Dear Community Members,

for nearly 70 years, the Peninsula Health Care District (PHCD) has fulfilled its commitment to the health of its constituents by building and operating the first Peninsula Hospital (opened in 1954); partnering with Sutter Health to build and operate the new Mills-Peninsula Medical Center (opened in 2011); identifying and addressing health priorities and gaps in needed services today; and planning for future health needs of our constituents tomorrow. As needs and health care delivery models have evolved, so has the District’s role in serving its residents. Our focus in 2016 was to: • Use the countywide 2016 triennial health assessment and other relevant market surveys, trended data and stakeholder feedback to inform PHCD priorities and activities. • Partner with community-based organizations to address current needs. • Identify gaps and collaborate with

stakeholders and clinical experts in developing and implementing a response. • Construct a 124-unit assisted living and memory care senior housing facility (ongoing). • Develop a master plan for the Peninsula Wellness Community - a housing and health services campus west of the Mills-Peninsula Medical Center. Our Commitment To Transparency As a testament to our commitment to ethical and sound business practices, we have been recognized for transparency and good governance by the Special District Leadership Foundation and the Association of California Healthcare Districts.

Access to Basic Health Care and Mental Health Services

1,080 uninsured adult medical/dental clinic visits.

866 low-income children ensured health and dental coverage.

245 children screened for developmental delays. 236 families and children affected by domestic violence received mental health services.

1,545 hours of therapy and assessment services for families affected by homelessness.

2,121 hours of care for youth struggling with drug and alcohol abuse.

5,530 dental care visits provided to Denti-Cal covered children and adults.

Results

77 percent reduction in post-traumatic stress symptoms in domestic abuse clients. 100 percent demonstrated learning at least two new coping skills.

85 percent of children and 90 percent of adults receiving outpatient mental health services reported improvement in mood, anxiety levels, concentration, and focus.

86 percent of clients receiving outpatient mental health services for treatment for substance abuse issues agreed or strongly agreed that they were now able to deal with their problems without substance abuse.

100 percent of uninsured, insulin-dependent diabetic adult clients were provided with glucometer and test strips.

4,000 students, parents and community members received nutrition literacy and education.

Results

1,500 students participated in 10 hours of added physical activity, dramatically impacting their attitudes and habits around health.

630,000 pounds of food delivered to homebound seniors and individuals with disabilities. 9,200 days of Adult Day Health Services. 7,296 senior peer counseling group and individual sessions. 2,313 unannounced visits to assisted living and health care facilities and 1,580 complaints investigated and closed.

93 percent of respondents reported the meal assistance program helped them better manage their health.

4,100 students and community members attended a presentation on suicide prevention and intervention services.

Reduce Health Risks through Education and Prevention

72 percent of seniors who received peer counseling services reported they felt satisfied with life and 75 percent reported feeling hopeful about the future.

93 percent of complaints from residents in assisted living and health care facilities were resolved to the satisfaction of the resident.

3,600 residents utilized the suicide hotline. 80 percent of health education participants reported improvement in overall health and confidence in using exercise to self-manage health problems. 80 percent of clients receiving health assessments were able to maintain normal vital signs and remained compliant with medications and 75 percent of clients showed a decrease in emergency room visits and achieved significant improvements in overall health.

PHCD is addressing health care needs by investing in several important initiatives.

Peninsula Wellness Community is an 8.2acre District development west of the hospital between Trousdale, Marco Polo Way and Davis Drive. The development plan is currently in the environmental impact evaluation phase. A master developer partner will be selected in 2017 and construction could start in 2019. The Community will offer housing for older adults, health support across generations, and working spaces for professionals and researchers.

Teen Mental Health Project is a three-year collaborative with the San Mateo Union High School District and Stanford’s Youth Mental Health and Wellbeing Center to develop and launch a school-based teen mental health service that will identify problems early, get students needed help promptly, and improve coordination between school, providers, and parents. The District committed $1.5 million to this project, which will serve seven high schools and 8,500 students.

The Apple Tree Center for Dental Health is a non-profit dental center that serves all ages, all levels of mobility, and all levels of dental coverage. The Center also has mobile dental care capabilities to serve schools, group homes, and long-term care facilities.

The Trousdale, the District’s 124-unit assisted living and memory care facility is under construction and scheduled to open in early 2018. The Trousdale programs and services will provide supportive, inspired senior living in the heart of the Peninsula.
