Bringing Back the Family Meal Why gathering around the dinner table can be important to your kids’ health Our modern grab-and-go culture has made family meals less common. But a recent study has shown a link between the number of meals children and teens share with their families at home each week and their nutritional health. Kids who eat at home with their family at least three times a week are 12 percent less likely to be overweight, 24 percent less likely to eat unhealthy foods and 35 percent less likely to develop an eating disorder. The upshot? Shared meals don’t just support a healthy family dynamic—they can also make kids healthier.

Get the 4-1-1 on Aortic Stenosis Pioneering treatment at John Muir Health avoids open-heart surgery THE PROBLEM • What aortic stenosis does to the heart: Wears it out. Severe narrowing or obstruction (stenosis) of the aortic valve in the heart forces the heart to work harder to push blood through the valve and on to the aorta, the largest artery in the body. Eventually, the heart’s muscles weaken, increasing the person’s risk of heart failure. • What it feels like: Severe shortness of breath, chest pain, fainting/dizziness, fatigue, rapid/irregular heartbeat. • Who it strikes: Maybe you or a loved one. About 1.5 million Americans have aortic stenosis; most are elderly. TREATMENTS • What TAVR is and how it works: Transcatheter aortic valve replacement (TAVR) is a remarkable new procedure now available at John Muir Health. Doctors replace

About Heartburn, GERD and Cancer Learn more in this Q&A with Dr. Wilson Tsai, co-director of John Muir Health’s Thoracic Surgery Program

What, exactly, is GERD? WT: GERD [gastroesophageal reflux disease] is a condition in which contents of the stomach, including stomach acid, backwash into the food pipe or esophagus. This problem, also called acid reflux, can irritate and damage the esophagus if it continues. Heartburn is one symptom of GERD. Other symptoms, which can come without heartburn, include problems swallowing, shortness of breath, voice changes, bronchitis or frequent pneumonia. What are some of GERD’s risk factors? WT: Obesity is a big one. It’s on the rise in this country and so is GERD. Another big risk factor is a hiatal hernia, a condition in which the upper part of the stomach bulges through an opening in the muscle that separates the stomach from the chest. About 95 percent of people with a hiatal hernia also have GERD.

Is there a link between GERD and cancer? WT: Yes. Esophageal cancer is the fastestgrowing cancer in the United States, and it’s directly related to the increase in GERD. Essentially, the esophagus isn’t designed to handle the acidic environment of the stomach, which is why it’s important that GERD gets treated. What are common treatments for GERD at John Muir Health? WT: The first step typically involves lifestyle changes, such as stopping smoking and drinking alcohol, avoiding certain foods and drinks, and maintaining a healthy weight. If symptoms persist, various drug therapies may be undertaken. If neither of these gets good results, surgery may be recommended. With new, minimally invasive surgery, the surgeon repairs tissue in the abdomen through several tiny incisions.

When should a doctor be consulted about GERD symptoms? WT: If acid reflux persists for one or two months or there are some of the other symptoms—such as bronchitis or recurrent pneumonia. The most important thing to know is that John Muir Health can treat all aspects of this condition, from cancer causing to benign reflux.

Avoiding Heatstroke

For athletes—and everyone else—prevention is the best treatment As medical director of John Muir Health’s Urgent Care Centers, Dr. Jesus Saucedo has seen his share of the bumps and bruises endured by athletic types. “I see a wide range of patients—weekend warriors, young kids, college students,” he says, “and very often, their injuries stem from not knowing enough about the sport and what’s involved. Once they’re hurt, they may not know how to keep things from getting worse or when they should see a doctor— their own or one of the doctors at our Urgent Care Centers.” To help you avoid this situation, check the following guide to some common sports-related injuries and remedies, based on Saucedo’s advice.

Ankle Sprain • Details about the injury: Ankle sprains often occur during sporting activities or when walking or running on an uneven surface. The foot can turn inward, stretching or tearing the weak ligaments (connective tissues that join the end of one bone to another) on the outside of the ankle. • Home remedies: Use the RICE (rest, ice, compression and elevation) method to relieve pain, reduce swelling and speed healing. • When to visit your doctor or an Urgent Care Center: Consult a doctor if the injury causes severe pain or swelling or you can’t tolerate any weight on the area.

Heatstroke can affect anyone. It’s caused by prolonged exposure to high temperatures or by doing physical activities in hot weather. Left untreated, it can lead to injury to your muscles, kidneys, heart and brain—and even death. To avoid heatstroke, experts advise taking the following precautions: • Before you head outside, wear light-colored, lightweight, loose-fitting clothing and a widebrimmed hat. • Use sunscreen with a sun protection factor (SPF) of 30 or more. • To prevent dehydration, drink extra fluids. The general recommendation is to drink 24 ounces of fluid two hours before exercise; right before exercise, consider adding another 8 ounces of water or sports drink. During exercise, you should drink another 8 ounces of water every 20 minutes, even if you don’t feel thirsty. • Cancel or reschedule outdoor activity. If possible, spend time outdoors during the coolest hours of the day—in early morning or after sunset.

Shin Splints • Details about the injury: A shin splint is the pain you feel along or just behind the tibia bone in front of your lower leg. What’s the cause? Too much force on your shinbone and connective tissues that attach your muscles to the bone. Shin splints are common in runners and in people who play games with sudden stops and starts, such as basketball, soccer or tennis. • Home remedies: Rest your body so underlying tissues can heal. To reduce the pain and swelling, ice the shin as frequently as possible. When you feel up to exercising and the pain is gone, increase your

activity level slowly. If the pain returns, stop exercising right away. • When to visit your doctor or an Urgent Care Center: If you’re still in pain, even with rest; you aren’t sure whether your pain is caused by shin splints; or you don’t improve with home treatment; and/or you think you may have a broken bone.

Tennis Elbow • Details about the injury: Tennis elbow is an overuse injury of the arm, forearm and hand muscles that results in elbow pain. FYI: It’s called this because tennis players often get this injury, but you don’t have to play the game to get it. • Home remedies: Rest your fingers, wrist and forearm muscles to allow your body to heal. Apply ice when you first feel pain. Do simple warm-up and stretching exercises to keep your tendons (connective tissue) from getting stiff. If you have any pain, stop the exercises.

• When to visit your doctor or an Urgent Care Center: If you hurt your elbow, make sure to consult a doctor immediately if you have severe elbow pain; you cannot move your elbow normally; your elbow looks deformed or bruised; your elbow begins to swell; you have signs of nerve injury; you feel numbness, tingling or weakness in the elbow; or the injured arm feels colder to the touch than the uninjured one.

Backache • Details about the injury: A common cause of this nagging pain is the twisting motion in golf, lifting weights, playing tennis. • Home remedies: Apply ice; rest; no overuse. • When to visit your doctor or an Urgent Care Center: If you have nerve injury, numbness, tingling sensation or searing pain in the back. Go immediately to the emergency room if you have bowel or bladder incontinence, worsening pain or swelling.

How to

Raise a Happy Teen Following the 5 Cs can make life a whole lot easier for offspring—and parents

O

n the millionth time you’ve told your teen to take out the trash and the trillionth time she’s rolled her eyes, you may forget how much is going on in that growing body of hers. “Lots of changes happen simultaneously in adolescence—hormonal and other biological changes, changing neurocircuitry in the brain, not to mention all the social and psychological turmoil,” says Dr. Yanni Rho. To help teens navigate that uncertain territory, Rho and Dr. Arman Danielyan, who are child and adolescent psychiatrists at John Muir Health, offer the following advice to parents.

COMMUNICATE. “It’s really important for parents to communicate with their teen; it has to go both ways,” says Rho, who is medical director of John Muir Health’s Adolescent Outpatient Program. “The same goes with respect: If parents are asking for respect from teens, they also need to give it back.” BE CURIOUS. “Teens appreciate your trying to understand what makes them tick,” says Rho, “It’s important not to impose upon them who you think they are and what’s important to them.” CHERISH. “Positively reinforce thoughts and behaviors you appreciate

and want to see more of,” says Rho. BE CLEAR. “Setting clear rules and expectations is essential,” says Danielyan, “but they should always be based on what’s appropriate for the specific child.” CONSULT. Consider getting professional help if you have concerns about your child’s mood or behaviors. Most of the psychiatric illnesses in adults originate in childhood or adolescent years. “If there’s no illness, no one will treat anything and that’s great,” says Danielyan. “But if there are signs of mental illness in your child, then at least you can be counseled about

the options available for intervention. Remember, studies show that for many childhood mental health problems, the earlier you start treatment, the better the chances are the child will get better—and sooner.”
