Franklin Community Health Network Franklin Community Health Network is a locally controlled, nonprofit, integrated network of rural health care providers, created by Franklin Memorial Hospital in 1991. Its affiliates include Franklin Memorial Hospital, Franklin Health, Evergreen Behavioral Services, Healthy Community Coalition of Greater Franklin County, and NorthStar EMS. Franklin Memorial Hospital is a progressive, not-for-profit community hospital whose mission is to provide high quality, costeffective, patient- and family-centered health care. The hospital actively works with the community and with other health care providers integrating services to ensure the optimum health of people within its service area. Franklin Health is a multi-specialty group practice sponsored by Franklin Memorial Hospital that includes nine medical and surgical practices. Franklin Health is the largest provider of health care services in the region. The practices are linked together by technology, an integrated medical record, and a common concept of patient- and family-centered care. Evergreen Behavioral Services provides 24/7 emergency mental health response and community outreach services in Greater Franklin County. Its crisis team helps both children and adults stabilize a crisis by tailoring interventions to each person’s unique needs, strengths, and situation.

Healthy Community Coalition of Greater Franklin County has a mission to measurably improve the well-being of all people using a coordinated public heath approach of education, health promotion, and outreach. Healthy Community Coalition offers health screenings, health information, programs and events that prevent disease and improve quality of life. Its community outreach efforts appear in every town and corner of the region. NorthStar is the regional ambulance service of Greater Franklin County. NorthStar follows its mission of providing respectful and excellent patient care, positive community activities, and good stewardship of resources. NorthStar responds to more than 5,000 calls per year in 71 communities spread over 2,800 square miles. With state-ofthe-art equipment and ambulances, the service is ready, responsive, and reliable.

Franklin Community Health Network has grown exponentially in the range and sophistication of services offered since its inception. Services are integrated to achieve a medical home model that supports population-based health care and focuses on prevention, primary care, mental health services, and care management.

A message from the president This annual report highlights our accomplishments over the past year toward the vision of Franklin Community Health Network “being the best we can be.” Inside you will read about significant accomplishments that demonstrate our values of pride, innovation, caring, and excellence in the work we do every day. Indeed we have much to be proud of. This past fiscal year was a pivotal year for our organization. Throughout the year, the Franklin Community Health Network board of directors engaged in a process to analyze strategic options for the network’s future and concluded that membership in the MaineHealth system would assure the delivery and quality of health care in the greater Franklin County area. The board believes that to be successful in the future, to offer the kind of high-quality care that patients expect, to expand access to wellness and population health promotion in a rapidly changing health care reform environment, community hospitals like Franklin Memorial cannot go it alone. This decision to become a member of MaineHealth best positions our organization to assure the provision of health care services for our communities for the next generation. We anticipate that our membership into the MaineHealth system will be finalized by October 1, 2014. This past year brought a welcome surprise when more than $617,000 was received from the Estate of S. Paul Washburn, a resident of Livermore Falls for most of his adult life, and beloved supporter of Franklin Community Health Network and a former hospital board member. In recognition of his support, the Androscoggin Valley Medical Arts Center was dedicated in his memory on August 28, 2014. See more information on this donor inside on pages 18–19. In closing, this annual report is a tribute to our health system employees, physicians, nurses, and community members who volunteer and contribute to the good work of Franklin Community Health Network. I would also like to extend a special recognition to the Franklin Community Health Network board of directors who have volunteered hundreds of hours and immeasurable expertise in strategic planning this past year to assure the success of the organization in the future.

Introducing our newest providers Benoit Blondeau, MD General Surgeon Dr. Blondeau completed a fellowship in hepato-biliary surgery and multi-organ transplantation at Recanati/Miller Transplant Institute at Mount Sinai Hospital in New York. Prior to that he completed a fellowship in vascular medicine from CHU Tours in France. Dr. Blondeau completed his residency in general surgery at University of Missouri in Kansas City after receiving his medical degree at University Francois Rabelais in Tours France. He is board certified by the American Board of Surgery and holds certification in hospice and palliative medicine. Dr. Bloundeau holds a master’s degree in business administration.

Nicole Donahue, CPNP pediatric Nurse Practioner Nocole Donahue is a nurse practitioner who received her master’s degree in pediatric nurse practitioner studies from Purdue University in West Lafayette, Indiana. She earned her bachelor’s degree in nursing from the University of Maine. Besides providing primary care, Donahue has worked as a pediatric charge nurse at the Barbara Bush Children’s Hospital at Maine Medical Center. She has also served as an adjunct lecturer in undergraduate pediatrics at the University of New England.

Gregory Emmons, MD Oncologist/Hematologist Dr. Emmons is board certified in internal medicine by the American Board of Internal Medicine and holds a specialty certification in medical oncology and hematology. He earned his medical degree from the University of New England College of Osteopathic Medicine in Biddeford. He then completed his residency in internal medicine at Maine Medical Center in Portland and a dual fellowship in hematology and oncology at Baystate Medical Center, the western campus of Tufts University School of Medicine in Springfield, Massachusetts.

Our health care practitioners listen to and honor patient and family perspectives and choices. Patient and family knowledge, values, beliefs, and cultural backgrounds are incorporated into the planning and delivery of care.


We are advancing the practice of patient- and family-centered care and creating partnerships in all health care settings. Oscar Fischer, MD Anesthesiologist Dr. Fischer received his medical degree from the University of Illinois at Chicago College of Medicine. While in medical school he worked as a neuroscience researcher designing surgical techniques and instruments for retrograde mapping of the 8th cranial nerve. He remained in Chicago for his anesthesia residency. Most recently Dr. Fischer provided a broad range of anesthesia services at Twin County Regional Hospital in Galax, Virginia.

Paul Frey, MD, MPH Cardiologist Dr. Frey is certified by the American Board of Internal Medicine with subspecialties in internal medicine, cardiology, and interventional cardiology. He received his medical degree from the University of Massachusetts Medical School, followed by a residency in Internal Medicine at Johns Hopkins Hospital. Dr. Frey has completed Fellowships in Cardiology and Interventional Cardiology.

Richard Renzi, MD Emergency Medicine Dr. Renzi received his medical degree from the University of Bologna in Italy. He completed his internal medicine residency and later a Fellowship in critical care medicine at Miriam Hospital in Providence, Rhode Island. In 2008, Dr. Renzi was named as one of Rhode Island’s Top Docs by the Rhode Island Monthly publication. In 2007 he was honored by the Consumer Research Council of America as one of America’s Top Emergency Medicine Physicians.

Our health care practitioners communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete and accurate information in order to effectively participate in care and decision-making.

The Inpatient Experience

P

atients at Franklin Memorial Hospital receive care from our highly qualified medical/surgical team that includes doctors, a physician assistant, nurses, nursing and medical students, pharmacists, physical therapists, respiratory therapists, medical specialists, a care manager, dietician, and others who help make our patients’ experiences positive, comfortable, and healing. Single rooms offer patients privacy and quiet and are equipped with white boards to enhance communication among patients, families, and health care providers. Amenities include a private bathroom, telephone, television, and wireless Internet access. Hospitalists take care of patients from admission through discharge. They are specially trained physicians that care for patients during a hospital stay. The hospitalist staff remains in contact with each patient’s primary care clinician, sending regular updates and asking for additional information as needed.

Nearly 40 hospitalbased registered nurses hold advanced certifications in at least one of the following areas: emergency critical care, medical/surgical care, oncology, chronic disease management, critical care, wound care, and diabetes education, among others. Each of these certifications requires years of clinical practice in the specialty, the completion of continuing education credits, and passing rigorous exams. Doctors and nurses, together with a physical therapist, respiratory therapist, dietician, and other care team members meet every morning to discuss discharge plans to ensure that patients have the services they need when they leave our hospital. A care manager arranges for these services for patients such as home care visits, rehabilitation in nursing care facilities, and continued physical therapy.

Living Our Values: pride Caregiver of the Year In May, Rebecca Gagnon-Pillsbury MSPT, ATC, CLT was named our Caregiver of the Year. This annual award honors a caregiver who on a daily basis demonstrates extraordinary commitment to the delivery of care to patients. Rebecca Gagnon-Pillsbury Rebecca is a highly skilled physical therapist with unique expertise in lymphedema treatment. Her patients have survived cancer and now deal with longstanding complicated diagnoses. Rebecca is caring, considerate, and confident with these patients who are usually seen in two-hour sessions, requiring extensive hands-on manual lymph drainage for the entire treatment. EPIC Award Established This past year the EPIC awards were developed by the Spirit Committee as a way to recognize employees who demonstrate on a daily basis their commitment to FCHN’s values of Excellence, Pride, Innovation and Caring. Four recipients are selected each quarter from the peer-submitted nominations and presented with trophies and monetary gifts at a board meeting. Each also has a designated parking spot of his choice for the next three months. In addition, each quarter all nominees and those who nominated an individual are invited to a reception where certificates are awarded to runners-up, and winners are announced.

Excellence in Nursing Award Kristen Welch was the recipient of the organization’s third annual Excellence in Nursing Award. Nursing leaders and peers nominated nine individuals for the award that was established to recognize outstanding contributions by nurses who support the advancement of nursing. Kristen was cited for being an extraordinary person and clinician who epitomizes nursing at its best. She is highly skilled and self motivated, consistently looking for the next challenge while always displaying the highest of standards. Welch has worked in multiple hospital departments and made contributions to patient care excellence in each. Arsenault Installed as MHA Board Chair Rebecca Arsenault was installed as chair of the Maine Hospital Association Board of Directors on June 18 at the association’s Summer Forum at the Samoset Resort in Rockport. As chair, her primary duties are leading the governance process of the association. Arsenault who has been president and CEO for Franklin Community Health Network and Franklin Memorial Hospital since January 2009 succeeds outgoing chair Michelle Hood, president and CEO of Eastern Maine Healthcare Systems.

Pride Stefanelli Achieves Certification in Health care Compliance In December, Regina Stefanelli successfully completed the Compliance Certification Board’s (CCB) Certified in Healthcare Compliance examination, thus earning the “CHC” designation. As compliance officer Regina oversees that clinical and business Regina Stefanelli services are delivered with integrity and in conformance with regulatory, accreditation, and ethical standards. This involves developing standards, coordinating compliance training and education, conducting or arranging internal audits, identifying compliance issues and trends, and investigating and resolving compliance complaints. Nursing Education The pursuit of higher education reflects dedication and commitment to advancing professional nursing practice and improving patient outcomes. This past year five staff members completed nursing classes to earn degrees: Pamela Caldwell, MSN; MaryAnne Goodwin, BSN; Rhonda Pierce, BSN; Debra Turner, BSN; and Rebecca Wood, BSN. In addition, 25 nurses earned national certifications in their specialty areas—validating their nursing skill and knowledge. Specialty certifications earned include: perianesthesia nurse, perioperative nurse, ambulatory perianesthesia nurse, critical care nurse, and lactation counselor.

Living Our Values: Innovation Pediatrics Adds Walk-in Services In March, Franklin Health Pediatrics started providing walk-in urgent care services for its patients from 7:45–8:45 a.m., Monday–Friday. This service is for patients having acute, shortterm illnesses or injuries that Nicole Donahue need prompt attention, but aren’t life threatening. These may include earaches, sore throats, coughs, and sprains, among others. This model has proven so successful that other Franklin Health primary care practices are considering doing the same. Nicole Donahue, CPNP, Franklin Health Pediatrics’ newest provider, is the primary clinician staffing the walk-in clinic. Nurse Navigator The Franklin CARES program, a pilot project funded by a paymentreform grant from the Maine Health Access Foundation, is working with uninsured and under-insured individuals in Franklin County to improve access to health care services. Health risk information is reviewed as patients sign up for financial assistance, so that appropriate care coordination and navigation begins right away. Health coaching assists patients in identifying and achieving personal health goals, and in making connections to appropriate social, medical, and behavioral health resources—critical aspects of achieving well-being and reducing costs of health care.

Healthy Community Coalition Awarded $2 Million Grant Healthy Community Coalition has been awarded a $2 million grant over three years from the U.S. Department of Education through the Carol M. White Physical Education Program to expand and develop youth physical and nutrition education programs in collaboration with the Mt. Blue Regional School District, MSAD 58, and the Stratton School. This new funding allows the community and schools to work together to create a gold-standard program of physical activity opportunities and nutrition education. Local programming for youth includes increased after school and summer programs. Programming also includes collaboration among existing projects such as 5-2-1-0 Let’s Go to enhance learning opportunities. Meditech Physician Documentation This past year inpatient provider documentation (PDoc) was implemented in Meditech that replaces dictation and allows providers to create electronic progress notes. Once a diagnosis is made, a dropdown menu of clinical protocol is provided that serves as a checklist, maximizing efficiency for the provider and enhancing care for the patient. PDoc boosts communication among providers and more importantly improves compliance with medical record standards. Another added bonus of its use is an impressive reduction in transcription expenses.

Innovation Most Wired For the fourth consecutive year Franklin Memorial Hospital was named among 375 of the nation’s “Most Wired” hospitals according to the results of the 2014 Health Care’s Most Wired Survey released in the July issue of Hospitals & Health Networks, the journal of the American Hospital Association. This award speaks volumes to the dedication of all of Franklin Memorial Hospital’s medical staff, nurses, and other employees who use technology and innovation every day to create an exceptional experience for our patients. The only other Maine hospital receiving recognition for four consecutive years is Maine Medical Center. NorthStar Establishes Paramedicine Project A proactive pilot project called House Call was established in November to target vulnerable patients for visits by emergency medical technicians (EMTs) and paramedics who make house calls to educate and reengage patients, monitor their condition, and if needed, provide treatment. Having EMTs perform these functions within patients’ homes help prevent emergency room and doctors visits, ambulance transport, and hospital admissions and readmissions. Primary care doctors refer patients to the program who fit one or more of the following criteria: have been newly released from the hospital; have had a recent surgery; have multiple chronic conditions; or have safety concerns in the home.

Living Our Values: caring domestic Violence Awareness Ambulance In April, Franklin Memorial Hospital unveiled a NorthStar ambulance painted in a purpleand-white color scheme and adorning symbolic oversized ribbons to bring awareness to the issue of domestic violence. The ambulance is another representation of our organization’s ongoing commitment to domestic violence awareness, community education, and support services. Police have reported that having local resources available and supporting awareness increases reports of domestic violence—an important step in breaking the chain of violence. Information Sessions on the Health Insurance Marketplace In December, Franklin Memorial Hospital, in collaboration with Western Maine Community Action, hosted two onehour information sessions followed by one-on-one free assistance in signing up for health insurance coverage through the Marketplace. The Health Insurance Marketplace accessed at www.healthcare.gov is a new way to find health coverage that fits one’s budget and needs. Through the Marketplace, residents can access quality health plans, make apples-to-apples comparisons between different options, and access new tax credits that could lower monthly premiums.

Services Expand with Vascular Clinic Franklin Memorial Hospital and Maine Medical Partners MaineHealth Cardiology now provide monthly vascular clinics at the Androscoggin Valley Medical Arts Center in Livermore Falls for patients with conditions such as atherosclerosis, nonhealing wounds, deep vein thrombosis, varicose veins, lymphedema, and peripheral vascular disease. Dr. Paul Frey is the cardiologist who provides the vascular specialty services. As part of the new service, diagnostic vascular imaging services are also provided by a certified vascular technologist. Dr. Frey specializes in peripheral vascular disease, a slow and progressive circulation disorder in which organs may not receive adequate blood flow for ordinary function. Care Support Nurses Care coordination is a core element of the Patient-Centered Medical Home model and requires effective, well-educated nursing staff. This past year a restructuring of Franklin Health medical practices took place to allow for a care support nurse within each primary care practice. Care support services require dedicated nursing time, separate from the dayto-day tasks in a busy practice. Working closely with providers, care support nurses give patients the education and support they need to manage their chronic diseases such as asthma, high blood pressure, diabetes, and congestive heart failure, among others.

caring County Prescription Drug Lock Box Through support from Wal-Mart, Healthy Community Coalition purchased a prescription drug lock box that is secured at the Farmington Police Department. Unwanted or expired over-the-counter and prescription medications can be discarded in the drug lock box during regular business hours, 8 a.m.–5 p.m., Monday–Friday. This unit provides a safe and easy way for Franklin County residents to dispose of their unwanted medications year-round. electroconvulsive therapy Now Available This past year Dr. Arthur Dingley, a psychiatrist at Franklin Health Behavioral Services, began providing electroconvulsive therapy (ECT) in the hospital’s ambulatory care unit. ECT is the most effective treatment for severe depression that has not responded to medication and psychotherapy. ECT uses tiny doses of electric current under brief general anesthesia so that those receiving treatment do not feel pain or discomfort. At the outset, treatment is provided three times weekly for three weeks. Then fewer follow-up maintenance treatments are provided at an interval that provides continued benefit and reduces the risk of relapse. New Services Launched at Evergreen Adult case management and children’s targeted case management are two new services launched this past year by Evergreen Behavioral Services. Both provide support services for individuals who have severe mental health conditions that interfere with medical treatment compliance. Qualified mental health workers provide intensive support including: home visits, and coordination and advocacy to attain transportation, housing, medications, medical equipment, and home care services for these patients.

Living Our Values: Excellence Ultrasound service Accredited In May, Franklin Memorial Hospital, including its satellite location the Androscoggin Valley Medical Arts Center, was awarded its first three-year term of accreditation in ultrasound as the result of an extensive review by the American College of Radiology. With this new accreditation, all modalities of imaging offered at Franklin Memorial Hospital are now accredited. This includes: adult and pediatric CT (Image Gently Site), MRI, nuclear medicine, and mammography at the Martha B. Webber Breast Care Center. Hospital Awarded “A” Grade Franklin Memorial Hospital was honored with an “A” grade in the spring 2014 update to the Hospital Safety Score, which rates how well hospitals protect patients from accidents, errors, injuries, and infections. The Hospital Safety Score is compiled under the guidance of the nation’s leading experts on patient safety and is administered by The Leapfrog Group, an independent industry watchdog. This second consecutive “A” recognition underscores that the hospital is making tremendous strides in providing the highest quality of care to the residents and visitors of Greater Franklin County. This positive affirmation by an independent reviewer only strengthens our determination to continue our efforts to excel at all levels of our health delivery system.

Pulmonary rehab Certified In October, Franklin Memorial Hospital’s Pulmonary Rehabilitation Program received its first-ever certification by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). The hospital was recognized for its commitment to improving the quality of life by enhancing standards of care. AACVPR-certified programs are recognized as leaders in the field of cardiovascular and pulmonary rehabilitation because they offer the most advanced practices available. Pulmonary rehabilitation programs help people with conditions such as COPD, emphysema, chronic bronchitis, and other respiratory diseases, recover faster and improve their quality of life. Livermore Falls Family Practice Receives PCMH Level 3 In January, Franklin Health Livermore Falls Family Practice received recognition from the Patient-Centered Medical Home (PCMH) 2011 program for using evidencebased, patient-centered processes that focus on highly coordinated and long-term care. The PCMH model designates a primary nurse-provider team that coordinates patient care across a continuum of settings ranging from the primary care office to specialty clinics to inpatient areas. The practice became the first Franklin Health primary care practice to receive Level 3, the highest status. Since then Franklin Health Internal Medicine has also achieved Level 3 status.


excellence Hospital Earns “Top Performer” Recognition In October, Franklin Memorial Hospital was named as one of 1,099 hospitals in the U.S. earning the distinction of “Top Performer on Key Quality Measures” for attaining and sustaining excellence in accountability measure performance. The ratings were based on an aggregation of accountability measure data reported to The Joint Commission during the 2012 calendar year. The hospital was recognized for exemplary performance in using evidence-based clinical processes on the following measure sets: acute myocardial infarction (heart attack), heart failure, pneumonia, outpatient services, and the surgical care improvement project. National Quality Measures for Breast Centers Program™ In March, the Martha B. Webber Breast Care Center achieved the status of Certified Participant in the National Quality Measures for Breast Centers Program™. This quality program measures the performance of more than 33 National Quality Indicators™ of data, such as timeliness of care from screening mammograms to diagnostic mammograms, surgical timeliness of care from time between needle biopsy and initial breast surgery, and pathology report completeness, among others. Certification by this program reflects the staff’s dedication in providing patients with the highest quality breast care possible. Staff are now actively working on meeting requirements to achieve the next level of certification, status as a Certified Quality Breast Center.

Donor Spotlight S. Paul Washburn This past year, more than $617,000 was received from the Estate of S. Paul Washburn. On November 21, 2013 Paul died peacefully at his home at the age of 92. A quiet and unassuming man, Paul had been a resident of Livermore Falls for most of his adult life. Paul married the former Ellen Tobey of Kittery Point. She predeceased him in 2000. Together, they enjoyed many years of shared interests in community and fraternal organizations including the Odd Fellows.

Paul was employed at International Paper Company in Jay as a bookkeeper in the business office for many years. He was a longtime member of the First Baptist Church in Livermore Falls, and an active long-standing member of the Franklin Memorial Hospital Board of Directors. On August 28, the Androscoggin Valley Medical Arts Center was dedicated in memory of S. Paul Washburn, whose generous bequest to Franklin Memorial Hospital came with his enduring wish to have a health center built in a prominent Livermore Falls location. This wish came to fruition in January 2012.

Franklin Community Health Leadership Award Each year, Franklin Community Health Network presents the Community Health Leadership Award to an individual who has worked to improve the health of his or her community and its members. Through the award, FCHN recognizes the unsung heroes who work to ensure that those around them achieve their greatest health potential. Peter Doran Peter Doran arrived in Maine in 1966 and began looking for ways to put his theory into practice suggesting that there was a link between the challenges in rehabilitation and the lack of health education in Maine’s public schools. Settling with his wife Lois in Belgrade, Peter hit the ground running as a consulting psychologist for the state. Within six months he was appointed to direct the Commission on Rehabilitation Planning, and by 1970 he had worked with the Maine legislature to bring about a major reorganization of rehabilitation services. Peter began at the University of Maine Farmington in 1971 to help develop programs in rehabilitation services, and within six years founded the university’s community health education program, the only one of its kind in Maine. For the next 19 years, he placed thousands of students in the health education and rehabilitation services’ programs in internships with Maine health agencies, and connected students with rewarding jobs in health education. Two of these former students, Jill Berry Bowen and Gerald Cayer, have also been recipients of the Franklin Community Health Leadership Award. In 2006 Peter received an honorary degree of Doctor of Humane Letters at the college’s 153rd Commencement. He additionally successfully co-chaired the fundraising campaign for the Education Center that houses the school’s College of Education, Health and Rehabilitation. In their own community, the Doran’s began the campaign that created the Belgrade Regional Health Center, a nonprofit health care facility. Peter has also served on a long list of health committees and boards including the American Lung Association of Maine, Maine Lead Poisoning Prevention Advisory Committee, Maine Asthma Council, and Maine Indoor Air Quality Board. His collective contributions over the years have helped Maine make great strides in health education and prevention.
