How to Register On-Line Registration 1. In order to register through On-line registration, you will need a main contact client number, a family PIN number and a credit card. 2. If you do not have a main contact client number and a family PIN number, please call (480) 503-6200. 3. Visit www.GilbertRecreation.com to register On-line. Payments can be made with a Visa, Master Card or American Express Touch-Tone Registration If you do not have a main contact client number, individual client numbers and a family PIN, please call (480) 503-6200. 2. After calling Touch-Tone registration, (480) 503-6225, follow the recorded prompts to register for your selected classes. 3. Your registration is not complete until you enter your credit card information for payment. Payments should be made with a Visa, Master Card or American Express.

Mail-In Registration After selecting the classes you wish to register for, then: • Complete the registration form on this page. • Only one person per registration form. Forms may be photocopied. • Enter the class code along with other required information, i.e. name of class. • Be sure to sign the assumption of risk waiver in ink. • Registration fees must be submitted with registration form. • Your registration form is an application; it does not assure placement in a class. • If your class choices are full, you will be placed on a wait list for that class. Registration Reminders · If you are on a waitlist for a class, and an opening becomes available, you will be contacted by the Community Services Department. · Community Services Department is committed to being fair and equitable to all participants, therefore we are unable to make exceptions to any class policies. Scholarships A scholarship program is available to help economically disadvantaged children. If you are interested in applying for a scholarship for your child(ren), please call the Gilbert CAP office at (480) 892-5331. They are located at 1140 N. Gilbert Rd. Ste. 109, Gilbert, AZ 85234.

Class Guidelines • For your child's safety, children must be accompanied into the facility as well as to and from the classroom. • Participants should dress appropriately according to the type of class they are registered for. Tights/leotards and or loose clothing is recommended for dance/movement classes. Closed-toe shoes are a must for all classes. • Parents of children wearing diapers or pull-ups must remain in the facility during their child's class. • Parent/Tot classes-Parents do not need to register, but do need to be present in the classroom. Only registered children are allowed in the classroom. • Only registered participants may remain in the classroom once class has started. Parents, family members and friends are asked to wait outside the classroom quietly while class is in session. If a participant is in need of a caregiver to remain in the classroom during class hours, prior approval must be obtained by the program coordinator before registration. • A participant will be allowed to register for a class as long as the participant reaches the required age anytime during the session, unless otherwise stated in the class description. • Photos & Videos of Participants: The Town of Gilbert often takes pictures of participants in our programs. Some of these photos may be used for advertising in our publications and or other types of advertising. By registering you are granting permission to the Town of Gilbert to utilize these photos/videos as marketing and promotional material. • Please Note: Class Instructor names are subject to change from what is published. If a class or program is cancelled, participants will be notified one week prior to class start date (unless unforeseen circumstances arise) and offered an alternative choice, if available. Costumes, props and books will be incorporated into this magical basic ballet class. A new fairytale will be incorporated into their learning along with building coordination, rhythm and vocabulary. TWINKLE TOES Class is full of imagination and fun while introducing the fundamentals of dance techniques in ballet, tap and tumbling. Civic Center Drive, Gilbert, AZ, 85296 March 7-March 25. All rosters must be received by March 25 by 5pm to be considered for the lottery. Postmarks will not be accepted. PROOF OF RESIDENCY: All 2011 Priority I teams must submit proofs of Gilbert players residency: copies of drivers' licenses, deposit slips, utility bills, etc.

LOTTERY: On Saturday, April 2 the softball lottery will be held at the Southeast Regional Library (Greenfield and Guadalupe). Registration for each category will not begin until the previous category is complete. Attendance at the lottery is required.

PAYMENT: All teams must have payment for the entry fee at the lottery at the time of their selection on April 2. Only one check per team will be accepted! Checks should be made out to: Town of Gilbert

LATE REGISTRATION: Teams that do not submit completed rosters by the March 25 deadline may register for any remaining league openings at the lottery site 11:30am on April 2. LEAGUE STRUCTURE *A mandatory $25 A.S.A. sanctioning fee will be charged to all teams for 2011. All efforts will be made to have rosters available at the ball fields before the end of each season for player signatures. Notification will also be sent to managers before each scheduled organizational meeting. However, it is the responsibility of each team to ensure they have a completed roster submitted in accordance with the league deadlines. All league schedules are based on the total number of team entries. 8 Team Leagues Single header will include a 7 game schedule. Double header will include a 14 game schedule.
