STEPHEN’S STORY

Medical Condition: Brain Tumor When twitching jaw muscles and a numb right thumb were followed by three severe seizures, Stephen Krahn met Dr. Mateo Ziu, who discovered the problem. Stephen had a brain tumor located in a tricky spot that controls speech and movement— heart-wrenching news for this master organist who relies on his agile fingers. “I was frightened to say the least but impressed by Dr. Ziu’s skills and confidence.”

To minimize Stephen’s risk of paralysis and speech loss, Dr. Ziu performed an awake craniotomy. The unique procedure allowed Dr. Ziu to react to Stephen’s speech and movements during surgery. “I listened to Brahm’s Symphony No. 1 to mentally prepare,” said Stephen. “There was no pain.”

The great majority of Stephen’s tumor was removed and the remaining portion should respond well to chemotherapy. Since the surgery Stephen hasn’t had a single seizure. “I recently played the piano and all the parts work! I just landed a new job as a church organist. I’ve got a lot left to accomplish on this earth.”

Teaching hospitals foster a team approach to care.

A neurosurgical oncologist, a neuropsychologist, and a radiation oncologist, among others, worked together in Stephen’s treatment.

FACTS: Over the next 10 years, Central Texas will need approximately 1,700 new doctors to care for a projected population of 2.4 million.

Medical Condition: Diabetes and liver disease Gloria Chapa knows Austin. She loves Austin. From her first breath at University Medical Center Brackenridge to 30 years of service at UT Austin, she’s embraced her community. Now, in a unique way, her community is embracing her.

Gloria is sick—the kind of sick that has taken away what’s dear.

Due to uncontrolled diabetes, memory loss, and liver disease, she is disabled. “I hate not being able to work. I learned that when you work honest, you walk with pride.”

Gloria’s doctor referred her to Seton Total Health Partners, an innovative program that is transforming how care is provided, while improving outcomes and lowering costs. Her Seton care team, including her health promoter Katherine Edmondson and dietitian Rosimeiry Jamalabad, regularly visit Gloria in her home.

It’s working. Gloria has changed her diet, lowered her blood pressure and maintained her blood sugar levels, which improves her memory. By staying on course, going back to work is a real possibility.

Gloria visited the ER 11 times in 2013 for complications from diabetes and liver disease. Since participating in Seton Total Health Partners, Gloria has visited the ER only once.

FACTS:

Programs like Seton Total Health Partners have grown from a collaboration between Seton and Travis County’s Central Health to create new, cost-effective care models that eliminate gaps and focus on chronic disease and prevention.

Services to Austin’s most vulnerable, that have long been separate and disconnected, will be integrated with the help of patient navigators or “health promoters.”
