ภาวะปากแห้ง (Xerostomia) เป็นอาการผิดปกติของการทำ�หน้าที่ ของต่อมน้ำ�ลาย ทำ�ให้น้ำ�ลายออกมาน้อยเกินไป มีทั้งแบบถาวรหรือ ชนิดหายกลับเป็นปกติได้ เกิดได้จากหลายสาเหตุ เช่น การขาดนํ้า เนื่องจากสูญเสียนํ้าจากร่างกายมากเกินไป หรือความเครียดทำ�ให้ ต่อมน้ำ�ลายผลิตน้ำ�ลายน้อยลง นอกจากนี้ยังพบว่าอาจเกิดจากการ รับประทานยาบางชนิด จำ�พวกยากดประสาทและยาลดความดัน โลหิต ที่พบบ่อยที่สุดคือผู้ป่วยที่ได้รับผลข้างเคียงจากการบำ�บัดด้วย รังสีบริเวณใบหน้าและลำ�คอ โดยเฉพาะผูป้ ว่ ยมะเร็งริมฝีปาก ช่องปาก และคอหอย ส่งผลให้เกิดการฝ่อลีบของเนือ้ เยือ่ ต่อมนํา้ ลายและทำ�ให้ ความสามารถในการผลิตน้ำ�ลายลดลงอย่างมาก ผู้ป่วยที่อยู่ในภาวะ นี้มักมีอาการปากแห้ง การหล่อลื่นในช่องปากลดน้อยลง เกิดแผลใน ปากได้ง่าย การรับรสเปลี่ยนไป และเสี่ยงต่อการเกิดฟันผุ สิ่งเหล่านี้ ล้วนส่งผลให้ผปู้ ว่ ยมีคณ ุ ภาพชีวติ ทีไ่ ม่ดี รับประทานอาหารไม่ได้ เกิด ภาวะทุพโภชนาการ จากสถิตขิ องสมาคมรังสีรกั ษาและมะเร็งวิทยาแห่งประเทศไทย พบว่ามีผปู้ ว่ ยโรคมะเร็งริมฝีปาก ช่องปากและคอหอย มารับการรักษา ด้วยรังสีในแต่ละปีเป็นจำ�นวนมาก ซึง่ มีผลข้างเคียงคืออาการปากแห้ง นอกจากนี้ ประชากรสูงอายุของประเทศส่วนใหญ่ได้รบั การรักษาโดย ยาควบคุมความดันโลหิต เบาหวาน และโรคอืน่ ๆ ซึง่ ก่อให้เกิดอาการ ปากแห้งได้เป็นจำ�นวนมาก การรักษาภาวะปากแห้งนัน้ เป็นการให้การ รักษาตามอาการ ได้แก่ ให้ผู้ป่วยจิบน้ำ�บ่อยๆ ให้ยากระตุ้นการไหล ของน้ำ�ลาย หรือให้สารทดแทนนํ้าลาย อย่างไรก็ตาม การรักษาใน ปัจจุบนั ยังคงมีขอ้ จำ�กัดเรือ่ งผลข้างเคียงจากการใช้ยากระตุน้ การหลัง่ ของนํ้าลาย รวมทั้งสารทดแทนนํ้าลายในปัจจุบันนั้นยังคงมีราคาสูง น้ำ�ลายเทียมจากธรรมชาติ น้ำ�ลายเทียมจะมีส่วนประกอบ ได้แก่ บัฟเฟอร์ อนุพันธ์ของ เซลลูโลส (เพือ่ เพิม่ ความเหนียวและความสามารถในการให้ความชุม่ ชื้น) และสารให้กลิ่นรส (เช่น ซอร์บทิ อล) อย่างไรก็ตาม น้ำ�ลายเทียม 28

ry mouth or xerostomia is resulted from an inadequate flow of saliva. The illness could be chronic or curable. It is caused by dehydration, or stress that reduces salivary gland’s activities. In addition, it could be side effects of several drugs such as sedative and anti-hypertensive drugs. The most frequent causes are side effects of radiation therapy on faces and throats, particularly patients who suffer with lip cancer, oral cancer and esophageal cancer. Radiation leads atrophy of salivary gland tissue and tremendously reduces saliva production. Patients suffer with dry mouth have lesser oral lubrication, and are highly susceptible to oral ulcer. They taste less, and have higher risk of tooth decay. Patients also have low quality of living. In addition, unable to eat leads malnutrition. According to studies of the Thai Society of Therapeutic Radiology and Oncology, many patients who suffer from lip cancer, oral cancer and esophageal cancer had radiotherapy. The therapy’s side effect is dry mouth. In addition, many elder populations have hypertension drug, diabetes drug, and others which cause dry mouth. They can be treated by just symptomatic treatments such as drinking very often, having drugs that stimulate saliva secretion, or using saliva substitutes. However, treating of dry mouth is restricted because side effects of saliva-stimulate drugs, and saliva substitutes are costly. Natural Essences as Artificial Saliva Artificial saliva normally contains a mixture of buffering agents, cellulose derivatives (to increase stickiness and moistening ability) and flavoring agents (such as sorbitol).
