MESSAGE FROM THE AACHC CEO AND BOARD CHAIR Hello and thank you for your interest in Arizona’s Health Center program and our Annual Report. No one will dispute 2020 was a year of unprecedented changes and challenges for our country as we continue to battle COVID-19. We have certainly never heard, “you’re on mute,” more than in the past year! While challenges came in all sizes and from unexpected places, the Arizona Alliance for Community Health Centers and its members addressed them all. Never before in the history of the Health Center program have we faced a pandemic like COVID-19. But just like the many natural disasters encountered over past decades, Arizona’s Health Centers stood tall and rose to the challenge. From the start, we mobilized our Health Centers to assure our communities received the needed care including providing critical COVID-19 testing sites around the state. Alliance staff spearheaded work with our state partners to bring millions of dollars in funding to support these testing sites. At the same time, staff worked with Direct Relief, Protect Arizona, Arizona Coalition for Healthcare Emergency Response (AzCHER) and many other organizations to deliver vital Personal Protection Equipment (PPE) across the state to assure our providers and staff could do their jobs as safely as possible. We cannot thank these and many more of our partners enough for their efforts. None of this work could have been accomplished without Arizona Health Centers’ own Health Care Heroes. From providing frontline workers for testing sites to completely changing how patient care is delivered, our Health Care Heroes delivered! The dedication of these selfless individuals is a testament to the Health Center program and truly makes Arizona a better place to live. The Health Center program mission to increase access to care remains strong and growing. The most recent Uniform Data Set (UDS) indicated significant increases in patients to nearly 750,000 patients served, or 1 in 10 Arizonans. Our Health Centers continue to expand into new communities as well as add needed services including dental, behavioral health and substance use disorder. Most importantly, Health Centers migrated to new and innovative services such as telehealth to assure our patients continue to have access to their healthcare provider during the pandemic. 2020 was also a year of change in leadership for the Alliance. After over a decade as Chief Executive Officer, John McDonald retired. On behalf of Alliance staff and all our Health Centers, we wish John a wonderful retirement. His work will not be forgotten. He left the Alliance and Arizona’s Health Center program in a better place from when he started. As you look through our 2020 Annual Report, we hope you are encouraged by the work the Alliance is doing to improve the lives of all people living in Arizona. We look forward to partnering with you to continue these efforts in 2021.

TRAINING AND TECHNICAL ASSISTANCE AACHC offers many opportunities for training and technical assistance to support our four core pillars - PCA Base Grant, HCCN, Outreach & Enrollment, and Workforce, as well as all supporting programs. After February of 2020, we pivoted to virtual learning for all events due to the pandemic in order to continue to provide valuable education opportunities.

HEALTH CENTER CONTROLLED NETWORK

The Network, made up of 28 Health Centers in Arizona and Nevada, is focused on leveraging the use of Health Information Technology tools, Data Analysis, and Integration to:

COVID Supplemental Funding: Created Business Intelligence Self-Service Dashboards (tri-state, AZ, NV, UT) to track data analytics around COVID-19, based on identified needs around data-based emergency response, tracking of clinical measures, and decision-making processes.

HCCN also developed Clinical Guidelines for research-based customized workflows to meet the needs of Health Centers while providing patient care during COVID-19.

OUTREACH AND ENROLLMENT SERVICES

Outreach and Enrollment (O&E) provides support and technical assistance to all AACHC members and receives funding from CMS Navigator and Connecting Kids to Coverage programs. The O&E department focuses on healthcare coverage and access, increasing health literacy and equity in local communities. Activities below reflect both grant programs which consist of 11 sub-recipients (8 are Health Centers). In addition to providing direct services to consumers, the O&E department provided trainings, marketing/messaging materials (generating over 1.8 million media/outreach impressions), and resources/tools to sub-recipients and other community partners.

RURAL HEALTH PROGRAMS ARIZONA RURAL WOMEN'S HEALTH NETWORK (AZRWHN) AZRWHN is a collaborative partnership that works towards addressing issues in Arizona's rural areas that affect women and their family members.

Mission: To build our network partners' capacity to cultivate and promote innovative policies, practices, and services that improve the health of women in rural Arizona

Implemented Screening, Brief Intervention, and Referral to Treatment (SBIRT) in 7 rural communities that are served by Canyonlands Healthcare throughout northern and eastern Arizona

In partnership with ACESDV, revised Sexual Violence Training Curriculum for Community Health Workers and conducted two online trainings

Recently awarded a Rural Communities Opioid Response Planning Grant
