What is palliative care? Palliative care is specialized care for patients living with serious illnesses. It is patient-centered and focused on providing relief from the physical and emotional symptoms often associated with disease progression. Palliative care can be delivered in conjunction with curative care and treatment plans are created to match the goals of care expressed by the patient and family, regardless of the stage of disease. Using a multi-disciplinary approach, our specially trained team aims to improve quality of life for the patient, caregiver and family.
What makes us different? ProMedica Palliative Care is dedicated to improving quality of life by assisting individuals who need assistance with a serious, progressive illness. By drawing on the extensive experience of our interdisciplinary professionals, the ProMedica Palliative Care team can help minimize the impact of illnesses such as lung disease, heart disease, cancer, chronic kidney disease, dementia, vascular disease and diabetes. Working with your own medical provider, we provide an extra layer of education and support. We begin by learning about you and about what is important to you right now and as your illness progresses. In this way, together, we can develop a personalized plan of care focused on your individualized goals. Symptoms that may benefit from palliative care: • Pain • Shortness of breath • Constipation • Diarrhea • Nausea • Loss of appetite • Fatigue • Difficulty sleeping • Anxiety and/or Depression • Increased confusion At ProMedica Palliative Care, we may also assist with: • Education about and completion of Advance Directives • Coordination across various care settings (transitions of care) • Disease specific education • Emotional/Spiritual support • Access to local community resources
