Patient-Centered Primary Care Collaborative of Central Ohio participation criteria for listing health plans and self-insured employers
Enhanced Payments: Purchaser commitment to payment reform for NCQA recognized patient-centered medical home (PCMH) practices – each purchaser will be asked to provide us with a written document that outlines the type (not amount) of enhanced payments for PCMHs in their provider network. Note: enhanced payments can be any type of incentive based methods (PMPM, pay-forperformance, risk-sharing agreements, etc.)
PCMH Improvement Dashboard: Purchaser commitment to share aggregate utilization data for covered lives in participating PCMHs with Access HealthColumbus for purposes of measuring improvements over time. Note: current measures are ER visits, hospitalizations, hospital readmissions, and generic prescription utilization.
Participation in Joint Planning Sessions: Collaborative with others to assist Access HealthColumbus planning activities for advancing the work of the Patient-Centered Primary Care Collaborative of Central Ohio
Participation in Quarterly Learning Sessions: Participate in quarterly learning sessions hosted by Access HealthColumbus
