Life is filled with surprises! Your period should not be one of these surprises. Heavy Menstrual Cycles

In this edition, I will be discussing the issue of heavy menstrual cycles. For most women, their menstrual cycle occurs about once every 28 to 30 days. The period may last three to seven days. The flow is described as moderate. Menses start somewhere around eleven to fourteen years old. This monthly gift will continue until fifty-one.

Unscheduled Bleeding

Bleeding outside of your normal cycle day or heavy scheduled cycles are considered abnormal. Unscheduled bleeding is vaginal bleeding at times other than your normal menses. This may be patterned or totally irregular in nature. This can be spotting to period like in flow. If the flow is excessive, it may result in anemia (low hemoglobin).

Heavy Cyclic Bleeding

Heavy cyclic bleeding (plain old heavy periods) is when a woman has regular periods with heavy flow and possibly longer duration with more bleeding days. This may be associated with cramps or pelvic pain. Pain may become severe enough and or bleeding profuse enough to change what you do on a daily basis. To be more specific, pain and/or bleeding can keep a woman from her doing her normal, everyday activities. Women may become fearful to leave their home, as they worry about bleeding through clothing in public. This can force a woman to lose work time, kid time, husband time etc. The cost of sanitary products and replacement clothing may also play a role in how this affects a person.

Evaluating Heavy Periods

In this article, I will be concentrating on heavy periods or menses. The evaluation of this problem starts with a

detailed history. The history can provide direction for selecting diagnostic testing. It can sometimes be difficult for a woman to keep an accurate record of her period. I find it helpful to use a menstrual diary. The patient is given a copy of this and asked to complete and return to the office with it. Initial lab testing includes complete blood count, thyroid hormone and a pregnancy test. An ultrasound of the pelvis will enable a noninvasive way to view the uterus and ovaries. If one is over the age of thirty-five, a tissue biopsy of the endometrium or a D&C may be needed. The pathological report will exclude diseases such as endometrial cancer or hyperplasia. If testing revels a diagnosis of benign heavy menses (hypermenorrhea) then a treatment plan can be made.

Medical and Surgical Treatment Options

Treatment options shall be separated into medical and surgical categories. Both options are designed to control cycle duration and flow. Most commonly, birth control medications are used. Combined oral contraceptives are easy to use and provide cycle control. Other contraceptives that can be used are the vaginal ring, patch or the depo provera shot. The intrauterine device containing a contraceptive medication is a good alternative. It is placed by a doctor in an office setting and provides long-term cycle control. With this alternative, about 30% of users do not get a menstrual cycle. The other 70% usually have a light spotting type of period. The patient has no need to take a pill or place a patch. Surgical options can also be considered. Endometrial ablation is the use of various devices to destroy the endometrium. These include a balloon with circulating hot water, circulating hot water, cryotherapy and electrical current. I use the balloon device or the device employing electrical current as my devices of

choice. Usually these types of procedures are performed in an outpatient surgery setting. The procedure itself lasts only several minutes. Post-op course is usually unremarkable and short. Return to activities is rapid, with very little down time. Post-op pain is usually restricted to menstrual-like cramps. The cramps can be controlled with non-steroidal medications such as Naproxen. The response to treatment is varied and highly individualized. Most patients have no further periods. The amenorrhea rate (no periods) approaches 70%. Of those that have a return of their cycle, it most commonly light in flow and/or short in duration. The benefit of this type of surgical intervention is related to the lack of medication side effects. Birth control medications do have some undesirable side effects. The most notable are decreased libido (sex drive) and weight gain. These are real issues for most women.

Although New Jersey experienced quite a mild winter this year, warmer temperatures are always welcome. In fact, winter practically passed us right by this year with not only mild temperatures, but also very little snowfall. The reason for this is something we have all been hearing much about: the polar vortex. This circulating stream of air forms over the Arctic during winter months and this year it has stayed in place. This keeps its cold air locked up north. Lucky for us we have felt very little of that brutal bone-chilling air. I am particularly excited about the coming of spring. For one thing, it means I get to enjoy the fresh outdoors with my pup and soak up some much needed Vitamin D. Did you know that our State is home to 11 National Parks? The list includes Ellis Island, Thomas Edison’s Essex County residence, and even a section of the 2,185- mile long Appalachian Trail which stretches across 14 states. Committing to ‘Memory’ – Designer Turns Her Baking Hobby Into a Full-Blown Career

AnnMarie Cooney AnnMarie Cooney did not grow up dreaming of becoming a baker. Visions of sugar plums and intricately decorated cakes were not dancing through her head as a young girl. By trade, Cooney was a kitchen designer, but retired early to raise her daughters. “With three girls, my job was to raise good women,” Cooney says. It was a special request from her daughter that would eventually turn her into a highly sought-after cake designer and baker. “It all started because my daughter wanted a cake,” Cooney says with a smile. Cooney’s middle girl, was born with health issues that resulted in special needs. So when she wanted a circus cake for her fifth birthday, Cooney made her the best circus cake around. That was 14 years ago, and her girls quickly caught on to their mom’s special talent. At every birthday, they requested a special cake. “It became, ‘what can we do next’ and ‘how can we top that,’ Cooney says. Then she began making cakes for friends. Cooney’s husband had to build an additional kitchen at her house just to accommodate all of the special cake requests. When Cooney’s youngest daughter entered kindergarten six years ago, Cooney decided she wanted to go back to work. “But, I needed to figure out what I was going to do,” Cooney says. Sadly, Cooney’s grandmother, with whom she was very close, passed away during this time. “I was sitting at home, just very sad, and my cousin called me to meet her at the park with her kids,” Cooney says. However, in the midst of grief, she didn’t feel up to it. “My cousin sad, ‘come on grandma would want us to.’ So I went, Cooney says. On the way to the park, she saw the real estate sign. She checked it out and immediately knew that it would be the perfect location for her bakery, A Sweet Memory Cake Shoppe & Tea Room. “My grandmother left each of her grandchildren a small inheritance and it was just enough for me to open my business,” Cooney explains. Cooney spent about five years at that location before moving to another larger space six months ago just two miles down the road in Waretown. A beautiful picture of her grandmother has watched over her since the day she opened. It’s difficult to simply call Cooney’s creations “cakes.” Some designs are so intricate that they look like art. Cooney and her team created a three-dimensional depiction of Van Gogh’s “Starry Night” that captured the essence of this masterpiece with sweet, swirly curves and lines of vividly hued icing. Then there’s the adorable panda birthday cake that could easily be mistaken for a cuddly toy. The list of unique, edible masterpieces goes on and on, That’s because they average a minimum of 30 specialty cake orders per week, mostly by working with different

American Girl teas, unicorn teas and more. They even carry a line of tea geared towards little ones. “We even have a dress up section that ladies and little ladies love,” Cooney says. A little help from above has turned Cooney’s one-time hobby into a business that brings customers joys bite by bite. The business that Cooney started out of her home has grown into a full-time operation with a staff of eight. “We work hard and we work a lot, but we really do love each other,” Cooney says. She says they have become a great team and she couldn’t do all of this without them. Cooney’s brother-in-law, Paul Cooney, recently joined the team as chef and baker. There’s also Sarah Rod, the head decorator, and Cindi Reider, head fondant who works in fine detail. “Sarah and Cindi are so talented, they really are the best around, they’re artists,” Cooney says. Together, what they create can easily be considered art, but at the end of the day-or party or wedding or afternoon tea- and after the last bite, their masterpiece has been turned into a sweet memory.

However, they also have a long list of loyal customers. “Some of our customers started with a gender reveal and now we’re doing their fifth birthday cake,” Cooney says. Customer Kathy Helm of Waretown got her grandson’s birthday cake from Cooney close to five years ago. “It was perfect, I brought a couple pictures of what I wanted and they just did their thing,” Helm says. And she has been coming here ever since. “I’m in sales, so I bring my clients treats from here all the time,” Helm says. She’s also brought her granddaughter to Sweet Memory’s princess tea parties. “Our clients really do become our friends,” adds Cooney. In addition to the special-order cakes, A Sweet Memory, has a selection of seasonal cakes, mini desserts, cookie sandwiches and other baked goods readily available daily. Other creative creations include their cake pies; cakes topped with pie filling and decorated to look exactly like a pie. “Our baking is very seasonal,” Cooney explains. Winter flavors includes a tasted snowball cupcake, which is a chocolate cake filled with marshmallow and chocolate fudge, frosted with coconut buttercream, rolled in toasted coconut, drizzled with chocolate and topped with marshmallows. Or their gingerbread scone with maple cream. Or the orange, cranberry Christmas scone with orange cream. “My grandmother used to always have her ‘lady friends’, as she called them, over for tea,” Cooney explains, adding that she wanted to honor her grandmother by adding a high tea to her bakery. “We squeezed one table in our previous location and began hosting afternoon teas.” They were all so well-received Cooney has expanded her operation to include a full-time tea room. Decorated with a lovely mix of china and flowers, the tea room serves tea, lunch and snacks from 11 a.m. to 4 p.m. Wednesdays through Sundays. They also offer a high tea, but reservations are required. “Our clients have really created the charm of this place,” says Cooney, who adds almost of her china, platters and place setting have been donated by her customers. High tea ($27.95) is served on tiered platters and begins with an individual pot of tea, chosen from their selection of more than 50 teas. Followed by seasonally inspired salad and homemade soup, four varieties or tea sandwiches, freshly baked scones served with their specialty creams, and completed with a selection of their miniature desserts. “Everything from our meats to spreads, our dressings and chutneys, our soups are all made fresh in-house,” Cooney says. Cooney also places an emphasis on healthy fare. “We source organic whenever possible and offer a large selection or vegetarian friendly dishes,” she says. With the extra space the tea room has added to A Sweet Memory Cake Shoppe and Tea Room, Cooney is able to host special events. “We’ve done cake-decorating classes and other ladies events,” Cooney says. They’ve also hosted princess teas,

Financial Management The SECURE Act

By this time many of you may be aware that recently Congress enacted legislation that included the SECURE Act, taking effect January 1, 2020. SECURE stands for Setting Every Community Up for Retirement Enhancement. How is that for Washington speak? As background, the SECURE Act [H.R. 1994] passed the House last year on a bipartisan basis, by a vote of 417 to 3, but then stalled in the Senate. A few Republican Senators objected to unanimous consent for the bill. In December, Congress enacted tax-and-spend legislation to which the SECURE Act was included. The Act includes a number of liberalizations that should promote retirement savings. For example, the prohibition on contributing to a traditional IRA upon reaching age 70½ has been eliminated. Required minimum distributions from IRAs and qualified retirement plans may now be delayed to age 72, rather than age 70½. To pay for the changes that result in delayed tax collections, the Congress changed the rules rather drastically for inherited IRAs. Under the old rules, one who inherited a traditional or a Roth IRA was allowed to take minimum distributions from the account over his or her lifetime. For example, a 50-year-old could spread the payments out over 34.2 years. If a great-grandchild inherited the account, the payout period could be as long as 80 years! This planning strategy was known as the “stretch IRA,” and it was understandably popular. To assure that the IRA was not invaded prematurely, some people paired the stretch IRA with a trust plan. The SECURE Act largely eliminates the stretch IRA. The general rule now is that the IRA assets must be distributed over the ten years following the account owner’s death. Exceptions are made for these designated beneficiaries: • a surviving spouse; • a minor child or children; • a disabled beneficiary;

• a chronically ill individual; and • beneficiaries who are less than ten years younger than the account owner (such as a brother or sister). The exception for the minor child lasts only until he or she reaches the age of majority, because then the ten-year rule kicks in. For the other categories of designated beneficiaries, the delay in distributions ends at death when a ten-year distribution must begin. Eliminating the stretch IRA accelerates the taxes on the retirement savings, shortens the deferral period, and it also makes it more likely that the distributions will occur during the beneficiary’s high earning years instead of being delayed until retirement. If a large IRA will be a significant element of your estate, you should consider meeting with your estate planning advisors early this year. I turned 70½ in 2019, so am I excused from taking a required minimum distribution now? Or next year? Sorry, the change does not affect you. Only those who turn 70½ in 2020 or later get the benefit of waiting to age 72 for required minimum distributions. You will have to take a minimum distribution for the 2019 tax year (if you haven’t already) as well as the 2020 tax year. I inherited an IRA three years ago from a parent and have been taking minimum distributions over my lifetime. Does that have to change now, do I take the rest over ten years? Good news, the new law does not affect you. IRAs inherited from those who died after December 31, 2019, are the ones affected by the new law. My children are adults, but my grandchildren are not. Can they get the benefit of waiting until they reach their majority for the ten-year rule to apply? Sorry, no. The exception only is for minor children of the account owner, not grandchildren, nieces, or nephews. These more remote relatives will need to withdraw everything from the account over ten years.

Brainwashing—the Power of Propaganda By Charles E. Meusburger, M.D.

One of the most powerful methods to control an individual or a multitude is via “Brainwashing;” a misleading term at best, because there is nothing hygienic or cleansing about it. The objective of “brainwashing” is the willful indoctrination of others for the purpose of largely unconscious and then conscious manipulation to control beliefs/ thoughts/emotions in order to morph them into false, biased facts as a means to control the behaviors and actions of one person and/or entire nations. Many examples over the course of history exemplify this insidious process going all the way back to the Old Testament Biblical stories and the enslavement of the lesser Jews under Pharaoh, etc. Cults and gangs have infamously used the technique to advance their agendas such as the KKK, Jonestown, the Manson family, mob rule and the Mafia. All have one thing in common indoctrination with lies. Bullying to the extreme, and childhood incest, are two very powerful methods of indoctrination and intimidation. Of course, the most powerful way to influence a mind is to attack it in its youth. A young developing mind has little to no cognitive ability to process or understand what’s happening. All they have are powerful emotions and porous defenses. The extent, severity and duration of either of these two devastating early life experiences can produce long lasting negative self-beliefs, shame, guilt and potentially crippling pathologies due to the indoctrination perpetrated by the assailants. The result is to make it the victims’ fault which subsequently leads to self-loathing and selfdefeat followed and reinforced by intimidation of further harm. This includes physical or even life-threatening behaviors that galvanize the damage and pain that goes on years after the abuse itself has ended. Violations never heal quickly and have no boundaries. One could argue that learning in general is a form of brainwashing because learning entails teachings from one source to another source about what those sources believe is imperative for another source to know. Personal teachings are often about how one should behave under certain circumstances involving societal expectations. These teachings are influenced by culture, beliefs and historical events, but fundamentally they are intended to keep us safely within acceptable social and cultural boundaries. However, pathological brainwashing can be described as being indoctrinated without

the ability to apply one’s own thought processes. It is purely and intentionally one-sided. Learning allows one to ponder, be curious and to apply concept to practicality in order to extrapolate digestible knowledge and direction based on choice. Brainwashing is about no choice while believing choice is alive and well. Brainwashing is used globally as a means to sell products and services. Marketing is a means of persuading people to choose or not choose a product or service. Certain products or services create a demand where there was no demand and become downright addictive in nature. For example, cell phones, texting, and apps of all kinds for all kinds of things. We are constantly bombarded with messages to convince us to choose certain foods, drinks, vacations, diapers, cars, clothes, exercises, insurances, and you name it, there is a push strategy to pull consumers in. However, it is also the gift of free enterprise when the enterprise is free. To the contrary, brainwashing can be designed as rhetoric and propaganda intended to steal free enterprise, free will and freedom overall. Entire nations and cultures have done the same in order to reign supreme, mostly at everyone else’s expense; examples are the Zealots, the Romans, the Huns, organized religion, Apartheid, the third Reich, Communist China and yes, even the United States - more than once, perhaps, more so today. Hidden agendas, ulterior motives, sadistic fantasies, fatalistic insecurities, nihilistic dogmas, and suspension/surrender of morality, decency, empathy and common sense all occur to varying degrees. Indoctrination alone however, will not be readily sustainable without an essential counterpart, Intimidation. Intimidation needs to occur in some form, as a proximal second layer of negative influence right after indoctrination has gained a grip; then it’s ‘game on.’ Direct intimidation with painful stimuli can be used effectively, but often resistances can increase from the direct methods with push back or even rebellion. Indirect, often times a form of vicarious intimidation is more effective with greater compliance and diminished organized resistance, i.e. when you witness - visually, audibility, or are informed of the direct method of intimidation of another. Ensuing fear, dread and obedience result - even false gratitude that it didn’t happen to you. Therefore, no deviation from the indoctrination - or else. Man’s addiction to control is the most compelling and powerful addiction there is. This addiction drives and is at the core of all other addictions of mankind. The need to control through Indoctrination and Intimidation is the most insidious and dangerous one. It has no compassion, empathy, civility or fairness. Therefore, it is a

mental rape that is perpetrated for specific, selfish motives and, often secondary gain for the assailants. Cults, gangs, organized religion, militaries, classrooms, mass media, entire nations and ideologies, even overly aggressive parenting that perpetuates generational predjudices - just to name a few, all operate using these tried and reliable methods. Orwell created his classic literary version - “1984.” We are 36 years past that fictitious milestone year, but its heralded ominous reality is well underway and consistently evidenced - almost daily - in the world we live in - including this “free country.” Frightening as that may be, inertia, apathy, learned helplessness and fear promote its persistence and progression. Beware the Ides of complacency and ignorance for they are far more deadly than the sword. So, what to do about this that steals truth and trust and promotes propaganda for gain at everyone else’s loss? From the psychiatric perspective, the trauma created by either subtle or more overt forces needs to be assessed by professionals with training and experience to help reset the balance and help restore hope and peace of mind for the individuals afflicted. Individual treatment can come in many forms, i.e. psychotherapies, medicines, spiritual supports, self-empowerments and heightened awareness and education. On a more macro level involving brainwashing afflicting masses, debriefings and retraining may be utilized over time; but most often political will and serious changes of culture and ideology are necessary, though seldomnly are such exerted efforts available by the many and for the many that require it. -Serves 8

5 lbs of beef chuck short ribs 1 1/2 cups ketchup 1/2 cup white vinegar 1/3 cup brown sugar, packed 1 T Worcestershire sauce 2 tsp lime zest 1 1/2 tsp dry mustard 3/4 tsp garlic powder

• In large Dutch oven, cover short ribs with water and bring to boil over high heat. • Reduce heat to low, cover and simmer for 2hrs. • Cover and refrigerate before eating. • About 1 hr before ready to serve, heat grill to medium. • In smal bowl combine ingredients. • Place ribs on grill for 20-25 minutes. • Basting occasionally with sauce mixture and turning often.

Once it is sown, we nurture it daily until it takes root in our subconscious. Discovering your Sankalpa will help you live out your life’s purpose. Like cultivating a plant, it takes time and patience to grow and blossom. The word itself means determination, conviction and resolve. But it is not about achieving a certain thing in a time frame. Sankalpa is broader and more encompassing than that. It is a steady, internal energetic shift that changes you. Be clear about what you desire and keep an open mind about outcomes sustain effort and faith.[?? does this make sense?] However, once you make a decision, the whole world conspires to make it happen. Your intention will empower your life, manifesting itself in abundant ways. Make it heartful and wise, beautiful and kind. Sankalpa is a simple but specific intention, usually a spiritual resolve. It is a blank canvas from which everything starts. Kalpa means imagination of the mind, idea or creation. Life is an art form and you are the creator. Bring awareness and attention to your art. Everything depends on your motivation and intention. What you tell yourself reflects where you are coming from. It begins as a desire. You are that deep, driving desire. As is your desire, so is your intention. As is your intention, so is your will. Then your thought manifests as the word. The word manifests as the deed. The deed develops into habit. Then the habit hardens into character. So it becomes your destiny. Plant the seed in your imagination. Visualize the seed as the earth presses up against it, caressing it. It draws the earth into itself. The soil offering its minerals to the seed. Seed and soil flowing into one another. Moisture pierces the seed’s green skin, much to its joy. The sun bakes through the soil and heats the love inside our seed. Who is in charge here? Is the outside world invading the world inside the seed? It’s all coming together—upside down and inside out. The seed can be looked at in and of itself or it can be seen as a point in a process. Where does one thing end and another begin? You are down in the soil where thoughts grow, arise, and die away. The foundation and the source are giving rise to ten thousand things, the whole manifest universe. Learn to have faith in and outside of yourself. There is a perfect home for everything inside of you. And inside of you, there is a perfect home for everything outside of you. Just let it go, and let it in. In and out, like the breath. Things flow into each other and back to themselves, gaining themselves through the other and the other through themselves. It’s natural. Everything falls into its proper place after spending some time apart. We just fit together and it’s perfect. Your fulfillment gradually manifests as seeds that fit perfectly with your surroundings.

As our environment cultivates us, we cultivate our environment. Broker the exchange between the two to manifest the interpenetration of inside and outside of self and other. All you have to do is dissolve in activity, in relationship with your surroundings. So the inner world can flow out and the outer world can arrive within. There is a natural balance, a dance between embracing and releasing. You turn your surroundings into yourself. The life in flowers and the life in you are the same. You are two halves of the same experience. The flowers that take root within you and blossom are not fundamentally separate. They are within each other and occupy one cosmos together. We are never more than a breath away from the home we share with the entire universe. Begin by letting go of expectations. You cannot receive until you create some space in your life to make room for the new. Create freedom in the home, body, and mind. Have a clear view of what spirit is calling for and faith in the abundance of the universe. Inside this sacred space you can develop faith that will be provided for. Causes Of Heel Pain

A common cause of heel pain is plantar fasciitis, a strain of the ligament that supports your arch. Plantar fasciitis often develops due to overuse, such as lots of running or wearing shoes that are unsupportive or too tight. Flipflops, high heels, and worn-out tennis shoes are often to blame. A heel spur, which is a growth of bone on the front part of the heel bone, is another common cause of heel pain. A spur forms when the plantar tendon pulls away from its attachment to the bone and a calcified spur forms. Causes include wearing worn-out or poorly fitting shoes, carrying excess weight, running on hard surfaces, or gait abnormalities. Do Artificial Sweeteners Help with Weight Loss? Written By Barbara Greenling, DNP, MSN, APN-C

Do you drink diet soda, versus regular, thinking you’re doing your body a favor? Are you under the impression that artificial sweeteners are a healthier alternative, especially for weight loss? You are not alone. Recent statistics show that close to a third of Americans say they use artificial sweeteners daily. Typical examples include aspartame (Equal, NutraSweet), sucralose (Splenda), and stevia (Truvia, Pure Via). They’re all available in packets and are also added to soda, yogurt, and other foods.

But do these sugar substitutes actually help you lose weight? New research suggests they do not. In fact, these zero-calorie additives may have the opposite effect — and possibly even increase the risk of high blood pressure, diabetes, and heart disease. Researchers reviewed 37 studies that included a total of more than 400,000 people. But only seven were “gold standard” randomized controlled trials, which compared people who used artificial sweeteners with a control (placebo) for six to 24 months. Those studies found no clear proof that the fake sugars helped overweight people shed pounds. Perhaps a different approach to dropping the pounds is in order. Instead of focusing on replacing sweetness in your food and drinks, maybe re-training your palette to less – sweet alternatives would be best in the long run. For example, unsweetened iced tea is far more thirst-quenching and refreshing than the sweetened version. You may actually find you prefer it if given the chance! Start to experiment by leaving your comfort zone a bit and trying new ways to limit and enjoy your sweets. This is especially important if you are overweight so as to prevent bigger problems down the line. If, however, you’ve already been diagnosed with type 2 diabetes and you’re overweight, you should get started as soon as possible on a weight loss plan. It

is important to work with a medical professional or registered dietitian to help you figure out a plan that will work for you—a healthy meal plan, physical activity, and realistic goals will help you reach a healthy weight.

They may need to use medications or insulin to keep their blood glucose level in the normal range, but they should also still eat healthy foods and increase physical activity. Everyone should strive for a healthy weight. Losing weight can help lower your body’s insulin resistance, but if it doesn’t help you achieve better blood glucose control, that doesn’t mean you’ve failed—or that you should give up. Losing weight and then maintaining a healthy weight are healthy choices for life—whether you have type 2 diabetes or not.

State Mandated Vaccines Several years ago, the State of New Jersey issues a regulation that all eleven-year-old has to be protected against two bacteria, pertussis and Neisseria Meningitidis. Pertussis causes whopping cough. Neisseria Meningitidis causes lethal meningitis. These are diseases you want protection against. The Adacel vaccine protects against pertussis, diphtheria and tetanus. The Menactra vaccine protects against Neisseria Meningitidis. • Neisseria Meningitidis is a relatively rare bacteria, but, if someone does contact the infection, she/he is very sick. Despite the best medical care possible, the fatality rate is ten percent. And even if one survives the infection, he/she has a 30 percent chance of being crippled for life. We’re talking loss of an eye, an arm, or hearing-that sort of crippled. • Pertussis is a much more common infection than Neisseria. And to add to the problem, human beings are the only known host for pertussis. The only way to protect yourself is to be vaccinated. I recommend all adults get the Adacel vaccine. • Whooping cough is an obnoxious beastie. If you contract it, you do not acquire permanent immunity. You have about ten years of protection, and then you can get it again. Four of our families have come down with pertussis in the past few years. The members of each family were coughing for two months. The Chinese call pertussis the cough of one hundred days. People break ribs this way. And it is imperative that new parents get the Adacel vaccine. The incidence of pertussis has increased from 3,000 cases annually in the 1970s to 25,000 cases in the 2010s. And the fatality rate has almost tripled. Those succumbing to pertussis are babies under six months of age. The babies are getting pertussis from their parents and caregivers. Other Important Vaccines • Gardasil. Another vaccine that is important for adolescents to get is Gardasil. This vaccine protects against cancers caused by the Human Papilomavirus. Originally, the vaccine was created to protect women from cervical cancer. Further research has demonstrated that the Human Papilomavirus causes other cancers in both men and women. The virus causes penile cancer in men, and mouth, tongue, and anal cancer in men and women. There are over 33,000 new cases of Human Pipilomarivus cancers every year. • Flu Shot. Influenza is an annual problem. As of today, we do not have any information the severity of this year’s flu. Last year’s flu season was mild. The good news is that the pediatric fatality rate from the flu has been dropping. Evryone six months of age and over should get a flu shot, as long as the individual is not allergic to the vaccine. It is particularly important for pregnant moms to get the shot. This will protect the baby because the mom’s antibiotics will cross the placenta and protect the baby. After birth, babies under six months of age cannot get a flu shot. And no, the injectable influenza vaccine cannot give you the flu. I urge all parents to give their children the protection those vaccines provide, and the long life the vaccines help ensure.

Volunteers in Medicine of South Jersey Volunteers in Medicine of South Jersey (VIM) operates free clinics in Atlantic and Cape May Counties to provide free medical care to lowincome, working residents of South Jersey who do not have health insurance or the means to pay for health care. VIM’s team of more than 100 volunteer physicians, nurses, social workers, dieticians, administrators, and students focuses on primary health care, disease prevention, health promotion, and patient education. Who is eligible for VIM’s services? Anyone who does not have usable health insurance (private insurance with a deductible >$2,500, Medicaid or Medicare) and earns no more than 300 percent of the Federal Poverty Guidelines (approx. $37,500 per year for single; $50,700 per year for couple). How can you support VIM? Providing the gift of good health is one of the most profound ways you can help those in need in our South Jersey communities. Jacqueline Meiluta, Executive Director of Volunteers in Medicine, was born and raised in Burlington County. She and her husband, Alec Vint, have considered Sea Isle City home for the last 20 years. After a very successful career in the for-profit world, Jackie chose to bring her skills to the non-profit world. She found her true passion at Volunteers in Medicine where she leads a team of 100 volunteers and dedicates her time to ensuring care for our neighbors in need. Under her leadership, VIM expanded into Atlantic County. Jackie is a fierce advocate for access to healthcare for all and is often invited to speak on this topic. Another passion is workforce development. In 2009, she was instrumental in the development of an open-shop apprenticeship program for metal building construction. Reliance Medical Group Barbara was born in Trenton, eventually settling in Atlantic County in 2005 while working on her medical degrees. Many years of diverse experience working in the medical field as she continued her education finally culminated in a Doctor of Nursing Practice degree in 2013. She joined Reliance Medical Group in 2014 and now has her own practice in Egg Harbor Township, an accomplishment of which she takes immense pride. She is affiliated with many hospitals including Atlanticare Regional Medical Center - City Div, and Shore Medical Center. Barbara takes special interest in her community, with a practice focused on family, veterans and the trans-gender population. She personally has volunteered in the Wounded Warrior project since 2009, and donates her time and resources to the American Cancer Society and the American Foundation for Suicide Prevention. Karen grew up in the Philadelphia suburbs and moved to South Jersey when she married her husband John, of 30 years! Karen and John, and their children Christopher and Brooke, then moved to Florida where they spent the last 24 years until moving back to South Jersey in May 2018. Karen’s heart had never left Philadelphia, where her parents, family and friends are. Karen and John bought a fixer upper on the Mullica River in Green Bank, New Jersey, where John is utilizing his lifelong building skills and are doing a complete renovation. Lisa Ledden Nurse Practitioner, Certified Dermatology Lisa Ledden has been a nurse practitioner specializing in dermatology since 1996. Before this, she had a broad background in civilian and military medicine. After graduating from the University of Massachusetts in 1985, she was awarded a military commission and accepted into a competitive internship with the U.S. Air Force. As an officer, she practiced trauma and emergency/critical care nursing and was part of a field medicine deployment team. She was awarded the Air Force Distinguished Service Medal in 1991. After seven years of service, Lisa left the military as a captain to pursue a civilian career. She accepted a position as nurse manager of the Heart Group of Oklahoma (the largest cardiac practice in central Oklahoma) and subsequently managed emergency services at Milford Memorial Hospital in Delaware while pursuing a master’s degree in nursing at Wilmington College in Georgetown, DE. At that time, she developed a special interest in dermatology. While continuing to practice in dermatology, she was an adjunct professor in the graduate program at Wilmington College. In 1998, she relocated to Michigan to practice dermatology and expanded her knowledge of cosmetic and surgical dermatology. Dr. Peggy Avagliano Atlantic Medical Imaging Dr. Peggy Avagliano came to Atlantic Medical Imaging (AMI) after completing her fellowship training in Women’s Imaging at the University of California San Francisco. Dr. Peggy is President of the AMI Foundation, a charitable organization that is dedicated to enhancing the level of healthcare for community residents by collaborating with local community groups to support programs that address critical health needs. Burnout is Harmful to Your Health

B

urnout is a state of emotional, mental, and physical exhaustion that results from chronic, unrelenting pressure, work, overwhelming demands or stress. People who experience burnout have often dedicated themselves completely to their work and tasks at hand. In fact, they likely push themselves, daily. However, at some point they begin to question that it is no longer possible because the demands of the ongoing work are too large, or greater than can be accomplished within reasonable resources. Burnout oftentimes drives people – through dedicated, tireless, exhaustive measures – to begin to question the demands. It can result in a complete loss of enthusiasm for everything and detachment from people as well as feelings of being ineffective. Signs and symptoms develop gradually and often can be subtle at first. As well, they can relapse and remit, or even smolder; but with time, they can get worse and worse if they are not managed or addressed properly.

What are causes?

Burnout can take place at work, home, school, or even related activities. Here are some common factors identified by experts in the field: • Dedication to work (employment, home, school, community affiliation) to complete their tasks and goals with excellence – or “all they have to give it.” Experiencing ongoing pressure, stress, demands with little to no relief. Add to this, it may be accompanied by a lack of appreciation, recognition, fairness, control, sense of community, or ethical behaviors by coworkers, associates, colleagues, administration or family members. And, too, working, studying or giving excessive hours to the cause, or having excessive demands. • Personal life and lifestyle: Working in overdrive, or not taking or having “me time” or time for family and friends. Alternatively, having excessive obligations to family and friends (e.g., being a caretaker); lacking a support system; being sleep-deprived or having poor health habits. • Personality: Very much aware of what they have to do along with great dedication and commitment to tasks. Get a lot done and can take care of a lot of work. May even press the bar a little higher for themselves as they work tirelessly to accomplish all the work.

What are symptoms?

• Early on, those with burnout may feel less energetic, but over time, they experience constant weakness or fatigue. Fatigue is excessive tiredness and your body’s way of telling you to slow down. • Fatigue, combined with the stress, pressure or demands that trigger burnout, makes it less likely that you will engage in routine physical activity, consume a nutritious diet, and get good, quality sleep. Together, this can enhance burnout. • Forgetfulness, difficulty concentrating, and a lack of focus or difficulty coping. • Nervousness, worry, and anxiety. Over time, anxious thoughts can become debilitating, preventing you from performing optimally – and stealing your joy. Some people report having panic attacks – an abrupt onset of fear or discomfort. • Detachment from friends, family, and job, which can eventually lead to isolation.

• Sleep disturbances. • Physical manifestations such as headaches, fainting, chest pain, palpitations, shortness of breath, or nausea. And, too, increased susceptibility to germs due to a weakened immune system that results from stress, sleep deprivation, decreased physical activity, and poor diet. • Resorting to unhealthy behaviors such as smoking, alcohol, drugs.

What are some tips?

• Manage stress, reboot, recharge, rejuvenate. Take time and figure out ways to make changes, and where possible, enlist help. Utilize vacation days or sick leave to allow time to rest. If self-employed, take the initiative to work fewer hours or consider taking on fewer projects. Parents, consider getting a sitter, an exchange system with another mom, part-time daycare, or family members to get a break. • Plan R&R time weekly. • Say goodbye to unnecessary obligations or deadlines – and be cautious about adding more to your “to-do” list. • Reassess priorities with “me” and “family” time. • Set healthy boundaries with your job, career, school or special project, and make changes when needed. Talk with a trusted source about the realities and resources to get help (or extensions) for certain assignments or projects. • Connecting with loved ones for support is important. They may pick up on signs and symptoms of your burnout before you do. Listen to them. • Walk it off. Consider starting with a 15-minute walk or jog to relieve stress and release endorphins. • Get a good night’s sleep. Set a bedtime and stick with it. It’s easy to stay up late so we can catch up with the million trivial things we need to do. However, being sleep-deprived can be a cause of burnout as well as slowing efficiency and perpetuating the cycle of being burnt out. • Unplug and find another outlet: listening to music, gardening, photography, yoga, prayer, meditation, cooking, reading for pleasure, spending time with family or friends, or exercising. • Be specific, write it down, then tell others about it. Saying that we will work less, exercise every day, and spend more time with the family and friends are great goals, but make them a reality. Being specific helps make it more likely to adhere to our goals and less likely to cut corners. • Take a technology break – while a useful tool, it can be also be a harmful disturbance.

Mary Philbrook

I recently had the pleasure of attending a seminar hosted by the Atlantic County Bar Association wherein the topic was women in the legal profession. There was a panel of three New Jersey attorneys and two female judges. Everyone on the panel was dynamic and quite accomplished in their areas of expertise. I have Njwomenshistory.org. close friends (two were on the panel) who are amazing women attorneys who have been “mistaken” for secretaries and court stenographers. All of those positions noted are crucial for day-to-day operations in a law office. However, when you have attended literally seven years of school, three of which are in law school, and successfully passed the bar exam, a woman deserves the respect of reaching the status of being called a lawyer, an attorney or a counselor. Part of the presentation included data on the first females in the legal profession in the State of New Jersey. As I began to research the notable and remarkable women, the first female attorney had a background that deserved the spotlight.

Mary Philbrook was born in 1892 in Washington, DC. Mary’s family relocated to New Jersey when Mary was in elementary school. Mary attended schools in Jersey City, New Jersey and ended up dropping out of school to become a stenographer. For those readers who are not familiar, a stenographer is an individual who types on a machine and manually records court hearings, depositions or other types of oral testimony. It is interesting to note that back in the 1880s a person did not need to have a law degree to sit for the bar exam. Mary applied for admission to the New Jersey bar in 1894. Mary was initially rejected by the Bar Association, who cited, “[a] woman is not, by virtue of her citizenship, vested by the Constitution…with any absolute right…to practice as an attorney.” The news of the New Jersey Court’s ruling soon spread and female suffragists from New Jersey strongly advocated for women’s rights to become practicing attorneys in the State of New Jersey. By 1985 the legislature passed a law permitting female attorneys to practice law in New Jersey. Mary Philbrook became the first female sworn into the bar. Mary began her practice in a law firm in Jersey City before opening her own practice. Mary was a driving force in lobbying for the first women’s college in the State of New Jersey, which is now known as Douglass College and is affiliated with

Rutgers University. It is also noted that Mary had a lead role in the formation of Legal Aid services in the State of New Jersey. Mary Philbrook also earned the title as the first female attorney appointed to practice law before the State Supreme Court in 1906, a mere 12 years after her admittance to the state bar. Mary Philbrook took Disasterpodcast.com. inspiration from Alice Paul after the passage of the 19th Amendment and became an activist in support of the passage of the Equal Rights Amendment. Mary was a true equal rights activist. Mary is also known for her crucial activism in the changing of the New Jersey Constitution. At the age of 75 Mary led a group of lobbyists at the Constitutional Convention to change the word “he” in the New Jersey Constitution to “person.” It would not be until 1979 that the New Jersey Supreme Court would rule “that sex discrimination was constitutionally prohibited in the State of New Jersey.” Interestingly enough, Mary may not have been aware that she was not just advocating equality for women but all members of society in her quest to secure equal rights. There are a significant number of intelligent, hard-working women attorneys who make “lawyering” look easy. However, we must not take for granted that in addition to the day-to-day rigor of being a female attorney, there still remains the “second shift” in balancing families, children’s sporting events, school activities, homework, keeping up with social events and, if you get lucky, some self-care. It was quoted best by Eleanor Roosevelt: “No one can make you feel inferior without your consent.” I dedicate this feature to my dearest female attorney friends, who are extremely powerful and inspiring leaders in the legal profession!

Rita King, LCSW is the Associate Director of Justice Involved Services at Jewish Family Service. In this role, Rita currently supervises a Re-Entry program for incarcerated individuals as well as a community based diversion program for consumers experiencing mental illness and involvement with the criminal justice system. Rita is a “De-escalation for Individuals with Special Needs” training facilitator and has trained approximately 1,200 law enforcement professionals to date. Rita is also a CIT trained mental health professional. Rita has a strong passion for women’s history and in her free time she continues to raise awareness of the importance of women’s history through lectures and community speaking engagements.

Pets not only provide their owners with companionship, but also with several health benefits. They may help lower blood pressure, heart rate and cholesterol levels and ease tension, anxiety and depression.
