Highlights for 2022

NO CHANGE TO EMPLOYEE PREMIUMS FOR MEDICAL, DENTAL AND VISION INSURANCE

• Employee premiums, deductibles and copays/coinsurance will not change in 2022 for medical, dental and vision insurance. • Importantly, although medical insurance costs for the university are expected to increase from 2021 to 2022, employees will see NO CHANGES to their premium costs in 2022 because the university was able to offset the increase through effective management of our Total Rewards spending and absorb the remaining portion of the expected increase in its operating budget.

WELL-BEING VENDOR — MARQUEE HEALTH

• Marquette University is moving the wellness vendor partner from Virgin Pulse to Marquee Health. • The Marquee Health program and activities will have a continued focus on physical, mental, spiritual, emotional and social well-being. • Employees and spouses participating in our medical plan will have the opportunity to earn the same amount of

HSA or FSA dollars for completing wellness activities.

CHRONIC CONDITION MANAGEMENT — TRIA HEALTH

• Marquette University is partnering with Tria Health, a condition management program that is a free and confidential benefit for eligible employees, spouses and dependents enrolled in our medical plan who have diabetes, heart disease, high cholesterol, high blood pressure, mental health disorders, asthma/COPD, osteoporosis or migraines. • Eligible members who actively participate in the program will have an opportunity to receive discounted medications and equipment for managing specific chronic conditions. Tria Health will contact eligible members directly via phone and email with more details on the program.

VOLUNTARY SPOUSE AND DEPENDENT CHILD(REN) LIFE INSURANCE — METLIFE

• Employees who are enrolled in the Optional Life Program can purchase voluntary life insurance for their spouse and dependent children through MetLife. • The spouse benefit can be purchased in $25,000 increments, not to exceed the lesser of $75,000 or 100% of the employee’s Optional Life election, and a flat $10,000 benefit for each covered child age 26 or younger. • If the employee is newly enrolling in Optional Life, they are required to go through medical underwriting.

Underwriting for their spouse is required if the benefit exceeds $50,000. No underwriting is required for children regardless of age.

ACCIDENT COVERAGE — METLIFE

• A new voluntary benefit offered through MetLife will provide payment directly to you if you or any family member who you choose to cover experiences an injury due to an accident. This is in addition to any benefit received under the medical plan.

LONG-TERM DISABILITY CARRIER — METLIFE

• Voluntary Long-Term Disability coverage will move from Northwestern Mutual to MetLife; employees will see a reduction in premiums with no change in coverage.

OTHER CHANGES IN COVERAGE

CURRENT NEW FOR 2022

Insulin pumps and pump supplies covered by Navitus for diabetes care Benefit will be administered by UMR (rather than Navitus) to provide discounted provider rates that will benefit members and the Plan.

Continuous Glucose Monitors Covered by Navitus for Diabetes Care

50-year-old age requirement for colonoscopies Deductible is waived currently for: • Inpatient hospital visits • Newborn hospital stays • Infertility benefits • Hearing aids • Foot orthotics • Acupuncture Benefits will now be administered by UMR and Navitus (rather than Navitus only) to provide a benefit through the medical and prescription drug Plan. Removing the age 50 requirement: colonoscopy will be covered at any age based on medical necessity.

Deductible and Coinsurance will apply to these services to ensure consistency in coverage across all three medical plans offered through Marquette.
