Special Circumstances for Waiver of the PSU Health Insurance Plan Fees for health insurance will appear on your student bill each term and cannot be waived unless allowed by one of the following special circumstances: • • • •

You are on a J visa and have adequate health insurance purchased before you arrive in the U.S.. Your government or embassy pays for your insurance through an outside company. You have insurance coverage through your US employer during your practical training period. You have insurance through your spouse’s employer.

Applying for Waiver of the PSU Health Insurance Plan To obtain a waiver of the Supplemental Plan coverage and fee you must do the following: • • •

Ensure that you have the minimum coverage required by law (see below). Submit an online waiver request through the Wells Fargo Insurance Services Waiver Form. Check your student billing account for the removal of the Health Insurance Plan fee.

To waive the PSU Health Insurance Plan, your insurance coverage must meet all of the following requirements: • •

• • • • •

Does your plan provide primary care services within 25 miles of PSU? For example, if you get sick and need to see a doctor, will your plan pay for your medical bills if you see a doctor within 25 miles of campus? Does your plan have less than a $1,000 yearly deductible? A deductible is the amount of money the insurance company has you pay first, before the insurance begins to pay for your medical bills. Please note: if you are a J-1 student, your immigration status requires that your insurance plan’s deductible cannot exceed $500 per incident. Does your plan offer Inpatient and Outpatient Mental Health benefits? In other words, does your plan cover any counseling visits or treatment programs for alcoholism, depression, eating disorders, etc. Does your plan pay for at least 80% of Inpatient Hospitalization? For example, if you have to stay in the hospital for a few days, will your plan pay 80% or more of that hospital bill? Does your plan cover at least $100,000 medical maximum? Does your plan have prescription coverage? Does your plan have coverage for medical evacuation and repatriation?
