All sites are licensed by the Department of Health Services (DHS). Department of Economic Security (DES) funding available for those that qualify, call (602) 771-0014. STEP 2: Complete the Emergency Information form leaving no blanks and provide a copy of your child’s current immunization records. This information can be turned into the NHS office or your child’s AMPM program. Children may not attend without this information. STEP 3: Staff will process your registration, charge 1st weeks fees and email a receipt for these charges. PAYMENT: A $20 non-refundable registration fee is required for all participants. *See parent handbook for detailed information. NOTE: School offices and AMPM sites can NOT accept registrations. YOUTH ADVISORY BOARD Peoria Youth Advisory Board Make a difference in your community, make new friends, learn leadership skills, and get involved!

If you are currently in grades 7th-11th and a City of Peoria resident, you are eligible to join. The Youth Advisory Board consists of nineteen local youth who advise the City Council, through the City Manager, on recreation programs, events, ordinances and proposals pertaining to youth within our community. Representatives are appointed by City Council, from all areas of the City, from local, charter, and private schools. Fitness/Dance Room

We welcome fitness buffs, and dancers alike, to enjoy the Fitness Room at the Community Center. Dancers may schedule the use of the room in 45 minute blocks between the hours of 1 & 3 p.m. on Sat. Use of the Fitness Room is for practice that does not involve any paid instruction classes. If you are 16+ you may use the fitness equipment with a signed fitness waiver, which may be obtained from the front desk. There are absolutely no guarantees of placement based on special requests provided at the time of registration. No exceptions due to the high volume of special requests provided. We are unable to switch children to a different team once they are have been placed due to the many moving parts and large amount of special requests received. Participants will be contacted by their volunteer coach after the pre-season coaches meeting on Sept. 14. Practices/Games All teams will be provided one, one-hour practice per week Mon. - Thur. The day of the practice will be chosen by the volunteer coach. Practices will start the week of Sept. 23. All games will be on Sat. starting Oct. 5. Game times and locations will be determined based on venue availability and total registration numbers. Age Groups (Playing Up) Please make sure you are registering for the correct age / gender / division for your child. Participants may not play in a younger age group than what they are eligible for. Participants may play up an age division if they meet the following criteria: 1. Meet the minimum age requirements for the youngest age division for the sport. 2. Are less than 12 months from the age cut-off for the age division they want to play. (Ex. An 8 year old may register for the 9 & 10 division.) The age cut-off date for the 2019 Fall season is Sat., Oct. 5 (1st day of games).

The Moving Wall Veterans Memorial

Rio Vista Park  Sept. 5th-9th, 2019 The Moving Wall is a half-size replica of the Vietnam Veterans Memorial in Washington, D.C. It was devised by John Devitt after he attended the 1982 annual commemoration ceremonies celebrated in Washington for Vietnam veterans. He felt that he needed to share his experience with those who did not have the opportunity to go to Washington.

Two structures of The Moving Wall now travel the United States from April through November, spending 5 or 6 days at each site. Now is your chance to experience the historic Wall for yourself.

If you are not completely satisfied with the activities and programs offered, we will assist you in the following ways:

Transfer you to another activity or class immediately. Credit your account in the computer immediately, which then can

be used by any family member for any recreation program in the future.

Refund your money. If you paid by credit or debit card, the amount

will be credited to your account. There will be no cash refunds. If you paid by cash or check, refunds take approximately three to four weeks to process and will be mailed to the address on the account. Refund checks will be made payable to the head of the household unless otherwise directed.

Class General Policies

• Assume you are registered and attend the first class. • NO confirmations will be sent. • Other than Parent Tot classes, parents remain outside the classroom. • Non-registered individuals are not allowed to attend classes.

Active Adults General Policies

Registration for trips is on a first-come, first-served basis. PAYMENT IS DUE AT TIME OF REGISTRATION. Times and, occasionally prices, are subject to change. Cancellations: For events where a ticket has been purchased or space booked from an outside agency (theater, travel company, etc.), we cannot issue a full refund, we can only try to resell your ticket, or you may sell it yourself. In some cases, a partial refund will be mailed to you in approximately 3 - 4 weeks. Transportation is by school bus, city van or charter bus. All trips require a minimum number of registrations. Arrive at least 15 minutes before departure time.

NO refunds, credits or transfers will be issued after the program or session has concluded. Refunds will be given for registration fees only.

Ramada and Field Rentals

Park ramadas, softball fields and multi-purpose fields are available to rent. Call (623) 773-7137 for more information.

Resident Rate

If where you permanently reside or live is located within the corporate limits of the City of Peoria and you pay property taxes to the City, you are eligible for the resident rate. Proof of residency is required at the time of registration. Acceptable proof is a copy of your property tax statement showing the physical location of your residence or we can look this up at www.maricopa.gov.

Notice to Persons with Disabilities

The City of Peoria Community Services Department strives to make its programs, services and facilities accessible to individuals with disabilities. If you would like to request modification that would allow you or a family member with a disability to participate successfully in a program, contact the Community Services Department at (623) 773-7137 and request that an application for modification be mailed to you. The city requires two weeks to implement a reasonable modification. Participation in a desired program may not begin until the request for modification has been thoroughly reviewed. Therefore, complete an application two or more weeks prior to a desired start date.
