Necessary Documentation ll applications from persons applying for credit transfers must be accompanied by: • 100 point identity check/proof documentation supporting the clients’ competencies for their Earthmoving and Particular Crane &/or Licence to Perform High Risk work. This evidence must consist of: Coal qualifications: a) Originals to be sighted by OHSA or copies certified by a Justice of the Peace or Commissioner for Declarations include: ­ Statement of Attainment/s for the coal competencies the applicant would like transferred to a WHSQ certificate; or ­ Training log book or a computer print out of the training log book (example provided) for the coal competencies the applicant would like transferred to a WHSQ licence; and b) A letter signed by the registered mine manager/senior site executive, specifically stating that the person named in the letter was “trained and assessed in accordance with the National Black Coal Industry Training Package”. (example provided). 2. Assessment To determine equivalence, reference should be made to the Translation Tables. Any qualification/s that fall outside the scope of the Translation Tables can be referred to OHSA for assessment/refusal. For an example of how to use the Translation Tables for Coal Competencies see examples provided. Gap Competency Required: When a Gap Competency is required for credit transfer (see Translation Tables) of a Coal competency, OHSA will provide the applicant with a “Prescribed Occupation Gap Competency Assessment Summary – Coal Competencies” (see example). If OHSA is unable to complete the assessment, a list of RTO’s will be provided that can do the gap assessment. The applicant must then go and complete the gap competency training. Once completed, the applicant must return the assessment summary to OHSA so that their application can proceed. 3. Principal Inspector (Licensing) Endorsement After all information has been received and a recommendation has been made by OHSA regarding the application for credit transfer, the application and supporting documentation will be scanned and emailed, attention to the Principal Inspector (Licensing) for final approval. The Principal Inspector will make a determination as to the accuracy of the assessment process and advise OHSA via return email within 10 working days. Necessary Documentation 4. Applicant Not Eligible When an applicant is found to be ineligible for a credit transfer, OHSA will communicate this decision to the person applying for a credit transfer. 5. Credit Transfer for Earthmoving and Particular Crane (EPC) licence class Where an applicant has been deemed eligible to hold a particular certificate for an EPC licence class/es, a ‘G’ form (form 36) and Earthmoving and Particular Crane form (EPC Assessment Summary) will be completed. OHSA will complete sections A, B C & D of the Earthmoving and Particular Crane form (EPC Assessment Summary) and sections 1, 2, 3, 6 & 7 of the ‘G’ form (form 36). The applicant will complete section E of the Earthmoving and Particular Crane form (EPC Assessment Summary) and sections 4 & 5 of the ‘G’ form (form 36). Once completed, the forms and the prescribed fee will be forwarded to Workplace Health and Safety Queensland (WHSQ) by OHSA for processing. 6. Credit Transfer for High Risk Work Licence (HRWL) class Where the applicant has demonstrated eligibility to hold a HRWL, OHSA will complete sections A, B, C & D of a HRWL assessment summary and issue a statement of attainment. These documents will be given to the applicant. The applicant must complete section E of the HRWL assessment summary and take the documentation to an Australia Post outlet, with the prescribed fee & a passport size photo for submission.

Information to Applicants on the lodgement of forms from July 01 2008 High Risk Work Licence applications need to be lodged by the applicant at an Australian Post Outlet. The High Risk Work Assessment Summary needs to be completed for these classes. In addition these applicants will require a Statement of Attainment. Applicants then attend an Australian Post outlet and complete a Form F1 Application. Applicants also require a passport quality photograph and 100 points of identification. The applicant will be provided with two Statements of Attainment as an original copy will be lodged with the Post Office.

Earthmoving and Particular Crane Certificates From 1 July 08 the licence classes regulated in Queensland that do not fall within the National Standard are to be called Earthmoving and Particular Crane Certificates (EPCs). The classes of work that will become EPCs are as follows:
