ADVANCE DIRECTIVES
We all value our independence and the right to make decisions for ourselves. One important area in which we exercise independence is choosing mental health treatment and other health treatment we receive. No one likes to consider the possibility of becoming unable to make their own decisions, even on a temporary basis. Although it is difficult to confront these issues, by doing so we can help ensure our wishes are honored in the future. The decision whether to have an advancedirective is up to you. Once you determine your wishes, the next step is relatively simple and inexpensive or free, and can be part of your person-centered planning process.
What is an Advance Directive?
It is a written legal document that allows an individual to specify their wishes for psychiatric treatment if they should become unable to make those decisions in the future. Michigan law allows the designation of a health care advocate to represent an individual should they be unable to make their wishes known. The directive may specify medications that are helpful or are to be avoided or preferences for location of treatment, such as a specific hospital.
How would an Advance Directive relate to my treatment with Easterseals MORC?
If you choose, your case manager/clinician will assist you in connecting to a resource that will help you to develop your advance directive. With your permission, your advance directive can be copied and made available to your case manager/clinician. Your case manager/clinician will then help you to distribute your advance directive document to all caregivers and health care professionals identified in your document and within the surrounding agencies (e.g. hospitals, crisis centers, doctors, etc.).
