Dear Neighbors, Autumn is upon us and it is a busy time in the hospital and in the community. We are preparing for our Annual Season of Giving as well as finalizing our Community Health Needs Assessment. I am inviting you to attend a community meeting on Thursday, November 10 at 6:30 p.m. in the Hurvtiz Board Room. The meeting will include information about our needs assessment, our landscaping updates, our campus improvement plans for 2017, our environmental sustainability projects, our continued efforts to respond to neighborhood concerns, our recent projects with several community agencies and our upcoming Season of Giving. As president of The Miriam Hospital, I remain committed to community relations as we strive to be a good neighbor. A critical piece of community relations is promoting and responding to our Neighborhood Hotline. Since 2005, the hotline has been a resource for neighbors to use when a need arises to share a complaint, concern or comment. Neighbors can use the hotline by calling (401) 793-4040 or emailing TMHneighbors@lifespan. org when the need arises. I encourage all of you to use the hotline whenever you need to tell us about something that is happening in the neighborhood, especially as it relates to our impact on you. I am pleased to announce that The Miriam Hospital will be accepting applications for the 2017 Community Investment Grants. The deadline for the grant application is Friday, November 18. You can learn more about our Community Investment Grants on page 3 of this newsletter. I encourage you to share the grant application with any non-profit or agency that serves the Summit and Mount Hope neighborhoods. Our strategic efforts in helping youth in grades 6 through 12, improving community health, and revitalizing the community are all categories of relevance for the grants. Since 2006, we have invested over $650,000 in our community and we are proud to continue this legacy of delivering health with care. The Miriam Hospital recently completed a Community Health Needs Assessments. In partnership with the Lifespan Community Health Institute, The Miriam Hospital held six forums in the community this past summer, in community-based locations such as The Rochambeau Library, Olney Street Baptist Church, and Laurel Mead. The forums gave us an opportunity to hear from you about your top health concerns. Six forums were organized for The Miriam Hospital with the help of four community liaisons. Thank you to each person who participated in our community forums. Come to our community meeting on November 10 at 6:30 p.m. to learn more about the Needs Assessment as well as other Miriam Hospital happenings. On August 27, we participated in the annual Mount Hope Billy Taylor Day. The Mount Hope Neighborhood Association is a recipient of The Miriam Hospital Community Investment Grant and each summer holds a summer basketball youth league. The culmination of the league is Billy Taylor Day, and this year five of our physicians formed a team to compete against the top neighborhood players in the Billy Taylor Day games. The doctors played their best game but lost to the youth team 49 to 47. After the game, the doctors talked with the youth and their families about health, fitness, and any question the community had for them. We are grateful to the Mount Hope Neighborhood Association for the opportunity to play and we hope to be back next year. The Miriam Hospital Season of Giving will soon be here. Did you know that our employees actively give back to the community every year during this campaign? In November 2015, our employees donated over 300 coats to support the annual “Buy Nothing Day.” The next month, employees generously donated over 300 gifts to share with children living on or below the poverty line in the Mount Hope Neighborhood. As the 2016 Season of Giving kicks off, we invite our next-door neighbors to join us in the spirit of helping neighbors in need. From donating gently used coats for the annual Buy Nothing Day, to volunteering with our gingerbread kits or through buying a gift for a child, we invite you to join us in delivering care to the community. I am pleased to announce that The Miriam Hospital Community Investment Grant funding cycle for 2017 is now receiving applications. Our focus areas for investing include programs for youth development and education in grades 6 through 12, health and wellness programming, plus community environmental improvement and community revitalization projects. Please see page 3 for a more detailed list of applicable categories available for funding. The deadline for submitting a grant application is Friday, November 18 at 5 p.m. I am available to meet with potential grant requestors on Mondays, Wednesdays and Fridays. Environmental and beautification projects benefiting the development of the community Please write a brief cover letter requesting consideration for a Miriam Hospital Community Affairs Grant. Briefly outline the focus area for which your request will be used. Please state in your letter that you will complete an evaluation within 30 days of the conclusion of your project. All requests must include a detailed budget. Requested grant monies are to be utilized for costs of the program with no more than 20% going to paying for staffing or general overhead costs.

Please summarize the mission and focus of your organization and project. State the amount of the donation request (no more than $12,000 per organization). Please outline the proposed use of contribution; explain the goals of your community project. When available, include your organization’s 501c (3) documentation. Include the following information where applicable: Federal Tax ID Number: Please estimate how many individuals your project will indirectly and directly serve. Please define the issues that the project will affect. Provide supporting documentation and statistics, which factually demonstrate the issue as a measurable community need. Describe how the money donated will impact the community. Please outline the measurable achievements and benchmarks of your program. Provide a targeted timeline for your project. (Please note that all organizations requesting funding of any kind from the Community Affairs Committee must fill out the request form at least 90 days prior to event, or monetary need.) List by agency name any other partners in this project and provide signed memoranda of agreement from all partners. Please indicate all financial and “in-kind” donations partners. Briefly explain how you will promote your program to the community as well as acknowledge The Miriam Hospital as a community partner with regard to the funding of this grant. If your request for funding is accepted, you will be required to send us an acknowledgment letter for the receipt of the funds within 30 days of receipt. You will also need to provide us with an invoice for the funds so that we can process your payment. Please note that processing can take up to 21 days. Please include the following items with your contribution request: • Formal proposal on organization’s letterhead • IRS determination letter of 501 (c)(3) status • W9 tax form • Board of Directors listing • Proposed budget for project (no more than 20% of which can be allotted for staffing costs) • Staffing needs for project

*Note that if you received grant funding the previous year, a final report for the outcomes and measurements of the success of your program must be submitted by November 4, 2016. Hope or Hope neighborhoods.
