Looking back at the past year, 2012 was no exception. In 2012, SJRMC received dozens of awards and certifications for quality care, compassionate service, and innovative offerings. In January, Thomson Reuters identified SJRMC as one of the Top 15 Health Systems in the country and among the Top 5 Small Health Systems in the nation. In addition, our Plymouth Campus received the Top 100 Hospital Award. These awards measure a variety of indicators, including mortality, complications, readmissions, and performance. Both of these awards were unsolicited and without application. In addition, certifications received in fiscal year 2012 included Joint Commission Stroke certification and cancer accreditation by the American College of Surgeons Commission on Cancer. Our Mishawaka Campus successfully completed the triennial Joint Commission Certification. The outstanding efforts of our information technology teams were recognized with the receipt of industry-sponsored awards, including Most Wired 2012, HIMSS Analytics Stage 6 Certification and home to one of the

Top 25 Clinical Informaticists in the nation, Devin Zimmerman, MD. The entire health care industry is undergoing rapid change, driven in part by medical discovery, technological advances, and by the requirements of the Affordable Care Act. Does

Stress Really Cause

Heart Disease?

It’s been a hot debate for years. But today, most researchers believe that stress and other psychological factors may raise the risk of developing heart disease. “Scientists don’t know exactly how stress is linked to heart problems,” says Shamiram Badal, MD. “But it can contribute to high blood pressure, increased blood clotting, heart rate problems, and plaque buildup in the arteries.” Some situations and personality traits make stress—and the risk of developing heart disease—more likely.

A Link to Anger Over the years, scientists have tried to identify whether some people are more affected by stress than others. One group that stands out: people with a type D personality. Quick to anger and often anxious, type D’s tend to have negative emotions that they suppress. Researchers reason that these emotions may be a response to emotional stress.

Embrace Good Stress Stress isn’t all bad. In fact, in smaller doses—and when managed—stress can be good for you. Stress can provide the “oomph” we need to get things done. To use “good” stress to your advantage: • Use visualization. You can prepare for a presentation or other event by visualizing it in your head, almost like a dress rehearsal. • Keep a positive attitude. This helps you to see stress as a healthy challenge, not a major obstacle. • During busy times, make sure you sleep well, eat right, make time for exercise, and take breaks. This can help you stay productive, happy, and healthy.

Pressure Ulcers:

A Sore Subject No One Talks About

Pressure ulcers, wounds that develop when there is prolonged pressure between the skin and bone, are no longer referred to as bedsores. Often people don’t even realize they can get a pressure ulcer until it happens.”

Signs & Symptoms In addition to a lack of mobility confining someone to a bed or wheelchair, other risk factors for pressure ulcers include age, poor nutrition, diabetes, obesity, incontinence, and conditions that prevent blood flow and cause lack of sensation. Pressure ulcers occur when a person remains in one position for too long without shifting his or her weight, which decreases the blood supply to the area, causing skin and tissue to break down. In lightskinned individuals, the first visible sign is a red area on the skin that doesn’t fade. Dark-skinned people may first notice persistent areas of red, blue, or purple with a different skin temperature or texture, which are often accompanied by pain or itching. If not properly treated, these areas may evolve into wounds that can extend down to bone.

What You Can Do “Most people have no idea how to prevent pressure ulcers,” says Dr. Royer. “For example, the first thing many people do after discovering they are going to be confined to a chair for a prolonged period of time is go out and buy a doughnut-type pillow, which is absolutely wrong because it can harm tissue.”

To reduce the risk of developing pressure ulcers: • Use commercially available pressure-reducing cushions and mattresses (but avoid those that resemble egg-crate foam). • Change position every 15 minutes when sitting and at least every two hours in bed. • Place a pillow under your calves to prevent your heels from touching the bed and use pillows and foam wedges to keep your ankles and knees apart. • Use mild soaps when bathing and avoiding heavy scrubbing. • Apply skin moisturizers to prevent dryness. • If confined to bed, do not raise the head of the bed more than 30 degrees and use an overhead bar to assist in movement. • When treating incontinence, cleanse skin, use a topical moisture barrier, and select absorbent briefs or underpads that provide a quick-drying surface next to the skin. Seek medical treatment for nonhealing pressure ulcers that show warning signs of infection. These include increased pain at the wound site; redness or swelling spreading away from the wound; a foul wound odor; change in color or amount of drainage from the wound; or fever, chills, nausea, or vomiting. The primary treatment for chronic pressure ulcers is to relieve or reduce the pressure. Other care may involve removing the nonliving tissue from around the wound with surgical instruments or with newly developed dressings and chemicals.


Fighting Fibroids

Without Surgery

It used to be when a woman was diagnosed with uterine fibroids, the only remedy was a hysterectomy. “Today women have choices,” says Tracy Byrne, MD. “There are several treatment options that allow women with fibroids to avoid major surgery, preserve their uterus, and ease chronic symptoms like heavy menstrual bleeding and debilitating pain.”

Fibroids are benign tumors in the uterus. They can grow to the size of a cantaloupe and cause excessive bleeding, severe pelvic pain, and an enlarged abdomen. Symptomatic fibroids are estimated to occur in 20 to 40 percent of women older than age 35. The rate is even higher— nearly 50 percent—for African-Americans.

Sustained Relief

How UFE Works 1. A flexible tube is inserted into the femoral artery through a tiny incision (less than a quarter inch) in the groin. 2. Using real-time imaging, the catheter is guided to one of the two uterine arteries. 3. Tiny beads of plastic or gelatin sponge are released, clogging the smaller arteries that feed the fibroids. Without blood to feed them, the fibroids will shrink. 4. The procedure is repeated using the same incision.

Every individual is different, but several large clinical studies found that UFE eased symptoms in 85 to 95 percent of women. The procedure is effective for multiple fibroids and large fibroids. Minimally invasive, UFE requires only a short hospital stay, usually one day, and most women return to their normal activities within a week. Relief of symptoms occurs gradually over two or three months. When a patient is given surgery as her only option, Dr. Byrne recommends that she ask about UFE and consult with one of SJRMC’s experts in the technique, which is covered by insurance. “UFE is a wonderful treatment option for women suffering from fibroids,” says Dr. Byrne. “A large majority can find relief with this innovative procedure.”

Embolization: A Major Advance in Women’s Health “Uterine fibroid embolization (UFE) is an exciting new treatment in which tiny particles are injected into the blood vessels feeding the fibroids to block the blood flow,” explains Dr. Byrne. “Once the fibroids are deprived of blood, they usually shrink within a few menstrual cycles.” UFE is performed by interventional radiologists at SJRMC. This type of doctor uses imaging techniques to guide procedures that would be impossible with conventional surgery.

Saint Joseph Welcomes the Following Pediatric Subspecialists Pediatric Endocrinology & Diabetes Andrew Riggs, MD, Medical Director Dr. Riggs is a graduate of Indiana University School of Medicine and completed his pediatric residency at the University of Minnesota and fellowship in pediatric endocrinology at Washington University in St. Louis and St. Louis Children’s Hospital. He has a specific interest in diabetes and has authored numerous articles on diabetes in children. Dr. Riggs is also very active in the diabetes community in central Indiana and is the medical director of the Diabetes Youth Foundation of Indiana. Samara Rahhal, MD Dr. Rahhal completed her pediatric residency at the Cleveland Clinic. She completed her fellowship in pediatric endocrinology

at Riley Hospital for Children, where she was an award recipient at the fourth annual Scholar’s Day. Dr. Rahhal’s experience at Riley Hospital encompassed all areas of pediatric endocrinology.

Pediatric Cardiology Sanjay Parikh, MD, Medical Director Dr. Parikh received an MD from the Medical College, Baroda/ Maharaja Sayajirao University Baroda, India. Dr. Parikh is certified by the American Board of Pediatrics and its subboard of pediatric cardiology. He is a fellow of the American Academy of Pediatrics and the American College of Cardiology. He completed a pediatric cardiology fellowship at Indiana University Hospitals/Riley Children’s Hospital, Indianapolis and a senior fellowship at Children’s Hospital, Boston. He served for five years as a faculty member at

Riley Hospital, where he was also associate director for the cardiac catheterization labs. He has extensive experience in cardiac catheterization and performed the first stent placements in pulmonary arteries in children and coil embolization for ductus ateriosus in Indiana. Sports-Related Concussions:

Increased Awareness and Knowledge Could Save Lives

Stephen Simons, MD Saint Joseph Sports Medicine Institute On staff at SJRMC

Amid heightened national interest in the long-term effects of sports-related concussions, Stephen Simons, MD, a pioneer in the sports medicine field, is seeing a spike in the number of athletes referred for return-to-play evaluation at the Saint Joseph Sports Medicine Institute. A new Indiana law requires an athlete who suffers a concussion to undergo an evaluation by a health care professional trained in the evaluation and management of head injury before returning to play. That brought 40 patients to Dr. Simons in the first six weeks of football season alone, compared with a total of 62 in the 12 months before the law took effect in July. Concussions occur more frequently in football, wrestling, and lacrosse and can happen in other sports like baseball and softball.

“Fortunately, parents are a lot more educated and there’s more concern about when we consider it safe to return an athlete to sport,” says Dr. Simons, who credits NFL attention for the awareness. “There was a time when the knowledge about concussion was much less and the dangers of it were not as well-known. Now we’re seeing the opposite. Parents aren’t at all rushing their kids back to play.”

Assessment Methods Have Improved Indiana is a national leader in adopting such a rule, patterned after a similar law passed in Washington three years ago. In recent years, sophisticated computer-based neuropsychological testing has also allowed physicians to inexpensively establish ordinary baselines for athletes in such areas as reaction time, short-term memory, and ability to concentrate. After physical concussion symptoms have subsided, the tests can reveal whether those functions have returned to normal. “It’s much more sensitive than me sitting and asking questions for their reaction time, essentially for their brain recovery,” Dr. Simons says, adding that Notre Dame has used the equipment for a decade, and local high schools started three to four years ago.

Spot the Signs of a Concussion A concussion can result from a fall or sudden blow to the head. Common warning signs include poor concentration, difficulty following instructions, and memory loss. A sufferer may or may not lose consciousness. Expedites Emergency Care During emergency situations, residents of the greater Michiana region are now able to send prearrival health information to local emergency rooms, thanks to ICE Michiana—a free smartphone app with a unique “PUSH” functionality. “The idea started with an internal discussion focused on the need for both emergency rooms and first responders to have critical patient information as early on in the care process as possible,” says Gregor Staniszewski, director of Saint Joseph Regional Medical Center Emergency Services. “We came together as a group to discuss how we could leverage technology to this end.”

How It Works

The ICE Michiana program consists of two parts—an app for both iPhone and Android users and a tube for households without these smartphones.

For Smartphone Users

The ICE Michiana app for the iPhone and Android will provide a secure, digital format for storing patient identification, emergency contacts, and medical information. During an emergency, first responders can quickly review pertinent medical history on-scene and send time-sensitive information directly to participating emergency rooms (ERs). The push function also allows smartphone users to send their own information directly to a participating ER prior to their arrival. Note: During an emergency, call 911. “Specifically, this push technology sets the ICE Michiana app apart from other ICE apps available in the marketplace,” says Jason Jaronik, MD, SJRMC EMS medical director and Emergency Department physician. “Whether it’s a first responder or the incoming patient sending his or her information, both circumstances allow hospital teams to prepare as needed.”

Area first responders are trained to look for these identifiers and use the information inside while providing care. They will also bring the tube to the ER during transport.

A Robot-Assisted Helping Hand Though it is often called a “robot,” the da Vinci System cannot act on its own—surgery is performed 100 percent by the physician. The da Vinci System consists of a computer console and robotic arms with flexible wrists that hold a tiny


System: camera and microinstruments. During surgery, the surgeon sits at the computer console viewing a highly magnified threedimensional image of the patient’s anatomy while using hand-and-finger devices to precisely direct the robotic arms. “Wristed instruments and fingertip control provide a degree of motion and dexterity that is vital in complex suturing and dissection,” says Dr. Method. “We can do things that we just aren’t able to do with standard instruments.”

Pioneering Virtually Scarless Surgery The da Vinci System enables surgeons at SJRMC to offer single-incision surgery for gallbladder procedures. “Where we traditionally made three or four incisions to remove a gallbladder, now we make one, which is then hidden by the belly button. Patients have a less painful recovery and pretty much no scar,” explains Edward

And in terms of patient recovery, it’s like night and day.” While clinical studies support the effectiveness of the da Vinci Surgical System when used in minimally invasive surgery for specific indications, individual results may vary. A New Level of Diagnostic Accuracy Pankaj A. Patel, MD

The American Cancer Society estimates that more than 277,000 people will be diagnosed with cancers of the digestive system this year, and nearly 139,000 will die. Up to 60 percent of those deaths could be prevented through early screening. “Once cancer is diagnosed, the key to treatment is exact staging of the primary tumor’s size and whether the disease has spread to other areas of the body,” explains Pankaj A. Patel, MD. In the past, hard-to-reach tumors could not be clearly staged without major surgery. But with the introduction of a new technology called endoscopic ultrasound (EUS), physicians at SJRMC are now able to diagnose and stage such tumors without surgery. “An endoscopic ultrasound allows precise staging and can even detect tumors when other tests are inconclusive or conflicting,” says Dr. Patel. “It can also scan nearby structures, giving us a more comprehensive view. This is critical for effective treatment planning.”

An Advanced Approach EUS combines endoscopy and ultrasound to create more detailed images of the digestive tract and the surrounding tissue and organs. During the procedure, a thin, flexible tube called an endoscope that has a built-in ultrasound probe is guided to the area of concern. The physician uses sound waves to create images of the digestive tract. Because the EUS can get so close to the organs, the images are clearer, more accurate, and more detailed than those provided by other techniques. EUS allows doctors at SJRMC to see organs and structures not typically visible during GI endoscopy. This detailed imagery is used to diagnose and stage abnormalities in the esophagus, liver, pancreas, stomach, lymph nodes, and bile ducts. “Endoscopic ultrasound can find tumors less than one-quarter inch in size, which may not be visible on a CT scan,” says Dr. Patel.

Pinpointing the Source of the Problem

In some patients, EUS can be used for a needle biopsy of a cyst or lesion to help determine whether it’s benign or malignant. This technique is known as a fine-needle aspiration (FNA). “EUS with FNA has revolutionized our ability to diagnose and stage cancers of the digestive tract and assess the pancreas,” says Dr. Patel. “It’s a much less invasive way of imaging and doing biopsies that avoids damage to other organs, while giving us the information we need to determine the most effective line of therapy.”

SJRMC is one of the few medical centers in Michiana that offers EUS, and its GI doctors are some of the most experienced in performing it. The procedure is available at the Mishawaka Campus. Ernst passed away on January 24, 2012, at age 98 and, as part of her estate, left a gift to the Auxiliary & Volunteer Services area of the hospital. Membership Chair Rosie Braunsdorf says the records indicate that Ernst joined the Auxiliary in 1969 and worked until her health issues became too much for her in 2004. Ernst delivered patient mail, took the book cart to the floors, and when her husband retired, she recruited him and the two of them worked as volunteers in the surgery waiting room. Ernst’s generous gift will go a long way in helping charitable programs within SJRMC. We are thankful to her for the time she spent enriching the lives of our patients and for the legacy of her gift, which will help many more patients in the future.

Sharon Scuzzo Genetics Fund Helps Women in Need In 2006, The Foundation of SJRMC established the Sharon Scuzzo Genetics Fund in honor of Sharon Scuzzo, a community leader who passed away from breast cancer. The fund is specifically designed to help women who are at high risk of developing breast or ovarian cancer. The mutation of human genes BRCA1 and BRCA2 has been linked to hereditary breast and ovarian cancer. By being tested early, women have the opportunity to know if a harmful BRCA1 or BRCA2 mutation is found. This gives them several options to help manage their cancer risk. Funds have been able to help women receive testing that they otherwise would have been unable to afford.

Inspired by Faith Faithful Lives Supports Pediatric Services The attendees for the 2012 Faithful Lives Celebration held in November were treated to an evening highlighting the dedicated staff of SJRMC’s pediatric programs, including our inpatient pediatric unit, our neonatal intensive care unit, our pediatric therapy department, and our pediatric specialty clinics. The event was a wonderful opportunity to recognize the hard work and incredible care they provide to help children in need. Monies raised will be shared among the departments to address their greatest needs.

Secret Sisters Society Luncheon Celebrates 11 Years The 2012 Secret Sisters Society Luncheon celebrated its 11th year at a new venue and by honoring one of its founding members, Joan Lennon. The event drew a crowd of more than 950 people and continued its tradition of serving lunch followed by a style show featuring models from young to older, all of whom are cancer survivors. Proceeds from the Secret Sisters Society Luncheon are used to provide mammograms to uninsured and underinsured women. The program is unique in that the women can have their mammogram done at a site convenient to them from any one of the local hospitals, including Saint Joseph, Memorial Hospital, or Elkhart Hospital.
