SERVICE CENTERS Prince George’s County Department of Family Services Administration on Aging The Administration on Aging provides a wide range of services and programs to enrich the lives of older adults to maintain the highest quality of life with independence and dignity. Services and programs are provided that support aging in place, consumer choice and promote dignity and respect. There are special programs designed to support adult children caring for their aging parents and relatives. Elder Abuse Prevention Project This project provides public education to the community and professionals serving older adults in an effort to prevent neglect and elder abuse. Family Caregiver Program The program provides assistance and support to people caring for their aging parents, relatives and friends and those that are involved with long-distance caregiving. Services include information and referral, assistance services, supportive counseling, emergency assistance, respite care and

community education. Foster Grandparent Program This program provides resources for older volunteers to work with physically, mentally and emotionally challenged children in centers and schools throughout Prince George’s County. Health Promotion-Living Well This program offers senior citizens a series of workshops to help them with diagnosed or self-diagnosed chronic conditions while learning skills to manage the symptoms and get the most out of their life. Topics covered in the workshops include: medicines, nutrition and physical activity; communication skills with your health care team; handling frustration and problemsolving; future planning; and relaxation methods. Maryland Access Point MAP is a new initiative that links older persons and adults with disabilities from all income levels with local health and support services. It is the single point-of-entry to access resources for lifelong independence. Whether the question is understanding Medicare, where to find

leisure time activities, how to care for a frail older relative, how to locate senior housing, assisted living or a nursing home — MAP is the place to start. Money Follows the Person MFP will help people transition from an institution to community living in an apartment, private home or small group setting. The program initiatives increase outreach to individuals in institutions and decrease barriers to transition. MFP provides peer mentoring, enhanced transition assistance, improved information technology, housing assistance, flexible transition funds and the addition of waiver services to existing waivers. Ombudsman Program Promotes the highest quality of life and care possible for nursing home residents. The ombudsman investigates and seeks resolution of problems that affect residents’ rights, health, care, safety and welfare. Public Guardianship Program Assures the ongoing safety and well-being of the elderly by professional case

managers when the Area Agency on Aging has been appointed Public Guardian by the Circuit Court. Retired and Senior Volunteer Program Provides volunteer opportunities for those seniors age 55 and older. More than 600 volunteers serve at more than 65 nonprofit organizations and government agencies. Senior Assisted Living Subsidy Program The program provides financial assistance to qualified low to moderate income seniors residing in assisted living facilities throughout the county. This program helps to defray the high cost of this service and delays or prevents nursing home placement. Senior Community Service Employment Program The program helps older adults find employment by building skills and confidence through paid community service positions. Participants must be 55 or older and meet income eligibility criteria. Senior Health Insurance Program The program provides

comprehensive health insurance counseling and assistance to senior citizens, persons with disabilities and their caregivers. Senior Nutrition Program Provides nutritious meals at senior centers and nutrition sites throughout the county and delivers meals to homebound seniors as needed.
