MEMBERSHIP MEDICAL SUSPENSION REQUEST

1. Golf Cart “Medical Flag” policy for players with disabilities. a. A Member who cannot adhere to the normal cart rules on the golf course due to a handicap or physical ailment may request a “Medical Flag.” b. An application for “Handicapped Cart Privileges” can be obtained from the Pro Shop and must be completed and returned with a Member’s physician letter and returned to the Pro Shop for approval. The application must indicate whether the disability is temporary or permanent. If temporary, an end date must be specified. c. If the use of a “Medical Flag” is approved, a flag shall be purchased in the Pro Shop and kept permanently in your bag or on the cart designating that the Member has special cart privileges. d. A DMV Handicap license plate qualifies for a “Medical Flag.” e. A player who qualifies for a “Medical Flag” must attend an orientation session with the Professional Staff to review the rules for use. f. The “Medical Flag” expires one year from the date the application is approved, or the end of the disability period, whichever comes first. The player is responsible for renewing this privilege.

2. Standard cart restrictions apply – Utilize the 90-degree cart path rule or the Gate System when applicable. 3. Must abide by the “Carts on Cart Path Only” rule when in effect. (No exceptions) 4. Shall not drive on any newly sodded grass areas, wet areas, or ground under repair. 5. Drive no closer than 30 feet from the green. 6. Must not drive between a bunker and the putting surface. 7. Carts must remain on the paths on all par 3s and shall stay on the cart path around all tees. 8. Shall not drive into bunkers, washes, or desert surrounds. 9. Stay on cart paths whenever possible. 10. If two players with Medical Flags are in the same group, both players shall ride together in the same cart. (Subject to change at the discretion of the Director of Golf) 11. The Golf Course Superintendent has the final say regarding the use of carts when conditions make it likely to damage the course. 12. The Views Golf Club may suspend the privilege to use a Medical Flag.

MEMBERSHIP MEDICAL SUSPENSION REQUEST

Should a current annual golf member of The Views Golf Club become incapacitated during that year, several conditions must be met before receiving consideration to obtain credit for this time:

1. Within two (2) weeks of sustaining an accident or a medical condition, the member must submit written documentation describing the situation which prohibits golf activity to the Director of

Golf. 2. A physician’s statement must be submitted stating the nature of the accident or medical condition including when the treatment began and when the patient was released.
