Insufficient sleep in Americans is considered a public health issue

A

re you getting enough sleep? Do you feel rested after you sleep? If statistics are any indication, you are not.

Dr. Kevin Swartz, an ear, nose and throat specialist at Swedish Medical Center who is boardcertified in sleep medicine, said insufficient sleep in America has really become a public health issue over the last decade. “In fact, it has long been identified in the sleep academic community and by public safety entities to be a serious issue,” Swartz said.

Recent data released by the Centers for Disease Control and Prevention (CDC) indicates that greater than a third of American adults, an estimated 35.2 percent, are clinically sleepdeprived, which means they are getting less than seven hours of sleep per night on average. Adults aren’t the only ones causing a public health concern. The CDC also released new data on how middle and high school students are not getting enough sleep. According to the report, an estimated 58 percent of middle school students and 73 percent of high school students are getting insufficient sleep. “The primary effect on adolescents is in learning, memory and behavior,” Swartz said. “Sleep deprivation has been linked to poor school performance, behavioral issues and increased motor vehicle accidents. Studies have shown that eating habits favoring sugar, processed foods and fats are associated with sleep deprivation. Psychological illness such as depression, anxiety and increased risk of attempted suicide are also higher.” Describing adults as “grown-up adolescents,” Swartz said sleep deprivation can have the same negative impact on their lives. “Adults experience similar issues with reduced memory, task initiation and management, and cognitive performance. In addition, there are strong associations with depression and anxiety,” he said. “Finally, chronic disease both becomes more likely and, if present, more difficult to treat in this population.” Swartz said the causes for insufficient sleep are all over the place from being busy to technology to health issues.

“Obviously, our modern lifestyle of trying to fit too many things into our 24-hour day are to blame,” he said. “But also, things like staring at blue screens, eating poorly, not exercising and stress can contribute to insomnia, which will ultimately shorten our sleep duration leading to deprivation.” Insufficient sleep could also be due to what a person does for a living. Swartz said shift workers suffer from constantly shifting sleepwake times that can ultimately lead to reduced sleep duration. Sleep deprivation is no laughing matter. In fact, Swartz said not getting enough sleep could lead to anything from a heart attack or stroke, to increasing a person’s risk of being obese and developing coronary artery disease, asthma, COPD, chronic kidney disease, depression, arthritis and cancer. Many of the most common causes of insufficient sleep can be addressed without having to see a medical professional. Swartz stressed that sleep must be “habitual,” which means sticking to a schedule, making sleep an even bigger priority than exercise, eating well and managing stress.

The CDC recommends getting more than seven hours of quality, restful sleep per night. To really make that happen, it might mean getting to sleep well before midnight, which means reducing stimulations such as bright lights, laptops, television, mobile phone and heated conversations at least 30 minutes prior to sleep. “Being a good sleeper starts with what is done during the day,” Swartz said. “Diet, exercise, management of stressors are all key for the set up to healthy sleeping. Healthy habits are a great way to coin the solution. Creating a routine that supports all the promoters while eliminating the detractors to good sleep is a pathway to success.” However, Swartz stressed that if a person feels he or she has done all they can to increase duration and still have issues with daytime excessive tiredness, it may be time to talk with a medical professional: Sleep deprivation could be a symptom of a condition or sleep disorder that requires treatment or assistance.

Q: When should I start brushing my child’s teeth? When should I take them for their first visit to the dentist? A: Parents should start with wiping the gums of their babies. When the baby teeth start erupting a soft cloth should be used to wipe the tooth. A finger brush that the parent wears over their index finger can be used to brush the baby’s teeth. Every child should have their first dental appointment by age one. These appointments will give the parent the opportunity to learn more about their child’s dental needs and the child will begin to understand what to expect at dental appointments. At this age, the child should sit in their parent’s lap during the appointment. Q: There are so many choices, can you tell me which toothpastes are the best for my children, myself and my older parents? A: There are toothpastes on the market that do not have any fluoride. These should be used by children under the age of 3 who do not have the ability to spit on demand. Until age 3, most children will swallow toothpaste that can cause medical problems. In children who are older than 3-years-old, toothpaste can be used. This toothpaste will come in a variety of flavors such as bubble gum and strawberry. As these might be more desirable flavors to a child, it will make it easier to encourage them to brush more regularly. For teens and adults, most toothpaste on the market will be acceptable to use multiple times a day. Should a person have sensitivity to either hot or cold temperatures, causing their teeth to hurt, then they should consider using toothpaste without whitening or toothpaste designed for sensitive teeth such as Sensodyne, Crest Sensitive or other brands that make tooth-

pastes for sensitive teeth, which will help with the sensitivity they are experiencing. Q: Should you pull a child’s loose tooth that just keeps hanging on? A: When a child has a loose tooth you can assist them in pulling it out. Some parent use methods such as eating an apple or just playing with the tooth until it is so loose that you can just tug on it and pull it out. If you notice that the erupting tooth is coming out and the primary tooth still has not come out, you may need to make an appointment and have the dentist pull it out. Q: Can a water pick replace flossing altogether? A: I think water picks are a good adjunct to flossing for some patients, but not necissarily a replacement for flossing. Q: If teeth seem super sensitive to cold or warm sensations, what could it be? When should I see a dentist?

A: You should see a dentist right away because if the issue continues, the nerve could die and infection may follow. Q: How often should I get my teeth cleaned and checked by a dentist? A: You should be seeing your dentist at least twice a year for cleanings. You may need to make additional appointments if you have any dental work that needs to be completed. Q: Are electric toothbrushes better? A: An electric toothbrush is better to use than an manual toothbrush. Any tooth brush with sonic technology such as the Sonicare ultrasonic toothbrush, will clean your teeth better. Although, the cheaper versions of this toothbrush can also be helpful. Most brushes make sounds indicating when you should move to another part of your mouth, requiring you to spend the recommended two-minutes brushing. Q: Are dental implants a good

alternative to dentures? What are the pros and cons? A: Generally, yes, implants are a good alternative, but not in every case. Placement and success of implants depend on many factors, which can be explained by your dentist during the evaluation of the procedure. Q: What are some of the biggest misconceptions people have when it comes to dental health? A: Most people think you need to go to the dentist to only take care of your teeth, but what they don’t realize is keeping your teeth and mouth healthy is also related to your heart’s health as well as the effect on your total health. Q: Besides regular brushing and flossing, what advice or tips do you have to do extra to keep my teeth and gums healthy? A: Diet is also a big factor since certain food, especially processed sugar, can create an environment to promote growth of the bacteria that are harmful to your oral health.

To practice, place one hand on your chest and the other on your belly. Breathe in slowly through your nose into your belly. You should feel the hand on your stomach being pushed outward while the hand on your chest remains still. Hold the breath for a few seconds, and then push it out through pursed lips, like you were whistling. Repeat for three slow, deep breaths to instantly feel less stressed.

Swedish Medical plays key role in getting cyclist back on track W

hen you are active in walking, running, hiking and cycling like Paula and Stan McClure, having knee surgery can be a tough pill to swallow. However, as the Centennial couple will tell you, having the great medical team at Swedish Medical Center, and the drive to get back to their regular activity can help. Stan joked that Paula is probably more of a “heroine” in their situation, given that she had knee replacement surgery in November, and she was out three months later doing her daily six-mile trek. At 71, Stan is recovering from the first surgery on one knee in January, while mentally and physically preparing for the second knee replacement on Feb. 26. “The reason to have knee replacement surgery is definitely different for everyone,” Stan said. “For me, I tore my ACL 20 years ago and the cartilage in there has just wore down faster, causing some arthritis.” Knowing they had to have the work done, Stan said he’s a researcher and he took to the internet to find a medical facility and doctor he was comfortable with. That research Dr. Joseph took him to SwedAssini ish Medical Center where he discovered 37-year-old Dr. Joseph Assini. “When you read about this doctor, he’s 37 and has already done more than 1,000 procedures,” Stan said. “This guy is amazing. He got my first knee replaced in 62 minutes. That’s incredible to do what you have to do to make this happen.” Assini said it was important for both Paula and Stan to have realistic goals going into the procedures. “Both are very committed to staying in shape,” he said. “They had realistic goals that we discussed pre-op and this is very important in making sure patient satisfaction is maximized. Each of them are in great shape aside from the respective joints. Being in good health can really make

recovery easier. However, for many patients getting their knee or hip replaced helps them exercise again, which in turn, gets them back to better overall health.” Assini said patient care is a top priority, and through the Swedish Medical Center joint program, cases are completed more efficiently, with better patient satisfaction and improved outcomes. Stan joked that he can’t blame Dr. Assini for the pain that comes with knee-replacement surgery, but knows the doctor did a great job and is hoping to be back at his high level of activity by April. And, high level is exactly what Paula and Stan do. Besides Paula walking more than six miles per day, Stan has spent years cycling and being active. Stan has done 5 tours of the annual, RAGBRAI, or The Register’s Annual Great Bicycle Ride Across Iowa. Stan said there’s something special about starting the 500-mile, 7-day trek, by dipping the back-bike tire in the Missouri River, and finishing the tour by dipping the front tire in the Mississippi River. “Doggone I hate it, but I think I might have to skip the bike tour this year,” Stan joked. “But, I should be back to cycling form by mid-April. I might have to skip jogging after I

Stan, a Vietnam Veteran, said he cycles to bring awareness to the ever-increasing rate of Veterans committing suicide with multiple organizations. “What many people don’t realize is 65 percent of Veterans who commit suicide never get in to Veteran Affair Services,” Stan said. “We’ve got to do better, and I know I can be out there to make a difference.” In the meantime, as he recovers, Stan is still carrying on his work with local Veterans, serving as the Post Service Officer with VFW Post 4666.

Stan McClure, 71, is currently recovering from kneereplacement surgery, and preparing for a second. The 71-yearold looks forward to getting back on his feet to carry on his work with Veterans.

CDC recommends vaccine as key to preventing flu Take time to get a flu vaccine. (CDC) ­THE CDC recommends a yearly flu vaccine as the first and most important step in protecting against flu viruses. While there are many different flu viruses, a flu vaccine protects against the viruses that research suggests will be most common. Flu vaccination can reduce flu illnesses, doctors’ visits, and missed work and school due to flu, as well as prevent flu-related hospitalizations.

• Everyone 6 months of age and older should get a flu vaccine every year. • Vaccination of high risk persons is especially important to decrease their risk of severe flu illness. • Children younger than 6 months are at high risk of serious flu illness, but are too young to be vaccinated. People who care for infants should be vaccinated instead. • Prevent spreading germs • Try to avoid close contact with sick people. • While sick, limit contact with others as much as possible to keep from infecting them. • If you are sick with flu-like illness, CDC recommends that you stay home for at least 24 hours after your fever is gone except to get medical care or for other necessities. • Cover your nose and mouth with a tissue when you cough or sneeze. Throw the tissue in the trash after you use it.

• Wash your hands often with soap and water. If soap and water are not available, use an alcohol-based hand rub. • Avoid touching your eyes, nose and mouth. Germs spread this way. • Clean and disinfect surfaces and objects that may be contaminated with germs like the flu. • Take flu antiviral drugs if your doctor prescribes them • If you get the flu, antiviral drugs can be used to treat your illness. Antiviral drugs are different from antibiotics. They are prescription medicines (pills, liquid or an inhaled powder) and are not available over-the-counter. Antiviral drugs can make illness milder and shorten the time you are sick. They may also prevent serious flu complications. For people with high-risk factors, treatment with an antiviral drug can mean the difference between having a milder illness versus a very serious illness that could result in a hospital stay. Studies show that flu antiviral drugs work best for treatment when they are started within 2 days of getting sick, but starting them later can still be helpful, especially if the sick person has a highrisk health condition or is very sick from the flu. Follow your doctor’s instructions for taking this drug. Flu-like symptoms include fever, cough, sore throat, runny or stuffy nose, body aches, headache, chills and fatigue. Some people also may have vomiting and diarrhea. People may be infected with the flu, and have respiratory symptoms without a fever.

Q&A with a local orthodontist Dr. Hilary Neiberg Baskin, owner of family-owned and-operated All About Braces and Orthodontics, has provided orthodontic care in Metro Denver for more than 20 years.

Q: What are the steps I can take to take care of my teeth and oral health while wearing braces? A: It is especially important to maintain good oral hygiene habits while wearing braces. This includes daily brushing and flossing. Aids like superfloss and floss threaders are important tools. Waterpiks or waterflossers are also great with braces. A daily fluoride rinse is also advised. Braces also means more frequent cleaning visits to your regular dentist. Rather than every 6-12 months, I recommend seeing your dentist every 3-6 months. Keeping your teeth clean is always important and can take a bit more time with braces, with the effort to prevent white spots, cavities or periodontal issues. Q: How have options changed for choosing braces over the years for both children and adults? A: While many of the options available today (clear aligners, clear braces, metal braces, customer aligners, lingual braces) have been available for many years, the technology employed for each of these options has advanced significantly. Unfortunately, along with these reliable methods used by orthodontists around the world, there has been an increase in do-it-yourself and gimmick products, making unproven claims. It’s important for safety and best practice reasons to speak to your orthodontist about options he/she has available that have been academically proven. Q: Should I get braces for my child before all of their permanent teeth have come in? A: It depends. The American Association of Orthodontists recommends that you have your child see an orthodontist at age 7. That does not mean that all 7-year-olds should be treated. It does mean they

Left untreated, many conditions that require treatment continue to worsen, possibly contributing to periodontal disease, joint pain or chipping and wear of teeth. Q: Are choices increasing for adults to get braces? How long do adults generally wear braces? A: As noted earlier, while in the broad context the “choices” for orthodontic treatment have been around for a while now, each of them have improved significantly, making many of them now available to a far broader range of patients. Currently, the national average for wearing braces is 27 months. Treatment time fluctuates greatly based on bite, difficulty of correction and patient compliance. In our practice, we hope to complete treat-

ment anywhere from 6 months to 20 months. We also work with other health care specialists to address joint pain, sleep apnea, periodontal concerns and, of course, aesthetic concerns. Q: At what age is best to see an orthodontist for the first time? The American Association of Orthodontics recommends that you first see your orthodontist at age 7. Q: What are the benefits of early intervention? Early orthodontic care (often called interceptive, or phase 1 orthodontics) can help avoid far more serious conditions and the potential need for surgical intervention down the road. As children are still growing, we can affect jaws and skeletal structure more easily and comfortably before puberty. Growth allows an orthodontist to address more significant developmental and dental issues at an early age as well such as crossbites and impactions. Q: What are the food restrictions with braces? What happens if you ignore them? A: Broadly speaking, while wearing braces you should avoid foods that are hard, sticky or chewy. These types of foods can break the braces off of your teeth. Chewy foods have the added risk of getting stuck in your braces, where you may be unable to brush them away, creating an added risk for plaque and cavities.

Q: What are some common misconceptions about braces and orthodontists? A: People get nervous about getting braces put on. Although there is some pressure felt hours after the braces are placed, often far less than most patients expect, getting braces put on is not an uncomfortable process. People also do not always realize how an orthodontist is different from a general dentist. An orthodontist spends an additional 2 or 3 years after dental school to specialize in growth and development, misaligned teeth, bite concerns and tooth movement. We work in partnership with general dentists, pediatric dentists, oral surgeons, periodontists, endodontists as well as physicians to optimize oral, and physical health. Q: What advice do you give parents who are looking at options for their children? A: Follow professional guidelines for having your child seen by a specialist by age 7. You do not need a referral from your general dentist to see an orthodontist. Like any health decision, you may want a second opinion before finalizing a decision on treatment. There is significant variability in price, treatment techniques and reputation across the market. While the orthodontist your dentist recommends may be fantastic, given these factors, a second consultation is prudent. Then you can select the practice your family is most comfortable with and provides you with the best value for your investment. The most important factor is to make sure that you are choosing a specialist, a member of the American Association of Orthodontics.
