A New Level of Acute Care: The Intensive Medical Management Unit

C

areOne at King James is proactively addressing evolving, acute care needs with a new Intensive Medical Management Unit (IMMU). This innovative unit provides a heightened degree of medical attention with increased nurse staffing for the initial transition period. Patients must receive clearance for medical stability from their attending physician to transition to our general sub-acute unit. Incoming patients will be transported directly from the hospital to our CareOne IMMU, which provides a 1:10 RN/Patient ratio and 1:10 CNA/Patient ratio to support this elevated care setting. The 10-bed IMMU is where all new acute admissions will be placed for the first 48 to 72 hours for assessment and specialized care. The IMMU is designed as a transitional care unit to assist patients’ adjustment from acute to post-acute care for continued rehabilitation. Our IMMU state-of-the-art medical technology and resources include: • ANCC Board Certified Family Nurse Practitioner • Dynamaps • Non-Invasive Open Ventilation System • Telemed • Electrocardiograms • 24/7 Physician Access for Patients • Specialist on Staff o Cardiac Care Center: Dr. Vlahos o Pulmonary Program: Dr. Parhar o Medical Director Geriatric Board Certified: Dr. S. An intensified Transitional Care Program translates to fewer hospital readmissions. CareOne is one of the first rehabilitation centers in New Jersey to align with the Affordable Care Act and Accountable Care Organizations with an advanced model of care—the Transitional Care Program. This innovative effort was designed to ensure the coordination and continuity of care during the transition of patients between healthcare settings and as health needs change. Findings from multiple clinical trials have consistently demonstrated the positive impact of the Transitional Care Model on the outcomes of older adults while reducing total costs of healthcare, including less out-of-pocket costs for patients. The model has resulted in fewer hospital readmissions with reported improvements in physical health, functional status and quality of life. More patients are moving from hospital to rehab to home with more specialized care from CareOne The cornerstone of Transitional Care with Intensive Medical Management is the CareOne Care Navigator.
