MESSAGE TO THE THE COMMUNITY COMMUNITY MESSAGE TO

This past year has come and gone rather quickly for your local hospital as we’ve embraced a number of changes, implemented new technologies and continue to plan strategically for the future. In August, we launched Epic, our electronic health record (EHR) system with the support of Providence Health and Services. This truly was one of the largest and most comprehensive changes Clark Fork Valley Hospital (CFVH) has seen in its history. Our staff worked tirelessly to ensure the transition went smoothly and was as seamless as possible for our patients. With this advancement came a helpful tool called MyChart so patients can access their records securely online and maintain a more active role in their care. We welcomed the addition of three new providers to our Family Medicine Network care team; Andrew Roberts, DO in Thompson Falls, Kim Schwittay, APRN in Hot Springs and Nick Lawyer, PA-C in Plains. We also partnered with the International Heart Institute of Montana to bring on three consulting cardiology specialists when Dr. Sharon Nichols, Cardiologist moved her practice in December. Pulmonary Rehabilitation classes began earlier this year and have already shown to be quite beneficial for patients. There is much for us to be excited about in the coming year as we are expanding our services to include mental health. Kate Whipple-Kilmer, PhD will start in August. Her practice will be based from our main campus in Plains and she will travel throughout the Family Medicine Network for consultations and treatments. Efforts are underway to offer a comprehensive approach to chronic pain management. This will include medical treatment, rehabilitation therapies, interventional procedures when needed, social support as well as other non-pharmacological treatments. Healthcare payment methods are beginning to change dramatically, and we know some of you are seeing your health plans become restrictive and your out of pocket costs increase. In response to the anticipated changes in reimburstment models, we are seeing organizations, including ones in Montana, begin to align in ways that few thought possible a few years ago. New technologies continue to be developed and we look to incorporate those whenever possible. Finally, with the recent completion of a Sanders County Community Health Needs Assessment (CHNA), we look forward to partnering with you and community organizations to improve overall health in our communities. The CHNA can be used to start the necessary conversations, but it will take the work of all of us to reach the goal. CFVH FOUNDATION The Clark Fork Valley Hospital Foundation is a charitable organization whose only purpose is to support CFVH and its related entities fulfill their mission to improve the health of Sanders County residents. The Foundation has provided nearly $225,000 in donations over the past six years. In 2013 the Foundation donated almost $11,000 towards the purchase of new dining tables for Long Term Care residents and kitchen equipment for Dietary Services. To facilitate projects like these, the Foundation conducted two fundraising efforts in 2013; a Gala event in March and the Golf Classic in September. The Gala featured musical entertainment by The Singing Sons of Beaches and an evening of food and fun at the Sanders County Pavilion for over 80 attendees. The Golf Classic, which was in its 6th year, was held on Sunday of Labor Day weekend and hosted 62 golfers for the day. Tremendous support from local and regional businesses helped make it successful and over $12,500 was raised. Each year the Foundation is asked to fund hospital projects and services that are not covered in the budget for the year but directly benefit the patients and guests to the facility. Your tax-deductible donation does make a difference. Past purchases continue to save lives and enhance the care the hospital can provide. Salaries, Benefits and Contract Labor comprise approximately two thirds of the hospital’s total operating expenses. Clark Fork Valley Hospital is the largest employer in Sanders County. When salary dollars are spent locally, it is estimated they generate a multiplier effect of six times as they circulate to support the local economy. FIRSTHAND HANDEXPERIENCE EXPERIENCE FIRST Last Wishes and a Peaceful Passing Patient Testimonial

Earlier this year, my husband John was rushed by ambulance from our home in Paradise to Clark Fork Valley Hospital when he started experiencing a sudden onset of poor balance resulting in multiple falls. He was assessed by hospitalist, Frank Towsley, PA-C, who kept him for observation and testing so John’s condition could be monitored. John spoke his last words to me that evening in the hospital when he said, “See you in the morning.” By morning his condition had become so severe, he had gone into a coma and was declining quickly. I spoke with Dr. Ronald Black, his attending physician that day, about a DNR and expressed John’s wishes to never be kept alive by machines. I also shared with Dr. Black that John did not want to die in a hospital. As John’s condition worsened, Dr. Black made sure to follow through on all of our wishes. I will never forget the kindness he displayed as he and three others wheeled his hospital bed out into the courtyard of the hospital so John could pass away peacefully under the vast Montana sky. I described John’s surroundings to him. I told him he was not in the hospital and that he was laying beneath a beautiful weeping birch tree. His breath became shallow and his oxygen levels dropped. I told him it was ok to go. I would miss him dearly, but it was ok to go. He passed away shortly after with a warm evening breeze softly sweeping over his face. I feel so fortunate to have been at CFVH during this time. We were treated with such compassion and I felt like we were truly taken care of by our closest friends. Dr. Black and all the other care providers at the hospital gave John and I our last special time together. John did not have to die in a hospital and I will forever be greatful for that. - Charlayne Tiemens . Paradise, MT


Pulmonary Rehab Provides Motivation for Healthy Living Patient Testimonial

After suffering a pulmonary embolism in May 2013, I became completely dependent on my oxygen tank and had to use it 24/7. After seven weeks of Pulmonary Rehabilitation classes at CFVH, I am so pleased to say that I only have to use my oxygen when I nap and sleep through the night. This has changed my life. I am an avid gardener and the thought of having my oxygen attached to me all the time was so depressing. I made up my mind I wasn’t going to live like that and now I feel so free without a tank and cords at my side all hours of the day. I am excited to catch up on all the things I’ve been missing; gardening of course and all the chores around the house that I haven’t been able to attend to in a while. Initially, my goal was to regain my independence from my oxygen tank. I have made such great strides at that, now my goal is to kayak this summer, something I haven’t been able to do for the last three years. Going through Pulmonary Rehabilitation classes has not only helped my health but I have built relationships with other classmates and felt a true camaraderie with them. We celebrate each others little victories and even though we have a wide spectrum of diagnoses, we’ve helped each other work through difficult days and are making a difference in each others lives. Often times, I don’t feel like coming but I do because I want to be with the group. They are such great motivators for me and I’m not sure I could have had the same success without this kind of rehab environment. Community Benefit

As a tax-exempt non-profit healthcare provider, it is an important part of our mission to maximize and measure the benefits we provide to the communities we serve. Some of the benefits we provide are more obvious in nature, such as charity assistance and sliding scale discounts to low income patients which amounted to $878,347 in 2013. Other community benefits are not as well known, but are nonetheless diverse and include:

Student Sports Physicals Program Encourages physical activity and wellness through youth athletics 2014 marked the third year CFVH donated 100% of its proceeds from the Student Sports Physicals Program to local Sanders County Booster Clubs. Clubs benefitted from donations anywhere from $450 - $1,700, with total donations amounting to over $4,300 across the county. With childhood obesity and other serious health issues on the rise, CFVH wants to encourage community youth to get moving and make physical activity and wellness a focus in their lives earlier rather than later.

Gretchen French, Surgical Technologist With experience working at larger medical facilities, I now see what a unique care experience CFVH offers to its patients and I feel so fortunate to be a part of an organization that puts their patients at the center of all they do. The care is personalized to each individual patient and they get much more face time with their nurses and providers. I feel this results in better continuity of care and ultimately better outcomes. As a Certified Surgical Technician, I not only care for my patients before, during and after their procedures, but I oftentimes see them during their pre/post op appointments in the clinic. I get to be the familiar face they need to bring them comfort as they undergo surgical procedures that can oftentimes be scary or unnerving.

Norma Cole, Clinical Laboratory Technician Never having enough money to go to school to pursue a career in family counseling opened the doors for me to stay close to home and begin working in the medical field. I was able to get the hands on experience I needed here at the hospital to advance little by little. My CFVH mentors encouraged me from the time I was a junior in high school working in the dietary department to continue to strive for excellence and work hard to get the training I needed to grow in my career and become a phlebotomist. Looking back, over the last 30 plus years, I know I wouldn’t have gotten that same support anywhere else. I have had the opportunity to raise my family in the community I grew up in and build a career that I am proud of. I have seen firsthand what CFVH provides for its patients and the level of care my family and I have received is beyond words. I love my job and quality is my highest priority. My patients aren’t just anyone walking through our doors, they are my friends and neighbors.
