Strategically Moving into the Future


From the President’s Desk Greetings, As I look back at all we have accomplished in the last year and our goals for the upcoming five year strategic plan, I am amazed at the numerous projects we have undertaken and completed around our campus in twelve short months. The flurry of excitement and activity is the result of thoughtful coordination and consideration of residents, members and guests. Through the vision of the Sisters of Charity, Bayley opened its doors in 1990 with a ministry to serve older adults. With our mission of providing a continuum of care to our seniors, we strive to provide compassion and quality of life, rooted in the values of respect, honesty and excellence. At the same time we strive to act justly to all those to whom we minister: our residents, families, volunteers, staff and the public. We are blessed with a thriving community and serve approximately 310 residents on campus, 1500 members in our adult day, fitness club and outreach programs, 324 staff members and 200 volunteers. We are also a corporate work partner with DePaul Cristo Rey High School. We foster a person-centered culture with a strong spiritual component in a home-like environment. To that end, some of our recent projects this past year include: an extensive renovation in our main building including lobbies, dining rooms and a state-of-the-art physical therapy room. We also have implemented an expanded, hospitality model of meal service for residents and their guests.

To ensure the funds needed to provide for our residents, for many outlive their resources, we launched an Endowment initiative. As of this time, we are well on the way toward our goal of $30 million. This past year, Bayley provided approximately $2.4 million in uncompensated care. Another project underway is a major technology upgrade. We will soon have a wireless infrastructure, current hardware, software and tech support needed to communicate in the new world of managed care, accountable care organizations and electronic health records. And perhaps more importantly, a way to keep our residents connected with their grandchildren! We face many hurdles-changing consumer preferences, dwindling funding streams and government regulation. We stay involved in our professional associations to ensure innovation, fiscal stewardship and best practices as we meet current and future challenges. Our Board, Advisory Committees and staff follow a Five Year Strategic Plan which, guided by our mission, details proactive goals to meet the needs of our residents and ensure that Bayley flourishes. QAPI: Instilling Quality at Every Level

A

lthough Ohio recently created a state mandate that nursing homes must participate in the continuous improvement program, Quality Assurance Performance Improvement, known as QAPI, Bayley already had a similar program in place. “We know we can never sit back, relax, and think about all that is good at Bayley,” said Paul Kocsis, vice president of health and wellness. “Therefore, we strive to identify where performance improvement is needed and then improve the processes that contribute to our residents’ quality of life on a continuous basis.” The scope of the QAPI program encompasses all departments of Bayley including: Accounting, Activities, Administration, Admissions, Campus Services, Development, Dietary, Environmental Services, Finance, Health & Wellness Services, Human Resources, Marketing, Medical Records, Nursing, Nutrition, Outreach, Paint Shop, Pastoral Care, Social Services and Support Services. The overall goal of Bayley’s QAPI Program is to improve the quality of care and services delivered to our residents, improve resident/family satisfaction, and meet the needs and expectations of residents, families and other stakeholders. In order to support this overall goal, the following objectives need to be met: consistency in caregivers, reducing readmissions to hospitals, focusing on person-centered care, stabilizing pain, preventing falls, infections and pressure ulcers and controlling medications.

If a staff member sees a process that needs to be improved, they can call for a Performance Improvement Project, known as a PIP, meeting with all the stakeholders in the process and gathering input from each one.

“When people hear all sides of the process, you start to see light bulbs go on,” said Jeannette. “The ideas for improvements start flowing.” A recent success story of a PIP centers around nutritional supplements. A question surfaced about the amount of supplements Bayley was purchasing to administer to residents. Aides and Nurses were consulted and provided the information that residents were usually offered supplements at meal times. The problem recognized was that residents either drank the supplement or ate their meal but could not ingest both. Residents were less than thrilled with the taste of the supplements as well. After studying the liquid supplements and involving the kitchen staff along with our dietary consultant, it was determined that by bringing the entire process in-house, Bayley could make our own supplements.

To focus on such widespread goals, Bayley has introduced QAPI to the entire staff.

The result is that we can use fresher ingredients, such as peanut butter, bananas and oranges to create better variety of flavors. Each day the supplement is a different flavor, and we can make a more nutritious product at a much lower cost. The supplements can also be served once per day in the middle of the afternoon, which leaves meal times for eating only.

“Every staff member has a say in things that might not be working as well as they should be,” said Jeannette Bredestege, RN BSN, who implements the requirements of QAPI. “By giving them a voice, they have buy-in.”

“Our residents are the real winners from this PIP,” said Jeannette. “But the best thing about this plan is that our goals are always fluid. As conditions, requirements and regulations change, our goals can too. The plan is ever changing.”

Mr. Fix-it - Can Do It!

E

very Wednesday from 11am – 1 pm Jack Middendorf opens up his St. Joseph’s Fix-it Clinic on the second floor, next to the beauty shop. He puts out his sign and lays out his tools, and waits for customers. Known around Bayley as Mr. Fix-it, Middendorf, offers to fix just about anything. Over the years, Jack has done everything from welding a wheel chair, to rewiring a desk lamp, to fixing broken tables and chairs. He gets called on quite often to repair intricate jewelry pieces, replacing hard-to-open clasps with magnetic closures. Everyone around Bayley knows that if something is broken, Jack is the guy to call. “I fixed a broken monstrance for the chapel a few months ago. The little glass door that holds the host in place wasn’t closing right. I was able to fix it with a magnet,” he said. “They ask for all kinds of things. I even adjusted a guy’s artificial leg once.” Jack started working around Bayley about two years ago when his wife Marylyn, who suffers from Alzheimer’s disease, became a resident. He came to visit all the time and just started fixing things while he was here.

When Jack opens up for business, he makes sure his customers know the price up front. “They have to send me more business,” he said. “There is no charge. I just want to fix anything that needs fixing and I want them to help me spread the word.” Bayley resident Joe Book was in the shop recently getting a bolt reconnected to his motorized scooter. Joe has been to the shop for other repairs. “I come back because he does a good job and he knows what he is doing,” said Joe. “Plus, the price is right.” Jack and Marylyn are both life long West-siders. He said he graduated from Elder and Marylyn from Mercy.

Jack replaces hard-to-open jewelry clasps with magnetic clasps. “Everybody loves those,” he said.

“No matter how many degrees you have, people around here just want to know where you went to high school,” he said with a laugh. And Jack has plenty of degrees, a Bachelor’s degree from University of Cincinnati and a Master’s of Business from Xavier University, along with a successful career with Franciscan Health Systems and PNC Bank where he retired in 1995 as Senior Vice President and Chief Operating Officer. After retirement he found himself bored after just a few weeks and took on a position as Director of County Facilities for Hamilton County. Jack thought he would help out on a few projects, but ended up staying for seven more years. In all of these positions, Jack found he had a knack for fixing things. So he focused on making his gifts available for others.

“They tell me great stories,” he said. “I get so much more out of it than they do.” As Marylyn’s condition worsened, Jack said his role of caregiver grew, to the point that his own world was very small. After she moved into Bayley, Jack knew he had to redevelop his own life. “I know Marylyn is well taken care of and she is at peace,” he said “I thank God every day that she is here at Bayley.” To rebuild his life, Jack became more active in his church, Our Lady of Visitation. So how did he decide to create this ministry at Bayley? “I think if Jesus didn’t have anything else to do, this is what he would be doing,” he said.

Jack says he loves to talk to people who patiently wait while he makes his repairs. They start talking and that is when the fun starts for Jack.

Person-Centered Dining

T

he ingredients to a good meal are not always just about the food. Bayley has recently completed an entire transformation of our dining services, the entire process, from the kitchen to the plate, has been upgraded. Just as important to what is being served is where it is being served. New furniture, updated décor and even new china, glassware and table settings, give each dining area the look of a fine restaurant. And, just as in a fine restaurant, our residents are receiving five star service from a wait staff who take their order from a menu and deliver it to them. “Even though we have always had good food, we knew that we could improve the presentation,” said Carol Blevins, Bayley food service director. “Now all of our dining rooms are warm and inviting.” With input from residents, family members and employees, a Dining Task Force was created. The group created a vision for the team to include these points: • The Dining Areas will be functional, welcoming and comfortable. • Meal time will be a pleasant dining experience that residents can look forward to. • Residents will be offered familiar foods that are presented in a consistent and appealing manner. • Residents will be able to eat what they want, when they want and where they want. • All residents will be treated as individuals with dignity and respect. After months of research and construction, the days of bringing a pre-filled tray to a resident came to an end. On July 9th the new style dining service came to fruition. In 2012 the Assisted Living Dining Room received a similar transformation, doing away with a serving line and bringing in table service. But with the

upgrade to each dining room, every level of our skilled nursing wings have been transformed as well. Although the day’s main entrée is still prepared in the Bayley kitchen, it is delivered to localized pantries throughout the building. The pantries are stocked with breakfast and sandwich fixings and a griddle for making burgers or a grilled cheese. Blevins explains that when a resident comes to the dining room they can choose the special of the day or items taken from a menu of Daily Alternates that are available all day, every day. The resident can order exactly what they want. In another move to provide more choices, the meals are now served with a two hour window. • Breakfast 7:30am – 9:30am • Lunch 11:30am – 1:30pm • Dinner 4:30pm – 6:30pm This allows residents to eat on their schedule, at the times they choose. Blevins goes on to say

that with the stocked pantries, if a resident should happen to miss a meal they can come in anytime and choose something from the Daily Alternate menu. A server is always on duty in each pantry. Bayley residents are enjoying the changes. Camille Paolercio said she loves the new drinking goblets that replaced the water glasses they used to have. “I have arthritis in my fingers and this is so much easier to hold,” she said. “Also I really think the dining room is lovely, it feels kind of ritzy.” Not only did Bayley make these physical changes to the dining process, the Task Force also developed new menus and expanded the list of items available every day. They also evaluated the products that were regularly purchased, the recipes

used and the methods of food preparation with the goal of serving the highest quality meals. Going from tray service to person-centered dining was not an easy feat, but everyone involved can see the positive results. “These changes give us the opportunity to satisfy more residents,” said Carol. “And that is what we are here for.”

Bayley’s Endowment: Ensuring Our Mission for the Future

I

n 2010, the Bayley Endowment Fund was created to become a perpetual example of our mission to provide compassion and quality of life to those we serve. This effort ensures that no resident is asked to leave because of an inability to pay. This past fiscal year saw an average cost of uncompensated care rise from $35,000 to $43,000 per Medicaid resident. These residents are most in need of our medical and lifestyle care. To counter this increasing burden and to address the needs of future residents, the Board and staff of Bayley have overseen the growth of this endowment fund to its current total of $13.6 million. To offer options to those who wish to contribute to the endowment, four funds have been established: • The Pastoral Care Fund-created to continue the spiritual outreach of our Catholic-Christian ministry at Bayley.

• The Adult Day Fund-created by Jim Schubert as a legacy to his wife, Kathy, in honor of the vital services provided to members and care-givers by the Adult Day program. • The Benevolent Care Fund-created to provide care and services to Assisted Living residents. • The Bayley Endowment Fund – created to address the needs of our Health Care residents. Gifts to the Endowment may be made through planned giving vehicles or cash gifts. As with every gift made to Bayley, it can be done as a tribute to another loved one or friend.

Bayley Adult Day: Helping Members and Families

B

ack in 2010 Karen Knapp realized her dad George could not be alone during the day any longer. So she began to look into adult day programs. “When we came to Bayley Adult Day, I knew it would be perfect for Dad,” she said. “The pleasant staff, the cleanliness of the facility and the fact that they could provide for clients at many levels, really impressed me.” George, a former school teacher and athletic director at Batavia School, loved the interaction with staff and other members. “He was used to being with people all day, and I think he missed that when he retired,” said Karen, who lives with George in Bridgetown. “Participating in activities really settles him down and he is calm when he gets home.” Bayley Adult Day, opened in 1985 at the Sisters of Charity Motherhouse. It was called Eldermount, using a combination of the words ‘elderly’ and ‘mount’ because the program is adjacent to Mount St. Joseph College. Ellie, our favorite golden retriever, spends her days visiting with our Adult Day members and enjoying all the attention they give her. Ellie, who belongs to Chris, the social worker at Bayley Adult Day, comes to work with her almost every day. In this photo, our member Marlene, spends some time with Ellie.

Kathy Baker, who is now Bayley’s Director of Marketing and Special Events started out at Eldermount back in 1989. “Part of the mission of the Sisters of Charity is to address the needs of the underserved in the community with respect, honesty and excellence,” said Kathy. “And now 28 years later we have served thousands of members and their families, fulfilling that mission.” The program was created to give caregivers a break and give seniors who need supervision a secure environment as well as social interaction. Also, with a nurse on-site, all medications and health-related issues can be monitored. “We make sure our members receive their medications when they are supposed to, and we can test blood sugar and vital signs throughout the day, if needed,” said Lynn Lander, Bayley Adult Day nursing supervisor since 1997. “Our nurse aides are with our members throughout the day and they report any pertinent change in condition to the RN on duty. The families feel very comfortable having them at Bayley Adult Day knowing that they are well supervised and with friends.” Of course that peace-of-mind is priceless to family members but it has always been the activities that kept George coming. Karen said her dad enjoys the field trips and special events, like when school children come to sing. But the most important thing to Karen and her family is the wonderful staff. “They care about Dad and all the members like they are their own family,” she said. “I just can’t say enough about them. LeadingAge Ohio Art Show In July, Bayley was proud to host the 2013 LeadingAge Ohio South Region Art and Writing Show. We received more than 200 pieces of art made by seniors who are affiliated with LeadingAge facilities. The first, second and third place winners moved on to the state competition in Columbus. Hundreds of people came to see the show and also voted on their favorite piece for the “People’s Choice” category. 10th Annual George Knittle Memorial Bayley Golf Classic

In May, Bayley presented the 10th Annual George Knittle Memorial Bayley Golf Classic at Western Hills Country Club. We welcomed 130 golfers and raffled off a 4-seater golf cart, a Golfer’s Dream Weekend in West Baden, Indiana, a Wheel Barrow of Cheer and a $1300 + Split the Pot. Hosted each year by Fox News Anchor Bill Hemmer, the classic is named after Bill’s grandfather and former Bayley resident George Knittle.
