CESAREAN SECTION: WHAT TO EXPECT?
The caesarean section is a surgical procedure in which one or more incisions are made through a mother’s abdomen and uterus to deliver one or more babies. Most commonly known as Cesarean section or C-section, this procedure should be carried out because of medical reasons, that is, to take care for the health of the mother and her baby in case of complications. However, the World Health Organization has highlighted an important rise all over the world in the last years of this procedure compared to the Vaginal Birth. Particularly, in the U.S. cesarean delivery rate reached a high of 31,4% of all births in 2009 and it is one of the most frequently performed surgeries in the country.

Regarding the rise of the Cesarean section several possible reasons are associated. Among others woman´s unmotivated requests for this procedure, changes in pregnancy conditions (i.e. the increase of multiple births, the rise of pregnant women´s age, the growth of obesity incidence), fear of malpractice liability, side effects of common labor interventions, lack of information from the healthcare institutions and professionals and incentives to perform a procedure economically more convenient can be mentioned. In summary, many actors are intervening and influencing the mothers respect to the decision making process that they have to face during the pregnancy. The society continuously sug-
gests that the delivery mode should be a free choice of the mother after being correctly informed by her doctor, but at the end, the women decide based on nonuniform criteria and in some cases partial information. Sometimes they have to deal as well with the pressure of the healthcare system, including their doctors. The issue has become a global concern for both the healthcare system and the scientific community and it is being widely diffused by journalism, non-profit organizations and parenting websites. Beyond the basics, the C-section is being analyzed by its cost, implications and the side effects for the mom and for the baby. In this regard, it is possible to find a huge amount of infor-
mation on the web from different sources that finally will arrive to the moms, who have to face one of the most difficult decisions of their pregnancy: what is the safer mode of delivery for me and for my baby? To set the framework of our research, we started by testing the query “c-section” on Google Trends to see the activity on the web. Then, we reviewed some “related searches”, such as, “after c-section”, “pregnancy after c-section” and “c-section rates”. The graphic provided below, shows a constant flow along time but it was observed that since 2008 there has been a strong activity with the two last queries, which coincides with the worldwide movement around the issue.

A comprehensive analysis was necessary to set the controversy within the subject; that is why the project includes an extensive fieldwork with Web search engines like Google and Google Scholar. The latter is described as follows:

ISSUE APPROACH: USING THE WEB FOR DATA COLLECTION
After a wide exploration, we realized that one of the biggest dichotomies inside the issue is the mode of delivery when a woman has already had a c-section: the ELECTIVE REPEAT C-SECTION (ERCS) or the VAGINAL BIRTH AFTER CAESAREAN (VBAC). Thus, we expand our research to other sources like You Tube, I Tunes and Google Play, in order to evaluate other channels through which it is possible inquiring about the controversy.

This research aimed to explore the controversy in the scientific field to starting outlining the situation. As expected, the majority of results come from medical and scientific journals, some of them from Books Google and very few from Universities. The 38% of the articles are PRO-VBAC, 13% are PROERCS and 49% are NEUTRAL.

TO VBAC OR NOT TO VBAC, THAT IS THE QUESTION
A very detailed research is being carried out in order to frame the controversy about the decision making process. In the course of the exploration it was possible to see how a second pregnancy for a woman that has already been subjected to a c-section, implies a complex dichotomy between two modes of delivery: the ERCS and the VBAC. Indeed, the controversy dates back from 1916, when Dr. Edwin Cragin presented a talk entitled “Conservatism in Obstetrics” to the Eastern Medical Society of New York and said: “once a caesarean, always a caesarean”, meaning that the first surgery would
necessarily lead to a second one, implying further risks. This thesis has been debated since the 1980s, when the American Congress of Obstetrician and Gynecologists presented the Vaginal Birth After Caesarean (VBAC) as a strong option for those pregnant women that have had a c-section previously. To sum up the general perception is still flowing in opposite directions. Nevertheless, the actors involved in the controversy are not quite specific about their position, whether PRO-VBAC or PROERCS. Many of them describe the procedure, talk about risks and benefits, but few
of them dare to take part and assume a radical position about the issue. At the end they just expose the facts and let the decision in women´s hands. In view of this situation, our research is aimed to understand the environment that enclose the women who have to decide what will be the best mode of delivery for their babies after a previous c-section. The main questions are: what kind of information are the women receiving related to the subject, what does it say and what are the sources.

After the cleaning process we obtained a corpus of 246 url, then, we categorized the actors involved by country and by category. This categorization allowed us to set an initial overview by defining what are the most active actors and where do they come from. A remarkable frequency is evident in PARENTING WEBSITES, BLOGS & FORUMS (32%), MEDICAL ASSOCIATIONS (17%) and NON PROFIT ORGANIZATIONS (11%). In the meanwhile the countries that speak more about the issue are USA (73%), UK (7%), AUSTRALIA (7%) and CANADA (5%). To conclude the textual analysis, we have evaluated the relationship established between the categories. This analysis was carried out through a network in which the proximity of the items and the thickness of the link indicate the utilization of a similar language. We analyzed the results of video research for 5 different queries (PREGNANCY AFTER C-SECTION, HOW TO DECIDE VBAC VS. C-SECTION, VBAC SIDE EFFECTS, ELECTIVE REPEAT CESAREAN, VBAC) obtaining a maximum of 40 videos per each one. This quantity was established in view of the scarce amount of pertinent videos for some queries (i.e. ELECTIVE REPEAT CAESAREAN SECTION has just 11 videos) and also because the aim is to provide a balanced information to the readers.

The previous visualization allowed us to determinate the quantity of subjects in the video previews’. The most relevant are HEALTH PROFESSIONAL, WOMAN, SURGERY and PERSONAL TRAINER. The subsequent analyses were focused on the first three, excluding PERSONAL TRAINER (as not relevant to the aim of the research). At this stage, talk about surgery, health professional and woman was just an estimate. Thus, the categories were redefined, finding the protagonist of the videos, discovering initial cataloging mistakes (i.e. health professional were mothers). Then, the analysis was conducted towards the engagement level from the You Tube users, by the evaluation of visualizations, comments, likes, and dislikes. From theses parameters was calculated the value of interaction rate, allowing us to outline an interaction level with the video, no matter if positive or negative. This criteria was always associated to the number of interactions ( to a low “visualization” and high “comments” value corresponds also a high “interaction rate” value). It is possible to note that the highest interaction rate´s values are present mostly in the vlog respect to other typologies of video).

The analysis was focused on the previews as the first visible information element for the mother, that allows her to get a initial impact about the video and to decide an information source to which approach. Since the preview is basically a fixed image, was possible to evaluate it in detail, establishing the context and the type of shot used. In order to understand what kind of information the moms can receive from the video, we analyzed the description that the users of YouTube have given to their own video. In this way it was possible to comprehend what actors communicate in a complete and detail mode. Then, 5 categories were defined to establish the typology of the description:
· Additional / more information: these are the descriptions that provide additional information regarding the title. · Elementary / basic information: these are the descriptions that explain the content of the video keeping the connection with the title. · Trivial / inconsistent information: the users include texts that do not refer to the content of the video or merely gives thanks and greetings. · Link: in the description they are present the link to the social profile of the users, their websites or the hospitals, journalist and institutions that are involved in the video. · No description: the user does not input any kind of comment. In such a way, it was possible to note who is paying more attention to details, informing the user in the most effective way.

A wide exploration of the controversy allowed us to recognize the close correlation between the c-section rise in the last years and the activity on the web according to the indicator reported by Google Trends. It is evident that lately the society has had a great interest in this subject, specially because actors from different positions are taking part in the discussion. Even if the actors are not very radical or explicit in their positions, they play a remarkable role. For instance Healthcare & Government Institutions tackle the issue from the public health perspective, emphasizing on the VBAC as a safe procedure that can avoid a dangerous surgery and safe lives. The actors’ network let us know how this category acts as information “supplier”, while the categories are more likely information “spreaders”. This means that the suppliers are entitled to bring official information like guidelines of the procedures, studies and statistics, that later will be diffused by the rest of actors. The role played by the Parenting websites, blogs and forums is as well very important because they are closer to the parents and
that is why they might be more influencing. However, they merely explain the conditions, risk and benefits for both procedures, but they let the future decisions about the mode of delivery in woman´s hands. By the other side, some non – profit organizations and specialist blogs are leaders on exposing “unknown” or “real” information. Their aim is to provide evidence-based sources and support regarding the issue. In most cases this organizations are PROVBAC and try to enlighten the women about a procedure that is seen as a “dangerous option”. Some of this websites are recommended by Government Institutions as a source of information. Nevertheless, some PRO ERCS actors were found. They emphasize that the VBAC is a Government strategy to reduce the c-section rates and that in some cases it is performed even if the women have not the required conditions. The kind of language differs according to the category of the actor. Those who talk directly to the mothers (like NON – PROFIT ORGANIZATIONS and PARENTING WEBSITES, BLOGS & FORUMS) use a common

language and often tackle the issue in a reassuring way, because they are aware of the difficult situations that the moms have to deal with during the pregnancy. On the other hand, the categories addressed mainly to health professionals and academic community use specialist language, prevailing scientific and medical terms, that may be considered as “shocking language” by moms. Nevertheless, it is worth pointing out that these actors use this kind of language not because they want to scare the public, but because it is the right way to refer to the subject. In any case, as we could see the web offers a variety of information for the users by exploring specific search engines like Google Scholar and video – sharing websites like YouTube. In the first case, due to its scientific nature it was possible to see how the controversy is tackled in a very objective way, presenting many studies performed by universities and hospitals that mostly conclude that further studies must be done about the controversy. This is a source of information often consulted by academics and professionals, instead of mothers.

Meanwhile, the research on You Tube showed how this is an open channel that allows publishing different kind of contents. After analyzing the typologies, the actors and the protagonist of the videos, we were able to conclude that there are three main kind of videos: those held by health professionals (presenting the subject through interviews), the surgeries (explicit images and explanations about the procedure) and those held by women, which in most cases are the protagonist of the videos. It was possible to evidence that You Tube is a very consulted source of information, maybe because of their visual contents, that represent an advantage compare to the regular websites that just provide textual information. Also is a source very close to the moms because most of the videos are testimonials and provide a closer approach to the woman´s position in the discussion. It is important to mention that the impact produced by the images and videos are fundamental on the woman´s perception about the issue.
In response of our research question, we can say that the mothers receive a wide range of information from the web, some of which enlighten them about the doubts that can exists respect to the mode of delivery they have to choose. The value of the information comes, not just from the contents, but also from the provider and the way in the facts are presented to the public. In conclusion, it is possible for the moms to find a more “friendly-closer” environment on the parenting and non –profit websites and on You tube, rather than the government and medical institutions, also because the latter are not directing their speech to the moms, but mostly to other institutions and professionals. The governments, the healthcare system and the society play a basic role in the framework of the controversy, but at the end the decision is in woman´s hands.
