Canine/Feline Adoption Application and Pet Ownership Contract The Society appreciates your willingness to adopt a homeless dog or cat. In order to initiate the adoption process, we kindly ask you to complete the following Adoption Application and Pet Ownership Contract. The information you provide will facilitate the society’s endeavor to make successful matches between our homeless pets and your family. VeTerinAry CheCk-UP: Most shelter animals have unknown medical histories, a quarantine period is recommended if you have other pets at home. Also, a visit to your vet is recommended. Although we treat the animals in our facility for intestinal parasites, it is not guaranteed that they do not have intestinal parasites. 30 DAy reTUrn POLiCy: You may return your pet for any reason within 30 days from the date of adoption and elect to receive an Adoption Credit Voucher. The voucher may be redeemed toward the adoption of another animal of your choice. The voucher is valid for 90 days from issuance. Please note: Adoption fees are not refundable Upon redemption of the Animal Credit Voucher, if you select a pet with a: 1. Lesser adoption fee, the difference will not be refundable 2. Greater adoption fee, you will be expected to pay the difference SPAy/neUTer reqUiremenT: Animals adopted from the Humane Society of Grove and Grand Lake are spayed or neutered before they leave our facility. This may delay your pick up date if the animal has not been altered when you fill out the adoption contract. As the new owner, you agree to take full responsibility for the care and well-being of this pet. Once the adoption is finalized, you will be responsible for all medical bills. if for any reason you are unsatisfied with this pet, you may return the animal to our facility.

Statement of Understanding and Agreement: All pets making the transition from the shelter into a new home need time to adjust to a new family and may require housetraining and behavior modification. Information regarding the history, health, and behavior of adopted animals may not be available or accurate.
