da Vinci Robotically Assisted Surgery, now at LMH

T

o provide a full range of surgical options, Lawrence Memorial Hospital (LMH) is now home to the da Vinci Surgical System®. By combining computer and robotic technologies, the da Vinci robot provides assistance to surgeons in ways that ultimately benefit patients, often resulting in faster recovery times and less post-operative discomfort. That’s because robotically assisted surgery requires only a few small incisions (1-2 centimeters) – and in some cases, only one incision. It also provides enhanced visualization and control for surgeons, making some hard-to-reach areas more accessible than with traditional laparoscopy. With da Vinci, the surgeon operates from a nearby console, while highly sophisticated software translates the surgeon’s hand 4

movements to the robot’s miniaturized instruments at the bedside. The da Vinci robot cannot be programmed to perform any surgical maneuvers on its own, but the software provides automatic motion scaling and tremor reduction to improve precision and dexterity. “The da Vinci robot is amazing technology,” says Chad Tate, MD, board-certified general surgeon who performs da Vinci procedures at LMH. “It has very intuitive control and more range of freedom than the human wrist. Everything is magnified to a great extent, which allows for very precise control of surgical instruments.” Tiffinie Mercado, MD, board-certified OBGYN physician who also performs da Vinci surgery at LMH, agrees, adding that the system’s enhanced visualization can be advantageous to surgeons and patients alike. “With that level

of magnification comes improved clarity of the tissues, which can enable better outcomes in many cases,” she explains. “I had very little pain – I felt great almost immediately!� says Karen Dixon, 58, recalling her experience with da Vinci surgery at LMH in early June 2013. “In fact, a lot of physicians and nurses who saw me the next day were amazed that I was doing so well right after major surgery.� Due to a recurring pancreatic pseudocyst, Dixon needed a Roux-en-Y cystojejunostomy – a procedure for placing a permanent drain between the cyst and her intestine. Traditional laparoscopy and open surgery were among her treatment options, but Dixon’s surgeon, Dr. Chad Tate, strongly recommended da Vinci robotic surgery. “It was clear he had great confidence in da Vinci, and I had great confidence in him,� Dixon says. “So in the end, it was an easy decision. da Vinci offered my best chances at a fast recovery – and that’s exactly what it delivered for me!� Dixon’s procedure required five small incisions, and all were healed almost completely within 10 days of surgery. While open surgery would have resulted in a week-long hospital stay, Dixon spent only two nights at LMH after da Vinci surgery and was able to resume all her normal activities at home very quickly. “I feel fantastic!� she says. “Based on my own experience as a patient, I would definitely recommend da Vinci surgery to my family and friends.�

Kim says that her knowledge of exercise and nutrition has helped, but she just needed the motivation. “Motivation for me is doing work for charities that I love,” says Murphree.

3. If the decision has been made to not continue as you are, then surgery will be needed. The options of surgery are: A. Realignment of the joint to take more weight on the good portion. This is done by breaking the bones and realigning them. This is a good procedure in the younger population, usually age 45 and below. It is a hard operation to go through, and only gives partial pain relief. B. Simple debridement, or so-called arthroscopy of the joint to smooth out the rough spots and take away some of the arthritis. The short-term relief of this procedure sometimes is good, but the long-term is very poor. This would only be offered to you if you had a very mild case of

There are a number of alternatives for your arthritic problem. They are generally speaking, as follows: 1. Tolerate the problem as you have. Toleration means using every kind of method of making your knee feeling better, including anti-arthritic medications, pain pills, heating pads, walking aids, limitation of activities, rub on lotions, etc. 2. Injections: As the first type of injection, usually cortisone injection to see how that works. If not beneficial, then a trial series of what’s called viscosupplementation, a lubricant of sorts. It benefits 50% of people.

Complications include: 1. Infection - This is by far the most serious complication. Antibiotics and sterile techniques are used to prevent this. Infections can occur after the surgery, but also can occur late. This happens when bacteria get into the blood stream and land in the operated area. Although rare, it is very serious, •

The main objective of total knee replacement is pain relief. and could ruin the joint. If you have infection present anywhere in your body, you should be on antibiotics after having had a total joint replacement. Certainly before any dental surgery or procedure is performed, we strongly recommend prophylactic antibiotics be prescribed and continued afterwards. Other problems, like prostate operations or infections around the feet should be treated with antibiotics. 2. Phlebitis is something that anyone can get after any surgery. There is a greater tendency after extremity surgery and, in order to prevent this, sequential compression devices will be put on your leg the first 48 to 72 hours after surgery. 3. Lack of range of motion and need for a manipulation Sometimes some patients have difficulty in getting their bending of the knee back. On rare occasion, manipulation needs to be carried out to get that. Range of motion from zero to 105 degrees would be considered more than adequate. You will not get full range of motion back in your knee. 4. There is a possibility of the components loosening and needing to be replaced. This is now becoming a less frequent problem. However, it may occur, and can even occur within the first year. Patients are usually admitted the day before the operation, and stay in from 10 to 14 days on average. Anesthesia given may be general or spinal anesthesia, the patient having his/her choice. The operation lasts approximately two hours. Post-operatively the knee hurts, but narcotics are given for pain relief. Intravenous medication is usually continued for approximately 48 hours. After having a total knee, one should not run or jump, but can engage in swimming, golfing, riding a bike, etc. The ultimate durability of these joints is not known. The main objective of total knee replacement is pain relief. If you do not consider your pain serious, you should not go through the operation. If you do feel the pain is affecting the enjoyment of your life and everyday living, then you should consider the operation. It frequently boils down to that question, which the patient needs to ask himself. •

Chances are you’ve been down the weight loss road before. And chances are just as good that you’ve gained it all back. But don’t despair; there are ways to avoid the dreaded yo-yo. Harley Pasternak, best-selling author and the go-to trainer for A-list stars, answers some common questions about how to lose weight – and the tools needed to keep it off. What’s the biggest mistake people make when it comes to weight loss? Too many people set unrealistic expectations from the start. Extreme diets don’t work. You can’t sustain that kind of deprivation over the long haul, and you’re setting yourself up for failure. For sustained weight loss, you need a diet and exercise plan that you can maintain. It starts with setting realistic goals. Check with your doctor first, but for most people, losing one to two pounds a week makes sense. How should I get started and pick the right plan? Instead of starving yourself or risking injury by over-exercising, focus on being active throughout the day. Park your car farther away from your destination, take the stairs, make social plans walking distance from home or work; every step adds up. When deciding on a diet plan, do your research. How can I eat healthy when I’m so busy I don’t have time to cook? Planning ahead is a must if you’re going to avoid temptation. Spend some time on Sunday evening preparing simple, healthy meals for the week. Keep staple ingredients like fruits, veggies, Greek yogurt, beans and whole grains like oats and quinoa at home at all times. What tips or tools I should use to keep myself on-track and motivated? I’m a big fan of using technology to stay fit. I like mobile apps, which help track calories and exercise. They take the guesswork out of losing – and make it more fun because you can get instant support. I hate to exercise, and gyms are too expensive. What can I do? Protect Young Eyes in the Technology Age

Whether it’s a tablet with an educational purpose or a big screen displaying the latest video game, the use of electronic technology is skyrocketing among kids. In fact, according to the Kaiser Family Foundation, children ages eight to 18 spend more than seven and a half hours with electronics every day. Unfortunately, all of that screen time can cause eye fatigue, and ultimately have an impact on your child’s overall vision and eye health. To view things closer, our eyes automatically adjust by drawing inward; our pupils get smaller to focus, and our eye muscles adjust so we can see a clear image. As a result, extended use of electronic screens can cause tired, blurry or irritated eyes. Intense focus on a video screen also leads to a diminished blink rate, which can result in eye injuries. Although there is no scientific evidence that computers and handheld electronic devices

directly cause vision problems, using these devices wisely can help prevent eye fatigue and strain, as well as associated headaches, blurred vision and dry eyes. To help protect your child’s vision, consider these tips from Ameritas, a leading provider of dental, vision and hearing care plans: • Know that prolonged use of electronic devices can exacerbate underlying eye conditions, so electronics should be used in moderation. Limit screen time to two hours or less a day (including watching TV, playing video games and using mobile phones). • Encourage intentional blinking while electronic devices are in use to help refresh eyes with natural moisture that helps prevent bacterial infections, dry spots and corneal breakdown. • Reduce additional eye strain by managing glare from windows and using low-watt bulbs in light fixtures.

• Keep computer screens 20 to 28 inches away from the face. • Practice a rule of 20s to give eyes a rest. Every 20 minutes, ask your child to look at least 20 feet away for 20 seconds before refocusing attention up close again. • Move around and change positions periodically while using a device. • Watch for signs of eyestrain while electronic devices are in use, such as squinting, frowning at the screen or rubbing eyes. • If vision problems or discomfort arise, schedule an appointment with an eye doctor for a professional evaluation. When taking into account time at the office in front of a computer screen, many adults regularly use electronic devices for as long as, or even longer than, their children. Following the same advice not only sets a good example, but it can help protect your own eye health.
