MESSAGE FROM DEAN’S DESK

The activities in SSSMCRI campus during the period of the 3 rd quarter of this year (between July and September 2017) have been a mix of Curricular, Co curricular and extracurricular activities making the environment in tha campus bubbling with energy in both staff and students. The day of 14th July will be remembed forever here at SSSMCRI as the unique facility viz Cadaver Lab got inaugurated and 1st Arthroscopy workshop was also conducted. On the same day new 2


generation MRI machine also got commissioned in our hospital and was dedicated for the service of the surrounding rural masses. An academic/ research MoU was signed between SSSMCRI and Agni College of Engineering and Technology Chennai on 20th July, to work together in the field of Biomedicine. A chain of academic events consisting of Orientation programs for the newly joined UGs, CRRIs and PG students, Hands on training workshop on Evidence Based Medicine for our HoDs apart from routine monthly Scientific Academic Forum, CRRI Scientific Forumwere conducted as scheduled earlier. Breast feeding week was organized by the Dept of Paediatrics and in return the dept got recognition as a Baby friendly hospital. On the eve of 71st anniversary of our national independence, the Tri colour was hoisted on 15 th August by the dean, with participation of staff and students. As per the mandate of the UGC, ‘Swachhata Pakawada’ was observed by the institution between 1 st Sept and 15th September , where in various Swacchata activities were conducted by our staff and students, not only in the campus but also in the surrounding areas and the rural and urban health centrs of the college. The student’s Council of the college organized the Teacher’s Day celebrations on 5 th September and a senior teacher, Dr Ramsamy, was invited as the chief guest, who gave an inspirational speech. Senior faculty member of the college Dr Swauyamjyoti was felicitated on the occasion and all the teachers took an oath of dedication on the occasion. The institution welcomed Dr Vievekanand, a well known and experienced surgeon as the new Medical Superintendent of the teaching hospital. All the departments also performed Ayudhpooja in the depts. on 28 th Sept and in the same evening a grand pooja was organized in the hospital and our respected Chairman Madam graced the occasion. With around 70 Publications in One quarter, the SAIGENESIS reflects the buzzling Research works accomplished within the campus. The visit to Harvard by a team of Students from SSSMCRI along with students from MGMCRI is an event with notable significance in this issue. The Massive and complete data about the Swachhata Pakhwada (15 Day Event) which expands to around 30 pages as a seperate additional book is a remarkable testimony to the commitment of SSSMCRI to the cause of Nation Development.

SCIENTIFIC EVENTS Resource Persons in Academic Events 1. Dr. Saurabh Shrivastava, Associate Professor, Department of Community Medicine and Dr Anusa A.M, Assistant Professor, Department of Psychiatry were resource persons for the first Hands on Training workshop conducted on “Evidence-based Medicine” (EBM) on 24 th August 2017 organised by the Medical Research unit at SSSMCRI. 24 faculties from preclinical, para-clinical and clinical departments attended the workshop. 2. Dr. Saurabh Shrivastava, Associate Professor, Department of Community Medicine, was the resource person for a FDP on Scientific Communication - “How to write a Research Article”. The interactive session was attended by more than 60 faculties, senior residents, tutors, and PG students. 3. Dr. Maduram, Associate Professor, Department of Pharmacology was a resource person and 13


gave a lecture on ‘Documentation in Clinical trial’ at the Golden Jubilee International Conference of IPS, Southern Region held at MGMCRI, Puducherry on 5th July 2017 4. Background: Surgical specialty requires adequate exposure of training and enormous practice of surgical skills to become successful in their field.

Surgical postgraduates’ ambition is to get

accommodated into the safe hand of an experienced surgeon and his supporting team for his future bright carrier. Moreover he has to be familiar with day to day trending newer developments and surgical techniques to compete the Milieu exterior in terms of his stability, viability cum surgical practice. For exclusive surgical training in newer surgical methods, available methodology being Surgical Skills Training (SST) in cadavers. If the cadaver is with life like color, pliability, operability 39


and increased working time no other method could be matched

in terms of the ambience and

medium in which a trainee is exposed 1. Even though live animals and low and high fidelity simulators are used for SST, the overall outcome is in low profile compared to embalmed cadavers with high cost and compromised anatomic fidelity 2. Materials and Methods: Out of the three types of embalming methods for cadaveric training available namely Formalin Solution method (FAS) 3, Thiels Solution method (TS) 4.and Saturated Salt Solution method (SSS) 5. we have planned to undertake a study outcome of preserving two donated bodies from MGMCRI using SSS of the International Cadaveric Training Centre of our college namely Sri Sathya Sai Medical College Ammapettai under Sri Balaji Vidyapeeth University Puducherry with due ethical committee clearance. The outcome will be evaluated in terms of color, pliability of the tissues, range of movements of the joints, cost, bacterial and fungal culture tests and compared with the other methods available in the literature. Results: The data collected is to be tabulated meticulously under various headings namely color, pliability of the tissues, range of movements, cost and bacterial and fungal cultures tests and analyzed appropriately. Discussion: Within the available methods of embalming of cadavers for surgical skills training FAS method has its own demerits as unavoidable smell,

stiffness, change of color. TS method is so

cumbersome that it has five steps of preparation namely Stem A, Stem B, Stem C, Stock and Final Solution requires considerable cost instead of giving natural color and Pliability 6. SSS method is a preferred one because of its less cost and giving similar outcome as TS method 7. Conclusion: We have firsthand experience from our ICTC under the program Advanced Surgical Training and Research Academy (ASTRA) a Knee arthroscopic workshop, as the cadavers were supple, suitable for the arthroscopic procedure. The cadavers were fresh, maintained at −20 degree C, allowed to be used after thawing. There was sloughing of the skin which was unavoidable and with mild unpleasant smell

was managed by room sprays (based on the feedbacks from the

INTERESTING CASE REPORTS: DEPARTMENT OF DERMATOLOGY,VENEREOLOGY & LEPROSY RARE CASE OF CHILDHOOD CUTANEOUS TUBERCULOSIS INTRODUCTION Inspite of the advent of of highly effective antituberculous drugs; Tuberculosis continues to be a significant medical problem in India. India has an incidence of 2.2 million cases of TB out of a global incidence of 9.6 million cases according to recent statistics of WHO. Cutaneous TB constitutes about 1.5% of all extra pulmonary tuberculosis. The prevalence in children varies from 18 to 54% in India. There is no gender predilection and the infection occurs with increased frequency in 10-14 year age group.The Protective efficacy of BCG is debatable and not yet fully defined. Of all the clinical types, scrofuloderma (SFD) is the most commonly encountered variant followed by lupus vulgaris (LV) and tuberculosis verrucosa cutis (TBVC). Though childhood cutaneous tuberculosis has an increased frequency in age group of 10 to 14 years, it is rarely seen below 5 years of age. Hence, we present a case of cutaneous Tuberculosis for its rarity of occurring in a 3 year old. CASE REPORT A three year old, accompanied by his parents came to the Dermatology OPD with presenting complaints of an asymptomatic elevated lesion progressing in size over the medial aspect of right foot adjoining the sole for duration of one year. No history of trauma to the foot, No history of pain 41


or discharge from the lesion. No history of preceding systemic symptoms prior to the onset of lesion. No history of systemic illness or previous exposure to Tuberculosis. No history Tuberculosis in the family. Parents give history of children being bare foot while playing outside. Immunization history is up to date. No recent history of upper respiratory tract infection or treatment for the cutaneous lesions. On examination of the patient with the parents consent; patient was conscious oriented, Afebrile, No pallor, Icterus, lymphadenopathy or edema. Local examination of the skin; A single hyperkeratotic irregular shaped scaly plaque measuring 5 X 3cm present over the right foot adjoining the sole. Lesion shows healing at one end and progression over the other areas. No other skin lesions elsewhere.

Differential diagnosis of Lupus Vulgaris, Mosaic wart, Tuberculosis

veruccosa cutis and hyperkeratotic Eczema was postulated. Investigations such as Complete blood count (CBC), liver function test, erythrocyte sedimentation rate (ESR), mantoux test, X-ray of chest, ultrasound of the abdomen and biopsy of the lesion were done. ESR and mantoux were elevated. The mantoux test was performed using 1 tuberculin unit (0.02 mg of purified protein derivative) and read at 72 hours. Induration of more than 10 mm was suggestive of infection with Mycobacterium tuberculosis.

The skin biopsy showed markedly hyperkeratotic, hyperplastic and acanthotic squamous epithelium. The upper dermis was diffusely infiltrated by lymphocytes and plasmocytes amidst which are seen in a few scattered granulomas formed of epithelial cells. Some of the granulomas contained Langhan’s giant cells and foreign body gaint cells. The features were consistent with Tuberculosis verrucosa cutis.

In view of the biopsy report, after obtaining Pulmonologist and Ophthalmologist opinion; the patient was intitiated on Category I ATT paediatric kit which consists of Tab. Isoniazid 10mg/kg, Tab. Rifampicin 15mg/kg, Tab. Pyrazinamide 35mg/kg and Tab. Ethambutol 20mg/kg per day for two months intensive phase and adviced to continue Tab Isoniazid and Tab Rifampicin for four months as a continuation phase. Upon follow –up of the patient after two months, the cutaneous lesions showed improvement with no new lesions, hence the family was advised to continue ATT for the child and review on monthly basis.

This form of the disease, also known as warty TB, occurs due to exogenous inoculation of tubercle bacilli in a previously sensitized individual with good immunity. Clinically, it presents as verrucous papules and plaques, with the surface showing fissures or clefts that may extrude pus and, often, perilesional erythema. It may be difficult to differentiate it from hypertrophic LV as well as verruca vulgaris or common warts. While tuberculosis verrucosa cutis (TBVC) was once seen in 65.5% of the childhood cases from Hongkong, it is an uncommon form of the disease in Indian children. Although Arya et al. have reported TBVC in 15% of their patients (3/20), most of the other series from India have reported it to contribute to <4.5% of all cases of pediatric skin TB. Exposed body parts, in particular the lower limbs, are the most common site for warty TB in children, possibly due to inoculation by unnoticed trauma. In contrast to SFD and LV, lymphadenopathy is not seen. Histology shows hypertrophic changes like pseudoepitheliomatous hyperplasia, the presence of acute infiltrate in the upper dermis and characteristic tuberculoid granulomas in the mid dermis. AFB are rarely demonstrated. Diagnosis - The diagnosis of cutaneous TB is based on the characteristic clinical morphology of the lesions as well as laboratory tests. Common clinical differential diagnoses of skin TB include cutaneous leishmaniasis, leprosy, atypical mycobacterial infections, fungal infections like chromomycosis and sporotrichosis and sarcoidosis. Many of these conditions also show granulomas on histology and thus definitive diagnosis relies on the direct demonstration of tubercle AFB on stained smears or biopsies, isolation by culture or detection by PCR and related molecular techniques. Histopathology The hallmark of cutaneous TB histology is the presence of characteristic tubercular granulomas with epithelioid cells, Langhans giant cells and lymphocytes. However, characteristics such as the distribution of granulomas in the dermis, nature of cellular infiltrate, presence of necrosis and certain specific epidermal changes aid in classifying and diagnosing the variants of skin TB, which have been discussed before (vide supra). But, the difficulty of histological interpretation in TB is well recognized as many other diseases can produce a tuberculoid granuloma. A clinicohistological concordance was observed in 64-85.6% of the cases of childhood cutaneous TB Classical tubercular histology is seen more often in lesions of LV compared with SFD. Histopathology is especially useful for diagnosis of tuberculids where bacilli cannot be isolated by culture or demonstrated by AFB staining. Mantoux test 44


The Mantoux or tuberculin test is a good screening test to detect the presence or absence of tubercular infection. It is performed by injecting 0.1 ml (5 TU) of the purified protein derivative (PPD) on the volar surface of the forearm using a 27-gauge needle and measuring the induration after 48-72 h. Induration of 10 mm or more is considered significant, but indicates infection and not necessarily the disease. False-positive results may occur due to exposure to environmental mycobacteria as well as within 1 year of BCG vaccination. False-negative reactions may also be seen in patients with disseminated infection like miliary TB or in immunosuppressed states like severe malnutrition in children or HIV infection. Pandhi et al. reported 66 of 68 children (97.1%) with positive Mantoux test, with induration ranging from 10 to 32 mm. Kumar et al. observed that of their children with cutaneous TB, 91.8% patients with localized disease had positive Mantoux test as against only 50% of the cases with disseminated disease. Severe reactions resulting in vesiculation or ulceration have also been observed in children. Tuberculin test is more commonly positive in tuberculids, as reported in all cases of LS by Singal et al. On 7th September 2017, Hon’ble Prime Minister addressed the student Leaders convention on the occasion of Pandit Deendayal Upadhyaya’s Centenary Cleberation and 125 th Anniversary of Swamy Vivekananda’s Address at the Chicago World Parliament of Religious. The theme of Programme is young India, New India- A Resurgent Nation: From Sankalp to Sidhhi. The programme had the live telecast of our Hon’ble Prime Minister ‘s speech , to the Nation . It was observed as per UGC norms, in our hospital block ,4th floor with MBBS- 7th, 8th, 9th Sem students ,CRRI’S, AHS students along with faculties, MS and Dean from 10.00 am to 1.00pm on 7 th Sep 2017.

SWACCHA BHARATH Every 1st and 3rd Saturdays Swachh Bharat has been conducted for the 5th semester students, college surroundings were cleaned, and it is done under the guidance of PGs from the Department of 61

SWACHHTA PAKHWADA: As per UGC guidelines “Swachhta Pakhwada” (Cleanliness Fortnight) was organized by SSSMCRI from 1st Sep to 15th Sep 2017, Department of Community Medicine participated in the outreach activities – all faculties and postgraduates from the Department of Community Medicine coordination with other departments participated in the activity along with the students. The following activities done are cleaning the roads leading to institution, cleaning health system in public places, visit to Sembakkam village, and visit to nearby markets & orphanages and a debriefing session with the CEO of town panchyat. INDEPENDENCE DAY CELEBRATION: The Independence Day Celebrations were conducted on 15th August 2017 at SSSMCRI Campus. Honourable Dean Hoisted the National Flag. International Yoga Day – 2017 was celebrated in a befitting way at SSSMCRI on Wednesday, 21st June 2017. More than 100 staff and students participated in the celebrations and the following activities were conducted in the College Block. The events started with a demonstration and performing of Yoga by the staff and students at 8.00 – 9.00 AM, followed by a Video film on Yoga between 9.00 – 9.30 AM in Ground Floor Lecture Hall. The main event was held between 9.30 -11.00 am. Dean welcomed all the dignitaries and the other participants. An inspiring and motivating speech emphasizing the benefits of Yoga were given by Dr.R.Lavanya & Dr.Kalavathy, Sky World Communication Service Center, Aliyar Chennai. Dr Karthika J Vice Principal, proposed Vote of thanks. DOCTORS DAY ON JULY 1ST , 2017 The Doctors Day was celebrated on 1st July 2017 at the hospital lobby in remembrance of legendary physician Dr B.C Roy. The gathering was conducted in presence of Medical Superintendent, Vice Principal, Faculties, Staffs and Students. The program commenced on with the Master of the ceremony Hoormika (VI Semester) welcoming the gathering. The invocation song was sung by the VI SEMESTER Students. The Vice Principal Dr Balaji Rajagopalan gave the welcome speech. The Doctors Day speech was given by Professor and HOD, Pulmonary Medicine Dr Sundaramoorthy. The Presidential Address was conveyed by Medical Superintendent Dr Vijayalakshmi. Students from every batches displayed posters based on DOCTORS DAY, around 12 posters were displayed. WORLD BREASTFEEDING WEEK – 01.08.2017 TO 07.08.2017 World breastfeeding week was observed at Thiruporur UHTC and Sembakkam RHTC on 1 st week of August 2017 by the Department of Community Medicine. A health talk was given by the Pediatrics Assistant Professor and PG from Pediatrics & Community Medicine, and role-play was conducted by the CRRI’s. Breastfeeding mothers were given awareness and benefits of breastfeeding. This program conducted under the guidance of Assistant Professors from the Department of Community Medicine.

WORLD RABIES DAY-18/9/2017 Theme: “Zero by 30” World Rabies Day was observed at Sembakkam RHTC on September 18 th 2017 by the Department of Community Medicine. A health talk using charts was organized for the welfare of the patients attending the centre. Medicine and the Medical Officer. Participants were explained regarding the Do’s and Don’ts of dog bite, symptoms and clinical features of Rabies, treatment and Pre & Post exposure vaccination. WORLD ORAL HYGIENE DAY- 21/09/2017 World Oral Hygiene day was observed at Sembakkam RHTC on September 21 st 2017 by the Department of Community Medicine. A health talk using charts was organized for the welfare of the patients attending the centre. Health talk using charts were given to the participants by the interns posted at RHTC, under the guidance of Dr. Muthukumar, Assistant Professor, Department of Community Medicine and the Medical Officer. Participants were explained regarding the good and bad practices for oral hygiene and enlightened about the signs and symptoms of oral health problems. The Department of Ophthalmology has conducted 25 camps screeing a total of 903 patients, Total no of cataract surgeries done for camp patients is 27 and the total number of surgeries done is over 81. The Department of Ophthalmology conducted an Eye screening camp on 19 th September at Chengalpattu in association with IDFC.
