Energy applied to registered therapeutic processes In order to produce a therapeutic process through the use of energy, we need to respect the optimal dose regardless the mode. What is Tecarforesi transdermal delivery method Tecarforesi is transdermal delivery associated to Tecar (Capacitive and Resistive Energy Transfer). A painless method without side effects. This method allows to transport through the skin active ingredients with high molecular weight, without intermediation of blood circulation in the first phase (i.e. drugs, homeopathic products, phytocompounds). The system uses low electrical pulses as carrier (electroporation). The electric wave of stimulation is produced by a generator of constant current able to evaluate the changes of skin resistance, in stable and reproducible conditions. With suitable changes of electric parameters, this method makes possible the administration of substances through the skin, without altering the tissues and reaching optimal depths. Transdermal delivery of active principles in gel and H20 Transdermal delivery is a non-invasive mode of administration of drugs or medicinal substances through the skin, without the use of the needle, directly to the site of interest. The transdermal delivery uses current micro-pulses to speed the absorption, which occurs in a few minutes. The beneficial effect is quick and doesn’t create any problem to the skin texture, as instead happens with other methods, for example with the laser that damages the skin until renewal (the patient can’t return at once to his social activity). Transdermal delivery built on adequate energy parameters, has the ability to deliver at first active ingredients through extracellular way. In a second phase the active ingredients of lower molecular weight can penetrate into the cells stimulated by the energy charge, obtaining a greater performance on the tissue we intend to treat.

What is Pressure Wave method When the pressure waves cross the biological tissues they lose a small part of their energy. When the shock wave interacts with biological tissue some effects develop, in particular: •

Specifically, the main therapeutic effects of pressure shock waves are:

Analgesic: Pressure shock wave is widely used when we need an analgesic effect, as on pathologies of the locomotorium system. This effect is caused by the action of the heat on the tissues and also probably by a direct action of pressure waves on sensitive nerve endings.

Fibrinolytic The oscillations produced by the pressure waves on particles of biological tissues modify collagen structure.

Trophic The vasodilatation that follows the thermal increase, removing the waste, enables the oxygen and nourishing substances to reach the tissues. This implies an improvement of tissues trophism with subsequent reparation of tissutal damages and rapid resolution of inflammatory processes.

What is Tecar method It’s the application of an energetic transmission system in radio-frequency, applied to the human body as a curative method, which controls and quantifies the quality and quantity of the energy accumulated by the body. Tecar is therefore a medical device that uses the energy applied to therapeutic registered processes. Premise: in order to obtain these conditions with a physical principle of endogenous radiofrequency we have to give the role of main actor to the human body, so that the principle is suitable in real-time and meets the clinical and biological processes of the organism.

How Tecar works It’s a generator that uses the physical principle of the condenser at high frequency, by creating an electromagnetic field between active and negative electrodes, involving the human body in a hyperactivity of movement through the shifting of electrolytic charges within the body. The pain relief is obtained thanks to the analgesic action that is induced by the perception threshold of the algotrope nerve endings and the release of endorphins. The anti-inflammatory effect, anti-edema, eutrophic and stimulating for the cellular tissue is the increase in blood flow due to capillary and arteriolar vasodilatation. The effect of increased absorption of interstitial fluids, with consequent reduction of edema and of activation of cell replacement, is due to the change of the hydrostatic intra-capillary pressure. The therapeutic effect depends on the interaction of the laser light with the biological tissue concerned or: each type of laser emits light at a specific wavelength that interacts differently with the chromophores present in the tissue. Some chromophores are already present in the tissue (water, hemoglobin, melanin, proteins, nucleic acids) while others may be injected instead, fixating themselves on the tissue to make it photosensitive. Water Therapeutic Window

Interesting, from the point of view of depth treatment, turns out to be precisely the latter band of wavelengths, the so-called “therapeutic window”. The water here is absorbed in a very high percentage and laser radiation penetrates into the tissues without any interaction with specific chromophores. It therefore follows that the level of penetration of the laser depends directly on the wavelength of emission and on the laser power.

Depth and absorption Each laser emits its beam to a well defined wavelength, and obviously each different type of laser has the ability to reach a certain depth in target tissues. Laboratory experiments conducted in various Universities have shown an increase of absorption of oxygen by cell cultures subjected to irradiation with laser energy of different wavelengths. In particular, we have activation on two zones of the “spectrum of wavelengths”: between 600 and 1200 nm.

Absorption and dispersion From the point of view of “Physics”, the laser beam passing through the biological structures interacts through two mechanisms: absorption and dispersion. In fact, the cell of living tissue is presented as a semitransparent system (a kind of glass bowl with a semi-opaque content) that retains a certain amount of energy, and which transfers the energy in excess that hit it in part to “reflection” and in part for its “crossing” and transfer to the underlying layers.

Penetration and incidence Although the impact with the skin of the laser beam is perpendicular (and the photons are in parallel), as occurs gradually tissue penetration (for dispersion effect) the section of irradiated tissue has a vaguely conical development and made irregular by different density of the structures crossed.

These graphics allow to observe that the increase of the wavelength is not strictly correlated to an increase in the level of absorption by the tissues. As can be seen, most of the absorbed energy stops in the first skin layers (epidermis and dermis). An extremely important factor are the chromophores, or particles that absorb light radiation and that render the laser a device of high selectivity and precision for the cure of the disease. For the same amount of energy administered, the differences of application by virtue of the wavelength, determine the interaction with the chromophores present in the tissue. The wavelength of 810 nm is mainly absorbed by the melanin, instead of the 975 nm and 1064 nm that are mainly recognized by hemoglobin. For the same amount of energy absorbed by tissues and of depth, the interaction of the wavelengths will determine specific therapeutic responses, different from each other.

Biostimulation The biostimulation is an interaction that occurs at molecular level on the cells of tissues, changing and influencing the intracellular state and extracellular responses that occur during the inflammatory stage, in order to obtain the faster expression of the physiological potential of healing of wounds, all aimed at morphofunctional restoring of an organ. The term was introduced in medicine with the discovery of the properties of laser to improve the outcomes of diseases and of healing. In order to distinguish these lasers from those with surgical skills (cutting, coagulation ..), they are called “therapeutic lasers”. The term has never been introduced in dictionaries, but is used more and more often in the current language. The use of the term originates from the attention of medicine to a biologically guided surgery, where assistive technology, the use of biomaterials are half to influence physiological processes in order to obtain faster healing, stimulating the regenerative processes of tissues. For example, in dental surgery is important the choice of methods that allow to contain the loss of tissue, especially bone, guiding regenerative processes in terms of both quality and quantity, to improve the oral rehabilitations from functional and aesthetic point of view. The set of these techniques can be defined by the term “biostimulation”.

The connective tissue This section has been introduced as the therapeutic action of the laser is directed to the connective tissue. The laser is a highly selective device. The laser light allows stimulating the fundamental components of the extracellular matrix such as glucosamine sulfate, chondroitin sulfate and high molecular weight polysaccharides (glycosaminoglycans). As regards the chondroitin sulphate, experimental evidences demonstrate its chondroprotective activity (stimulation of proteoglycan synthesis, inhibition of collagenolytic activity, inhibition of the effects of IL-1 on the synthesis of proteoglycans and collagen, reduction chondrocyte apoptosis). The stimulation of chondroitin sulphate makes possible to stop the progression of structural damage. Connective tissue is a type of tissue of mesodermal origin that provides structural and metabolic support to organs and other tissues inside the body. The Skin Our skin is a medium of communication because it’s what the others see of ourselves and vice versa. The outermost layer of the skin, the epidermis, is characterized by wrinkles and furrows, and their design depend on several factors such as elasticity, the arrangement of skin appendages and, on hands and feet, fingerprints distinguish each of us. The outer skin is formed for the most part by keratinocytes, cells with particular resistance to mechanical insults and with very selective permeability against exogenous substances. The epidermis has melanocytes, containing melanin, which protects us from the sunbeam. Connected to the epidermis is the dermis, which forms the substrate for mechanical support and metabolic exchange, constituted by blood and lymph vessels. The dermis is composed of fibroblasts, collagen fibers, elastic fibers and ground substance that are the bricks which support the architecture of the skin: we have to act here to enhance the strengthen and keep the skin young. 20


Why the skin gets old? The skin gets old because the “bricks” of collagen and elastin reduce and the TEWL increases significantly. The dermis is a highly elastic layer, resistant to traction that must be protected, defended and nourished in order to remove the signs of aging. The ground substance is an amorphous gel which contains mainly water. The glycosaminoglycans dissolved, including hyaluronic acid, assemble collagen and elastic fibers: from here it is clear that an optimal hydration is fundamental. The so-called skin appendages, hair and glands, have the fundamental function of thermo regularization and allow the secretion of substances from the body towards the outside: without them, this step would be minimal because the skin is a waterproof organ. The hypodermis is the tissue interposed between the dermis and the muscular apparatus, formed by adipocytes, rounded cells that isolate the deeper layers and absorb them from external insults. The skin cannot be considered as a simple coating, but a real organ with necessary tasks: it’s a sensitive organ which supports, protects and maintains constant body temperature.

For all these reasons, it’s important to protect the skin and preserve it, the most possible, by using methods that nourish it in a non-invasive way.

Clinical case of therapy after tibialis anterior muscle strain. A.P. , basket player, 27 years-old, complained about pain in the anterolateral face of the left leg, after a quick change of direction of movement performed during a regular league match in June 2012. He came to the attention of the doctor the next day: from the visual inspection were obvious redness and swelling of the part referable to the left tibialis anterior muscle. The doctor recommended the athlete to undergo diagnostic ultrasound, performed on June 4, 2012. After ultrasound evaluation, the doctor decided to start a six sessions instrumental treatment with TECAR SIN following the treatment protocol, three times a week. During the acute phase the doctor chose the program P1 - power: 20% - using Winform Demusco gel. The device was applied on two proximal and two distal point of the lesion, duration of 4 minutes each point. The geometry of the positioning of the handpiece and neutral plate was rigidly adhering to the treatment protocol, so that the detected lesion was never be found between the two elements. On the area of the lesion was then performed program P1 for about 4 minutes - power: 20% - using Winform EDTA Gel 10% to facilitate absorption of the hematoma. After the third treatment the doctor changed the power of program P1, rising it from 20% to 60%, using Winform Demusco gel again, moving the handpiece all over the left anterior tibialis, then on the plantar fascia and sural triceps, and on hamstrings (posterior thigh muscles - the flexors of the leg) on last analysis, for the duration 12 minutes per segment. At the end of the 6 treatments setted, the doctor resent A.P. for a new ultrasound evaluation of post-rehabilitation, on 6 July 2012.

The medical device WS2 Therapy is an ultrasound at digital emission of last generation.
