What is

Live Young Wellness Spa?

If a person were to ask one of our long-time patrons, “What is Live Young Wellness Spa?” They would likely answer, “It’s a great place to get botulinum toxin and filler,” or potentially, “It is where I get my hormones managed. I love the team there.” While these are both true, Live Young Wellness Spa is so much more. At Live Young Wellness Spa, there are a few principals we hold dear.

First — we will serve our customers. We understand you have chosen us for your aesthetic and wellness needs. With hundreds of other options in the DFW area, you have chosen us. We hold that sacred. We will do our very best to serve you.

Another valued principle — if the treatment does not work, we do not sell it. There is a lot of snake oil, so to speak, in this business, and honestly, it is why I do not offer non-invasive body contouring treatments. As of early 2024, I have not been able to convince myself there is enough of a difference after the treatment to sell them to our customers. We work very hard to vet the new treatments on the market before offering them. We hope this helps build trust.

Finally — we will be the best at what we do. I explain to all new team members, if you were the top 1% of authors, you probably would have written the Harry Potter series or if you were the top 1% of quarterbacks, you would play for the Dallas Cowboys. Okay, maybe the Kansas City Chiefs, but you get the point.

My entire team understands they will need to continuously work to improve and get better at their job if they want to stay on the team at Live Young Wellness Spa. Our laser technicians will be the top 1% of their field. Our injectors, our customer service representatives, and our managers will all work and actively seek out opportunities

for growth and learning, so collectively we are all great at what we do. To put it another way, we will be top 1%ers.

More than just a location or a team, Live Young Wellness Spa is a lifestyle or ideal. The people who come here want to make the most of every minute of their lives and intently look for quality and purpose for the entirety of their lives.

We want to help people look their best and feel their best so they can maximize every minute doing what they love. Life is finite. With that understanding, we encourage goal setting and celebrate when those, sometimes elusive, goals are achieved.

It has been said, “In every older person there is a young person wondering what happened.” It’s true. But who cares? Keep pushing.

Evoking a Rolling Stone Tradition

For those who have heard my operating room play list, they know I love a wide variety of music.

Many years ago, I subscribed to Rolling Stone magazine, and every month I looked most forward to a two-page section on new music or new artists. There were specific song recommendations and a quick blurb about the artist. It was frequently one gem after another of songs and artists I had never heard. It kept me up to date, and at the time my kids were young, I felt like without it, I would enter a dark age of music so to speak.

How many can relate to endless Barney songs or Frozen soundtracks to keep your little ones calm while running errands? I recently re-subscribed only to find out Rolling Stone no longer has the short section with new song recommendations. Well, my kids are older now and I have decided to re-light the torch and attempt to keep those who enjoy new music but may not live in a world

[Rolling Stone] kept me up to date, and at the time my kids were young, I felt like without it, I would enter a dark age of music so to speak. “ ”

where they are introduced to it, up to date on the tunes.

Two things — 1) I am not a music major, and 2) I do not love all these songs, so please do not shoot the messenger. My process includes asking people younger than 25 years old to send me four songs from newer artists they have recently heard and liked. I listen to the songs several times over a period of weeks and then report. Please enjoy the following section and expect to see it quarterly.

JOSHUA QUIMBY, “Connecticut Fall”

Quimby’s glasses, hair, and mustache combo give him a bit of a serial killer look, but do not judge him on appearance. He is from a small town in eastern Connecticut. His song, “Connecticut Fall,” is undoubtedly straight from the heart.

His voice has a rough quality, but it seems to work. His lyrics, at least on “Connecticut Fall,” are elementary, but combined with his playful guitar chords, you may find your toes tappin’ to this one. This song did not make my operating room playlist, but it could probably find a home in a playlist reserved for family, back porches, burgers, and beers.

ZACH BRYAN, “Sarah’s Place”

Okay, most of you know Zach Bryan. If you don’t, then stop whatever you are doing right now and go listen to him. I don’t care if

you are talking to your wife, coaching your kid’s baseball team, or driving home from work, find him and listen. I recommend, in this order:

1. “Something in the Orange”

2. “Whiskey Fever”

3. “Oklahoma Smokeshow”

He cranks out one great song after another with intelligent lyrics. There is a reason he is selling out big venues like AT&T stadium. I had never heard “Sarah’s Place,” but Zach Bryan teams up with on this one Noah Kahan, who I like too.

LIL YACHTY, “A Cold Sunday”

This one has explicit lyrics so don’t listen if it offends you. I like the beat, but it seems rushed, almost like he did the song in freestyle and recorded it. Rap/hip-hop holds about 20% of my playlist real estate, but this one did not make the cut.

BACKSEAT LOVERS, “Kilby Girl”

The lead singer/ guitarist Joshua Harmon and guitarist Jonas Swanson met while waiting in line for an open mic in Provo, Utah, and later added their drummer and bassist. The verses are slower with decent lyrics but the song would have been better if they cut the instrumental by about 45 seconds.

Chance Encounter with Dog Changes Woman’s Life

It was a beautiful evening, and Ms. Smith (name changed at her request) decided to get some exercise and take her dog for a walk. Both were excited to feel the breeze and enjoy one of the few days Texas offers with seventy-degree temperatures. Unfortunately, her walk would not end as planned.

Ms. Smith has volunteered for years at a local shelter for dogs and has a wealth of experience in caring for animals — strays as well as her own. She was experienced with dogs to say the least. When she saw a forty-pound stray on the sidewalk ahead of her, the mutt had a tag and showed no signs of aggression, so she continued straight ahead with her own dog on its leash.

Every year about 4 million Americans are bitten by dogs — with almost half of the victims under the age of 14.

Once within a few steps, the mutt’s attitude shifted from Milo to Cujo. With lightning speed, the dog showed its teeth and lunged. Ms. Smith moved to protect her own dog scooping him up quickly and attempting to position her back to the attacking animal, as she had been trained. She was not quick enough, and the dog’s teeth came down on her face detaching part of her lip. The ani-

mal withdrew, but the damage was done, and blood began to pour down her chin. With her own dog thankfully unharmed, Ms. Smith, afraid to look in a mirror, headed to the ER.

Dog attacks are unfortunately not an isolated incident, and every year about 4 million Americans are bitten by dogs with almost half of the victims under the age of 14.

Injuries usually involve a crushing component as well as puncture wounds. Bacteria can embed in the wounds, so preventing infection is the first step. All dog bite victims should get a short course of preventative antibiotics and wound irrigation. The old surgery adage, “Dilution is the solution to pollution” means irrigating a wound will help prevent infection and it does. At home, you should wash the wound with soapy water.

It is really important to help decrease the bacterial load inoculated into the wound. Is washing it painful or uncomfortable? Yes, but do it. To close or not to close is a question emergency room doctors often ask themselves, and it is often a judgment call.

In my career, I have managed more dog bites than I can remember, and my approach changes a bit depending on the location and type of injury. Puncture wounds are more likely to get infected than tears or open wounds.

In areas where we have good blood supply, infection is less of a risk. On the face and

scalp, we have a wonderful blood supply so I typically irrigate these wounds well and close them with layers of sutures to help prevent scarring. Bites below the face, I usually leave open and do wound care and frequent washouts to prevent infection. After a few days, we can usually close the wounds surgically.

Ms. Smith made her way to the ER, and because a portion of her lip was missing, the perceptive ER doctor gave her antibiotics and a tetanus shot and referred her to me for closure. I saw her the next day, and under local anesthesia mobilized the remainder of the red portion of her upper lip to replace what was missing. Her scars are very immature, and with time, will likely be only minimally noticeable.

The best lessons are those we can learn through others. Remember to keep your face away from unknown dogs. Respect how quick they are. We all love our pets but in a fraction of a second, a dog can completely change a life.

Keep Moving Keep Moving

Hiking a Hidden Gem at Eagle Mountain Park

My father, who is in his early 70s, picked up his golf clubs again to make another go at playing the game. Over the last few decades he has developed a pattern of playing golf, getting frustrated and quitting, only to pick the clubs back up again a few years later. It reminded me of someone trying to quit smoking cigarettes. This last time, I really thought he was done. So when I received a text with an image of his new clubs, I had to ask, “Dad, why now?”

I will never forget his answer. “The older I get, the more I realize I have to keep moving. You can never stop moving.”

It’s true — the older you get and the less you stay active, the faster that final curtain begins to fall.

In the spirit of “keep moving,” this morning, I set out and hit one of our DFW local hikes. Eagle Mountain Park is 30 minutes northwest of Fort Worth and well worth the drive. Parking was easy, and I chose a five-mile loop, which would take me along Eagle Mountain Lake for a good portion of the hike. If you wanted a little more distance, you could make both loops of the trail, which would take you about seven and a half miles.

If you’re new to hiking, I highly recommend the All Trails app. The

app is free and has every hike in the country mapped out. For a reasonable fee, you can download the trail maps in case you are off the beaten path with poor cell service.

With my shoes tied and the 70-degree air flowing freely through my lungs, I took a left out of the parking lot and began my hike. A few minutes in, with the street noise far behind, it was easy to forget I was anywhere near the urban sprawl of the Dallas-Fort Worth metroplex.

The trail was not crowded, but other hikers were there enjoying the weather as well. The back of my neck played peek-a-boo with the sun as I went in and out of shady sections of the hike. This made for a nice change of temperature. The recent rains left my shoes a bit muddy with several low-lying areas of the trail covered in mud. This did not take away from what was great exercise and a wonderful hike. I’ll sacrifice a pair of old shoes every time to enjoy a lake view.

If you are looking for a way to spend a couple of hours getting exercise and taking in beautiful scenery not far from home, make the trip to Eagle Mountain Park. There are several benches with nice views set up along the trail in case a picnic is in the plans. Wear older shoes, especially if it has rained recently, and use caution as some of the wet portions of the trail were slippery.

Dogs are not allowed at the park, and the threatening signs in the park seem to indicate they mean it, so leave Rover at home.

If you follow any celebrity news outlet, you have more than likely heard of Ozempic or its generic counterpart — SEMAGLUTIDE .

No prescription medication has sparked as much curiosity and conversation since possibly Viagra. What started as a diabetic treatment in 2017 has now become a highly sought after (and FDA approved) weight loss treatment.

Public opinion on semaglutide is still up in the air, however. Critics may say it’s the lazy way to lose weight for those who don’t want to “work” for it. Proponents will argue it’s a life-changing treatment for some who have been putting in blood, sweat, and tears to get to a healthier weight without success.

As a physician assistant who has prescribed Ozempic since it entered the pharmaceutical space seven years ago, I have invested time into researching on behalf of my patients and listening to patients’ experiences with semaglutide.

WHAT IS DIABETES?

Since semaglutide was developed as a treatment for diabetes, let’s understand the disease it was created for.

Type II diabetes mellitus is a chronic disease that essentially causes patients to have high blood sugar. Consistently high blood sugar can lead to damage of the kidneys, eyes, heart, brain, and every other vital organ and process in the body.

One key player in this process is the hormone insulin. Insulin helps drive sugar (or glucose) circulating in our blood into our cells to then be utilized as fuel. Think of insulin as the key that unlocks the door into the cell so glucose can transfer from the blood into the cell where it can be utilized.

In type 1 diabetes, the pancreas does not make enough or any insulin. Semaglutide does NOT treat type 1 diabetes.

Type II diabetics are still creating insulin, however their body does not utilize it well. Prediabetes is the precursor to type II diabetes, where blood sugar and insulin levels have begun to rise but are not quite to the levels of type II diabetes.

The CDC estimates that 1 in 3 Americans have prediabetes, and up to 80% of them are

likely unaware of it. Prediabetes usually has no symptoms at all. As part of your annual physical, your primary care provider should draw fasting bloodwork. You will then have a snapshot of your blood glucose levels. A lab called glycated hemoglobin — also known as hemoglobin A1C — can give you an idea about what your average blood sugar has been for the last three months. This lab gives us a much clearer insight and makes it much less likely to “cheat,” as it is not affected by a few days of eating clean.

DIABETES, INSULIN, AND WEIGHT GAIN

Insulin is an essential hormone that manages our energy storage, and blood sugar levels. When you are hungry, your blood sugar levels are typically lower and therefore insulin levels are lower as well.

Because your body needs a certain amount of glucose in your blood at all times, another hormone produced by the liver assists in changing long chains of glucose, called glycogen, into individual units of glucose to keep your blood sugar level from getting too low.

Simultaneously, your body is breaking down fat to use as energy. When this process occurs, it signals to your body that you

need food, and therefore start to feel hungry. You then indulge and crush half a pizza. This triggers your pancreas to start working overtime and pump out insulin to help deal with the surge of glucose from the ingested pizza. When the level of insulin goes up in your blood, your body shifts from fat burning to fat storage.

When you have pre-diabetes, your cells are just starting to become resistant to insulin. Much like too many phone calls from an annoying relative, you begin to ignore the ringing. When insulin is constantly knocking on the doors of your cells, your cells stop answering.

This is not okay with your pancreas. It will secrete more and more insulin to get the blood glucose levels down a bit. Higher levels of insulin means a longer period of fat storage and you become… well, fatter.

How do we clasically treat diabetes? Typically, diabetes is treated with drugs that lead to increases in insulin directly or with injectable insulin.

Do treatments like this make you gain weight in the form of fat? They absolutely

can, but consistently elevated blood glucose quickly destroys organs, so the pros outweigh the cons.

Lowering detrimental levels of blood sugar is a non-negotiable in medicine. People who walk around with blood glucose levels in the 300s or higher have irreparable damage to their organs, which can certainly lead to earlier deaths.

We are limited to only a few drugs that can treat diabetes and reduce weight (rather than stagnate weight or even increase weight).

Semaglutide is one of the very few that can help weight loss, treat blood sugar concerns if present, yet not cause hypoglycemia in non-diabetic patients. This is quite a rare occurrence in the realm of diabetic medications.

HOW DOES SEMAGLUTIDE WORK?

Semaglutide treats type II diabetes by mimicking another hormone (GLP-1) to help the pancreas release more insulin when the body needs it. It also reduces the amount of sugar the liver produces and releases. The way it helps with weight loss is by sending signals of satiation to the brain.

It also slows down stomach emptying, further reducing the appetite. The signals of satiation to the brain and the delay in gastric emptying more than offset the added insulin, and your scale has less work to do every morning when you stand on it.

WHAT ARE THE MOST COMMON SIDE EFFECTS?

The most frequently reported side effects of semaglutide are typically gastrointestinal complaints, as that is one of the body systems the medication directly impacts. Nausea, constipation, diarrhea, stomach upset, headache, fatigue are all possible side effects that have been most frequently reported. However, the slow titration over months is what reduces the frequency and severity of these.

WHAT’S THE TREATMENT PROCESS LIKE?

Semaglutide is a once-weekly injection. The dose incrementally increases each month until the maintenance dose is reached (month five). The slow titration allows the body to adapt to the medication and reduces the occurrence of side effects. Some patients find they begin losing weight even on the lower doses, while others notice results when they reach mid to high doses.

There is currently not a concrete protocol on treatment duration. Most medical experts would agree that obesity is a chronic condition, and therefore, staying on semaglutide long-term would not be unreasonable. There could be two approaches to this, with neither being the wrong answer. The patient could either a) discontinue treatment once goal weight is achieved or b) lower the dose or frequency and continue to maintain the weight loss that was achieved.

WON’T THE WEIGHT COME BACK ONCE I STOP?

Whether weight loss is achieved through diligent dietary changes, a new exercise regimen or a prescription medication, once it is discontinued, the weight can climb back up. Although remaining on the medication at a lower dose is an option, most patients prefer to avoid chronic medications. That is why as a prescriber, I always explain to my patients that while semaglutide is an incredible tool, it is not a magic bullet.

I view the time that someone is on semaglutide as a cushion period to make lifestyle changes while staying motivated. What I have heard from patients repeatedly is that they “fell off the wagon” when attempting to lose weight in the past.

All at once, they began eating clean, exercising four times a week, cutting out all treats and truly giving it their all. After weeks of arduous meal planning, early morning workouts and politely declining birthday cake, the number on the scale barely budged. This experience would be very discouraging and could lead someone to give up the effort.

Enter semaglutide. The medication will help patients lose weight while they make the same lifestyle changes previously attempted, with the only difference being they are seeing progress. Fast forward to one year later, and the patient would like to discontinue the medication as they have achieved their goal weight. They have had one year of consistency to instill habits, change behaviors, and weaken cravings. Weight loss maintenance after treatment cessation is realistic and achievable in this scenario.

Okay, so semaglutide reduces appetite = less food intake. If I have never had success losing weight in a calorie deficit diet, why would semaglutide be any different?

This is a question I personally had and looked into when I started prescribing semaglutide for weight loss. The postulated answer involves a hormone called leptin, which is released by our fat cells. When enough leptin is released in a calorie deficit state, our brain gets the signal “we have a high amount of fat, let’s use it for energy.”

However, our body can develop leptin resistance, which means the brain does not recognize the high levels of fat. Semaglutide can actually increase the body’s sensitivity to leptin, and therefore burn more fat if combined with the lower calorie intake.

IS THE COST WORTH IT?

This is a question only the patient can answer. Although the cost of the medication has become notably more affordable over the last two years, it is still a factor to consider. Generally, the path to weight loss is full of expenses. There are countless other supplements, weight loss programs, gym memberships, and personal trainers.

Most will consider this “worth it” in the long term to get to a healthier lifestyle. Using a prescription medication like semaglutide should be no different, especially since there are studies proving its efficacy. The cost of life-long weight-related chronic conditions outweighs the cost of the semaglutide for the time period someone is taking it. Specialists visits, hospital stays, chronic condition medications over the years will all overshadow the cost of semaglutide.

FINAL THOUGHTS

Although decades of research proves that obesity is a complex lifelong disease, as a society, we are in denial. The age-old thought of “eat less, exercise more” is not wrong per se — however, we need to recognize that there are numerous variables in the equation. Just a few examples for any given individual — environment (family dynamics, early childhood habits, food resources), genetics (hunger and satiety responses, how easily we store or burn fat), medical conditions (hormone imbalances, PCOS) or side effects of medications (steroids, antidepressants).

The perception that those who are obese or overweight are just a result of bad personal decisions is one that needs to be refuted. We now have access to an effective and safe treatment that can improve both the quality of life, and quite possibly the quantity of life by reducing co-morbidities (high blood pressure, diabetes, high cholesterol, the list goes on). In my eyes, semaglutide is not a cheat or a quick fix. Instead, it is an effective tool to help optimize the health of individual patients, and therefore, our society at large.
