GENERAL GUIDANCE NOTES FOR YOUR APPLICATION FORM: It is important that you give a potential employer good first impressions, as your application form is your foot into the door of what could be a great working experience. 1 Practice writing your answers on a blank piece of paper before completing the application form to help avoid making mistakes. Mistakes on the form will not impress an employer. 2 Complete the application form in black ink and use CAPITAL letters. 3 Answer all the questions giving specific information that is relevant to the job you are applying for, avoid the use of â€˜not applicableâ€™ or n/a. 4 You should take a photocopy of your completed form as this may be a useful reminder should you be invited for an interview. 5 If you have any difficulty completing this application form, your local Job Centre Plus office (or Jobcentre) can assist you.

Are you looking for full time work (16 plus hours), part time work (under 16 hours) or casual work (just a few hours a week).

Complete with factual information, in CAPITAL letters.
Give details of driving licences held i.e. motorcycle, car, HGV, PCV. Also declare details of any points on your licence.
Give details of previous employment starting with most recent first. You should give as much detail as possible about your duties in each job. Use a separate sheet if necessary.
Give details of school, colleges, universities or other training establishments attended starting with the most recent first. You should list any examinations taken and any training courses attended, as well as grades and results awarded.
This section gives you the opportunity to sell your skills to the employer. You should include any relevant information that you feel makes your particularly suitable for the job you are applying for. This may include previous experience, voluntary work, hobbies, language skills and other relevant qualifications. If you have no formal qualifications, you might want to include anything you are good at. Use a separate sheet if necessary.
Ask whether you consider yourself to have a disability as specified by the Disability Discrimination Act, that is if you have a long term health problem or disability which has a substantial and adverse effect on your ability to carry out day-to-day activities. Also mention in this section if your disability requires any special arrangements should you be invited for an interview.
You should provide the names, addresses and contact details of two people who will act as character references for you. This is usually a previous employer and someone whom you have known for at least 5 years such as a family friend or neighbour, but not a relative. Please check with your referees that they are willing to provide a reference for you before entering their names and contact details on your application form.
