PRACTICAL NURSING (1100 HOURS, 43 WEEKS)

Why Now?

Licensed Practical Nurses (LPNs) are in demand more than ever. This program prepares you to provide essential medical care to patients of all ages in a hospital and clinical setting. After completing this program, you will be eligible to take the NCLEX-PN licensure exam, opening a world of opportunities in the healthcare industry.

WHAT YOU’LL LEARN

Students will learn the following in this 1100-hour training program:

• Nursing care for patients across all age groups: adult, pediatric, maternity, and geriatric

• Human anatomy and physiology

• Assessing and monitoring patients’ health status

• Fundamental nursing skills, such as medication administration, IV therapy, wound care, and infection control protocols

• Communication techniques for interacting with patients, family, and healthcare teams

• Hands-on experience through our unique clinical partnerships program at Southwest General Health Center

• Preparation to pass the national licensure exam to excel as an LPN

166-hour unpaid clinical will be completed during weekdays. May be required to complete (at student’s expense) immunization/ titers, drug screening, physical exam and/or background check prior to placement. Proof of personal health insurance may also be required.

Requirements for Completion: A “B” average (80%) or higher is required for lecture courses and a “S” (Satisfactory) is required for laboratory and clinical . Students must achieve a passing score on an exit examination. Three attemps to pass the examination are permitted. After three attempts, a structured review must occur prior to subsequent attempts. Required attendance rate of 90% is required for classroom lecture courses and 100% for clinical and laboratory experiences.

FINANCIAL AID

Students are responsible for submitting a Free Application for Federal Student Aid (FAFSA). The FAFSA is reviewed, processed by the U.S. Department of Education and sent to the schools of the student’s choice. Students should add the Polaris Career Center school code 016745 to their FAFSA. It is highly recommended that the student start the application process for all financial aid options well in advance of registering for a program.

FEDERAL PELL GRANT OVERVIEW

• The Federal Pell Grant is considered “gift-aid”, financial aid that does not require repayment.

• Federal Pell Grant eligibility will depend on a student’s financial need, the cost of attendance and the number of clock hours in the program the student is attending.

• The student’s SAI, reported on the Student Aid Report (SAR), is calculated from the prior-prior year (PPY) income information submitted on the FAFSA and is used to determine the amount of the Federal Pell Grant award.

• Federal Pell Grant funds are meant to assist students with tuition and will not cover tuition costs completely. Students may consider a Federal Direct Loan to supplement the cost of tuition (visit studentaid.gov for specific loan information). It is recommended that a student complete the student loan requirements at the same time the student completes the FAFSA.

FEDERAL DIRECT LOAN OVERVIEW

• There are two types of Federal Direct Loans: subsidized and unsubsidized.

• Subsidized loans are awarded based on need. Interest is paid by the federal government while the student is enrolled in school. Unsubsidized loans are not need-based. The student is responsible for all interest accrued on an unsubsidized loan. Federal Direct Loan awards are based on student eligibility and the number of clock hours in the student’s program.

• Repayment begins 6 months after student ceases to be enrolled in the program.

• Parents of Dependent Students may apply for a Parent PLUS Loan through the U.S. Department of Education for up to the student’s Cost of Attendance (COA) minus any need-based aid received by the student. The parent will need to visit studentaid.gov/plus-app/parent/landing to

complete an application and fill out the Parent PLUS Loan Master Promissory Note (MPN) online. We will get in touch with the parent shortly after the application and MPN have been completed. Please see chart at polaris. edu/financialaid for fees associated with the Parent PLUS Loan.

ADMISSION PROCEDURE AND REQUIREMENTS

Job & Career Training students must register in-person at Polaris Career Center Adult Education Department. Enrollment for Fall 2024 will open May 1, 2024 at 8 am by calling 440.891.7653 to make an appointment with Kathryn for in-person registration. Please note, walk-in registration may be available during standard office hours after our Open House, Wednesday, May 8 at 5 PM.

Appointments are recommended: call 440.891.7653.

Walk-in hours: Monday-Thursday, 8:30 AM-3:30 PM and Fridays, 8:30 AM – 3:00 PM

Evening appointments available by request. Please bring the following required documentation:

• High school diploma/official transcript from a State Department of Education approved high school, GED certificate or its equivalent. If the student’s diploma is from a foreign country, the student must have the diploma translated into English by an authorized, certifying official at an official translation service to determine if it is equivalent to a U.S. diploma. Evaluators charge for this service, and it can take several weeks. Please visit the National Association of Credential Evaluation Services at www.naces. org to view a list of evaluators.

• Valid United States of America government issued photo ID (i.e., driver’s license or state ID)

Police Academy

• Be a U.S. citizen

• Be 21 years of age or older within two (2) months of the completion of the program

• High school diploma, GED or equivalent

• Show proof of a valid driver’s license

• Complete a physical exam with satisfactory results

• Complete and pass the State of Ohio required physical fitness assessment

• Complete a satisfactory drug screening test

• Complete a background check with satisfactory results, including no conviction of felony, serious misdemeanor (m1) or domestic violence (even if expunged or sealed).

Practical Nursing

• An Adult Polaris Career Center application

• Pre-Entrance Examination with a basic level test score or higher as determined by the testing company/Program Administrator.

Tuition payment (amount is determined by the financial aid award)

Financial aid:

1. Student applies for financial aid

2. Student confirms the award amount(s) with the Financial Aid Coordinator, 440.891.7668

3. Financial Aid Coordinator applies anticipated award to student account

4. Student may register

5. Any balance due is paid in-full at time of registration or student may enter into a payment plan, with the initial payment due at time of registration.

PROGRAM PAYMENT PLAN

An interest-free monthly payment plan is available through the Adult Education Department for Job & Career Training Programs.

CANCELLATION OF PROGRAM POLICY

The Adult Education department must cancel programs that do not attain the required minimum enrollment. If a program is cancelled, all payment plans are null and void and full refunds are disbursed to the student within 45 business days.

REFUND POLICY

Refunds for programs cancelled by the school:

• If tuition and fees are collected in advance of the start date of a program and the school cancels the program, the school will refund 100% of the tuition and fees collected.

• If a student paid by check or cash, the refund will be disbursed within 45 business days of the program start date in the form of a Polaris Career Center check. If the student paid using a credit card, the refund will be credited within 45 business days of the program planned start date on the credit card initially charged.

Refunds for students who cancel on or before the first day of class:

• If tuition and fees are collected in advance of the start date of classes and the student does not begin classes or withdraws on the first day of classes, the school will apply a $100 cancellation fee.

• If a student paid by check or cash, the refund will be disbursed within 45 business days in the form of a Polaris Career Center check. If the student paid using a credit card, the refund will be credited within 45 business days on the credit card initially charged.

Refunds for withdrawal after class commences:

• Please refer to the Withdrawal Policy in the student handbook for the definitions of Official and Unofficial Withdrawals.

• Tuition refunds are prorated and based on the hours the program was held until withdrawal date. Books and supply fees are non-refundable. Materials and assessment fees are non-refundable if used prior to the withdrawal date. The school will apply a $100 cancellation fee.

• If a student paid by check or cash, the refund will be disbursed within 45 business days in the form of a Polaris Career Center check. If the student paid using a credit card, the refund will be credited within 45 business days on the credit card initially charged.

Any refund that totals $1 or less will remain on the student account as a credit balance.

VETERANS RECEIVING BENEFITS

POLICY FOR EVALUATION OF PRIOR CREDIT

Polaris Career Center will collect and evaluate academic and military transcripts, as well as any credentials held, from all veterans and eligible recipients of veterans benefits and determine if credit is applicable to the student’s program of study. Academic and/or military transcripts from previous institutions of training must be provided to Polaris Career Center within 30 days of the student enrolling for a VA-eligible program. Evaluations will take place by the Adult Education Director or designee. The student will be notified and informed via email within one week of receipt of all transcripts if any prior credit will be applied. If credit is granted, the program will be shortened and the cost will be prorated accordingly.

ATTENDANCE POLICY

Regular and punctual attendance is the greatest single factor in student success in a Job and Career Training Program. All students are expected to attend Polaris Career Center on all scheduled days and times of the program. Students are required to sign in and out daily indicating exact arrival, dismissal and lunch (8 hours or more) times that they are in attendance. Students are encouraged to be in attendance 100% of the time however, students can complete the program with 90% attendance. An absence should only be used for such instances as illness, emergency situations or personal or family obligations. Students should always notify their instructor of an absence in advance or as soon as possible. Students must contact their instructor to discuss any work that was missed. Some programs may have specific attendance requirements for successful completion.

Cosmetology Academy: Students must complete 1500 hours.

Manicurist: Students must complete 200 hours.

Medical Assisting: Student must complete 160 hours of practicum (Work-based activity) as a requirement of program completion per The Commission on Accreditation of Allied Health Education Programs.

Police Academy: Students registered in the Police Academy will adhere to the policies presented in the PEACE OFFICER BASIC TRAINING STUDENT

HANDBOOK administered by the state.

Practical Nursing: Students must maintain a 90% attendance rate for each class taken in the curriculum and complete 100% clinical and laboratory experience hours.

GRADING POLICY

A student’s academic progress will be measured according to the following scale:

Grades will reflect achievement in the following areas: written and program competency test scores, homework, class assignments, employability skills, class participation, attendance, lab performance, and externship evaluations.

An overall grade average of 70% (C) is required to complete a program.

Police Academy: Graded on a Pass/Fail basis under the auspices of Ohio Attorney General

Practical Nursing: Each course must be completed with a grade of 80% or better and laboratory and clinical experience completed satisfactorily before progressing to the next term.
