VON offers a wide range of community health care solutions, some of which are offered 24 hours a day, 7 days a week. Each VON site modifies its services to meet the needs of its local community. VON offers home care, personal support, and community support services to enhance each client’s quality of life by providing them with the personal assistance and support needed to make them comfortable in their own home. VON has been doing so for more than 100 years. Our professional staff works closely with clients, caregivers and all members of the health care team to develop a flexible and individualized plan of care. Some services are offered 24 hours a day, 7 days a week across Canada by specially trained Registered Nurses, Registered Practical Nurses and dedicated staff of Recreation Therapists, Personal Support Workers, Coordinators of Volunteers and Home Support Workers who provide homemaking, personal care, and respite care. Nursing Services Visiting Nursing and Shift Nursing Programs

VON offers a wide range of customized quality health care services. The VON Nurse works together with clients, caregivers and health care professionals to assess the individual’s health care needs and collaboratively develop a personalized plan of care to meet their needs. Our outcome based philosophy helps our clients live life to the fullest. All our highly committed, caring, nurses employed by VON are registered professionals, knowledgeable and experienced in all aspects of community health care. Every visit will consist of ongoing assessment, support, encouragement and holistic care.

Supportive Housing - Health starts at home This program enables the frail & elderly to “age in place� by providing personal support, homemaking and attendant services in a supportive housing environment by VON Personal Support Workers. This program supports clients who can no longer manage safely in their own homes but with the provision of individualized service, can continue to live independently with on-site assistance & 24 hour availability. Services include assistance, supervision or training with personal support for washing/ bathing, personal hygiene and preventative skin care. Activities of daily living, such as dressing, transferring & positioning, toileting, bowel routine, assistance with eating, assistance with pre-measured medications, medication reminders and exercising. Clients must have a valid Ontario Health card number to be eligible for the program. They must be able to direct their own care. They must live or be willing to move to a designated Supportive Housing site and meet tenancy requirements to rent an individual apartment. Clients require a volume or frequency of service more than CCAC can provide, need emergency response availability up to 24 hours a day. Have insufficient caregiver support, and without service, is at risk and would require placement in a long term care facility. VON has partnered with Safeguard to ensure 24/7 help for our clients. This program fills a large void in our community that currently exists for frail and elderly who require additional support however do not require long term care. This program also cuts down on hospital visits and continues to accommodate individuals to be able to age in place.

Home Help - Age in place The Home Help program is designed to help isolated seniors, seniors recovering from surgery, frail seniors and those persons with permanent physical disabilities, who do not have access to assistance, remain safely in their home by providing light housekeeping services. Upon acceptance, the client is assigned a Home Helper who can assist with light housework to maintain the functionality and cleanliness of the home. There is a modest fee for this service ranging from $5.00 to $20.00 per hour dependent on a financial assessment. This assistance may be provided weekly, bi-weekly or monthly. Assistance is flexible and based on individual need. The program objectives are to assist clients to “age in place� by helping them maintain a safe, hygienic and hazard free home. Our goal is to allow clients to maximize their independence in areas of their choice by allowing us to assist with essential housework. Services may include dusting, vacuuming, laundry, mopping floors, grocery shopping, cleaning of bathroom & kitchen and linen changes. By preventing accidents and encouraging safety, Home Help will reduce the need for untimely admittance to long term care. The Home Help program receives referrals from CCAC, community partners, family members, churches, physicians and clients themselves.

S.A.F.E. (Seniors Active in Falls Education) What is S.A.F.E.?

S.A.F.E. (Seniors Active in Falls Education) is a research and evidence based exercise and falls prevention program provided by VON Erie St. Clair under the direction of the ESC LHIN and the Ministry of Health. S.A.F.E., an hour long exercise class is facilitated by a certified senior fitness instructor who has been trained by the Canadian Centre for Activity and Aging, University of Western Ontario. Besides the prescribed components of Cardiovascular Conditioning, Strength Training, Muscle Endurance, Balance/Power and Stretching/Relaxation there is also an educational component which focuses on the main reasons for falls and appropriate intervention strategies to reduce that risk. S.A.F.E. is offered at no charge to participants. At the present time our focus is centered on bringing this program to the Rest and Retirement Homes throughout the Erie St. Clair District.

Meals On Wheels - More than just a meal! Meals on Wheels (MOW) helps those who are unable to shop for their own food or to cook, maintain their independence at home by delivering nutritious, delicious and affordable meals throughout Windsor. MOW also provides clients with a caring connection to the community. Our clients include seniors, people recovering from illness or surgery, and people with disabilities. We assist those who need short term (caregiver relief, recuperation, seasonal) or long term assistance. No long term contract necessary. We deliver hot, nutritious meals which accommodate a variety of diet types and texture modifications. Our meals are delivered by our friendly volunteers, Monday to Friday between 10:45 am and 1 pm. Frozen meals are available for clients who prefer flexibility, such as just heating a meal for weekends or holidays. Working in partnership with the Unemployed Help Centre, meals are prepared in a brand new state of the art kitchen, featuring fresh, locally grown produce and fruits. Meals include: soup, entrée, (including a protein, vegetable and starch), bread and dessert. Vegetarian, chopped, minced and pureed meals are accommodated. There is a fee of $6.25 per meal. Veteran’s can receive subsidization through Veteran’s Affairs.

Volunteer with VON Meals on Wheels

Volunteers at the VON Meals on Wheels keep the wheels turning at our organization! Participate once a week, once a month, or be on call! We will work around your schedule. Volunteers are the key fundamental source of our Meals on Wheels Program. They deliver and serve hot, nutritious meals to seniors, new mothers and disabled individuals who are unable to prepare meals for themselves. Our driver’s and server’s help people in Windsor continue to live independently and to “age at home”. They enrich the lives of our clients by providing a warm smile and a friendly face. Meals On Wheels is a vital service to our community, we provide a “check” on a client’s wellbeing along with a meal. At VON our Volunteers are valued, respected and appreciated! Deterring Emergency Time Offering Urgent Respite (DETOUR) VON DETOUR Program officially began operation on April 12, 2012. This program is designed to provide emergency respite services for a period of up to fourteen (14) days to Windsor Essex County seniors age 65 years or older; who are exhibiting a mental health problem(s) in maintaining their independence; requiring 24 hour respite supervision from Personal Support Health Workers as determined by the Emergency Department GEMs, Psychiatric Assessment Nurses (PAN), Physicians and Mental Health staff in the emergency department.

DETOUR Objectives: • T o support the senior’s needs to return home with improved compliance in managing their own care; improved quality of life and independence. • To assess and optimize linkages between community and professional services, caregivers(s) and family; ensuring a wrap-around service model of care. • To provide family/caregiver relief. • To show evidence in the reduction of senior patient Emergency Department visits and hospital admissions thereby directly and indirectly improve access and reduce overcrowding in hospital emergencies. • To ensure VON alongside Community Support Service and Professional Health partners will meet the growing demands of caring for seniors in our community and across Erie St. Clair. Chronic Pain Program Chronic Pain Management Assessment & Referral Program Mission:

Evolve and Implement an individualized, multidisciplinary team approach for clients living with chronic pain, to obtain efficient access to health care services, while enriching quality of life through empowerment. The VON Erie St. Clair Chronic Pain Management Assessment & Referral Program (CPMARP) is a LHIN funded program that aims to service people living in the areas Windsor- Essex, Chatham-Kent and Sarnia-Lambton. The program provides services to people who are experiencing non-malignant chronic pain and provide them with pain management options and resources. Chronic pain affects all aspect of our lives, both physically and mentally. That is why the CPMARP clinical team is diverse with different disciplines and your pain will be examined or explored by any or by all of our clinicians. The Nurse Practitioner and/or Registered Nurse will review your pain history, your completed diagnostic imaging and your medications. They will also review your past and present medical interventions and their effectiveness. A focused physical examination will be done when necessary. Their findings will allow them to provide to both you and your healthcare provider medically based treatment options. Some possible examples of recommendations could include medication changes, various therapies, referral to specialists and specialized procedures. The Occupational Therapist will explore how pain affects your day to day function, and will provide you with any resources to help improve your function including self-management strategies to help you live a more engaged life. An OT can identify activities or behaviours that aggravate pain and will recommend alternatives. Alternatives may include the teaching and the use of adaptive equipment like the use of a walker, a cane, or a reacher. Assistive devices will allow you to better function with the least amount of pain. Other potential recommendations may include the use of community wellness programs that will allow for exercise, strength or physiotherapy. The Social Worker will provide support and education on how pain affects your life to help you cope with the daily stress of living with chronic pain. Linking you to organizations that have supportive services for people living with chronic pain may be a recommendation provided to you. To participate in the CPMARP program it is necessary for you to be referred by either your primary health care provider or by a community health care agency. In addition you must also be deemed eligible.

To be eligible you must have:

1. Non cancer related pain 2. Not be considered end of life (palliative comfort care) 3. And must have chronic pain greater than 3 consecutive months. Please have your primary health care provider call 1-855-419-5200 to request a referral form. All clients must be referred. Once VON receives your referral, you will be contacted by the program’s Intake department and the necessary process will begin. You will be scheduled with the necessary appointments and assessed at any one of our assessment locations. • • • • •

Foot Care Program Our specially trained foot care nurses provide hands-on foot care in your home, including: • • • • •

Assessment; Clipping and filing of toe nails; Treatment of corns, calluses, ingrown nails, thickened nails; Padding as necessary; Preventative foot care for high risk clients with diabetes, arthritis and circulation problems; • Referral to doctors, podiatrists or other health care professionals as necessary; • Health teaching. Student Nutrition Program Ontario Student Nutrition Program, South West Region

The Ontario Student Nutrition Program (OSNP) in the South West Region is administered by VON Canada, Windsor- Essex Site. Our mission is to ensure that every student in the South West Region attends school well nourished and ready to learn. In doing so, VON works in partnership with 10 public health units, 12 school boards and several community partners across the region to deliver funding and support to over 480 school breakfasts and snack programs. In total, VON flows over two million dollars in provincial funding to support school nutrition programs across the region. Prenatal Education VON Prenatal Weekender Program

• Meets needs of busy parent- to-be by providing information over a single weekend! • Provides 10 hours of in-class instruction and a hospital birthing centre tour • Content covers information about late pregnancy, preparing for birth, medical and nonmedical pain relief measures, comfort measures, postpartum recovery, newborn care and safety, breastfeeding, how to access community resources, and more! • Includes a Healthy Beginnings and Breastfeeding Matters prenatal guidebook • Light snacks, juice and coffee provided • Classes taught by an experienced maternal child RN and IBCLC lactation consultant • Registration fee is 125.00. Subsidized rates available based on financial need • New! Parents can now register for the Breastfeeding segment at the end of Saturdays class for 65.00, inclusive of guidebook 2014 Class Dates: June 13-14, Sept 12-13, Dec 12-13 Registration Fee: $125.00 There are subsidies available in each class thanks to the generous support of Shopper’s Drug Mart.
