How to Register On-Line Registration 1. In order to register through On-line registration, you will need a main contact client number, a family PIN number and a credit card. 2. If you do not have a main contact client number and a family PIN number, please call (480) 503-6200. 3. Visit www.GilbertRecreation.com to register On-line. Payments can be made with a Visa, Master Card or American Express. Touch-Tone Registration 1. If you do not have a main contact client number, individual client numbers and a family PIN, please call (480) 503-6200. 2. After calling Touch-Tone registration, (480) 503-6225, follow the recorded prompts to register for your selected classes. 3. Your registration is not complete until you enter your credit card information for payment. Payments should be made with a Visa, Master Card or American Express. Mail-In Registration After selecting the classes you wish to register for, then: • Complete the registration form on line. • Only one person per registration form. Forms may be photocopied. • Enter the class code along with other required information, i.e. name of class. • Be sure to sign the assumption of risk waiver in ink. Registration Reminders If you are on a waitlist for a class, and an opening becomes available, you will be contacted by the Parks and Recreation Department. Parks and Recreation Department is committed to being fair and equitable to all participants, therefore we are unable to make exceptions to any class policies. We appreciate your cooperation and understanding. The Town of Gilbert has the right to cancel, postpone, combine classes, change rooms and/or instructors at any time.

A $25 service charge will be assessed on returned checks.

Refund and Cancellation Information *Refund Information– Requests for refunds for classes/programs must be received prior to 6pm, Tuesday, May 29, 2012. Unless otherwise noted in the program description. *Please call (480) 503-6200 for more information. The Town of Gilbert has the right to cancel any program due to insufficient enrollment. Early registration is encouraged to help prevent course cancellation. If a class or program is cancelled, participants will be notified one week prior to class start date (unless unforeseen circumstances arise) and offered an alternative choice, if available. If no alternative is available, a full refund or credit will be issued. Refund checks require 2-3 weeks processing time.

Class Program Guidelines and Policies GENERAL GUIDELINES For your child’s safety, children must be accompanied into the facility as well as to and from the classroom. Non-registered children must meet the minimum age requirement of the facility to be left unsupervised. Please see facility staff for age requirements. In lieu of the Town of Gilbert cancelling a class, we will make every effort to find a substitute to teach the class. In the event that a substitute cannot be found then a make-up class will be held. Make-up classes are held one week following the last scheduled date. Class will be made up at the same time, day and location. If a make-up class is not available then a credit for the missed class will be applied to your account for future registrations. Classes holding end of the session recitals will be held on the last scheduled class date even if a make-up class is scheduled. The Town of Gilbert has the right to cancel, postpone, combine classes, change rooms and/or instructors at any time. Please take a moment to update all of your contact information with a Town of Gilbert staff to ensure proper communication (i.e. cancelled class, injury/illness, late pick-up, etc.). If your child is sick with a cold or with flu like symptoms we ask that you keep them home for a minimum of 24 hours after symptoms have subsided. All participants must be temperature, vomit and diarrhea free for at least 24 hours before bringing them to class. Participants should dress appropriately according to the type of class they are registered for. Tights/ leotards and or loose clothing is recommended for dance/ movement classes. Closed-toe shoes are a must for all classes. A participant will be allowed to register for a class as long as the participant reaches the required age anytime during the session, unless otherwise stated in the class description. If you are on a waitlist for a class, and an opening becomes available, you will be contacted by the Parks and Recreation Department. Photos & Videos of Participants: The Town of Gilbert often takes pictures of participants in our programs. Some of these photos may be used for advertising in our publications and or other types of advertising. By registering you are granting permission to the Town of Gilbert to utilize these photos/videos as marketing and promotional material. We appreciate your cooperation and understanding. ROOM POLICY Only registered participants, instructors and Town of Gilbert staff are allowed in the classroom during class time. Parents are not allowed in the classroom unless it is a PARENT/TOT class. Exceptions are made for participants with special needs who require assistance. PARENT/TOT classes: Only registered participant and parent are allowed in the classroom. Siblings, including infants, unless registered, are not allowed in classroom. Parents do not need to register but will be required to sign a waiver of liability on the first day of the session. Children and/or parent/guardian should wait outside of the classroom until instructor invites you in. Classroom doors will open at the scheduled start time. Doors may remain closed to allow time for instructors to prepare for their upcoming class(es). Please refrain from using your cell phone when entering a classroom. No food or drink allowed in the classrooms except water in a sealed container. Due to limited classroom space, no strollers allowed inside classrooms. Stroller parking is available in the hallway outside of your classroom. RESTROOM NEEDS Parents are encouraged to take their children to the restroom before class. A 10-minute bathroom/drink break will be taken for preschool classes that are one hour or more. Instructors accompany the entire class on such breaks. Parents of children in diapers/pull-ups must remain in the facility while the class is in session. If a diaper/pull up needs to be changed during the class, the parent will be asked to take the child to the restroom. Children should be able to use the restroom independently if you plan on leaving the facility during the class. If a child’s parent is not available to take them to the restroom then a Town of Gilbert staff will escort the child to the restroom. BEHAVIOR POLICY Children who are harmful to others or extremely disruptive to the class will be subject to the following Behavior Policy: Verbal warning Removal from the activity Call parent; temporary removal from the program Permanent removal from the program LATE PICK-UP POLICY If parent/guardian is more than 5 minutes late for pick-up; the child will be brought to the front desk to be supervised by a Recreation Leader. Instructors will contact parent/guardian if late pick-up occurs more than 2 times in one session.

Parent /Tot (Birth - 6 years) Gymnastics **HEAD OVER HEELS GYMNASTICS-PARENT/TOT** Parents will learn to assist their little gymnast through warm-ups and stretching before jumping, climbing, crawling and rolling on our kid-friendly gymnastics equipment. Children are encouraged to explore the world of movement using mats and balance beams to build strength, balance and coordination. Participants can expect to participate with their babies in 25 minutes of cardio, plyos, and strength training. Nutrition tips will be provided at each class and participants will have the opportunity to share challenges and successes for the first five minutes of class. Participants may be asked to bring a car seat or stroller. Each month we will set out on a new adventure through the pages of our books, creating story inspired crafts and celebrating with silly songs that may just make us get up and dance. Each month will feature a new story, inspired by a monthly theme. All ages welcome!
