.

Management Discussion of 2010/2011 Financial Statements As part of the 2010/2011 University Hospital Foundation Annual Report we are once again pleased to provide expanded insight into the Foundation’s 2010/11 financial statements and performance.

Financial Performance Highlights As prudent stewards of our donors’ dollars, we work diligently to ensure that we efficiently and effectively raise and manage our donor gifts. The Foundation employs a variety of fundraising programs such as annual giving and major gift campaigns, direct mail, donor prospecting, special events and lotteries. Each fundraising method has certain expenses associated with it. Special events and lotteries have significantly higher costs than other fundraising activities, but the revenue they provide to the Foundation is substantial and invaluable. This past year the Foundation raised $24.4 million in gross revenue, which includes investment income of $6.2 million ($27.2 million and $9.4 million, respectively, in 2009/10). After $12 million ($8.4 million in 2009/10) in expenditures our net revenue, our primary measure of success, was $12.3 million ($18.8 million in 2009/10). The decrease in net revenue from the previous fiscal year can be attributed to a variety of factors. First, our investment income was lower by $3.2 million. Although our investment income in 2010/11 was exceptional, 2009/10 was extraordinary as we recouped much of the losses from the 2008/09 fiscal year. (See below for a detailed discussion of our investment income). Second, in 2009/2010 changes to the CICA (Canadian Institute of Chartered Accountants) guidelines mandated that certain lottery costs be expensed in the current fiscal year and not be deferred as in previous years. This accounting change significantly increased lottery expenses as reported in the 2010/2011 fiscal year. Due to this change, net revenue for Full House Lottery is lower by $1 million from the previous year. Third, after detailed and favorable market research, our foundation partnered with another hospital foundation on a second lottery: Dream Away. Unfortunately, the results were not as anticipated, incurring a net loss of $0.8 million (our share) on the Dream Away Lottery, reducing our overall share of net revenues from lotteries to $0.4 million. It should be noted that we will not continue this lottery in the 2011/12 fiscal year. Fourth, our net fundraising revenue is down by $1.5 million which can be attributed to a decrease in capital campaign revenues and a decrease in charitable giving nationwide. Recently there have been several media reports regarding charities and their cost of fundraising. As a licensed charity in Alberta, the University Hospital Foundation complies with guidelines established by Service Alberta to determine cost of fundraising. (The cost of special events and lotteries is not included in Service Alberta’s calculation of the cost of fundraising.) This fiscal year our cost of fundraising was 15% ($1,192,853), which represents an effective use of resources and assets. However, the University Hospital Foundation also has investment

income to offset all of its administrative and amortization costs ($1,206,824), as well as $794,635 of the $1,192,853 of the fundraising costs. In total of $2,001,459 of operational costs are paid from investment income. The Foundation’s Statement of Financial Position remains strong and healthy, with over $75 million in net assets ($66 million in 2009/10) invested in a diverse portfolio, which is managed by two investment firms. Once again we are pleased to report excellent investment returns due to favorable market conditions and sound investment management. In 2010/2011 our investment managers (Bissett Investment Management and Mawer Investment Management Ltd.) returned 11.7 % (22.5% - 2009/10). Their consolidated returns outperformed their benchmark as outlined in the Foundation’s Investment Policy by 2.1%. Their benchmark is comprised of 50% fixed income, 25% Canadian equities, 12% US equities and 13% international equities. As at March 31, 2011 our established funds totaled $36.2 million ($35.4 million in 2009/10). Established Funds are an important source of long-term revenue as the donation is invested, and the investment income is distributed annually to support priorities designated by the donor. The rate allocated for distributions to the Established Funds is based on the five year net annualized return on our long term investments. Due to continued excellent investment returns, we were able to keep the investment income allocation at 3.23% the same as in the 2009/10 fiscal year. In this and in the previous fiscal year, $1.1 million of investment income was allocated to the Established Funds which are listed in Schedule 1 of the Financial Statements. In the past year the Foundation distributed over $3.1 million ($9.3 million - 2009/10) to advance research and patient care at the University of Alberta Hospital and the Mazankowski Alberta Heart Institute. The Foundation is currently holding $8.3 million in cash and term deposits which will be distributed in the near future to satisfy the Foundation’s current funding commitments to Alberta Health Services (AHS). With changes at AHS, various projects are being reviewed and reassessed, requiring the Foundation to hold its cash for longer than expected. We anticipate that in the near future there will be a significant outflow of cash to fund a number of outstanding priorities, which will benefit current and future patients at our facilities.
