Clinical Nursing Team In response to a need identified through COVID-19, an extension of nursing support for the COVID-19 positive community has been launched. This team is known as the COVID-19 Clinical Nursing Team. The health navigation hubs supporting our COVID-19 positive community are increasingly being asked for nursing intervention by primary care. An in-person service will fulfil both COVID-19 and non-COVID-19 related health needs. These may include (but is not limited to) taking blood samples, administration of medication and prescribed treatment orders. The team will work alongside a GP to ensure appropriate care is accessible. Once a need is identified for in-person support by a doctor, a referral will be sent to the existing community hubs. A Registered Nurse from the COVID-19 Clinical Nursing Team will provide care, with guidance from the GP. Once care has been provided, notes will be completed in CCCM, and the GP will be notified. The service will be available weekdays between 8 am and 4 pm with on-call capacity for weekends.
