I’ve been homeless for 15 years. No one has been nice to me until now! Community Outreach Program Serves Homeless Population Joe broke down and cried when Livingston nurse David Cates, RN gave him a sandwich, a sleeping bag, and a goodie bag filled with water, band aids, face wipes, fruit bars and applesauce. David works with the CATCH (COPD Access to Community Health) Program, a grant-funded initiative, implemented through the Ventura County Health Care Agency (VCHCA). Inside This Issue:

WHAT IS COPD? • COPD refers to chronic lung disorders that result in blocked airflow in the lungs. It worsens over time, making it hard to breathe. • It is preventable and treatable! • In Ventura County 28,000 people have COPD. • Identifying and treating COPD in its early stages can drastically change/slow the progression of the disease. A simple test called a Pulmonary Function Test (PFT) can be performed to determine if someone has COPD. • COPD is the third largest cause of death in the nation. • Smoking is the number one cause of COPD.

uality home healthcare is embedded in Livingston’s core. Nearly every action or initiative we take starts with the question, “How will that improve the quality of our patient care?” Sometimes the quality of our patient care is reflected in referrals from Livingston patients to family members and friends or from our partners within the healthcare industry. Other times quality of care is measured by our low rehospitalization rate and pain management scores. Sometimes we receive recognition from third parties who have their own criteria of what “quality” means. Most recently Livingston was recognized as the only Ventura County home healthcare agency to receive a Four Star rating in both quality outcomes and patient satisfaction by the Center for Medicare and Medicaid Services (CMS). This is especially gratifying as the input for the star ratings comes directly from our patients. Patient satisfaction surveys are sent to individuals based on eligibility criteria set by CMS and then randomly selected by a Medicare approved vendor who manages the survey process. The results represent how patients respond to questions related to the care they received from our clinicians and other staff. Eighty three percent would DEFINITELY recommend us to family and friends. That is nine percent higher than the California state average. Also, within the last year we have been recognized as the Customer Satisfaction Leader of the Year by the Ojai Valley Chamber of Commerce for consistently delivering quality customer service. The Ventura County Board of Supervisors’ Healthy at Healthcare Committee selected Livingston to receive its Health Champion Award for providing home healthcare and hospice to Ventura County residents. These third party acknowledgements of our patient care are wonderful statements about Livingston’s hard work on a daily and patient-by-patient basis. You have counted on us to take care of you and your loved ones for seventy years. We believe it is Livingston’s commitment to quality home healthcare that has earned your trust and loyalty. Thank you for the privilege of caring for you and those you love. Laura K. McAvoy, Board Chairman Lanyard K. Dial, MD, President/CEO and Medical Director 2

Mission Statement Livingston Memorial VNA Health Corporation, Livingston Memorial Visiting Nurse Association and Livingston CareGivers recognize the growing need to provide services in the home environment for the sick, handicapped, elderly, terminally ill, and homebound. As community-supported non-profit organizations, they strive to provide professional and supportive home health and hospice care services of the highest quality in the most cost effective manner to the people of Ventura County to ensure a positive patient outcome. Livingston explores and develops health-related programs based upon community need and acceptance, economic viability and availability of resources to provide services. Livingston is sensitive to the financial circumstances of its patients and provides needed services, including those at reduced or no charge to indigent patients, based upon the available resources of the organization. The Livingston Memorial VNA Health Corporation is the parent organization of Livingston Memorial Visiting Nurse Association and Livingston CareGivers. Livingston Memorial Visiting Nurse Association is licensed by the State of California, certified by Medicare and Medi-Cal, and is accredited by The Joint Commission. Joint Commission accreditation is regarded as the most comprehensive review of patient safety and quality efforts in the industry. Livingston CareGivers is a non-medical provider of in-home assistance and is CAHSAH certified and not associated with The Joint Commission.

n recognition of the founding of Livingston Memorial Visiting Nurse Association in 1947 and in gratitude to Tom Pecht for his enduring devotion to the organization, a gala celebration is being planned. Please join us for an evening of family, friends and fun. You can count on delicious dishes, umbrella drinks, a beautiful lakeside setting, all while celebrating Livingston’s 70 years of service and honoring Tom. Don’t forget your classic Hawaiian dresses and shirts. Of course paniolos (cowboys) are encouraged and welcome. Livingston founding nurse Olivia Hathaway made her first house call on July 21, 1947. By 1955, the agency’s three nurses were visiting nearly 300 patients a year – and a third were indigent residents who could not pay. Seventy years later, Livingston remains Ventura County’s only non-profit provider of comprehensive home healthcare, medical hospice and in-home support. Today Livingston’s home healthcare professionals see nearly 5,000 patients per year and continue to provide care to the majority of the county’s uninsured and underinsured. Board Member Tom Pecht is many things: family man, farmer, business owner, cowboy, traveler, scuba diver, USC Alum, community leader, philanthropist and friend. He brings life and world experience to Livingston, where his steadfast leadership helps Livingston make Ventura County a kinder and more caring place to live.

They provide diet counseling and refer patients to services according to their needs. As David says, “Education is the biggest thing we do.”

Improving Lives, Reducing Costs “CATCH is also reforming the way in which we pay for the treatment of COPD, reducing costs by an estimated 22% overall. During the first year of the project, CATCH has been able to reduce Emergency Room visits by 18% and the number of critical vaccinations for COPD patients has increased by 25%. CATCH is improving the quality of life for COPD patients while reducing the costs of treatment,” says Dr. Renee Higgins, CATCH Principal Investigator. The goal of the CATCH program is not just to reduce emergency visits and hospitalizations, but to improve the quality of life for patients through smoking cessation classes and support, improved medication regimen, nutrition counseling and emotional support. n Pain

I have been told I have heart disease. Can you explain what it is and how I can reduce my risk of having a heart attack? Coronary artery disease (CAD) is the most common type of heart disease in the United States. For some people, the first sign of CAD is a heart attack. You and your health care team may be able to help you reduce your risk for CAD. Heart Attack Signs and Symptoms The five major symptoms of a heart attack are: n Pain or discomfort in the jaw, neck, or back. n Feeling weak, light-headed, or faint. n Chest pain or discomfort.

or discomfort in arms or shoulder. n Shortness of breath. Other symptoms of a heart attack could include unusual or unexplained tiredness and nausea or vomiting. Women are more likely to have these other symptoms. When To Call 9-1-1 If you notice the symptoms of a heart attack in yourself or someone else, call 9-1-1 immediately. The sooner you get to an emergency room, the sooner you can receive treatment to prevent total blockage and heart muscle damage or reduce the amount of damage. At the hospital, health care professionals can run tests to determine whether a heart attack is occurring and decide the best treatment. Remember, the chances of surviving a heart attack are greater the sooner emergency treatment begins.

Reducing Your Risk for CAD If you have CAD, your health care team may suggest the following steps to help lower your risk for heart attack or worsening heart disease: n Lifestyle changes, such as eating a healthier (lower sodium, lower fat) diet, increasing physical activity, and quitting smoking. n Medications to treat the risk factors for CAD, such as high cholesterol, high blood pressure, an irregular heartbeat, and low blood flow. n Surgical procedures to help restore blood flow to the heart. Please send us your “Ask Livingston” questions. Call Gloria Forgea, RN, MBA, at (805) 642-0239 or email Gloria Forgea, RN, MBA

Personal Care is Peace of Mind

ivingston CareGivers has provided vital, non-medical, personal assistance in the home to residents of Ventura County for over 15 years. Our clients encompass not only the frail, disabled and elderly, but anyone and any age that needs personal help at home. When daily needs are managed and addressed, people can remain in their home, which is often a preferred option to long term placement. Many studies show that home is where the elderly prefer to be and do best. Livingston CareGivers provides numerous services, but the most important benefit we provide you with is the peace of mind in knowing that a trusted agency, part of the Livingston Family, is here for you and your loved ones.

A Gift for Later

Silver Lining Comes with Silver Hair

o you complain about the aches and pains of aging? Most of us do. Well there is at least one benefit to getting older; Charitable Gift Annuity (CGA) rates get better and better the older you are. A CGA is a contract between you and Livingston that exchanges a gift for secure, fixed payments for life to either you or you and your spouse/partner. You get an immediate charitable income tax deduction and whatever amount remains at your passing benefits Livingston. Also, a portion of the annual payment is tax free, which results in you receiving a higher effective rate of return when compared to a fully taxable investment. Your gift may be funded with cash or stock. If you fund the gift with appreciated stock some of the capital gains tax is eliminated immediately. The remainder of the capital gain is prorated over your entire life expectancy. Once you take out a gift annuity, the amount you receive annually remains the same for life, regardless of what happens to the economy or stock market. You must be 65 years or older and $25,000 is the minimum investment amount. Rates of return generally range from 4.5 to 8.5 percent, which compares very favorably to

short-term money market, CD and savings account rates. Your actual rate will be determined by your age, gender, and whether it is a one or two life annuity. For your peace of mind, Livingston partners with the California Community Foundation, which has been serving Southern Marc Wilde California nonprofits for over 100 years and holds $1.5 billion in assets. Your annuity will be safe and in very capable hands. Please contact Marc Wilde, CFRE at (805) 642-0239 x603 or to get your CGA rate of return and receive a free illustration of how a Charitable Gift Annuity can benefit you and Livingston, a real win-win for those of us who are getting a little older. Please note: Livingston is not qualified to provide specific legal, tax or investment advice. This column is intended to provide general gift planning information and education. Building Bridges

Sherry Doyle

t began 12 years ago when her friend, Eleanor Land, who ran the volunteer program in Ojai, offered volunteer training to Sherry Doyle. Always looking for a way to enrich her life, Sherry signed up for the class. She was soon “hooked.” Now, over a decade later, Sherry says the best thing about being a Hospice volunteer is the connection with people. She says it’s unique and special to be invited into someone’s life at such an intimate time. She loves that she sometimes becomes the bridge between the dying patient and their family, helping to connect them in such trying times. Sherry feels that being that bridge is an important role of a volunteer. Ojai Volunteer Coordinator Kathy Leary-Wilde says of Sherry, “She really steps in and shows up in any way she can to serve her patients and their

families. It’s obvious to me that Sherry brings a sense of security and safety to her patients in her confident and sensitive manner of caring and serving. Sherry is an Ojai treasure!” In her spare time, Sherry teaches art to kids at a variety of schools in the area. She also meets weekly with an art group. She is very close to her three grown sons and enjoys hiking and golf. When asked about volunteering with Livingston, Sherry states “I feel a sense of honor being part of this organization. The work we do collectively is so important and powerful. I am always aware that I want to honor that and be sure we get it right.” We are lucky to have such devoted volunteers as Sherry. If you are interested in volunteering for Livingston, please contact Maddy Hazard at (805) 642-0239 x775 or
