Ophthalmology Services For My Enlightening 30-Year Journey as an Ophthalmologist

hen I completed my fellowship training at Bascom Palmer Eye Institute in 1987 and began practicing ophthalmology in Hollywood July 1987, I was part of a new wave of ophthalmologists who brought the exciting technique of out-patient cataract surgery to Broward County. Our predecessors would admit patients to the hospital the night prior to surgery and keep them hospitalized for an additional night following surgery. Avoiding the hassles and expenses of hospitalization was attractive, but even more advantageous was avoiding the risks of hospital –induced infections. Out-patient cataract surgery became an almost instant success.

Outpatient Cateract Surgery

For the next decade, those of us on the “cutting edge” of cataract surgery in Hollywood were doing cataract surgery by a procedure called Extracapsular Cataract Extraction whereby we removed the nucleus of the cataract in its entirety. This necessitated that we make a large incision in the eye. The cataract was removed, followed by insertion of an intraocular lens (IOL). At the completion of surgery, five to seven sutures were required to close and secure the surgical incision. The eye was pressure patched. For the next three weeks, we encouraged our post cataract surgery patients with the following refrain: “Do not bend, do not lift, do not exercise, do not exert.” Finally, after three weeks our patients could become “free agents” again and resumed their normal lives. We began suture removal in the office, and by the sixth post-operative week the patient received a final new eyeglass prescription. The IOLs utilized in that era provided fairly good distance vision, but eyeglasses were needed to see near and intermediate distances, with fine tuning of distance vision.

“No Stich” Cateract Surgery

While moving cataract surgery to an out-patient basis was considered revolutionary at the time, an even bigger milestone transpired with the transformation of cataract surgery in the mid to late 1990’s to a highly improved technology called Phacoemulcification (phaco). Instead of removing the cataract nucleus in its entirety, with a hightech ultrasound we could break up the nucleus into tiny fragments and suck them out. This paved the way to tiny incisions in the eye and the advent of “no stitch” cataract surgery. Phaco brought quicker, safer, and better visual outcomes to the cataract surgery arena. With great pleasure, on the very first post-operative day, I was now able to encouragingly tell my patients, “You can now bend, lift, exercise and resume 100% of your activities!” Even more importantly, post-operative edema, infections, and inflammations plummeted with the new technology. Cataract surgeries were now routinely scheduled two weeks apart instead of two months apart. Concurrently, there have been remarkable enhancements in the instruments that we utilize pre-operatively in the office to ascertain the optimal custom fit intraocular lens for any given patient. The quality of these high tech ophthalmic devices and ultrasounds utilized pre-operatively for Iens determinations, as well as the expertise of the staff actually making the measurements, are of paramount importance for excellent visual outcomes.

30th Anniversary

From the onset of my career in 1987, through to the present time, we have been blessed with newer generations of intraocular lenses (IOLs) which provide superior vision. Another career milestone occurred within the past ten years with the arrival of the multifocal ReStor IOL. With the ReStor IOL, we can generally correct most patients’ cataracts

and enable them to see near, far, and everywhere in between without the aid of glasses or contact lenses. The ability to read, drive, work on the computer, and see without glasses is an incredible freedom that was never possible with traditional monofocal lenses. My journey in the world of cataract surgery in Hollywood has now hit three decades as I had my 30-year anniversary of practicing ophthalmology on July 1st! Perhaps the most incredible landmark and noteworthy step on this journey has been the landmark transformation in cataract surgery to the new era of laser-assisted cataract surgery that has commenced in earnest in the past five years. Femtosecond laser cataract surgery is highly reproducible, far more accurate, and far safer than older techniques of cataract surgery with a scalpel. The laser offers excellent precision thereby producing the absolute best visual outcomes in current cataract surgery. Furthermore, patients with low or moderate levels of astigmatism can have this astigmatism eliminated or greatly reduced at the same time as the cataract procedure, resulting in a markedly enhanced quality of vision and overall outcome. In essence, this is customized laser cataract surgery utilizing high-tech 3D imaging and tailoring the ultra-precise treatment to the unique size and shape of your eye.

My Own Cataract Surgery

On a personal note, even though I am a cataract surgeon, naturally I am not exempt from developing cataracts myself. In fact, in 2014, at age 56, I began to notice the litany of complaints that I have heard from my patients. First, I noticed that while driving at night, I was not as comfortable or confident as I was driving during the day. Second, oncoming headlights became bothersome, and eventually so did the rising or setting sunlight in my face

while driving. Third, I had always been an avid reader, able to read small print journals or novels for hours at a time prior to the inception of my cataracts. But in 2013 and 2014, I noted that reading tiny print became more of a chore and challenge, curtailing my reading duration. Finally, and most importantly by far, I needed to be positive that my vision was excellent to examine my patients and perform surgical procedures. I decided that it was time to proceed. In July 2014, I was the personal beneficiary of the wonderful transformations of modern ophthalmology that I described. I had advanced laser cataract surgery with the insertion of an ReStor lens in my right eye, with the same procedures to my left eye the following week. My vision is now phenomenal! I wore eyeglasses starting at six years of age and then began with contact lenses around age 13. Until age 56, I needed glasses or contact lenses for everything! Now, for the past three years, I wear no glasses, no contact lenses, nothing. Of course, I practice what I preach and always wear sunglasses with UV400 blocker and polarization outdoors during daylight hours for ocular protection. My quality of vision and depth perception are the absolute best of my entire adult life. I am delighted! Now at age 59 and feeling like the bionic man, I look forward to my next decade serving the community as an ophthalmologist on this thoroughly enlightening journey!

Celebrating Hispanic Heritage Month

t was a sold-out crowd, as many came out to be a “guest” at the Beauty and the Beast Jr. performance at the Art and Culture Center/Hollywood. The tale as old as time was just one of the organization’s 2017 Summer Arts Camp productions. Although the cast and crew pulled off an amazing show, BBX Capital Foundation was a behind-the-scenes superstar. This summer BBX Capital Foundation presented a $3,500 check to the Art and Culture Center/ Hollywood to support the organization’s 2017 Summer Arts Camp and its Broadway Actors’ production of Disney’s Beauty and the Beast, Jr. and Disney’s My Son Pinocchio, Jr. Lois Marino, Executive Director of BBX Capital Foundation, presented the check to Joy Satterlee, Executive Director of the Art and Culture Center/

It’s a Wrap – Utility Boxes Get Wrapped Throughout Hollywood COMMUNITY NEWS

R

esidents and visitors may have noticed some of the newly wrapped utility boxes at intersections throughout the City. The continuation of the beautification project to enhance the area and showcase the historic cultural arts vibe for which Hollywood is known is in full force. These public art projects have been popping up throughout the country to incorporate art into public places. Artists can express themselves while elevating

the cultural vibrancy of their cities. Typically, these utility boxes are large and grey, and pretty much an eye-sore., but beautification projects such as this one makes a necessity into a piece of art. The city of Hollywood was one of the first to jump on this trend. Since 2013, these traffic cabinets were receiving a pop of color. Images such as historic postcards of the beach, the Broadwalk, marine life, and even an anti-litter campaign adorned the corner boxes.

The 2015 project in Highland Gardens covered 15 boxes, and the new 2017 one has 24 planned with 7 already installed. The remaining 17 are awaiting approval from the Florida Department of Transportation (FDOT). “Advertising our beautiful beaches and beautifying areas of our city is a win/ win situation for neighborhoods,” stated Commissioner Kevin Biederman, District 5 Hollywood Florida. The images currently being used are provided by the Hollywood Beach Community Redevelopment Agency (CRA). The organization had an art contest for children who reside in Hollywood. They wanted youthful artistic creativity for these new installations. The decision was difficult as the Downtown Hollywood Mural Project review committee and Commissioner Hernandez were only able to select a few winners out of all the submissions. The new project will be installed by Car Wrap Solutions-The Perfect Image of Ft. Lauderdale. Each wrap is under $400, and is funded entirely by the CRA’s advertising budget. “By wrapping these utilitarian boxes with decorative, colorful, creative artwork it not only helps to beautify but also helps to unify the City,” says Joann Hussey, City of Hollywood Public Information Manager. “Teams of engaged residents are selecting the artwork to be installed in neighborhoods and the unsightly but necessary infrastructure gets a much-needed makeover and serve as a canvas for eye-catching art in a public place.”

Hollywood Resident is a True Warrior INSPIRING INDIVIDUAL

J

ulio Santiago is best known as “Dainamite” when performing, and better known as “The Puerto Rican Ninja” when competing on American Ninja Warrior. The Hollywood resident has been a contestant on the television series since season 7, three years ago. Although, he never thought that he would be competing in anything, especially in a nationally known hit series, since he was born with an undiagnosed heart problem which caused dizziness, fainting, and intense headaches. At the age of 24, he suffered a stroke and was finally diagnosed with Atrial Fibrillation (AFib). The condition is what causes irregular heartbeat (arrhythmia), fatigue, shortness of breath, blood clots, heart failure, and other heart-related complications including strokes. It has been estimated that 2.7 million Americans are living with AFib. Santiago was prohibited from exercising and instructed to take 800mg of Atenolol every day for the rest of his life. For ten years, Santiago followed the doctor’s orders, but was enthralled with an episode of American Ninja Warrior. The show motivated him to start training, and that was when a miracle happened; the illness just disappeared. He stopped medication, and was determined to compete and help others. However, it wasn’t easy. In the beginning, he couldn’t run more than 50 meters without feeling exhausted and out of breath, so he created a routine of exercises in between

the 3 miles he was running each day along with a special diet. This gave him the stamina to continue. After a month of continuous dedication, he lost 20 pounds and was dehydrated. Sadly, this put him back in the hospital. “At the hospital, the doctors couldn’t believe the test base on my 10 years with AFib. They didn’t find any sign of arrhythmia. It disappeared completely,” said Santiago. “Two days after I left the hospital I started training again, and I never had to take another pill or suffer any symptom of dizziness or faint. I’ve been blessed to be fully cured of a sickness I had from birth.” The Puerto Rican native has now competed in several local competitions and finished as a top competitor. This new way of life has inspired Santiago to help others. In addition to his personal business, Strong Grip LLC, where he teaches boot camp classes, trains others, and even hosts birthday parties, he is launching a new project to help those with addictions. “Based on how I trained and cured myself, I developed a workout and techniques that will help those with addiction to drugs and/or alcohol,” added Santiago. “It is a personal dedication method that will help replace bad habits with new habits. I’m hoping to get into many rehabilitation programs and help as many people as I can, and use my example to inspire the people to overcome obstacles.”

Hallandale High School Football Team Grinds and Shines at Miami Dolphins OTA Practice SCHOOL NEWS

s the school year came to an end, the Hallandale High School football team had the amazing opportunity to learn from the pros. The Miami Dolphins hosted the Chargers at their Organized Team Activity (OTA) practice at the Baptist Health Training Facility at Nova Southeastern University. After touring the facility, Twan Russell, Miami Dolphins Youth Programs Ambassador and Dolphins alum, spoke about the importance of teamwork and brotherhood. The team also had the opportunity to watch the Dolphins practice and meet with current players on the field. Following practice, the Hallandale athletes had lunch provided by Papa John’s. “I think it was a great experience. We had an opportunity to get our kids out here to show them how the pros do it. The visit gave my players motivation leading into the off-season workouts. This truly reflects their quote, “We Grind, We Shine”. “We finished the spring further than most would have expected to see the program at this point. With us finishing the spring in the fashion we did gave us as a unit a smooth transition into the offseason getting prepared for the season,” added Lovett. “The players believe in themselves and trust the coaches to better them as individuals on and off the field. These young men have developed a ‘REFUSE TO LOSE’ mentality. If we continue to focus on getting better every day we will be very successful during the upcoming season.” The Miami Dolphins Youth Programs’ mission is to transform the lives of youth football players by instilling character building and reinforcing safety standards that help to increase football participation and grow a new generation of Dolphins fans.

McArthur Special Needs Students Take on the Town

cArthur High School Exceptional Student Education (ESE) teacher, Eileen Santiago likes to provide real-world experiences for her students. Her goal as an educator is to teach math, reading, science, social studies and social skills beyond the classroom. Therefore, she is always searching for opportunities to do so. Santiago learned about Junior Achievement Biz Town through her 5th grade daughter who was working on the curriculum in her class at the time, and she knew this would be an amazing place for her students to visit. The indoor center contains a mini city with up to 18 public and private businesses, sponsored by the real businesses. The appearance is that of an actual marketplace to give students real life experiences such as operating a bank, voting for a mayor, selling merchandise, depositing paychecks, reporting, and interviewing business leaders. Biz Town, also known as JA World, had never worked with high school special needs students, so Santiago began adapting and creating a smaller version of the curriculum to fit the needs of her 9-12th grade students, as well as her students in the Postgraduate Alternatives for Secondary Students program. Before visiting the facility, students need to complete a pre-visit curriculum where they learn basic

economic principles to prepare them for their “jobs”. “My goal was for our students to work on basic life skills such as how to manage their own personal bank accounts,” said Santiago. “Students learned to write a resume, interview for a job, write checks and deposit funds, and work with other classmates as a team. After three months, we were ready to visit and run a town.” Upon arriving at Biz Town, Santiago’s students were given their workplace and job functions which they had to perform. Other 11th and 12th grade McArthur students came to volunteer to assist their classmates. Jobs ranged from retail sales professionals, accountants, business managers, and medical professionals to TV and newspaper reporters. The favorites among these Mustangs were the Humane Society and BB&T.

“The staff at Biz Town worked closely with us to be sure the students were able to complete their job duties,” added Santiago. “I would like to thank Junior Achievement for giving McArthur the opportunity to run a town for a day. Attending Biz Town is an experience that our students will never forget. Learning, having fun, and preparing for the real world all in one place!”

THE DONOWAY CENTER NEW HOPE FOR ADVANCED PANCREATIC AND LIVER CANCER PATIENTS Patient’s once diagnosed with locally advanced Pancreatic and Liver Cancers were given little hope and often offered limited treatment options all with certain short life expectancies despite the treatment they were receiving. Preliminary results have been very encouraging and have more than doubled many patients' life expectancy. Clinical trials are ongoing to use new novel agents including monoclonal antibodies and vaccines as the next line of treatment in this highly selected & heavily pre-treated group. A

s a hospital pediatrician, there is one symptom that I often see my patients exhibit and that is fever. Fever is not an illness within itself, but rather a symptom of an underlying disease. In most cases, fever is a sign of the body’s immune system trying to attack and get rid of an underlying illness. It is also a frequent reason for office, emergency room and urgent care visits that can amount to significant stress on parents, as they address the fever and try to figure out its source. To truly understand how a fever works, it’s important to first define it. Fever is any temperature greater or equal to 100.4F or 38C. Any temperature lower than that is not clinically defined as a fever. From a pediatrician’s perspective, we like to see the trend of the fever because it gives us clues to which direction the illness is heading. Remember that a fever is a symptom of an underlying disease. However, there are some exceptions. A small percentage of kids have a benign condition called febrile seizures, but outside of a specific condition, no amount of fever will cause a child to have seizures. Fever alone cannot cause brain damage or any long-term consequence. The most common cause of fever in otherwise healthy kids is a viral illness. If there are additional findings such as nasal congestion, cough, headache, vomiting, diarrhea or rash, the fever is the result of the body’s immune response to tackle the underlying virus. Persistent fever with no additional symptoms, especially in non-verbal children, should prompt an investigation for things such as urinary infection or bone/joint infection. So when should parents take their child to the doctor if he or she is experiencing a fever? ▸▸ Fever in anyone younger than 2 months. These babies are at high risk of serious bacterial infection due to their immature immune system. Anyone younger than 2 months with a fever should be evaluated as soon as possible. ▸▸ If the fever is accompanied with these symptoms: lethargy, drowsiness, stiff neck and irritability. These are signs of serious infections like meningitis and should be evaluated immediately ▸▸ When a child refuses to drink, has decreased wet diapers/urine output, and is experiencing persistent vomiting and diarrhea. ▸▸ Fever longer than 3-4 days without any improvement or other symptoms to explain the source of the fever. This includes coughing, runny nose, vomiting, diarrhea, rash or ear pain. ▸▸ In the era of vaccination, the rates of bacteremia (bacteria in the blood) and meningitis (infection of the fluid around the brain and spine) have dramatically decreased. Unfortunately, kids who lack this protection because they are unvaccinated are still at risk for these infections, and should be seen immediately should they develop fever ▸▸ Kids with chronic medical problems such as cancer, sickle cell, cerebral palsy and others. ▸▸ The greatest risk with fever is dehydration, so the most valuable action is encouraging fluids. Dr. Berkovich, a pediatric hospitalist at Joe DiMaggio Children’s Hospital in Hollywood, is a board-certified pediatrician, as well as a fellow of the American Academy of Pediatrics and member of the American College of Osteopathic Pediatricians and the Helfer Society. She received her medical degree from Nova Southeastern University, and completed her pediatric residency at Nicklaus Children’s Hospital, formerly Miami Children’s Hospital. The award recipient of the Dr. Arturo J. Aballi Educational Award and the 2016 American College of Osteopathic Pediatrician’s Outstanding Resident of the Year resides in Hollywood with her husband and two children.

ongenitally prominent ears are a cosmetic deformity that can have a severe emotional and behavioral effect on a child. When a child’s ear protrudes too far from the side of the scalp, a surgical procedure known as an otoplasty can be performed to enhance the position and appearance of the external ear. Five percent of children are affected by prominent ear protrusion. The majority of children genetically inherit the appearance from their parents. Prominent ears can present in a myriad of forms including cup ear, shell ear, bat ear, Machiavellian ear and lop ear. Most commonly children and adults will often camouflage the deformity with hairstyling. Timing of repair can be crucial in avoiding psychological damage. The ear achieves 85% of adult size by age 3 years and nearly adult size by 6 or 7 years of age. As an individual ages, elongation of the lobule (lobe) gives the appearance of continued ear growth. The most common deformity in prominent ears is an underdeveloped or poorly defined antihelix. This is the part of the ear fold that lies inside the outer rim of the ear. The otoplasty surgery is performed through a concealed incision along the back of the ear. Correction of protruding ears uses surgical techniques to create or increase the antihelical fold (just inside the rim of the ear) and to reduce enlarged conchal cartilage (the largest and deepest concavity of the external ear). The cartilage of the ear is reshaped by different suturing techniques. The excess cartilage can be excised and the ear repositioned closer to the side of the head. The procedure usually takes 60 to 90 minutes to perform. It is performed under local anesthesia with sedation or general anesthesia under the direct supervision of a board certified anesthesiologist. The patient can resume their normal activities within three to five days wearing a soft headband for one to two weeks to protect the ears. Ear surgery, otoplasty, offers almost immediate results in cases of protruding ears visible when the dressings that support the new shape of the ear during the initial phases of healing are removed. Children and adults who are concerned about the appearance of their ears can experience both the cosmetic as well as the emotional improvement of otoplasty surgery.

Local Chamber Leads the Way, Plans to Renovate Office Building by Hollywood Circle

T

he Greater Hollywood Chamber of Commerce will continue the economic development trend around the City of Hollywood by investing $60,000 on its office building. The renovations, scheduled to start late September, on the two-story building on the corner of US-1 and Taylor Street, will expand the reach of the ongoing transformation around the Circle, in Downtown Hollywood and some of the adjacent residential neighborhoods. “The Chamber wants to be a partner in economic development and there is nothing like leading by example and modeling the way! This is part of business leadership and we hope that our transformation will encourage and inspire oth-

ers to invest in the beautification of the US-1 corridor, one step, one corridor at a time,” said Anne Hotte, Hollywood Chamber’s CEO & Executive Director. “AIA is in good shape, US-1 is next, and in the meantime Dixie Highway and State Road 7 are creating the infrastructure necessary for healthy and sustainable development.” Thirty-six (36) board members, representing a multitude of industries and businesses from all over Hollywood, voted unanimously for the beatification initiative, allowing the Chamber to commit to the greater good of the community. “As we see developers invest in our Downtown and see how stunning Hollywood Circle (formerly Block 55) now looks, and the new residents, visitors and commerce it will bring to our Downtown, there is no limit to what Hollywood can become,” adds Hotte. The renovation plans include replacing windows with hurricane impact resistant glass, repainting the building with new colors, improving the surrounding landscape, removing the awnings and providing new signage and lights. Built in 1996 on city owned land, the 3,500-square-foot building also houses Costa Bella Realty Group and Idea 180. The Chamber hopes that other businesses will follow suit and be inspired by the transformation. ollywood resident and Nova High School senior Olivia Cohen was selected as a Bank of America Student Leader. The Bank of America Charitable Foundation selects only five exceptional high school juniors and seniors for this coveted position in the Bank of America Student Leaders® Program at the YMCA of Broward County. The choices are based on the students’ commitment to serving the community and making Broward a better place to live along with their outstanding academic achievements. The eight-week paid internship opportunity gives junior and senior level students the opportunity to develop and apply leadership skills through hands-on community work experience at local nonprofit organizations as part of the Bank of America’s Student Leaders® program. “In the third grade I came to the conclusion that the health I was fortunate enough to have was not something I could simply ignore or take for granted,” said Cohen. “Instead I decided to take action and create my own non-profit organization called Giving & Living.” The entire premise behind Olivia’s organization is that it is “For Kids, By Kids”. To date, Giving & Living has raised more than $18,000 for organizations such as Joe DiMaggio Children’s Hospital, Make-A-Wish, and Give Kids the World in Kissimmee, Florida. Today, it is much more than just a fundraising group but a

network of students who are interested in volunteering and seeking the right organization to give their time. In addition to the internship, these select individuals from Broward County had the opportunity to travel to Washington D.C. to join more than 200 other Bank of America Student Leaders from around the country for a week-long Leadership Summit. T

he vertebral column (backbone) is made up of 33 vertebrae. These vertebrae are then grouped into divisions called the cervical (neck), thoracic (upper back), and lumbar (lower back). A fibrous disc (intervertebral disc) connects each pair of the vertebrae. Healthy spines have discs that cushion the vertebrae allowing for normal flexibility and movement of the spine. Degenerative Disc Disease (DDD) is the weakening of one or more of these intervertebral discs. Unfortunately, the condition develops as a natural part of the aging process. These discs between the bones of the spine are made up of cartilage, fibrous tissue, and water. With age, the discs weaken, flatten, bulge, or break down. Sometimes there may be a genetic component in some individuals who suffer from DDD, but the true cause can be a combination of things, but usually it’s simple “wear and tear” or the result of an injury. Degenerative Disc Disease in the lower back, or lumbar spine, occurs when a disc is compromised and causes pain. The disc itself does not have a blood supply, so if an injury occurs it is unable to repair itself the way other tissues in the body can. DDD is so common that an estimated 30% of individuals ages 30-50 years old have some degree of disc space degeneration. Many may not ever even experience pain or have it diagnosed. After a person reaches the age of 60, some level of disc degeneration is considered normal and nothing to be alarmed about. ▸▸ Symptoms. Symptoms vary with each individual depending on the location and the type of disc degeneration. However, a primary symptom of the disease includes sharp and/or chronic pain in the back and neck. Other signs are weakness or numbness. As a disc undergoes change, the body may react and develop bone spurs as well. These bony growths may take up space, therefore compressing the nerves, and causing weakness in the arms and/or legs. ▸▸ Tests. If a patient presents any of the above symptoms, their doctor may order X-rays, an MRI (Magnetic Resonance Imaging), or a CT (Computed Tomography) scan. ▸▸ Treatment. The severity of the condition will determine the best approach for treatment before considering surgery. Nonoperative measures may include: heat and/or ice therapy, activity modification, medications, physical therapy to strengthen the muscles, and epidural injections. These measures are often effective in providing pain relief. Surgery, such as a discectomy, may be required for those patients not responding to any of the nonoperative measures, whose disc changes have resulted in spinal cord compression, who have a structural abnormality, or those who have such severe chronic pain. The procedure removes the disc and any associated bone spurs which are compressing the spinal cord and/or surrounding nerves. The removal makes room for the spinal cord and nerves to move freely again, and the pressure is relieved. Felix M. Many times, owners want to try listing at a higher price because everyone believes that their home is the most beautiful place and will naturally be worth more, especially if updates have been made. It is perfectly acceptable to do that but always be aware that it may not have the activity expected. When these situations happen, there are many factors that could be the reason: price, condition, or simply the “functionality” of a property. It’s recommended to reevaluate and correct before reentering the property on the market again. Many times, it happens that many people find the cheapest way to list their property and they find that they are alone in the process. This way may cause for the property to stay longer on the market without getting any positive results.
