We continue to enhance our services to meet community needs, focus on improving safety and providing innovative care. Medical Center of the Rockies Poudre Valley Hospital

For instance, PVH was recently re-designated for the fourth consecutive time as a Magnet Hospital for Nursing Excellence by the American Nurses Credentialing Center. PVH is one of ten hospitals in the nation re-designated four times as Magnet, the nationally recognized gold standard for nursing excellence. Magnet designation symbolizes years of hard work, tireless commitment, endless training, success and quality, especially for patients. Even now, 20 years after the Magnet program was launched nationwide and then moved onto the worldwide scene in 2000, only seven Colorado hospitals have been designated. MCR is one of those hospitals. The Loveland hospital earned its designation in October 2010 and is developing an application to submit for re-designation in late 2014. One significant endeavor completed in 2013 at PVH, MCR and other UCHealth hospitals and clinics throughout the Front Range was the implementation of a top-of-the-line electronic medical records system called Epic. UCHealth now has more connectivity along Colorado’s Front Range than any other healthcare organization including easy online access for patients. Through the internet, all patients can access My Health Connection—our Epic portal—through which they can view their own online medical records. The service also offers access to test results, appointment reminders, and the ability to communicate with physicians in a safe environment. Meanwhile, in late 2013, we added a new MCR unit dedicated to caring for women and children. The unit features 12 spacious and private patient rooms for children from birth to age 17, to women having gynecological surgery and new postpartum mothers and their babies. In June 2014, we will open our new cancer center, an $11-million, 30,000-square-foot facility on the Harmony Campus in southeastern Fort Collins. Our goal is to provide a better cancer-care experience that meets the needs of patients in one location. The cancer center is part of UCHealth’s expansion of inpatient and outpatient cancer treatment and support services in northern Colorado. In addition to the services that have traditionally been offered at PVH, we added inpatient cancer treatment in 2010 at MCR. The UCHealth cancer program also extended its medical oncology and outpatient infusion clinics to Greeley and Loveland. We are also underway with a project to replace the old section of PVH at the northwest corner of the hospital with a two-story, 113,000-square-foot-medical building. This will allow us to expand PVH’s emergency room to meet a projected increase of ER patients from the current 52,000 a year to 70,000. The new building will also increase capacity for the operating rooms, orthopedic services, infusion services, wound care, and women and children’s services. For patients, the presence of UCHealth in northern Colorado means more opportunities for highquality, compassionate care, improved medical record access and, among other benefits, new services such as the cancer center. Against the backdrop of a changing health care industry, a number of changes are taking place that will have a significant impact in Fort Collins and the surrounding area for years to come. I am pleased that construction of the Banner Fort Collins Medical Center continues to progress with a grand opening scheduled in April 2015. This new hospital will feature an emergency department, surgical services, lab and pharmacy services, an outpatient specialty clinic, labor and delivery rooms, medical imaging and a 24-bed inpatient unit. A medical office building is also planned for the campus located on Harmony Road and Lady Moon Drive in the southeastern part of town. While 24 beds may seem like a small number, we are building a sizable infrastructure so we can quickly expand as demand increases. This will enable us to better serve our rapidly- growing patient base in Fort Collins. Banner Health may be a new name for some of you, but we have operated medical clinics in Fort Collins since 2001 when we opened the Banner Health Clinic at 303 Colland Drive on Carpenter Road between U.S. 287 and Lemay Avenue. In September, we opened a Banner Health Center at 702-A W. Drake Road with physicians specializing in family medicine, pediatrics, internal medicine, allergy/asthma, and pulmonology. We also offer laboratory services and medical imaging in the facility, making it convenient for you to receive a variety of services under one roof. Our roots elsewhere in Northern Colorado run even deeper. We have dozens of health care clinics in Weld and Larimer counties. We operate North Colorado Medical Center, which evolved from the first hospital built in Greeley in 1904, and we own and operate McKee Medical Center which opened in Loveland in 1976. Much has changed since those two hospitals opened and the pace of change is accelerating. As the government continues its efforts to reduce the cost of health care in the United States, we expect Medicare reimbursements to decrease even as the volume of patients we serve continues to grow. In order to meet these potential financial challenges, we are driving cost out of the system by reducing variation and increasing reliability in all areas. This means our physicians use the same evidence-based practices system-wide to ensure you receive consistently high quality care. The results of these efforts are reflected in awards recently received by McKee and NCMC. The floods last fall devastated many areas of this community. To help support relief efforts, we worked to spearhead volunteers and have donated more than $300,000 to various Northern Colorado-based projects. Kaiser Permanente recently issued $1.4 million to 21 Colorado school districts through our Thriving Schools campaign in an effort to make good health a part of everyday life at school. Poudre School District is the recipient of a $100,000 Thriving Schools grant and will use this funding to improve staff and student wellness through projects such as movement break videos, bike fix-it stations at high schools and standing desks for employees. In early 2014, Kaiser Permanente also announced an $85,000 grant to the City of Greeley to develop greater infrastructure for safe bicycling routes. Those needing health insurance—and on the receiving end of its benefits—see the ACA as a delivery on the promise to reform health care. Others involved in the business of health care are not so quick to celebrate the new law’s arrival, and some compare it to an out-of-control nightmare that Americans have yet to see completely unfold. The arrival of the ACA is modifying every aspect of health care. Much like the consumers who must navigate health care exchange websites to purchase insurance, insurance agents and doctor’s office personnel are adjusting to the new business model. Peter Trozan is an independent broker in Fort Collins who specializes in selling health insurance. He’s been in business for 28 years, and he also writes the monthly Insurance Doctor article for The Coloradoan. His

interaction with clients today is quite different than before. “Since the implementation of ACA, everything has changed—during the underwriting process, I no longer ask medical questions since pre-existing conditions are no longer a factor,” he said. Instead, he asks for a client’s financial information to determine if he or she qualifies for government subsidies to offset the cost of private insurance, or Medicaid, the government-funded social health care program for low-

income individuals. For eligible participants, determining if they qualify for Medicaid and signing up for the benefit are often two different stories. “Applying for Medicaid has been a big problem,” Trozan said. The application typically requires two to three hours of client time working through issues via email, the internet, or sitting on hold on the phone. People sometimes turn to Trozan when they get stuck in the application process. Working through the Colorado system, however, has been a breeze compared to the federal government’s website, and there might be light at the end of the tunnel. “It (the Colorado system) has definitely gotten better, but it might be a year before it’s working smoothly,” he said. For private insurance policies, which are offered through Connect for Health Colorado, people have a decision to make based on premiums. An HMO plan, such as Kaiser Permanente, is less

expensive than coverage that offers variety when it comes to picking a doctor. “There’s a lot of philosophical tension where consumers are choosing to trade the freedom of choice for a lower price,” Trozan said. Some consumers are willing to pay a higher price to see the best specialist; if customers go with an HMO, they can’t anticipate where that specialist will be if they want to see someone outside their network. The ACA is also affecting how doctor’s offices do business. Richard “Mac” McMullen is the office administrator at Rocky Mountain Family Physicians in Fort Collins. He is responsible for the day-to-day operations for the group of eight health care providers and several office staff, and keeps his finger on the pulse of all aspects of the ACA. “This is the reason our patients are screaming at us,” he said, referring to the change in payment methodology that patients have experienced when coming in for an appointment since January 1. Until implementation of the ACA, most patients with insurance would make the co-pay for their appointment, and could typically expect to hand over about $25. Today, patients who buy health insurance from an exchange will

typically have a high-deductible plan (depending on monthly premium selected—the higher the premium, the lower the deductible), averaging around $3,500 annually for a nonsmoking middle-aged female, which means in addition to paying monthly premiums, patients will pay for health care out of pocket until they’ve met their annual deductible. “When patients come in with new insurance, and their policy has a very high deductible, they’re outraged (when asked to pay-in-full for their appointment, which averages $140),” McMullen said. The result of this patient frustration is that some are unwilling to stop and pay on their way out, forcing McMullen to change the payment procedure. The office staff now takes a significant portion of the appointment cost at the time of visit check-in, instead of after the patient has seen the doctor. “If that patient walks out and we send a bill for the full office visit, we have a hard time collecting anything.” McMullen points out that the work doctors do isn’t changing, but the methodology on how to pay for it is. “Our doctors are kept out of the business—the business of health care has changed, but the care patients receive hasn’t.” Patients don’t

necessarily understand that the cost they pay for office visits covers all the aspects of running a health care business. “My job is to keep the lights on. There are 27 employees here, and 27 cars in the parking lot, and 27 people paying mortgages or rent,” he said. Unpaid bills would force the office to shut down, which trickles down through the economy. Most people agree that health care costs are out of control and

that something needed to change. McMullen concedes that certain aspects of the ACA are good, but it’s not a cureall. “I’m all for Medicaid as a program for kids who need it—they need to get their vaccinations and to be able to see

a doctor, or for adults who have lost jobs and their insurance,” McMullen said. McMullen’s career in health care started over five decades ago folding sheets in the basement of a hospital. He eventually became a physician’s assistant, and then traded patient care to work on the business side. He finds the recent health care overhaul with its government mandates and paperwork to be disturbing. “I’ve been in medicine since 1960, and I have no way of describing what’s happening. If we don’t do due diligence, we won’t have jobs,” he said. “When something is such a train wreck, I don’t understand why it’s

still being pushed forward. The government’s agenda is to make sure everyone has the same kind of insurance and once you get on the train, you don’t want to get off. It’s all about control—if the government can control our health care, they control us, and we’re well on our way,” he said. Some obvious good news with the ACA for doctors is the opportunity for more caseload. More people with health insurance benefits than before will likely equal more patients and increased revenue—plus the bonus of more people having access to necessary medical attention—but doctors will also see more patients with Medicaid, meaning reduced payments for services and more paperwork. The big winners with the implementation of the ACA, however, are not the patients or the doctors, McMullen said, but the hospitals. Before ACA, consider a theoretical patient who had no health insurance and who developed appendicitis, then required surgery and a stay at the hospital. He’d receive a bill from the hospital, see the price tag, and either couldn’t or wouldn’t pay anything. The hospital receives $0 for the visit and must write off about $80,000 (or whatever the going rate might be). With the ACA, this patient now has health insurance and a $10,000 deductible. Instead of receiving a hospital bill for $80,000, he gets a statement for his $10,000 deductible, and the hospital receives $70,000 from the insurance company. The patient might still refuse to send the hospital a check to cover his deductible, but instead of missing out on $80,000, the hospital now only writes off $10,000. Insurance companies will also likely see a boost in their revenue as more

people are paying high premiums than before. Or, with a high-deductible option, some patients may choose not to see a doctor or will not meet their deductibles, so the insurance companies will not pay out anything on those policy holders. What will the future of health care look like? According to health care professionals interviewed, there will be fewer independent physicians because they simply either don’t want or can’t afford to keep up with the government mandates on how they do business. One example is the impending change to how doctors code medical records. Today’s system is the International Classification of Diseases 9 (or ICD-9), a database of diagnosis codes that doctors assign to indicate medical necessity that they submit on insurance forms to receive payment. The next revision to this system, ICD-10, is set to take hold in October of this year, and will increase the number of available codes from ICD-9’s 15,000 to 70,000. The benefit of this change is labeled as more specificity in the documentation of diagnosis; the downside is the increased time it will take health professionals to code paperwork correctly. Insurance companies deny payment if something is coded improperly, forcing health care providers to correct the form, resubmit it, and if it’s correct, they’ll receive payment. “This is where doctors are going to fall on the sword,” said McMullen. “They might not fill out the form correctly the first time, and it will cause a loss of revenue. Predictions are that doctors better have a line of credit for 3 to 4 months to live on as they wait for payments.” Their survival will depend on whether they have the staff and resources to implement the new coding procedure correctly.

The Affordable Care Act (ACA) and the consumer’s experience of taking the plunge into obtaining health insurance has been a hot topic since the implementation of the new law. Physicians are also managing the changes brought about by the ACA as it impacts how they do business. Opinions vary about this new business model: Some view the new law as a way of adding patients; others don’t like what they’re seeing and genuinely fear speaking out publicly A group that will likely feel the ACA’s impact is the independent practitioner. These physicians are not employees of a hospital, such as Kaiser Permanente or University of Colorado Health, and include lone practitioners or a group of doctors who belong to smaller practices. These doctors might affiliate with a hospital to perform surgery or deliver babies, for example, but remain self-employed. With the ACA, future predictions for the independent doctor look different depending on whom you ask. “Things haven’t changed much yet,” said Garth Nelson, an orthopedic surgeon who manages his own practice and performs surgery at Poudre Valley Hospital. However, the independent doctor might become a rare find in coming years. “Statistically, there are fewer graduates going into solo practice because of the onerous government rules and mandates. If you’re part of a group, you can better afford to hire someone to oversee the

changes.” These changes include past government mandates, such as HIPPA compliance, and the future changes that will likely involve electronic medical records and coding requirements. Dr. Nelson’s patient load hasn’t fluctuated much yet either, which might change now that more people have health insurance. His patients have commented that they appreciate that pre-existing conditions are no longer excluded from their insurance plans; many believe this modification to health insurance is one of the biggest patient benefits of the ACA. Dr. Mark Rotman practices family medicine and has been part of Associates in Family Medicine, a group of about 50 providers, since 1991. He was the ninth physician to join the practice. He views the ACA as favorable change for his caseload. “More people will have insurance, which should increase the number of primary care patients,” he said. with one doctor heading into hospital administration, and one moving over to Banner Health. Working within the hospital model has so far been mostly positive for Breyer. “There are good and bad aspects with all changes. As part of a bigger system, we’re operating efficiently. We still have the same day-to-day operations and the same staff. We haven’t noticed any real differences yet.” Other physicians agree that the hospital-managed care model is something lone practitioners might gravitate toward because it is simpler. “The benefit of a hospital is the safety and security of working for somebody—you can better control your workflow and hours, and your focus is based on patients (instead of managing your business),” Rotman said. Dr. Nelson adds that there are pros and cons of hospitals providing one-stop health care shopping. “It’s less personal, but it works well for some people,” he said. But the advantage of seeing a primary care doctor who is familiar with your family and all your history may be more difficult to come by. Breyer agrees with the opinion that there will likely be fewer independent physicians to choose from in the future, which made her group’s decision to merge with the hospital important. “No one in our group was ready to retire, and you’re taking a gamble if you choose to remain independent,” she said. She also added that a benefit of a more cooperative environment can lead to better care. “Medicine has become very complicated,” with patients being on different medications and seeing different specialists,

she said. “We need to have the tools to cooperate; integration is better for patient safety.” Future medical school graduates will need to consider their options when deciding how to practice medicine. Breyer said, “There is pressure to control costs, and a lot of independent physicians are feeling the pressure to keep their doors open. People need to be in agreement with a hospital’s mission,” if they’re going to join a group. Would Breyer make the same career decision if she

had to do it all over again? “I love medicine and being involved in caring for patients and their families,” she said. “It’s a great job, but right now there is a lot of uncertainty and headaches.” With the expense of medical education, new graduates need to plan accordingly. “I’d recommend becoming a doctor, but be careful,” she advises.

With the deadline approaching to sign up for insurance through the Colorado health exchanges, many residents are faced with choosing the plan that’s right for them and their families. Often, the alphabet soup of different plans and terms can be overwhelming. Here’s a brief primer on what some of the terms mean and how they affect you: Coinsurance: Your share of the costs of a covered health care service. Plans will often share costs 80/20, meaning the plan pays 80 percent, and you pay 20 percent after you’ve met your deductible. Copay: The fixed amount the insurance company requires you to pay for medical services. There may be separate copays for different services. Deductible: The amount you owe during the benefit period — usually a calendar year — for health care services before your health insurance begins to pay. If you have a $1,000 deductible, your insurance company won’t pay anything until you’ve met it. Some plans may have separate deductibles for specific services.

Flexible spending accounts or arrangements (FSA): These provide a way for you to set aside pretax dollars from your paycheck to pay for your share of insurance premiums or medical expenses not covered by the health plan. Employers may also make contributions to an FSA. Typically, money set aside in the account must be used within the year or you will lose the money. Health savings accounts (HSA): These are savings accounts designated for out-ofpocket medical expenses. Employers and individuals are allowed to contribute to a savings account on a pre-ax basis. One major difference between a Flexible Spending Account and a health savings account is the ability under an HSA to carry over the unused funds for use in a future year. In-network copayment: A fixed amount you pay for health care services to providers who contract with your health insurance or plan. They usually cost less than out-of-network copayments. Managed care plans: Managed care plans generally provide comprehensive health services to their members and offer financial incentives for patients to use the providers who belong to the plan. Examples of managed care plans include

health maintenance organizations (HMOs), preferred provider organizations (PPOs), exclusive provider organizations (EPOs) and point of service plans (POSs). Maximum out-of-pocket expense: The maximum dollar amount a person is required to pay out of pocket during a year. If your maximum is $3,000, you will be asked to share the costs with your insurance company (after you’ve met your deductible) until you’ve paid the $3,000. After that, the insurance company will pay 100 percent of the costs for the rest of the year. Under many plans, doctor and drug copays are not part of the out of pocket formula and, consequently, never go away. Preferred provider: A provider who has a contract with your health insurer to provide services, usually at a discount. Premium: The price of your health insurance. Premiums can be paid by employers, unions, employees, or shared by both. Primary care physician: A physician who serves as a your primary physician within the health plan. In a managed care plan, the primary care physician provides basic medical services, coordinates and, if required by the plan, authorizes referrals to specialists and hospitals. In 2014 many aspects of health care are being defined and redefined in the light of the Affordable Care Act, aka “Obamacare.” The new law isn’t just about insurance coverage; it’s about changing the way health care services are provided. A big focus is on preventive health care. By implementing more preventive health initiatives and providing broader access to care, health insurers and providers are hoping to halt or inhibit the advance of both chronic and acute health conditions. Let's take a deeper look into preventive care: As defined in the US National Library of Medicine, “Preventive medicine or preventive care consists of measures taken to prevent diseases, rather than curing them or treating their symptoms.” Additionally, many preventive care methods are aimed at substance abuse. Preventive care is often implemented at three levels: primary, secondary and tertiary. Primary prevention strategies are aimed at avoiding the occurrence of disease, secondary strategies are meant to diagnose and treat existing disease issues while they are in the early stages, and tertiary care methods are intended to reduce the impact and morbidity of existing disease by restoring function and improving disease related symptoms. These strategies are employed in a threetier model at universal, selective and indicated levels.

Universal prevention is aimed at entire populations, these types of programs are intended to prevent and educate entire groups about particular disease risks or substance abuse problems. Selective programs target a smaller group of people: those who are at above average risk for developing a particular disease or abuse problem. Indicated prevention involves screening participants for signs or symptoms that indicate early onset of said substance abuse or disease. Basic preventive services are provided on a universal level, yet some services that are “covered” by insurance have to be provided at an indicated level, meaning you have to meet requirements that put you at higher risk or are already having symptoms of the particular disease or abuse problem in order for your insurance plan to pay for the service. Many of these requirements are age or gender

based, yet some involve family health history, race or ethnicity, or personal lifestyle habits. WHAT'S THE BIG DEAL? The Affordable Care Act requires health plans to cover a wide array of preventive care services such as immunizations and screening procedures. »Well » Woman Visits: to obtain recommended preventive services WHAT'S NOT COVERED? Naturally there are loopholes and conditions that can prevent some services from being covered. The preventive services required to be covered by the Affordable Care Act apply only to insurance plans that were implemented on or after March 23, 2010. If you have a health plan that was in place before then you may be “grandfathered,” meaning you can keep your health plan with all the current benefits it provides without your insurer having to make the changes required by the Affordable Care Act. If your grandfathered plan does not cover a preventive service that would otherwise be covered under the ACA, you may have to pay out of pocket for that expense. A few other things a grandfathered plan may not cover are your right to appeal a coverage decision, protection of your choice of doctors and access to emergency care, and being held accountable through Rate Review for excessive premium increases. Other services or procedures might not be covered if you do not fit into the target population for treatment or screening. There may be specific age requirements or gender requirements that must be met in order for a screening or counseling to be covered. Many of the listed services are only indicated for those at “higher risk.” This can be determined by race, family history, previous health, or current health status. Whether you are in favor of or against the Affordable Care Act, it's here and there are more changes coming. Knowledge of your rights and coverage is power in the health care industry. Know your plan, know your benefits, and begin to take charge of your health.
